Description
本發明係有關於一種蛋白質或胜肽之持續釋放傳輸,特別是有關於一種以一疏水性、非聚合物載體材料進行共價修飾及配製以對蛋白質或胜肽進行有效持續釋放傳輸之組合物及方法。
疏水性、非聚合物材料,特別是,高黏度、非聚合物液體材料已有揭露作為用於生物活性化合物控制釋放傳輸的生物降解系統(Smith and Tipton,Pharmaceutical Research,13(9),S300,1996)。疏水性、非聚合物材料一般實質上不溶於水,其可為一於37℃黏度至少達5,000cp且於環境或生理條件下不產生均勻結晶的高黏度液體。當混合上述材料與一少量塑化溶劑時,混合物的黏度會較單獨非聚合物液體材料的黏度低許多,而此低黏度溶液可簡單地與一生物活性化合物進行配製,最後,得到的低黏度液體配方可立即施予一生物體,以形成一高黏度原位儲庫(depot)。
上述於原位形成包含疏水性、非聚合物液體載體材料的儲庫系統其典型代表例揭露於美國專利5,747,058、5,968,542、6,051,558及6,992,065。該等專利描述的組合物包括一例如蔗糖乙酸異丁酯(sucrose acetate isobutyrate,SAIB)的疏水性、高黏度、非聚合物液體材料、一溶於水或與水互溶的有機溶劑與一生物活性物質。上述組合物可簡單地製備並以一低黏度溶液型式施予一生物體。一旦進入體內,溶劑會消失或擴散進入周圍組織,導致非聚合物材料的沈澱或凝結,而形成一高黏度膠體、半固體或包覆生物活性物質的固體儲庫。之後,生物活性物質便藉由儲庫的溶解、擴散及/或降解而釋放。
然而,由於非聚合物載體材料的疏水性質,使得許多生物活性試劑,特別是,具有電荷與極性特性的親水性胜肽及蛋白質,可能無法與非聚合物載體材料相容,而導致一不穩定的液體配方。已知當包括一疏水性、非聚合物液體材料、一胜肽與一有機溶劑的組合物於環境條件下靜置一短時間周期時,會有相分離的現象發生。此外,由於蛋白質或胜肽的親核特性,致可能與一有機溶劑所形成溶液/懸浮液中的非聚合物載體材料產生相互作用或反應。此相互作用或反應將使非聚合物載體材料迅速降解並改變蛋白質或胜肽的化學性。配方中不穩定的載體材料及蛋白質或胜肽會阻礙一具有一合理儲存時間周期的適當組合物的製備以及使用配方以形成一依據施予(administration)達到預期釋放特性的一致性儲庫。再者,一爆炸釋放(burst release)為此類液體配方的典型特徵。此無法控制的最初爆炸可能為非預期,特別是對於窄治療指數(therapeutic index)的蛋白質或胜肽而言。雖上述配方已揭露使用某些聚合物添加劑,以調節釋放速率,然而,其結果並不全然令人滿意。
因此,實有必要發展一可解決上述問題且可於一長時間周期對一有效治療量的蛋白質或胜肽進行有效持續釋放傳輸的組合物。
本發明提供一種適合於原位儲庫系統之新穎液體組合物,以一控制、持續方式傳輸一蛋白質或胜肽。本發明組合物包括:一疏水性、非聚合物載體材料;一與水互溶之生物相容性有機溶劑,溶解該疏水性、非聚合物材料並降低該組合物黏度,以利其製備及施予之方便性;一蛋白質或胜肽,與一或多個配方效能促進化合物(formulation performance-enhancing compound)共價結合。其中該非聚合物材料實質上不溶於水且為一高黏度液體,其黏度於37℃至少達5,000cp且於環境或生理條件下不產生均勻結晶。本發明組合物選擇性地更包括一添加劑,以達到預期之釋放特性。本發明亦提供一種該組合物之製造及使用方法。
