Abstract
Description
本發明涉及尿酸性或痛風性疾病的預防或治療,更具體地說,本發明涉及用於治療或預防尿酸性或痛風性疾病的化合物或其組合物,此外,還涉及尿酸性或痛風性疾病的治療或預防方法,以及所述化合物或組合物降低尿酸和抗炎的用途、所述化合物或組合物製備尿酸性或痛風性疾病治療劑或預防劑中的用途。
尿酸高是人體內有一種叫做嘌呤的物質因代謝發生紊亂,致使血液中尿酸增多而引起的一種代謝性疾病,體內尿酸每日的生成量和排泄量大約是相等,生成量方面,三分之一是由食物而來,三分之二是體內自行合成,排泄途徑則是三分之一由腸道排出,三分之二從腎臟排泄。上述各種途徑只要有任何一方面出問題,就會造成尿酸升高。尿酸上升,會阻礙血液分泌尿酸的過程,導致尿酸無法排出。尿酸過高,還會引發其他疾病,例如痛風、腎病以及心血管疾病。
尿酸高會引起痛風,痛風是人體內嘌呤代謝出現
障礙,血尿酸的增加,導致反復發作的關節炎症。尿酸高還會引起痛風性腎病,痛風性腎病是因為血尿酸的過多產生,沒有合理的排泄出去,形成高尿酸血症所導致的腎損害。嚴重的尿酸高病人會導致腎衰竭。痛風(GOUT)是因為血尿酸增高導致組織損傷的一組異質性疾病,由於體內嘌呤代謝紊亂和/或尿酸排泄減少而引起,表現為高尿酸血症。正常人血漿中尿酸含量為20-60mg/L,超過80mg/L時,尿酸鹽晶體會沉積在關節、軟組織、軟骨、腎等部位,析出結晶,導致關節炎、尿路結石以及腎臟疾病,即“痛風症”。急性發作時,尿酸鹽微結晶沉積於關節而引起局部粒細胞浸潤及炎症反應。反復發作,關節變畸形,形成“痛風石”,在西方及日本早已成為常見病。近些年來,痛風也成為中國(尤其是沿海開放地區)的盛行病。
現有的研究提示,痛風急性發作是尿酸鹽晶體誘發的一個急性炎症過程,它起始於尿酸鹽晶體與常駐單核/巨噬細胞的相互作用,經過一系列炎症反應,最終在多個機制的參與下自發緩解。大多患者痛風發作與血尿酸水準的變化速率有關(上升或下降),而與穩定的血尿酸水準無關。血尿酸水準的急驟波動可使晶體發生體積或形狀的變化,使之在組織基質中鬆動,促進尿酸鹽晶體從已形成的痛風石沉積部位釋放,而引發炎症反應的正是脫落的微小晶體或局部新形成的結晶。單核/吞噬細胞與尿酸鹽晶體的相互作用是急性痛風啟動的主要環節。尿酸鹽晶體與常駐巨噬細胞相互作用觸發炎症,並誘導中性粒細胞和單核細胞浸潤,擴大炎症反應。
急性痛風的發作涉及多個炎症因數主要包括:IL-1β、IL-6、IL-18、TNF-α及IL-8(CXCL-8)等趨化因數。通過測定痛風急性發作不同階段患者關節液中炎症因數和抗炎因數水準發現,在炎症的早期和中期,關節液中IL-1β、IL-6和TNF-α等炎症因數水準和白細胞水準明顯升高。
目前IL-1又名淋巴細胞刺激因數,主要由活化的單核-巨噬細胞產生。IL-1分子有兩種不同的分子形式,IL-1α和IL-1β。局部低濃度時協同刺激抗原提呈細胞和T細胞活化,促進B細胞增殖和分泌抗體,進行免疫調節。IL-1β的前驅物由單核巨噬細胞和樹突狀細胞等免疫細胞產生,通過NALP3炎性體活化蛋白酶-1轉化為有活性的IL-1β並釋放從而介導炎症反應。IL-1β可誘發IL-6、IL-8的釋放,介導中性粒細胞的浸潤。此外,急性痛風患者對各種IL-1抑制劑的快速臨床反應也證明這一因數在痛風炎症中的關鍵作用。最近研究資料顯示白介素1β(IL1β)在痛風病人尿酸鈉(MSU)結晶沉積產生的炎症進程中起著重要的作用(Ann Rheum Dis 2009;68:1613-1617.)。
TNF-α(腫瘤壞死因數)是由單核細胞和巨噬細胞產生的多肽類細胞因數,在炎症反應、免疫系統的發展、細胞程式性死亡和脂代謝中起重要的作用。TNF-α也參與了包括哮喘、克羅恩氏病、類風濕性關節炎、神經性疼痛、肥胖症、II型糖尿病、自身免疫病和腫瘤等疾病的發生。在免疫反應中TNF-α是一個多功能的調節器甚至作為一個強烈的熱原性物質刺激中性粒細胞,改變血管內皮細胞的特性,調
節其它組織的代謝活性。
IFN-γ是(γ型干擾素)是由免疫系統中的T細胞和自然殺傷細胞產生的糖蛋白,IFN-γ的啟動效應可促進巨噬細胞分泌更高水準的促炎性細胞因數和更低水準的抗炎細胞因數,提高巨噬細胞殺菌和殺腫瘤的活性。
痛風病人的共同體徵是,血液中的尿酸濃度發生病理性增高。雖然並不是所有尿酸水準增高的人均會患痛風病,然而尿酸水準增高仍是痛風病的前提。尿酸濃度增高的原因有兩種:一是尿液中尿酸的排出減少;二是由於缺乏調節作用使尿酸的生物合成增強。
痛風分原發性和繼發性兩種,前者主要是酶缺陷引起的,常伴有高血壓、糖尿病、高血酯、肥胖、代謝綜合症和冠心病,有遺傳傾向;後者則是由腎病、血液病、用藥等多種原因引起的。
對於痛風如不積極預防或者治療不當,痛風會發作得越來越頻繁,症狀也會隨之加深,侵犯的關節也會越來越多,引發痛風-異質性(heterogeneous)疾病。尿酸鹽結晶沉積在關節形成痛風性急性關節炎;隨著多個關節同時受累,痛風會發展為慢性(長期性)關節炎。反復發作可造成關節永久性損害,包括長期疼痛和僵硬、活動受限及關節變形,當病情發展時,結晶在軟組織中沉積,並在皮下形成叫做“痛風石”的腫塊。結晶在腎臟沉積會引發腎臟病變,導致急性尿酸性腎病、慢性尿酸鹽腎病,造成嚴重的腎臟損害產生泌尿系統尿酸性結石。
