背景技术
带套囊的气管造口管(trach tubes)通常用来给住院患者较长的时间换气;气管插(ET)管可使用不足一周至10天的时间。气管造口通过咽喉插入气管,然后近端通常连接至呼吸空气的机械供给源,即呼吸空气来自呼吸机或呼吸器。套囊或“气囊”位于气管造口管远端上,并且封闭患者的气管,以便仅通过管来进行吸气和呼气。气囊也用来阻止液体分泌物向下通过、进入肺部,且可能导致呼吸机相关性肺炎(VAP)。分泌物保留在气囊上方,并可被周期性地移除,以帮助确保它们不进入肺部。

发明概述

发明详述
图1是带有套囊的气管造口管装置100的示意图。该装置100具有管105和可充气套囊110。管还具有近端120和远端115。管的远端附近具有凸缘(flange)125,其用来通过缝合将管连接至患者皮肤。凸缘也具有槽,用于连接围绕患者颈部的皮带,以利于将气管造口管保持在适当位置。管具有通过中心的用于吸气和呼气内腔,并且如果需要,近端可连接至呼吸机。一旦管通过气管口放置在患者气管中,就将气囊充气并将气管密封。使用用于向气囊充气的常规装置,该装置包括小充气内腔(未示出),该小充气内腔沿着管并通过凸缘连接至充气源。
图2是图1的装置100插入气管24且气囊110充气之后的示意图。凸缘125靠咽喉外侧放置,气囊110密封气管24,使得呼吸必须通过管105的内腔来引导。
图3表示了根据本发明的气管中的管上气囊,其中充气气囊180适合将气管口210以下区域205中的气管(即气管内腔200)密封,并避免将气管口以上的区域中的气管密封。这通过以下事实来实现:近端120上的连接点和管上可充气气囊180的远端115上的连接点不相邻,或者换句话说,呈一定角度(α)但非180度。这种气囊构造应当使得分泌物从开口215排出气管口。
更理想的是,气囊的各个区域具有不同的厚度。例如,希望区域“B”中的气囊壁比区域“A”中的气囊壁更薄。尽管发明人不应该受特定的工作理论的限制,但是通常认为当患者平躺着休息时,接触气管下壁195的气囊相对薄的第二部分“B”由于重力将在更易于收集分泌物的区域提供较好的密封。当患者平躺着休息时,由于重力,接触气管上壁190的气囊相对厚的第一部分“A”是较不易收集分泌物的区域。一旦患者在竖直方向移动时,分泌物应该能够到达气管口,并从开口215排出气管。
在概述部分所讨论的内容包括一种用于制造可具有不同壁厚的可充气气囊部件的方法。该方法包括提供由热塑性聚合物组成并具有内腔的原料管的步骤。当在模具中将该管预热至足以软化管材料的温度时,以引入原料管的气体充气从而大体均一地拉伸管材料,该管形成模具形状的气囊,且包括远端、远连接区、近端、近连接区、上区和下区。理想地,上区厚度为约15至约30微米,下区厚度为约5至约15微米。
可以使用Litematic设备来测量气囊的壁厚度。示例性的设备是MitutoyoAmerica Corporation的系列318Model VL-50A。根据厂商,Litematic设备使用探针针尖和刚性陶瓷基座,以0.01微米的分辨率来测量0和50.8mm之间的厚度。所使用的测量力是0.01N(1克)。本文用于测试的探针针尖是3mm直径的碳化物球接触点,其用作Litematic设备的“标准”探针针尖。
单层箔或膜的条带(strip)可用于确定每个样品的厚度。可以切割每个样品的气囊样本(未连接至气管造口管)来制备条带:首先,应该切除端部,以获得宽度约30mm的均一的带;然后,应该沿宽度方向切割每个带,以形成条带。应该在沿每个条带的长度方向的10个位置处的测量厚度,每个样品的条带(测量至少6个条带)的单个测量值应该在一起求平均值,并且计算各自的标准偏差。
图4和图5是由所公开方法形成的气囊的视图。当然,用来形成该气囊的模具与想要的气囊的形状相同,即管的至少一个端部偏离模具的中心线,以便可以形成偏置气囊。因而,图4和图5不仅是气囊的图,而且也是模具空隙空间的示意图。为了在偏离气囊中心线的端部上形成具有至少一个开口的气囊,模具必须是不对称的,即将管放置在模具中,使得其以直线穿过模具的空隙空间,管端部根据期望偏离模具的中心线。模具在每个端部上均有开口,管可以通过该开口突出并被夹紧。模具能够以两件或多件的形式被打开,或者模具可以是单件的。如果模具能够被打开,则将管放置在模具中,并封闭模具。如果其是单件的,则将管从任一端滑入模具中。
根据该方法,原料管在模具中被预热至足以软化管材料的温度。管可以具有壁,该壁的厚度是均匀的,且其尺寸(直径)将由期望的囊套尺寸来确定。例如,9号气管造口管可以以外径(OD)为8.61mm和内径(ID)为8.5mm的原料管来制备。将管放置在有足够材料且每个端部突出以使其被紧紧固定的模具中之后,将模具和管预热至50到120℃之间,理想的为60到80之间的温度。经预热的原料管通过牵拉端部纵向(轴向)拉伸。经5到60秒的时间,以空气、氮气或其他惰性气体向内(原料管内腔中)施加约0.5巴的压力,管应该被拉伸约50%至200%,同时维持温度。接着,经加热、拉伸的管被加压,即以空气、氮气或其他惰性气体向内施加约0.5到2巴之间的压力,同时使管在5到15秒的时间内回缩10%至50%,以形成气囊。该回缩/加压步骤使管被拉伸直到其与模具壁接触,但不继续被延伸以致非常薄并且破裂。
任选地,当气囊仍在模具中且其端部固定时,可以通过将其加热至温度为130-165℃达约30-90秒的时间,且在保持气囊充气的压力,例如约0.5巴来热定型。气囊可以在约20-50℃下冷却,并且随后从模具移出。如果模具是单件模具,那么应将气囊收缩,以便其可以被完好取出。气囊可以通过对管抽真空并排出气囊内容物来收缩。气囊收缩之后,其可以通过任一端部或者管进入模具的进入点从模具容易地取出。由此形成的气囊随后可通过已知方法连接至管。
当然,其他聚合物材料可以用来形成气囊部件。例如气囊部件可以由热塑性聚氨酯聚合物、热塑性聚烯烃弹性体、热塑性聚烯烃嵌段共聚物、SBS二嵌段弹性体、SEBS三嵌段弹性体、聚氯乙烯(PVC)、聚对苯二甲酸乙二酯(PET)及其掺合物和混合物形成。更理想地,可以使用聚氨酯,这是因为已经发现其与其他材料相比较对组织造成的刺激更小。有用的聚氨酯包括以商品名Pellethane获自Dow Chemical Company(Dow Plastics)的那些。Pellethane

