Summary: Transfer only charged calls

Initial Comment:
We need an option to only transfer those calls that have a value, either debit or credit, attached to them.  This will cause "free" calls to not show up on the clients invoices.  This will be a system wide setting.

Summary: Add support for rating DID calls from Thirdlane

Initial Comment:
We have a number of users that are using ASTPP to rate calls for Thirdlane installations.  There are a few adjustments that would make those easier to handle.  This could be done with a simple toggle switch in the configuration that marked if an installation uses the Thirdlane gui.

1.  Accountcodes are written into the "userfield" instead of into the accountcodes field.

2.  DIDs that are marked as billable have the DID posted into the "accountcode" field in the following format "DIDNUMBER-IN" ie "8881234567-IN".

Summary: Packages not included in max length calculations

Initial Comment:
an account is in a pricelist that has a package attached to it with free minutes to a certain number\

but during dialing this package is not taken into consideration, and if the account has less credit available than the call, it won't go through
while rating the package is taken into consideration

for now i'll tell them to put $1 credit into each account to make sure this works, but how do we get packages to come into play

here is when the credit_limit < the cost of 1 minute of the call (although if anything it should take < cost / increments)