痛風性關節炎發病率高、治癒難、反復發作,各種疑難病症令患者痛苦不堪,這種疾病被聯合國衛生組織列為21世紀人類十大頑症之一。
目前,多個流行病學研究證實,血尿酸是高血壓發病的獨立危險因素,血尿酸水準每增高1mg/dL,高血壓發病相對危險增高25%。長期高尿酸血症可破壞胰腺β細胞功能而誘發糖尿病,且有研究證實,長期高尿酸血症與糖耐量異常和糖尿病發病具有因果關係。
尿酸是冠心病死亡獨立危險因素:有研究顯示,無論性別,尿酸是普通人群冠心病死亡的獨立危險因素。血尿酸每升高1mg/dL,死亡危險性在男性增高48%,女性增高126%。血尿酸>6mg/dL是冠心病的獨立危險因素;血尿酸>7mg/dL是中風的獨立危險因素。
尿酸與腎臟疾病關係密切。除尿酸結晶沉積導致腎小動脈和慢性間質炎症使腎損害加重以外,許多流行病學調查和動物研究顯示,尿酸可直接使腎小球入球小動脈發生微血管病變,導致慢性腎臟疾病。
目前,抗痛風藥目前品種不多,臨床治療痛風主要以秋水仙鹼、抑制尿酸合成藥(別嘌呤醇、非布司他
(Febuxostat))、促進尿酸排泄藥(如丙磺舒、磺吡酮、苯溴馬隆、Lesinurad)、非類固醇類抗炎藥和激素為主。急性發病期主要使用秋水仙鹼、非類固醇類抗炎藥、激素,緩解期主要使用抑制尿酸合成藥、促進尿酸排泄藥。但是,這些藥物在治療上都存在療效差、副作用大的缺陷。
2012年美國風濕病學會(ACR)痛風治療指南推薦的一線降尿酸藥物是別嘌醇和非布司他,非布司他首次作為一線藥物推薦,丙磺舒是至少對一種黃嘌呤氧化酶抑制劑禁忌或不能耐受,才作為一線的促尿酸排泄藥用於降尿酸治療。另外,ACR指南建議在開始有效的抗炎治療後就可以開始降尿酸治療。
因此,目前仍需研究開發能夠治療或預防尿酸性或痛風性疾病的新型藥物。
WO2005077950公開了本發明通式(I)所示的化合物,通過激動HM74A(又稱GPR109A)受體來治療血脂異常、II型糖尿病等疾病。但是該專利文獻沒有公開或暗示所述化合物可以用於預防或治療尿酸性或痛風性疾病。
WO2011057110公開了黃嘌呤衍生物通過激動HM74A受體來預防或治療腦缺血等疾病。
US20130150383公開了黃嘌呤類化合物用於治療牛皮癬的作用。
US2015080418A1公開了黃嘌呤類化合物用於神經組織疾病。
WO9316699A1公開了黃嘌呤化合物用於治療真
菌感染。
WO9920280A1公開了黃嘌呤化合物用於治療皮膚瘙癢。
EP0389282A2公開了黃嘌呤化合物在腦代謝、神經保護以及血管異常等方面的作用。
文獻(Expert Opin.Ther Patents 2009,19(7),957-967)公開了黃嘌呤衍生物激動GPR109A治療血脂異常、II型糖尿病等疾病。
文獻(Curr Atheroscler Rep 2013,15:325,1-10)公佈了GPR109A具有介導血管炎性的藥理作用。
文獻(PLoS One.2014 Oct 17;9(10):e109818)表明GPR109A具有治療帕金森病的效果。
但是上述這些文獻沒有報導過黃嘌呤類化合物在降低尿酸、預防或治療痛風方面的作用。
本發明的目的在於提供一種降低尿酸、抗炎、治療或者預防高尿酸血症、痛風、痛風性炎症、疼痛和尿酸性腎病的新方法。
為了實現上述目的,本發明人進行了悉心研究,結果,出乎預料地發現一類黃嘌呤類化合物可以降低尿酸、抗炎,進而有效地預防或者治療高尿酸血症、痛風、痛風性炎症、疼痛和尿酸性腎病,從而完成了本發明。
具體而言,本發明涉及下述技術方案。
(1)式(I)所示化合物、其藥學上可接受的鹽、其溶劑化物或者含有它們中的任何一種的藥物組合物降低尿酸、抗炎的用途以及在製備用於治療和/或預防高尿酸血症、痛風、痛風性炎症、疼痛和尿酸性腎病的藥物的用途,
其中R1選自氫,C1-4烷基,其中所述的C1-4烷基未被取代或者任選被一個或者多個鹵素原子、氰基和CF3取代;R2選自C1-10烷基,C2-6烯基,C2-6炔基,其中所述的C1-10烷基、C2-6烯基、C2-6炔基未被取代或者任選被鹵素原子、氰基、C3-7環烷基取代;R3選自鹵素原子、氰基。
(2)上述(1)的用途中,R1表示氫或者甲基,R2表示乙基,環丙基乙基,環丙基甲基,丙基,2-甲基丙基,丁基,3-甲基丁基或者戊基,R3表示氟原子或者氯原子。
(3)上述(1)的用途中,所述化合物選自:8-氯-3-戊基-3,7-二氫-1H-嘌呤-2,6-二酮,8-氯-3-丁基-3,7-二氫-1H-嘌呤-2,6-二酮,8-氯-1-甲基-3-丁基-3,7二氫-1H-嘌呤-2,6二酮,8-氯-1-甲基-3-戊基-3,7二氫-1H-嘌呤-2,6二酮,

採用本發明的上述技術方案,既具有抗炎作用,抑制高尿酸患者炎症的發生、發展,減輕痛風患者、痛風石患者、痛風性炎症患者以及尿酸性腎病患者的炎症反應;又可以通過降低尿酸,進而降低痛風、高血壓、糖尿病、高血酯、肥胖、代謝綜合症、冠心病和腎臟損害發生的風險,從而達到減輕患者痛苦,治療或預防尿酸性或痛風性疾病的目的。
特別地,炎症反應是尿酸升高之後的一個病症,本發明的化合物具有預料不到的降低尿酸的作用,因此可以從根本上緩解尿酸升高引發的炎症反應,從而發揮抗炎的作用。

術語“預防”是指在有風險發生該疾病或障礙的物件中防止該疾病或障礙。