实施例
使用由名称为Pellethane2363-90A、硬度计硬度为90A(ASTM D-2240)的Dow聚氨酯制备的原料聚氨酯管。在224℃、2160g(ASTM D-1238)条件下,该聚氨酯具有110℃的软化温度(ASTM D-790)和30g/10min的熔融指数。将外径(OD)为8.61mm和内径(ID)为8.5mm的管放置在如图4所示的具有空隙空间的单件模具中并将其夹在模具外的端部。将该模具和管预热至温度为约60℃。一旦达到平衡,在微小内压下,使用氮气在0.5巴下,在约10秒的时间内将管拉伸约75%。当向管内施加2巴的压力时,使管回缩约三分之一以形成气囊。在约0.5巴的压力下,以约140℃的温度将气囊热定型约90秒。在约45℃下冷却气囊,对管内腔抽真空,以使气囊收缩,并将管和气囊通过端部之一从模具移出。
再参见作为所形成的可充气气囊部件250的透视图的图4和作为相同气囊的侧视图的图5,该可充气气囊可包括远端255、远连接区260、近端265、近连接区270、上区275和下区280。如上所述,理想地,上区厚度为约15至约30微米,下区厚度为约5至约15微米。图6是可充气气囊部件250的另一个实施方案的透视图,图7是相同气囊的侧视图。如图6和图7可以看到的,可充气气囊可包括远端255、远连接区260、近端265、近连接区270、上区275和下区280。图6和图7的气囊以与图4和图5的气囊相同的一般手段形成;将原料管插入模具中,以便该管以直线穿过模具。形成的气囊连接至气管造口管造成近气囊开口和远气囊开口彼此偏离180度。
理想地,图4至图7所示的上区厚度为约15至约30微米,理想地,下区厚度为约5至约15微米。气囊上区275至下区280的尺寸范围可以为约50毫米至约25毫米,且理想地可以在约35毫米至约30毫米之间。远端255至近端265的尺寸范围可以为约60毫米或更多至约25毫米,且理想地可以在约40毫米至约30毫米之间。当然,预期该尺寸可以更大或更小。
具有30微米或较薄(例如,从上区中的15至30微米至下区中的5至15微米)或甚至更薄的壁的可充气气囊套囊的一个优点是,这样的套囊呈现出非常小的轮廓,与具有较厚膜的气囊,例如厚度大于30微米的气囊相比较,在充气之前会更贴近轴。常规厚度的气囊具有实质的附加材料,其在插入期间需要穿过气管口。该附加材料需要求穿过的更大的气管口,造成外伤增加并可能影响患者最终效果。
原料管也可以具有不均匀厚度的壁。图8A和图8B示出不对称原料管300的横截面示意图。中心腔302的不对称度将取决于诸如热塑性聚合物的种类和管将承受的鼓风量(blowing)或拉伸量的因素。由于其不对称性,在鼓风之前可以旋转管,以便形成的气囊壁厚情况可以针对特定应用由用户微调。相比以与对称管相同的方法制造的的气囊的壁厚度,气囊中不同点处的壁厚度甚至更为不同。供选择地,管可以在不对称模具中定向,以便气囊中不同点处壁厚度基本上相同。

