を含み、反応溶剤の体積(ミリリットル)が、キトサンの少なくとも20倍の質量(グラム)である、高分子量カルボキシメチルキトサンを調製する方法が提供される。
自己癒合カルボキシメチルキトサン繊維の生成
A)合成
反応の直前に、クロロ酢酸ナトリウム(1.75g)を、4%水性水酸化ナトリウム溶液(7ml)中に溶解した。この溶液を、イソプロパノール(45ml)に追加して力強く加振し、混濁懸濁液を得た。この混合物を、キトサン繊維(1.50g)を収容した容器に追加し、このコンテナをシールし、約60rpmで18時間回転させた。
B1)ステップAの後に、今や透明化した反応溶剤から繊維を除去し、99:1のエタノール:水(200ml)を収容する容器に繊維を移した。この材料を1時間にわたり、15分おきに擾乱させた。1時間後に、この材料を取り出し、吸収性材料の複数の層の間において手で圧力をかけることにより、物理的に乾燥させた。大まかな乾燥の後に、この材料を周囲温度にて一晩真空乾燥させた。

自己癒合カルボキシメチルキトサン繊維の放射滅菌
実施例1のステップB2より得られた材料を、ガス透過性滅菌ポーチ内にパッケージングし、ガンマ照射によって30〜40kGyで滅菌した。ゲル透過クロマトグラフィによって滅菌前および滅菌後の材料の分子量を判定した。滅菌前の分子量は、約Mw700kDaであり、滅菌後の分子量は約Mw140kDaであった。材料の分子量の変化は、大きいものの、材料の物理的特性は、滅菌により大幅には変化しなかった。
自己癒合カルボキシメチルキトサン繊維のエチレンオキシド滅菌
実施例1のステップB2により得られた材料を、ガス透過性滅菌ポーチ内にパッケージングし、エチレンオキシド処理により滅菌した。ゲル透過クロマトグラフィによって滅菌前および滅菌後の材料の分子量を判定した。滅菌前の分子量は、約Mw700kDaであり、滅菌後の分子量は約Mw575kDaであった。材料の分子量の変化は、材料の物理的特性が滅菌により大幅には変化しないようなものであった。
自己癒合カルボキシメチルキトサン繊維の水吸収度
実施例3により得られた材料(100mg)を水(4ml)の中に1分間の間浸漬させ、引き上げた。過剰液体を排水させ、次いで、水和透明質量を秤量した。この材料は、著しい溶解を伴うことなく、水中においてそれ自体の質量の約25倍を吸収すると算出された。
自己癒合カルボキシメチルキトサン繊維の血清吸収度
実施例3により得られた材料(100mg)を血清(4ml)の中に1分間の間浸漬させ、引き上げた。過剰液体を排水させ、次いで、水和透明質量を秤量した。この材料は、著しい溶解を伴うことなく、血清においてそれ自体の質量の約13倍を吸収すると算出された。
水中におけるカルボキシメチルキトサン繊維の自己癒合
実施例3により得られた材料(100mg)を水(4ml)の中に1分間の間浸漬させ、引き上げた。過剰液体を排水させ、次いで、水和透明質量を4時間の間放置した。4時間後に、材料の各繊維は、自己癒合し、材料は、事実上等質の弾性ヒドロゲルとなり、荷重下においてその物理的構造を安定的に保持することが可能であった(図2)。
血清中におけるカルボキシメチルキトサン繊維の自己癒合
実施例3により得られた材料(100mg)を血清(4ml)の中に1分間の間浸漬させ、引き上げた。過剰液体を排水させ、次いで、水和透明質量を4時間の間放置した。4時間後に、材料の各繊維は、自己癒合し、材料は、事実上等質の弾性ヒドロゲルとなり、荷重下においてその物理的構造を安定的に保持することが可能であった。図2Aから図2Cは、創傷接触要素14内に創傷部滲出液が吸収されるステージを示す。基本的構成は、図1のものと同一であり得るが、図2の図面は、簡略化されたものである。図2Aは、創傷10内に新たに設置された清浄なドレッシングを示し、滲出液は、吸収されていない。図2Bは、一部に滲出液が満ちた創傷接触要素14を図示する。図2Cは、創傷部滲出液で飽和状態になった創傷接触要素14を図示する。しかし、創傷接触要素材料14の性質は、液体が、真空連結チューブ28によって、創腔24から、または実に創傷接触要素14自体から全く吸引されないものである。