術語“尿酸性疾病”是指與生物體內尿酸值異常相關的疾病,包括但不限於高尿酸血症或尿酸性腎病
術語“痛風性疾病”是指與痛風相關的疾病,包括但不限於痛風、痛風性炎症或疼痛。
原發性高尿酸血症,分為原因未明的分子缺陷和先天性嘌呤代謝障礙:先天性嘌呤代謝障礙分為以下四種情況:(I)5-磷酸核苷酸-1-焦磷酸合成酶(PRPPS)活性增加,引起5-磷酸核苷酸-1-焦磷酸合成酶合成過多,尿酸產生過多,遺傳特徵為X連鎖;(II)次黃嘌呤-鳥嘌呤磷酸糖轉移酶(HPRT)部分缺少,引起5-磷酸核苷酸-1-焦磷酸合成酶濃度增加,尿酸產生過多,遺傳特徵為X連鎖;(III)次黃嘌呤-鳥嘌呤磷酸糖轉移酶完全缺乏,嘌呤合成增多所致的尿酸產生過多,見於Lesch-Nyhan綜合症,遺傳特徵為X連鎖;(IV)葡萄糖-6-磷酸酶缺乏:嘌呤合成增多所導致的尿酸產生過多和腎清除尿酸減少,見於糖原積累病I型,遺傳特徵為常染色體隱性遺傳。
繼發性高尿酸血症:多種急慢性疾病如血液病或惡性腫瘤、慢性中毒、藥物或高嘌呤飲食所致的血尿酸產生增高或尿酸排泄障礙所致高尿酸血症。
痛風是由尿酸鈉(MSU)沉積所致的晶體相關性關節病,與嘌呤代謝紊亂和(或)尿酸排泄減少所致的高尿酸血症直接相關,特指急性特徵性關節炎和慢性痛風石疾病,主要包括急性發作性關節炎、痛風石形成、痛風石性慢
性關節炎、尿酸鹽腎病和尿酸性尿路結石,重者可出現關節殘疾和腎功能不全。
原發性痛風,多有遺傳性,但臨床有痛風家族史者僅占10%~20%。尿酸生成過多在原發性高尿酸血症的病因中占10%。其原因主要是嘌呤代謝酶缺陷,次黃嘌呤鳥嘌呤磷酸核糖轉移酶(HGPRT)缺乏和磷酸核糖焦磷酸鹽(PRPP)合成酶活性亢進。原發性腎臟尿酸排泄減少約占原發性高尿酸血症的90%,具體發病機制不清,可能為多基因遺傳性疾病,但應排除腎臟器質性疾病。
繼發性痛風,指繼發於其他疾病過程中的一種臨床表現,也可因某些藥物所致。骨髓增生性疾病如白血病、淋巴瘤、多發性骨髓瘤、紅細胞計數增多症、溶血性貧血和癌症等可導致細胞的增殖加速,使核酸轉換增加,造成尿酸產生增多。惡性腫瘤在腫瘤的放化療後引起細胞大量破壞,核酸轉換也增加,導致尿酸產生增多。腎臟疾病包括慢性腎小球腎炎、腎盂腎炎、多囊腎、鉛中毒和高血壓晚期等引起的腎小球濾過功能減退,可使尿酸排泄減少,導致血尿酸濃度升高。藥物如噻嗪類利尿藥、呋塞米、乙胺丁醇、吡嗪醯胺、小劑量阿司匹林和煙酸等,可競爭性抑制腎小管排泄尿酸而引起高尿酸血症。另外,腎移植患者長期服用免疫抑制劑也可發生高尿酸血症,可能與免疫抑制劑抑制腎小管排泄尿酸有關。
痛風性炎症是由於尿酸鹽沉積在關節囊、滑囊、軟骨、骨質和其他組織中而引起病損及炎性反應,它多有遺
傳因素和家族因素,好發於40歲以上的男性,多見於拇趾的蹠趾關節,也可發生於其他較大關節,尤其是踝部與足部關節。主要表現為關節的劇痛,常常為單側性突然發生。關節周圍組織有明顯腫脹、發熱、發紅和壓痛。
急性痛風性關節炎,多數患者發作前無明顯徵兆,或僅有疲乏、全身不適和關節刺痛等。典型發作常於深夜因關節痛而驚醒,疼痛進行性加劇,在12小時左右達高峰,呈撕裂樣、刀割樣或咬噬樣,難以忍受。受累關節及周圍組織紅、腫、熱、痛和功能受限。多於數天或2周內自行緩解。首次發作多侵犯單關節,部分以上發生在第一蹠趾關節,在以後的病程中,部分患者累及該部位。其次為足背、足跟、踝、膝、腕和肘等關節,肩、髖、脊柱和顳頜等關節少受累,可同時累及多個關節,表現為多關節炎。部分患者可有發熱、寒戰、頭痛、心悸和噁心等全身症狀,可伴白細胞計數升高、紅細胞沉降率增快和C反應蛋白增高等。
間歇發作期,痛風發作持續數天至數周後可自行緩解,一般無明顯後遺症狀,或遺留局部皮膚色素沉著、脫屑及刺癢等,以後進入無症狀的間歇期,歷時數月、數年或十餘年後復發,多數患者1年內復發,越發越頻,受累關節越來越多,症狀持續時間越來越長。受累關節一般從下肢向上肢、從遠端小關節向大關節發展,出現指、腕和肘等關節受累,少數患者可影響到肩、髖、骶髂、胸鎖或脊柱關節,也可累及關節周圍滑囊、肌腱和腱鞘等部位,症狀趨於不典型。少數患者無間歇期,初次發病後呈慢性關節炎表現。
慢性痛風石病變期,在痛風病人的發病過程中,會出現一種堅硬如石的結節,稱為“痛風石”,又名痛風結節。皮下痛風石和慢性痛風石性關節炎是長期顯著的高尿酸血症,大量尿酸鈉晶體沉積於皮下、關節滑膜、軟骨、骨質及關節周圍軟組織的結果。
這種尿酸鈉結晶沉積於軟組織,引起慢性炎症及纖維組織增生形成的結節腫。痛風石最常見於耳輪,亦多見於踇趾的第一蹠趾關節、指、腕、肘及膝關節等處,少數病人可出現在鼻軟骨、舌、聲帶、眼瞼、主動脈、心瓣膜和心肌。在關節附近的骨骼中侵入骨質,形成骨骼畸形,或使骨質遭受損毀。這種痛風結節也可在關節附近的滑囊膜、腱鞘與軟骨內發現。痛風石大小不一,小的如芝麻,大的如雞蛋。
內臟也可發生痛風石,主要見於腎臟實質。有時可見於輸尿管和膀朧。肝臟、膽囊、膽道和胰腺等處罕見。曾有報導在唾液中找到尿酸鹽結晶。腦、脾、肺未見痛風石。痛風石出現後慢慢由小變大,尿酸鹽結晶逐漸增多,內壓增高,常常會使局部皮膚膨脹、緊張、菲薄、發亮。加上尿酸鹽結晶的侵蝕作用,覆蓋其上的皮膚完整性受到破壞,抗牽拉性下降,一旦因摩擦、受壓、受凍以及創傷等原因可發生潰爛,“牙膏樣”白色尿酸鹽結晶物質就會從破潰的洞裡漏出來。破潰處可形成竇道或疹管。開口周圍組織由於尿酸鹽結晶的刺傲呈慢性炎症性肉芽腫。易繼發細菌感染,形成慢性化膿性病灶。破潰處由於血液循環差,細胞再生力弱,加上感染和慢性肉芽腫等原因,難以自行癒合。嚴重者可引起