因此,蛋白質或胜肽較佳係與一穩定蛋白質或胜肽的配方效能促進化合物共價結合,以增加與非聚合物載體材料的相容性,促進蛋白質或胜肽的釋放。一通合的配方效能促進化合物為親水性、親脂性或兩性。該化合物為一小分子或一高分子。適合的配方效能促進化合物藉由一可降解或非降解鍵與一蛋白質或胜肽結合。結合的結果較佳係維持未修飾蛋白質或胜肽其部分或全部的原生物活性。
之後,將結合的蛋白質或胜肽混合或分散於疏水性、非聚合物載體材料,或者疏水性、非聚合物載體材料較佳與一水溶性或與水互溶的溶劑混合,例如甲基吡咯烷酮(N-methyl-2-pyrrolidone,NMP)或乙醇,以形成一低黏度溶液,之後,使用此低黏度溶液溶解或懸浮結合的蛋白質或胜肽,以形成一均質溶液或均勻懸浮液。一般來說,此類包含一未結合的蛋白質或胜肽或其簡單鹽類例如乙酸鹽的配方,會進行快速的相分離。然而,不可預期地發現,本發明藉由共價鍵所形成蛋白質或胜肽與配方效能促進化合物的結合可避免相分離,維持配方的物理穩定性。此外,一般非複雜型蛋白質或胜肽或其簡單鹽類例如乙酸鹽在製備及後續儲存的過程中易發生化學降解,然而,可藉由本發明蛋白質或胜肽與配方效能促進化合物的共價結合來避免或降低此化學降解。本發明組合物增加的化學及物理穩定性可允許發展一具有一預期特性與一合理貨架儲存壽命的穩定產物。
當本發明液體組合物與一例如一生物體內生物流體的水溶液環境接觸時,溶於水或與水互溶的溶劑會消失或擴散進入周圍的水溶液或生物流體,同時,不溶於水的非聚合物載體材料會沈澱或凝結,以形成一高黏度膠體、半固體或捕獲或包覆蛋白質或胜肽的固體儲庫。由於溶劑的快速擴散,在儲庫形成的過程中,會觀察到蛋白質或胜肽的一典型高最初爆炸釋放。然而,不可預期地發現,本發明藉由共價鍵所形成蛋白質或胜肽與配方效能促進化合物的結合可大幅降低爆炸效應,並促進蛋白質或胜肽的全面釋放,此蛋白質或胜肽為相對於包含未結合的蛋白質或胜肽的配方。一旦形成儲庫,蛋白質或胜肽便藉由非聚合物載體材料的溶解、擴散及/或降解而自非聚合物基質釋放。
根據本發明,組合物可選擇性地包含添加劑,以修飾組合物,達到蛋白質或胜肽的預期釋放效果。添加劑包括,但不限定於,爆炸效應降低材料、釋放速率調節試劑、酸鹼修飾劑、抗氧化劑、穩定劑及其類似物。添加劑為聚合物或非聚合物材料,包括生物可降解或非降解聚合物、碳水化合物或其衍生物、有機或無機化合物。
本發明組合物為一黏性或非黏性液體或為利用一注射器或類似器具可施予的膠體。組合物以皮下、肌肉內、腹腔內或皮膚內的注射方式施予,以形成一原位儲庫。組合物亦以口服、局部性或經黏膜的方式施予。當施予至一生物體內時,根據本發明製備的組合物可控制蛋白質或胜肽持續釋放一預期時間周期。依據適當選擇的非聚合物載體材料及其他賦形劑,可控制蛋白質或胜肽持續釋放的時間從數天、數周至一年的時間周期。
為讓本發明之上述目的、特徵及優點能更明顯易懂,下文特舉一較佳實施例,並配合所附圖式,作詳細說明如下:
