Description
体液の採取および試験は多くの目的に有用であり、医療診断および処置のためおよびその他のさまざまな用途のために重要性が増し続けている。試験はさまざまな体液について実行することができ、ある一定の用途については特に血液および/または組織間液の試験に関している。医療状態を特定し、治療に対する反応を決定し、処置の進み具合を評価するために、このような流体は流体のさまざまな特徴、または流体に含まれている検体について試験することができる。
体液の試験は基本的に流体サンプルを取得する工程、サンプルを試験装置に転送する工程、流体サンプルに試験を行う工程、および結果を表示する工程を伴う。これらの工程は一般に複数の別々の機器または装置によって実行される。
流体サンプルを採取するための方法の1つとして、血液サンプルを引き抜くために中空の針または注射器を静脈または動脈に挿入することが挙げられる。しかしながら、このような直接的な血管血液サンプリングには痛み、感染、血腫およびその他の出血問題といったいくつかの制約がある可能性がある。さらに、直接的な血管血液サンプリングは定期的に繰り返すには適しておらず、非常に困難となり得、患者に自分自身に対して実行するように勧められるものではない。
体液サンプルを収集するための他の一般的な技術として皮膚に切り込みを形成して皮膚表面に流体をもたらすことが挙げられる。ランセット、ナイフまたはその他の切断機器を使って皮膚に切り込みを形成する。得られた血液または組織間液標本は次に小型チューブまたはその他の容器内に収集されるか、または直接試験片に接触して配置される。流体供給源として指先が頻繁に使用される、なぜなら指先は高度に血管化されているため大量の血液を生み出すからである。しかしながら指先はまた神経末端も大量に集中しているため痛みを伴う可能性がある。手のひら、前腕、耳たぶなどといった別のサンプリング部位をサンプリングに利用することができ、これらは痛みが少ない。しかしながら、これらはまた生成する血液の量がより少ない。したがってこれらの別の部位は一般に、比較的少量の流体を必要とする試験システムについてだけの使用、または切り込み部位からの体液の圧出を容易にするための工程が行われる場合に適している。
患者は切り込みを取り巻く領域に対して圧力をかけるなどして流体を切り込み部位へと促し切り込みから流体を搾り出すまたは噴出させるよう頻繁に指示される。切り込みからの体液の圧出を容易にするための機械的な装置もまた公知である。このような装置は、たとえば特許文献3〜5および特許文献7〜10に示されている。切り込みから体液の圧出を促進する代表的な市販の製品はアミラ アトラスト ブラッド グルコース システム(Amira AtLast blood glucose system)(商品名)である。
生成された流体の採取は、これ以後は流体の「サンプリング」と称するが、さまざまな形態をとり得る。一旦流体標本が切り込みにおいて皮膚表面にやってくると、サンプリング装置は流体に接触して配置される。このような装置は、たとえば、チューブまたは試験片が切り込みを形成する前に切り込み部位の隣に配置されるか、または切り込みが形成されたあと直ちに切り込みへと移動されるシステムを備え得る。サンプリングチューブは吸引によるかまたは毛管現象により流体を採取し得る。このようなサンプリングシステムには、たとえば特許文献6、11および12に示されているシステムが挙げられる。市販のサンプリング装置の例としてはロッシュ コンパクト(Roche Compact)(商品名)、アミラ アトラスト(Amira AtLast)(商品名)、グルコミター エリート(Glucometer Elite)(商品名)およびセラセンス フリースタイル(Therasense FreeStyle)試験片(商品名)が挙げられる。
体液サンプルは、当該分野において公知であるようにさまざまな特性または構成要素について分析され得る。たとえば、このような分析は赤血球容積、血糖、凝結、鉛、鉄などに向けられる。試験システムはサンプリングされた流体を分析するための光学的(たとえば反射率、吸収、蛍光、ラマンなど)、電気化学的、および磁気手段といった手段を備えている。このような試験システムの例としては、特許文献13〜15のものが挙げられる。
一般的な医療試験は血糖値の測定である。血糖値は血液を分析することによって直接、または組織間液といった他の流体を分析することによって間接的に決定することができる。糖尿病患者は一般に、彼らの糖尿病の性質および深刻さにしたがって一日に数回自分の血糖値を測定するよう指示される。測定された血糖値において観察されたパターンに基づいて、また食事制限、運動、およびその他の要因といった問題点も考慮して患者および医師は投与すべきインシュリンの適切なレベルを決定する。
上述のように、糖尿病患者は典型的には自分の血糖値を一日中モニタして自分の血糖が許容範囲内にあることを確認しなければならない。いくつかのタイプのサンプリング装置は血液などの体液を吸収しおよび/または試験するための媒体を含んでいる試験片の使用を必要とする。試験後、血液で汚染された試験媒体は生物学的危険物質であると考えられるので汚染された試験片に他の人々がさらされることを避けるため簡単に廃棄される必要がある。このことは特にこの人物がレストランなど家から離れたところにいる場合に不都合である。さらに、個々の試験片は有効期限の異なる他の試験片と簡単に混ざりあってしまう。有効期限の切れた試験片を使用すると間違った読み取りを行う場合があり、その結果、糖尿病患者に対する不適切なインシュリン供与量など患者に不適切な処置が施される可能性がある。

前記供給部(66)と保存部(68)との間に設けられ、当該体液に対して前記試験媒体テープ(64)の一部を露出する露出部(70)と
を備え、前記巻き取りリール(82)が、除去されずに前記保存部(68)に試験媒体テープ(64)が供給され得るように当該巻き取りリール(82)を一方向のみに回転させる、付勢カセット爪を係合する歯が刻設された部分を有してなることを特徴とする体液サンプリング装置。