膿毒血症,導致死亡。
痛風石是痛風的特徵性改變。痛風石的形成與病程、血尿酸水準有關。病程越長,發生痛風石的幾率就越大。高尿酸血症持續時間越長,越易發生痛風石。反之,痛風石越多、越大,表明高尿酸血症未得到很好控制,即病情越重。有些患者,病程雖已很長,但治療後血尿酸長期保持在正常範圍內,很少發生痛風石。痛風石的發生、痛風石的數量與大小,也是臨床判斷病情輕重和治療是否滿意的直觀指標。
皮下痛風石發生的典型部位是耳郭,也常見於反復發作的關節周圍及鷹嘴、跟腱和髕骨滑囊等部位。外觀為皮下隆起的大小不一的黃白色贅生物,皮膚表面菲薄,破潰後排出白色粉狀或糊狀物,經久不愈。
皮下痛風石常與慢性痛風石性關節炎並存。關節內大量沉積的痛風石可造成關節骨質破壞、關節周圍組織纖維化和繼發退行性改變等。臨床表現為持續關節腫痛、壓痛、畸形及功能障礙。慢性期症狀相對緩和,但也可有急性發作。
尿酸性腎病是由於血尿酸產生過多或排泄減少形成高尿酸血症所致的腎損害,通常稱為痛風腎病,臨床表現可有尿酸結石,小分子蛋白尿、水腫、夜尿、高血壓、血、尿尿酸升高及腎小管功能損害。本病西方國家常見,國內以北方多見,無明顯的季節性,肥胖、喜肉食及酗酒者發病率高。男女之比為9:1,85%為中老年人。本病如能早期診斷並給予恰當的治療(控制高尿酸血症和保護腎功能),腎臟病變可減輕或停止發展,如延誤治療或治療不當,則病情可惡化
並發展為終未期腎功能衰竭而需要透析治療。
慢性尿酸鹽腎病,尿酸鹽晶體沉積於腎間質,導致慢性腎小管一間質性腎炎。臨床表現為尿濃縮功能下降,出現夜尿增多、低比重尿、小分子蛋白尿、白細胞尿、輕度血尿及管型尿等。晚期可致腎小球濾過功能下降,出現腎功能不全。
尿酸性尿路結石,尿中尿酸濃度增高呈過飽和狀態,在泌尿系統沉積並形成結石。在痛風患者中的發生率在20%以上,且可能出現於痛風關節炎發生之前。結石較小者呈砂礫狀隨尿排出,可無症狀;較大者可阻塞尿路,引起腎絞痛、血尿、排尿困難、泌尿系感染、腎盂擴張和積水等。
影響腎結石形成的因素很多,年齡、性別、種族、遺傳、環境因素、飲食習慣和職業與結石的形成相關。機體的代謝異常、尿路的梗阻、感染、異物和藥物的使用是結石形成的常見病因。已經知道泌尿結石有32種成分,最常見的成分為草酸鈣。其他成分的結石如磷酸銨鎂、尿酸、磷酸鈣以及胱胺酸(一種胺基酸)等,也可以是以上各種成分的混合物。尿酸鹽結石尿液持續酸性,特點為質硬,光滑,顆粒狀,黃色或棕紅色,尿酸代謝異常。
急性尿酸性腎病血及尿中尿酸水準急驟升高,大量尿酸結晶沉積於腎小管、集合管等處,造成急性尿路梗阻。臨床表現為少尿、無尿,急性腎功能衰竭;尿中可見大量尿酸晶體。多由惡性腫瘤及其放化療(即腫瘤溶解綜合症)等繼發原因引起。
血尿酸升高沉積於關節可引起痛風性關節炎,進一步引起關節變形;沉積於腎臟可引起痛風性腎病、尿酸結石進一步引起尿毒癥;血尿酸升高可以刺激血管壁引起動脈粥樣硬化並加重冠心病、高血壓;血尿酸升高可以損傷胰腺B細胞,從而誘發或加重糖尿病。
高尿酸血症是痛風的發病基礎,但不足以導致痛風,只有尿酸鹽在機體組織中沉積下來造成損害才出現痛風;血尿酸水準越高,未來5年發生痛風的可能性越大。
本發明中,R1選自氫,C1-4烷基,其中所述的C1-4烷基未被取代或者任選被一個或者多個鹵素原子、氰基和CF3取代,更優選選自氫和未取代的C1-4烷基,進一步優選選自氫、甲基和乙基,最優選氫或甲基。
R2選自C1-10烷基,C2-6烯基,C2-6炔基,其中所述的C1-10烷基、C2-6烯基、C2-6炔基未被取代或者任選被鹵素原子、氰基、C3-7環烷基取代,更優選C1-6烷基,進一步優選乙基,環丙基乙基,環丙基甲基,丙基,2-甲基丙基,丁基,3-甲基丁基或者戊基,更進一步優選C3-6烷基,再進一步優選C4-5烷基,最優選丁基或戊基。

另外,本發明的通式(I)表示的化合物中,如果存在手性碳,則本發明包括基於該手性碳的任何立體構型形成的異構體,例如包括消旋體或任何一種鏡像異構體。而且,本發明包括全部可能出現的其他立體異構體。也就是說,本發明的化合物包括所有對映異構體、非對映異構體、平衡化合物、它們的任意比例的混合物、消旋體等。
本發明的通式(I)化合物可以按照各種公知的方法製備,沒有特別限定,例如可以如下述反應路徑圖所記載的那樣按照以下的反應步驟製備,但其製備方法並不限於此。

具體而言,如上述反應路徑所示,當R3代表鹵素原子時,在0-50℃優選20-30℃下,在溶劑DMSO、DMF、丙酮、二氧六環、乙腈、四氫呋喃或N-甲基吡咯烷酮,優選DMSO中,用烯丙基溴烷基化原料1,得到中間體A。然後,在濃度低的酸(例如醋酸、稀鹽酸或稀硫酸)和水的體系中,在0-100℃下,優選20-60℃下,使用亞硝酸鈉重氮化,接著水解形成中間體B。中間體B在0-100℃下優選20-60℃下,在DMF、DMSO、二氧六環、乙腈、四氫呋喃或N-甲基吡咯烷酮中,用鹵代丁二醯亞胺鹵化,可以得到中間體C。中間體C在0-50℃下優選25℃下,在DMF、DMSO、二氧六環、乙腈、四氫呋喃或N-甲基吡咯烷酮中,鹼的作用下,使用鹵代烴烷基化得到中間體D。其中中間體C和中間體D的製備先後順序可以互換。當R1代表烷基時,中間體D在0-120℃下優選50-100℃下下,在DMF、DMSO、二氧六環、乙腈、四氫呋喃或N-甲基吡咯烷酮中,鹼的作用下,使用鹵代烴烷基化得到中間體E。中間體D(R1代表氫原子)或中間體E(R1