本發明係有關於一蛋白質或胜肽以一配方效能促進化合物的共價修飾,以於疏水性、非聚合物材料中製備一蛋白質或胜肽的穩定配方,以避免或降低蛋白質或胜肽在製備及後續儲存過程中的降解及反應,促進蛋白質或胜肽的持續釋放效果。此發現引導發展適合於原位儲庫系統的預期組合物,以一控制、持續方式傳輸蛋白質或胜肽。
本發明提供一組合物,包括:一疏水性、非聚合物載體材料;一與水互溶的藥學上可接受溶劑;以及一蛋白質或胜肽,與一或多個配方效能促進化合物(formulation performance-enhancing compound)共價結合。本發明組合物選擇性地更包括一添加劑,以達到預期之釋放特性。本發明亦提供一上述組合物之製造及使用方法。
本發明使用疏水性、非聚合物材料作為一載體,以控制蛋白質或胜肽的持續釋放。為數眾多的藥學上可接受非聚合物材料已為本領域具有通常知識者所使用,以產生適合的藥學組合物,應用於不同蛋白質或胜肽的持續釋放傳輸。關於疏水性、非聚合物傳輸系統及其製備方法的典型代表例包括美國專利5,736,152、5,888,533、6,120,789、5,968,542及5,747,058。
適合的非聚合物載體材料包括,但不限定於,例如膽固醇硬酯酸酯的膽固醇酯類;例如甘油單油酸酯、甘油單亞油酸酯(glyceryl monolinoleate)、甘油單月桂酸酯(glyceryl monolaurate)、甘油單二十二烷酸酯(glyceryl monodocosanoate)、甘油單十四烷酸酯(glyceryl monomyristate)、甘油單癸酸酯(glyceryl monodecenoate)、甘油雙棕櫚酸酯、甘油雙二十二烷酸酯、甘油雙十四烷酸酯、甘油雙癸酸酯、甘油參二十二烷酸酯、甘油參十四烷酸酯、甘油參癸酸酯、甘油參硬酯酸酯及其混宜物的單酸甘油酯、雙酸甘油酯及參酸甘油酯(碳數為16~32);例如蔗糖雙硬酯酸酯及蔗糖棕櫚酸酯的蔗糖脂肪酸酯;例如山梨醇單硬酯酸酯(sorbitan monostearate)、山梨醇單棕櫚酸酯及山梨醇參硬酯酸酯的山梨醇脂肪酸酯;例如十六醇棕櫚酸酯及十六芳醇棕櫚酸酯的脂肪醇酯及脂肪酸;例如卵磷脂、磷脂醯絲胺酸(phosphatidylserine,PS)、磷脂醯乙醇胺、磷脂醯肌醇(phosphatidylinositol,PI)及其衍生物的磷脂類;神經鞘氨醇(sphingosine)及其衍生物;例如硬酯酸神經鞘磷脂(stearyl sphingomyelins)、棕櫚酸神經鞘磷脂及tricosanyl sphingomyelins的神經鞘磷脂;例如硬酯酸神經醯胺(stearyl ceramide)及棕櫚酸神經醯胺的神經醯胺;醣神經鞘脂類(glycosphingolipids);及上述材料的組合物及混合物。
較佳的非聚合物載體材料具有低結晶性及非極性特性,且為疏水。非聚合物載體材料更佳為一黏性液體。非聚合物液體材料較佳為疏水,實質上不溶於水且其黏度於37℃至少達5,000cp而於環境或生理條件下不產生均勻結晶。此處,”疏水或不溶於水”一詞係指一材料於25℃時水中溶解度低於1wt%。此處,”非聚合物”一詞係指酯類或混合酯類其用來形成酯類的酸性基團中無重複單元。用來形成酯類或混合酯類的酸性基團可包括一小數目的重複單元(寡聚物),例如二聚體、三聚體、四聚體及五聚體。一般來說,酸性基團中的重複單元必須少於5。