ex vivo創腔への図1の装置の適用。メスにより、ブタの脚関節中に直径5cm、深さ5cmの創腔を切削した。創腔の領域の筋組織に生理食塩水を注入して、実験の期間中に組織が十分に水和されるようにした。カルボキシメチルキトサン繊維の不織球からなる2つの創傷接触要素を創腔に詰め、中央に直径5mmの孔20を有する、Smith & Nephew Medical Ltd社により作製された接着性シリコーンゲルシートCica Care(商標)で創腔および充填材を覆った。コイル状真空ホースに装着されたルアーロック式嵌着物をこの孔内に挿入して、Virtual Industries Inc社により作製された電池作動式真空源Pac-Vac(商標)に連結させた。創縁において直ぐに収縮が観察され、不織球を、皮膚の表面と同一の高さにまで圧し下げた。擾乱を受けさせずに、このシステムを8時間の間定位置に置き、8時間後には、流体は、腔部のパッキングから全く出ず、その中に収集されていた。
図1に基づいて、Smith & Nephew Medical Ltd社により作製されたAllevyn Adhesive(商標)の創傷カバー要素14を有する装置を組み立てる。バイオプシーパンチを使用して、Allevyn Adhesiveの上部フィルム中に直径3mmの孔を作製した。Mini Valve International B.V.社により作製されたシリコーンエラストマードームminiValve(商標)のパーツ番号DO 072.004を、この孔の上に接着させた。Virtual Industries Inc社により作製される小型真空源であるPAC-VAC V3200(商標)を、真空チューブ28によってハンドメイドシリコーンゴムカップ30に結合させた。
実施例2のAllevyn Adhesiveに基づいて装置を使用する。メスによって、ブタの脚関節中に直径5cm、深さ5cmの創腔を切削した。創腔の領域の筋組織に生理食塩水を注入して、実験の期間中に組織が十分に水和されるようにした。カルボキシメチルキトサン繊維の2つの不織球からなる創傷接触要素14を創腔に詰め、実施例2において説明した修正されたAllevyn Adhesiveドレッシングで、創腔24および充填材を覆った。実施例2において説明した真空源をオンに切り替え、真空チューブカップを、Allevyn Adhesiveの上に中央に配置されたドーム弁を覆うように配置した。Allevyn Adhesiveドレッシングおよび創縁において緊縮が直ぐに観察され、不織球を、皮膚の表面と同一の高さにまで圧し下げた。擾乱を受けさせずに、このシステムを8時間の間定位置に置き、8時間後には、流体は、腔部のパッキングから全く出ず、その中に収集されていた。

その中に創傷部滲出液流体を保持するための吸収性創傷接触要素と、
前記創傷接触要素および創傷部位の上に気密シールを提供する創傷カバー要素と、を具備し、
前記創傷接触要素および前記創傷カバー要素は単一の要素へと組み合わされており。かつ、
前記装置は液体接触作動弁を具備すると共に、前記創腔からの液体移送は前記液体接触作動弁の閉鎖によって阻止される、装置。 - 前記液体接触作動弁は自己癒合材料を具備する、請求項1に記載の装置。

- 前記吸収性創傷接触要素は、流体吸収時に自己癒合することが可能な超吸収性材料を具備する、請求項1ないし請求項11のいずれか1項に記載の装置。
- 前記創傷カバー要素は液体流に対して不透過性であるが、水蒸気の移送に対しては透過性である、請求項1ないし請求項12のいずれか1項に記載の装置。
- 前記創傷カバー要素はポリウレタンである、請求項13に記載の装置。
- 前記創傷カバー要素は、それに塗布された感圧接着剤を有する皮膚接触表面を具備する、請求項1ないし請求項14のいずれか1項に記載の装置。