代表烷基)在0-60℃下,優選20-30℃下,惰性氣體保護下,在二氯甲烷、氯仿、四氯化碳或者丙酮中,加入嗎啉或者1,3二甲基巴比妥酸,在鈀催化劑催化下或者無催化劑催化下,除去烯丙基得式(I)所示化合物。
當R3代表氰基時,中間體B在0-50℃下優選25℃下,在DMF、DMSO、二氧六環、乙腈、四氫呋喃或N-甲基吡咯烷酮中,鹼的作用下,使用鹵代烴烷基化得到中間體C’。中間體C’在0-120℃下優選50-100℃下,在DMF、DMSO、二氧六環、乙腈、四氫呋喃或N-甲基吡咯烷酮中,鹼的作用下,使用鹵代烴烷基化得到中間體D’。中間體D’通過用LiHMDS或者NaHMDS進行消除反應,用DMF、醇類或水淬滅,得到中間體E’。中間體E’在乙酸酐、甲苯或苯中,鹽酸羥胺和吡啶作用下得到中間體F’。中間體F’在0-60℃下,優選20-30℃下,惰性氣體保護下,在二氯甲烷、氯仿、四氯化碳或者丙酮中,加入嗎啉或者1,3二甲基巴比妥酸,在鈀催化劑催化下或者無催化劑催化下,除去烯丙基得式(I)所示化合物。
上述鹵代丁二醯亞胺可以選自氯代丁二醯亞胺,溴代丁二醯亞胺,碘代丁二醯亞胺等。上述鹼可以選自碳酸鈉,碳酸鉀,碳酸氫鈉,碳酸銫等。上述惰性氣體可以選自氮氣、氬氣等。
上述各反應得到的中間體和目的產物可以根據需要通過有機合成化學常用的精製方法,例如過濾、萃取、洗滌、乾燥、濃縮、重結晶、各種層析法等進行分離、精製。
另外,中間體也可以不特別進行精製直接用於下一步反應。
得到的通式(I)表示的化合物也可以按照傳統的方法形成酸加成鹽、鹼加成鹽,以及各種溶劑化物,例如水合物。
各種異構體可以採用利用異構體間的物理化學性質差異的傳統方法分離。例如,消旋混合物可以通過與酒石酸等一般的光學活性酸形成非對映異構體鹽進行光學拆分的方法,或者採用光學活性柱層析法的方法等一般的消旋拆分法,獲得光學純的異構體。另外,非對映異構體混合物例如可以通過分別結晶或各種層析法等拆分。另外,光學活性的化合物也可以使用適當的光學活性原料進行製備。
本發明包括含有式(I)所示化合物、其藥學上可接受的鹽、其溶劑化物中的任何一種或多種的組合物。所述組合物可以含有一種或多種藥用載體。作為上述載體,例如賦形劑和稀釋劑,包括水、乳糖、葡萄糖、果糖、蔗糖、山梨糖醇、甘露醇、聚乙二醇、丙二醇、澱粉、樹膠、明膠、藻酸鹽、矽酸鈣、磷酸鈣、纖維素、水糖漿、甲基纖維素、聚乙烯基吡咯烷酮、滑石、硬脂酸鎂、硬脂酸、甘油、芝麻油、橄欖油、大豆油等各種油等。
本發明還包括上面所述的任一化合物或其藥學上可接受的鹽或其溶劑化物與一種或多種降尿酸藥物、抗痛風發作藥物和抗炎藥物的藥物組合物。所述降尿酸藥物選自減少尿酸生成藥物和促尿酸排泄藥物,所述抗炎藥物包括免疫調控非類固醇抗炎藥(NSAIDs)和糖皮質激素。所述減少
尿酸生成藥物選自黃嘌呤氧化酶抑制劑,選自別嘌醇、非布司他、聚乙二醇重組尿酸酶pegloticase,所述促尿酸排泄藥物選自尿酸鹽陰離子轉運蛋白1(URAT1)抑制劑,例如丙磺舒、苯溴馬隆、苯磺唑酮、lesinurad,所述抗痛風發作藥物選自秋水仙鹼,所述非類固醇抗炎藥選自非選擇性非類固醇抗炎藥和選擇性環氧化酶(COX-2)抑制劑,所述非選擇性非類固醇抗炎藥阿司匹林、貝諾酯、吲哚美辛、氨糖美辛、舒林酸、雙氯芬酸鈉、布洛芬、芬必得、酮洛芬、萘普生及吡羅昔康,所述選擇性環氧化酶(COX-2)抑制劑選自塞來昔布、羅非昔布及帕瑞昔布等,所述糖皮質激素選自地塞米松、氫化可的松、可的松、強的松、強的松龍、甲基強的松龍、潑尼松、潑尼松龍及倍他米松。
本發明的含有上述通式(I)表示的化合物或其藥學上可接受的鹽、其溶劑化物作為有效成分的藥物組合物的給藥方式,可以例舉通過片劑、膠囊劑、顆粒劑、散劑、糖漿劑等的口服給藥,或者通過靜脈注射劑、肌肉注射劑、注射用無菌粉末、注射用濃溶液、栓劑、吸入劑、經皮吸收劑、滴眼劑、滴鼻劑等的非口服給藥。另外,配製上述各種劑型的藥物製劑時,可以單獨使用該有效成分,或者與其他可藥用載體,即賦形劑、黏合劑、增量劑、崩解劑、界面活性劑、潤滑劑、分散劑、緩衝劑、保存劑、矯味劑、香料、包膜劑、稀釋劑等適當組合,採用傳統方法製成藥物製劑。
本發明還包括藥劑盒,其包含:上述組合物,以及包括一種或多種形式的資訊的說明書,該資訊選自顯示給
予該藥物組合物所針對的疾病狀態、藥物組合物的貯藏資訊、給藥資訊和關於如何給予該藥物組合物的用法說明。
本發明的藥物組合物的給藥量根據患者的體重、年齡、性別、症狀等有所不同,可以根據口服給藥或非口服給藥等適當選擇。
下面,通過實施例形式的具體實施方式,對本發明的上述內容作進一步的詳細說明。但不應將此理解為本發明上述主題的範圍僅限於以下實施例。

由試驗結果可知,化合物A、B、C、D在食蟹猴體內IV給藥的半衰期(t1/2z(h))差別較大,降尿酸實驗時間點設定根據t1/2z(h)進行設計。

每個化合物選用non-naïve食蟹猴3只,編號分別為1#、2#、3#,靜脈注射給予供試品,給藥前及給藥後1h、3h、6h、24h靜脈採血。室溫靜置,離心取血清,檢測血清尿酸含量。