本發明疏水性、非聚合物載體材料可為一或多個非聚合物酯類或混合酯類。一般來說,酯類形成自一具有至少20個羥基的聚醇,其與羧酸進行酯化。適合的聚醇包括具有2~24個碳原子的單官能基及多官能基醇類、糖醇、單醣、雙醣、寡醣與聚醚醇。聚醇亦可為十二烷醇(dodecanol)、己二醇、丙三醇(甘油)、甘露醇(mannitol)、山梨醇(sorbitol)、葡萄糖、果糖、蔗糖、肌醇(inositol)、聚丙三醇、聚乙二醇、聚丙二醇、聚乙二醇-聚丙二醇共聚物、聚乙烯醇及其類似物。
用於形成疏水性、非聚合物載體材料的羧酸包括具有2個碳以上的有機酸,例如脂肪酸。羧酸可為飽和、不飽和、芳香族(芳基或芳烷基)、線性或分支結構。羧酸可具有一或多個羥基或其他例如鹵素、硝基及其類似物的基團。羧酸包括乙酸、丙酸、丁酸、異丁酸、戊酸(valeric acid)、硫鋅酸(lipoic acid)、己酸、庚酸、油酸、棕櫚酸、硬脂酸、荳蔻酸(myristic acid)、苯甲酸、乙醇酸、乳酸、ε-羥基己酸(ε-hydroxycaproic acid)、辛酸、癸酸、十二酸、十四酸、十六酸、十八酸、花生酸(eicosanoic acid)、二十二酸(docosanoic acid)及其他脂肪酸。
本發明疏水性、非聚合物載體材料較佳具有生物可分解性,不會產生任何非生物可分解或毒性分解物。當混合疏水性、非聚合物載體材料與一水溶性溶劑時,可獲得一低黏度溶液。此低黏度溶液可再與一蛋白質或胜肽結合,而製備出本發明組合物。由於低黏度特性,使得組合物易於施予一生物體。可藉由簡單調整疏水性、非聚合物載體材料與溶劑之間的比例,獲得一預期黏度。
在一較佳實施例中,使用蔗糖乙酸異丁酯(sucrose acetate isobutyrate,SAIB)作為一疏水性、非聚合物載體材料。蔗糖乙酸異丁酯(SAIB)為一蔗糖的混合酯類,以兩個乙酸及六個異丁酸基團進行酯化。該酯類為非結晶,其黏度於30℃超過100,000cp。該酯類的黏度可藉由輕微增加溫度或添加溶劑予以大幅降低。本發明使用的蔗糖乙酸異丁酯(SAIB)購自美國Eastman Chemical Company及參照美國專利2,931,802所揭露流程進行合成。在一實施例中,將蔗糖乙酸異丁酯(SAIB)進行加熱並與一共價結合於至少一配方效能促進化合物的蛋白質或胜肽混合,以製備一懸浮液。蔗糖乙酸異丁酯(SAIB)亦可與多種不同的生物相容性溶劑混合,以獲得一易與一蛋白質或胜肽配製的低黏度溶液,得到一注射溶液或懸浮液。
可選擇使用在本發明組合物的適合溶劑具有生物相容性且可溶於水或可分散互溶。溶劑於水中的溶解度在25℃至少達1wt%,較佳為至少3wt%,更佳為至少7wt%。當與疏水性、非聚合物載體材料結合時,溶劑可大幅降低混合物的黏度(相對於單獨非聚合物載體材料來說)。此低黏度液體組合物可進一步與一蛋白質或胜肽配製,用於持續釋放傳輸。本發明適合的溶劑包括,但不限定於,丙酮、苯甲醇、丁二醇、己內醯胺(caprolactam)、己內酯(caprolactone)、二甲基亞碸(DMSO)、乙醇、乙酸乙酯、乳酸乙酯、丙三醇(甘油)、甘油縮甲醛(glycerol formal)、四氫呋喃聚乙二醇醚(glycofurol)、甲基吡咯烷酮(N-methyl-2-pyrrolidone,NMP)、聚乙二醇、PEG-300、PEG-400、烷氧基聚乙二醇、mPEG-350、碳酸丙烯酯(propylene carbonate)、2-吡咯烷酮、甘油三乙酸酯(triacetin)、檸檬酸三乙酯(triethyl citrate)與其混合物。