Claims (10)

1.一种制造具有不同壁厚度的气囊的方法,其包括:
提供由热塑性聚合物组成的原料管;
在不对称模具中将所述原料管预热至足以软化所述管材料的温度;
拉伸所述管;
用压缩气体向所述管充气,以拉伸所述管的材料来与所述模具的经加热的壁接触,同时使所述管回缩;
冷却完成的气囊;以及
从所述模具将所述完成的气囊移出。
2.根据权利要求1所述的方法,其中所述完成的气囊具有不同的壁厚度,其上区厚度为约15至约30微米,并且下区厚度为约5至约15微米。
3.权利要求1所述的方法,其中所述热塑性聚合物选自热塑性聚氨酯聚合物、热塑性聚烯烃弹性体、热塑性聚烯烃嵌段共聚物、SBS二嵌段弹性体、SEBS三嵌段弹性体、聚氯乙烯、低密度聚乙烯、聚对苯二甲酸乙二酯及其掺合物和混合物。
4.权利要求1所述的方法,其还包括向所述管充气之后,通过将其加热至足以定向无定形聚合物部分的温度和时间来热定型所述气囊的步骤。
5.权利要求1所述的方法,其还包括冷却所述管之后,对所述管进行排气以便收缩所述气囊的步骤。
6.一种制造具有不同壁厚度的气囊的方法,其包括如下步骤:
将具有内腔的原料管放置在不对称模具中,所述模具在每个端部具有开口,所述管通过该开口突出;
将所述管紧紧固定在所述模具的每个端部上;
将所述模具和管预热至在50和120℃之间的温度;
以空气、氮气或其他惰性气体向内施加约0.5巴的压力,经过5到60秒之间的时间将所述管拉伸约50%至200%,同时维持温度;
在约0.5到2巴之间的压力下,以空气、氮气或其他惰性气体向内施加压力来对所述管加压,同时使得所述管在5到15秒的时间内纵向回缩10%至50%以形成气囊。
7.权利要求6所述的方法,其还包括如下步骤:
当所述管仍在所述模具中且其端部被紧紧固定时,将其加热至130-165℃的温度长达约30-90秒的时间,并且保持气囊充气的压力;
在约20-50℃下冷却所述气囊;
将所述气囊从所述模具中移出。
8.权利要求7所述的方法,其还包括在将所述气囊从所述模具中移出之前,收缩所述气囊以便其可以被完好无损取出的步骤。
9.根据权利要求6所述的方法,其中所述原料管包括热塑性聚合物,所述管具有不均匀的壁厚度。
10.根据权利要求9所述的方法,其中所述热塑性聚合物选自热塑性聚氨酯聚合物、热塑性聚烯烃弹性体、热塑性聚烯烃嵌段共聚物、SBS二嵌段弹性体、SEBS三嵌段弹性体、聚氯乙烯、低密度聚乙烯、聚对苯二甲酸乙二酯及其掺合物和混合物。