血清尿酸降低率=(給藥前平均尿酸含量-給藥後平均尿酸含量)/給藥前平均尿酸含量*100%。
試驗結果:參考化合物A、B、C、D在食蟹猴體內IV給藥的半衰期(t1/2z(h)),分別測定化合物A給藥前、給藥後3h和6h的血清尿酸含量及血清尿酸降低率,化合物B給藥前和給藥後3h的血清尿酸含量及血清尿酸降低率,化合物C給藥前和給藥後24小時的血清尿酸含量及血清尿酸降低率,化合物D給藥前和給藥後3h的血清尿酸含量及血清尿酸降低率,具體結果如下表2~9所示。
由試驗結果可知,與給藥前比,化合物A、B、C、D可以顯著降低受試猴血清尿酸含量,說明式(I)所示化合物具有治療和/或預防高尿酸血症、痛風、痛風性炎症和尿酸性腎病的用途。

試驗結果:參考模型組尿酸(UA)在食蟹猴體內IV給藥的半衰期(t1/2z 0.54h)以及血清尿酸含量變化趨勢,給藥組選取血清尿酸含量檢測時間點為給藥前、給藥後2h和UA造模後1h、2h、4h,模型組和給藥組血清尿酸含量及血清尿酸降低率分別見下表3-1、3-2和3-3。

MSU懸浮液配製:1g尿酸溶於含6mL 1N NaOH的0.2L沸水中,調節pH值為7.4,室溫放涼,4℃過夜。離心蒸發乾燥MSU晶體,超音波處理後,顯微鏡檢查針狀晶體長度在5-25μm。5mg晶體裝入一個玻璃瓶,高壓滅菌。氣囊注射前無菌的MSU晶體加入5mL滅菌生理鹽水。
雄性SD大鼠適應性餵養1周後,隨機分6組,動物麻醉後背部注射24mL無菌空氣,4天后第二次注射空氣。第7天空白組、模型組灌胃給予溶媒10mL/kg,地塞米松組腹腔注射地塞米松5mg/kg,秋水仙鹼組灌胃給予秋水仙鹼5mg/kg,化合物A高、低劑量組份別灌胃給予100mg/kg和30mg/kg化合物A,給藥後1h,空白組大鼠背部氣囊中注射5mL生理鹽水,其他組大鼠背部氣囊中注射5mL 1mg/mL MSU晶體懸浮液。4h後取氣囊內灌洗液,一部分灌洗液用於細胞學分析,剩餘灌洗液8000rpm,離心15min,上清液-80℃保存至細胞因數IL-1β、IL-6,KC和TNFα檢測。試驗結果用mean±SEM標示,用t-檢驗進行統計分析。

由試驗結果可知,地塞米松和秋水仙鹼能顯著抑制由MSU引起的灌洗液總細胞數和中性粒細胞數。化合物A在口服30mg/kg和100mg/kg劑量下能顯著抑制灌洗液總細胞數和中性粒細胞數。地塞米松能顯著抑制由MSU引起IL-1β、IL-6、KC和TNFα的增加,化合物A在口服100mg/kg劑量下能顯著抑制IL-6、KC和TNFα的增加。該試驗證明化合物A在MSU誘導的氣囊試驗中具有很好的抗炎作用。

實驗方法:
1 將化合物用二甲基亞碸(DMSO)配製為5mM的母液備用。本次試驗共8個濃度,終濃度分別為5000nM、1250nM、312.5nM、78.1nM、19.5nM、4.9nM、1.2nM、0.3nM。
2 緩衝液配製方法:無氯HBSS緩衝液(125mM葡萄糖酸鈉,4.8mM葡萄糖酸鉀,1.3mM葡萄糖酸鈣,1.2mM KH2PO4,1.2mM MgSO4,5.6mM葡萄糖,25mM 4-羥乙基呱嗪乙磺酸(HEPES)(pH7.4));裂解液(100mM NaOH)
3 獲得穩定表達人尿酸鹽轉運體1(hURAT1)的轉染細胞:將人胚腎細胞(HEK-293T)細胞置於含有完全培養基的細胞培養板中,在5% CO2、37℃下孵育24h,用磷酸鹽緩衝液(PBS)沖洗細胞,胰蛋白酶消化,製備成單細胞懸浮液。取8×106細胞置於細胞培養板中,然後將TransIT-293 Reagent:DNA complexes(1.5ml Opti-MEM I Reduced-Serum Medium,15μg plasmid DNA,45μL TransIT-293 Reagent,混勻後室溫下孵育30min)逐滴加入到細胞培養板的不同區域,輕輕振盪細胞培養板使TransIT-293 Reagent:DNA complexes分佈均勻,在5% CO2、37℃下孵育48h。
4 hURAT1轉染細胞中14C標記的尿酸攝取實驗:(1)將hURAT1轉染的細胞鋪到Poly-D-lysine 96-well微孔板中,細胞密度為6×104/孔,在5% CO2,37℃下過夜。(2)細胞鋪到Poly-D-lysine 96-well微孔板中12h以後,用預先加熱的無氯HBSS緩衝液洗滌細胞3次,200μl/孔,並將微孔板中的洗滌液清除。(3)加入含Uric acid[8-14C](0.1μCi/well)無氯HBSS緩衝液,50μl/孔,然後加入待測化合物,5μl/孔,37℃孵育5min。(4)清除孵育緩衝液並加入100μl冰冷的無氯HBSS緩衝液終止Uric acid[8-14C]攝取反應。(5)用無氯HBSS緩衝液洗板3次,並清除孔中的緩衝液。(6)加入50μl/孔裂解液,600rpm條件下振盪10min。(7)將微孔板置於離心機上,1000rpm離心5min,然後吸取45μl上清液至Isoplate-96 Microplate中。(8)加入150μl/孔Ultima GoldTM XR scitillation cocktail並在600rpm條件下振盪10min。將Isoplate-96微孔板置於MicroBeta Trilux(PerkinElmer)中檢測14C的放射性強度。
5 計算IC50值
抑制率(%)=(陽性對照信號強度-化合物信號強度)/(陽性對照信號強度-陰性對照信號強度)×100,採用GraphPad Prism 5.0軟體進行分析,得出IC50值。
實驗結果:見表19。
lesinurad*,該化合物是2015年12月FDA批准的阿斯利康公司研發的治療痛風的新型藥物,其化學名為2-[[5-溴-4-(4-環丙基-1-萘)-4H-1,2,4-三唑-3-基]硫代]乙酸。
由表19可知,本發明化合物對hURAT1(人尿酸鹽轉運體1)具有良好的抑制作用。
(1)7-烯丙基-2-胺基-1H-嘌呤-6-(7H)-酮的製備
將鳥嘌呤核苷(2000g,7.07mol),烯丙基溴(1950.5g,16.20mol)溶於DMSO,氮氣保護下,室溫攪拌,18小時。37%的濃鹽酸(5L)加入溶液中攪拌1小時,加入2L甲醇,然後用2N的氫氧化鈉溶液中和,直到有固體析出,過濾,濾餅在烘箱裡乾燥得白色固體(1210g,收率89.6%)。
(2)7-烯丙基-1H-嘌呤-2,6-(3H,7H)-二酮的製備
將7-烯丙基-2-胺基-1H-嘌呤-6(7H)-酮(1200g,6.28mol)溶於醋酸(3L)和水(750mL)中,將亞硝酸鈉(1732g,25.1mol)的水溶液(600mL)滴入到上述體系中,反應攪拌3h,濃縮反應液到三分之一,靜止,析出固體,過濾,濾餅乾燥得淡黃色固體(875g,收率:72.8%)。
(3)7-烯丙基-3-丁基-1H-嘌呤-2,6-(3H,7H)-二酮的製備
將7-烯丙基-1H-嘌呤-2,6-(3H,7H)-二酮(400g,2.08mol),碘代正丁烷(422g,2.29mol)和碳酸鉀(345g,2.50mol)溶於無水DMF(1.6L)中,反應攪拌24小時,加入乙酸乙酯(2L)和2N的稀鹽酸(500mL),萃取,有機相乾燥,旋乾得淡黃色固體(185g,收率35.9%)。
(4)7-烯丙基-3-丁基-8-氯-1H-嘌呤-2,6-(3H,7H)-二酮的製備
將7-烯丙基-3-丁基-1H-嘌呤-2,6-(3H,7H)-二酮(160g,0.645mol)溶於無水DMF(800mL)中,加入NCS(94.7g,0.71mol),氮氣保護下反應攪拌24小時,旋乾,加入200mL乙酸乙酯,重結晶,過濾,濾餅乾得淡黃色固體(110.2g,收率60.5%)。
(5)8-氯-3-丁基-3,7-二氫-1H-嘌呤-2,6-二酮的製備
將7-烯丙基-3-丁基-8-氯-1H-嘌呤-2,6-(3H,7H)-二酮粗品(565mg,2.0mmol)、四三苯基膦鈀(104mg,0.09mmol)和嗎啉(775mg,8.9mmol)溶於20mL二氯甲烷中,氮氣保護下室溫反應12小時,濃縮,矽膠柱層析(石油醚:乙酸乙酯=1:1),得到淡黃色體80mg,收率16.5%。

將鳥嘌呤核苷(200g,0.707mol),烯丙基溴(19.51g,0.162mol)溶於DMSO(,氮氣保護下,室溫攪拌18小時。37%的濃鹽酸(500mL)加入溶液中攪拌1小時,加入2L甲醇,然後用2N的氫氧化鈉溶液中和,直到有固體析出,過濾,濾餅在烘箱裡乾燥得白色固體(125g,收率92.5%)。
(2)7-烯丙基-1H-嘌呤-2,6-(3H,7H)-二酮的製備
將7-烯丙基-2-胺基-1H-嘌呤-6-(7H)-酮(120g,0.628mol)溶於醋酸(1.5L)和水(150mL)中,將亞硝酸鈉(173.2g,2.51mol)的水溶液(300mL)滴入到上述體系中,反應攪拌3h,濃縮反應液到三分之一,靜止,析出固體,過濾,濾餅乾燥得淡黃色固體(85g,收率70.5%)。
(3)7-烯丙基-8-氯-1H-嘌呤-2,6-(3H,7H)-二酮的製備
將7-烯丙基-1H-嘌呤-2,6-(3H,7H)-二酮(2.10g,10.9mmol)溶於無水DMF(12mL)中,加入N-氯代丁二醯亞胺(1.60g,12.0mmol),氮氣保護下反應攪拌6小時,體系倒入水中,乙酯萃取,旋乾,得淡黃色固體粗品1.20g。
(4)7-烯丙基-3-戊基-8-氯-1H-嘌呤-2,6-(3H,7H)-二酮的製備
將7-烯丙基-8-氯-1H-嘌呤-2,6-(3H,7H)-二酮粗品(750mg)和碳酸鈉(383mg,3.61mmol)溶於無水DMF(10mL)中,加入碘代正戊烷(690mg,3.48mmol)反應攪拌24小時,體系倒入水中,加入乙酸乙酯(100mL)和2N的稀鹽酸(50mL),萃取,有機相乾燥,旋乾得淡黃色油狀物粗品
500mg。
(5)8-氯-3-戊基-3,7-二氫-1H-嘌呤-2,6-二酮的製備
具體方法參照實施例1中的(5),製得目的化合物80mg,收率4.6%。

(2)8-氯-1-甲基-3-丁基-3,7-二氫-1H-嘌呤-2,6-二酮的製備
將8-氯-1-甲基-3-丁基-7-(2-丙烯-1-基)-1H-嘌呤-2,6-(3H,7H)-二酮粗品(400mg)、四三苯基膦鈀(104mg,0.09mmol)和嗎啉(775mg,8.9mmol)溶於20mL二氯甲烷中,氮氣保護下室溫反應12小時,濃縮,矽膠柱層析(石油醚:乙酸乙酯=2:1),得到淡黃色體112mg,兩步收率23.1%。

(2)7-烯丙基-1H-嘌呤-2,6-(3H,7H)-二酮的製備
將7-烯丙基-2-胺基-1H-嘌呤-6-(7H)-酮(1584g,8.29mol)溶於醋酸(8.5L)和水(1500mL)中,將亞硝酸鈉(2277g,33mol)的水溶液滴入到上述體系中,反應攪拌過夜,有固體,過濾,固體用水洗滌,濾餅乾燥得白色固體(1086g,收率68.2%)。
(3)7-烯丙基-8-氯-1H-嘌呤-2,6-(3H,7H)-二酮的製備
將7-烯丙基-1H-嘌呤-2,6-(3H,7H)-二酮(2166.3g,11.3mmol)溶於無水DMF(8L)中,加入N-氯代丁二醯亞胺(1657g,12.4mmol),氮氣保護下反應攪拌24小時,加入EA,冷卻,抽濾,固體用EA洗滌,乾燥,得白色固體(1804g,收率70.6%)。
(4)7-烯丙基-3-戊基-8-氯-1H-嘌呤-2,6-(3H,7H)-二酮的製備
將7-烯丙基-8-氯-1H-嘌呤-2,6-(3H,7H)-二酮(200g,0.88mol)溶於無水DMF(1.2L)中,加入碳酸鈉(116.6g,1.1mol),加入碘代正戊烷(173g,0.87mol)反應攪拌4天,體系倒入水中,有固體析出,抽濾,固體用正己烷漿洗,抽
濾,乾燥得白色固體(197g,收率75.2%)。
(5)8-氯-3-戊基-3,7-二氫-1H-嘌呤-2,6-二酮一水合物的製備
將7-烯丙基-3-戊基-8-氯-1H-嘌呤-2,6-(3H,7H)-二酮(96g,0.32mol)、四三苯基膦鈀(13g,0.011mmol)和N,N-二甲基巴比妥酸(253g,1.62mol)溶於1L二氯甲烷中,氮氣保護下室溫反應12小時,抽濾,固體用氫氧化鈉水溶液溶解,用二氯甲烷洗滌,水相用稀HCl調pH至4,有固體析出,抽濾,固體烘乾,得白色固體60g,收率67.7%。
分子式:C10H15ClN4O3;分子量:274.1;質譜(M+H):257.1
1H-NMR(DMSO-d 6 ,400MHz):δ 0.84-0.86(t,3H),1.28(m,4H),1.63(m,2H),3.85(t,2H),11.22(s,1H),14.38(br.s,1H)。
Claims (15)
- 如請求項1所述的用途,其中所述與生物體內尿酸升高引發的疾病是尿酸性和/或痛風性疾病。
- 如請求項1所述的用途,其中,所述式(I)中,R1表示氫或者甲基,R2表示乙基,環丙基乙基,環丙基甲基,丙基,2-甲基丙基,丁基,3-甲基丁基或者戊基,R3表示氟原子或者氯原子。
- 如請求項1所述的用途,其中,所述化合物選自:8-氯-3-戊基-3,7-二氫-1H-嘌呤-2,6-二酮,8-氯-3-丁基-3,7-二氫-1H-嘌呤-2,6-二酮,8-氯-1-甲基-3-丁基-3,7二氫-1H-嘌呤-2,6二酮,8-氯-1-甲基-3-戊基-3,7二氫-1H-嘌呤-2,6二酮,8-氯-3-(3-甲基丁基)-3,7二氫-1H-嘌呤-2,6二酮,8-氯-3-(2-環丙基乙基)-3,7二氫-1H-嘌呤-2,6二酮,8-氯-3-(2-甲基丙基)-3,7二氫-1H-嘌呤-2,6二酮,8-氯-3-(環丙基甲基)-3,7二氫-1H-嘌呤-2,6二酮,8-氯-1-甲基-3-(3-甲基丁基)-3,7二氫-1H-嘌呤-2,6二酮,和8-氯-1-甲基-3-(2-環丙基乙基)-3,7二氫-1H-嘌呤-2,6二酮。
- 如請求項1所述的用途,其中,所述化合物為8-氯-3-戊基-3,7-二氫-1H-嘌呤-2,6-二酮或其水合物。
- 如請求項2所述的用途,其中,所述尿酸性或痛風性疾病是高尿酸血症、痛風、痛風性炎症、疼痛或尿酸性腎病。
- 如請求項6所述的用途,其中,所述高尿酸血症包括原發性高尿酸血症和繼發性高尿酸血症。
- 如請求項6所述的用途,其中,所述的痛風包括原發性痛風和繼發性痛風。
- 如請求項6所述的用途,其中,所述痛風性炎症包括急性痛風性關節炎、皮下痛風石和慢性痛風石性關節炎。
- 如請求項6所述的用途,其中,所述疼痛包括急性疼痛、慢性疼痛、頑固性疼痛和癌性疼痛。
- 如請求項6所述的用途,其中,所述尿酸性腎病包括急性尿酸性腎病、慢性尿酸鹽腎病和尿酸性尿路結石。
- 如請求項12所述的用途,其中,所述式(I)中, R1表示氫或者甲基,R2表示乙基,環丙基乙基,環丙基甲基,丙基,2-甲基丙基,丁基,3-甲基丁基或者戊基,R3表示氟原子或者氯原子。
- 如請求項12所述的用途,其中,所述化合物選自:8-氯-3-戊基-3,7-二氫-1H-嘌呤-2,6-二酮,8-氯-3-丁基-3,7-二氫-1H-嘌呤-2,6-二酮,8-氯-1-甲基-3-丁基-3,7二氫-1H-嘌呤-2,6二酮,8-氯-1-甲基-3-戊基-3,7二氫-1H-嘌呤-2,6二酮,8-氯-3-(3-甲基丁基)-3,7二氫-1H-嘌呤-2,6二酮,8-氯-3-(2-環丙基乙基)-3,7二氫-1H-嘌呤-2,6二酮,8-氯-3-(2-甲基丙基)-3,7二氫-1H-嘌呤-2,6二酮,8-氯-3-(環丙基甲基)-3,7二氫-1H-嘌呤-2,6二酮,8-氯-1-甲基-3-(3-甲基丁基)-3,7二氫-1H-嘌呤-2,6二酮,和8-氯-1-甲基-3-(2-環丙基乙基)-3,7二氫-1H-嘌呤-2,6二酮。