此處,胜肽為鍵結所形成的短鏈聚合物,定義為α-氨基酸。一氨基酸殘基與下一殘基之間的鍵結為已知的一醯胺鍵或一胜肽鍵。蛋白質為多胜肽分子(或為多重多胜肽次單元)。兩者的不同在於胜肽為短鏈分子,而多胜肽或蛋白質為長鏈分子。此處,胜肽、多胜肽及蛋白質可互為使用,表示為相同類型的分子。

較佳來說,蛋白質或胜肽與一配方效能促進化合物共價結合。共價結合的蛋白質或胜肽維持部分或全部生物活性,並增加原試劑的配方效能。此處所使用的配方效能促進化合物為可與蛋白質或胜肽共價結合的化合物,且結合的結果實質上可維持蛋白質或胜肽至少部分的生物活性。結合的蛋白質或胜肽可輕易地與非聚合物載體材料進行配製,而所得的配方相對於使用未結合的蛋白質或胜肽者更為均勻及穩定。上述的結合可增加物化穩定性及促進蛋白質或胜肽自包含疏水性、非聚合物載體材料傳輸系統的釋放效果。蛋白質或胜肽的共價修飾亦可延長體內處置半生期、降低抗原性及免疫性、防止蛋白質水解、增加生體可利用率及降低毒性。
此處,”配方效能促進化合物”一詞代表任何經共價修飾後可促進於本發明非聚合物載體材料中蛋白質或胜肽配方效能的分子。該化合物可為親水性、親脂性或兩性。該化合物可為一小分子或一聚合物。作為配方效能促進化合物的條件為經共價修飾後可維持蛋白質或胜肽部分或全部生物活性及與本發明非聚合物載體材料配製時,可促進效能特性。本發明配方效能促進化合物與蛋白質或胜肽的結合可維持蛋白質或胜肽至少10%的原生物活性,較佳為維持蛋白質或胜肽至少25%的原生物活性,更佳為維持蛋白質或胜肽至少50%的原生物活性。本發明配方效能促進化合物與蛋白質或胜肽的結合可促進配方的物化穩定性,同時降低最初爆炸釋放。
在一較佳實施例中,配方效能促進化合物包括任何親水性聚合物。親水性聚合物為水溶性且可為一線性或分支聚合物,其典型代表例包括,但不限定於,聚乙二醇、聚丙二醇、聚乙烯吡咯烷酮(polyvinylpyrrolidone)、多醣、糖及其類似物。此聚合物的分子量較佳介於200~50,000道耳吞。本發明使用的親水性聚合物一般來說具有至少一可用於與蛋白質或胜肽共價結合的反應基團,例如胺基、羧基、硫氫基、磷酸鹽或羥基官能基。有關結合(conjugation)與聚乙二醇化(PEGylation)的各種方法揭露於美國專利4,179,337、5,446,090、5,880,255及6,113,906以及M.J. 在一較佳實施例中,配方效能促進化合物為一疏水性或親脂性分子。一般來說,親脂性分子於20℃時水中溶解度低於1wt%。適合的親脂性配方效能促進化合物較佳選自由C4~36烷基、C4~36烯基、C4~36烷二烯基、生育醇(tocopherol)與固醇殘基所組成的族群。此處,”C4~36烷基”、”C4~36烯基”及”C4~36烷二烯基”所指係包括直鏈與分支,較佳為直鏈、飽和、單未飽和及雙未飽和的碳氫化合物(4~36個碳原子)。親脂性分子較佳對細胞膜具有高親和性且可與例如白蛋白的血漿蛋白反應,以延長修飾的蛋白質或胜肽的體內半生期(相對於未修飾的蛋白質或胜肽而言)。親脂性配方效能促進化合物為一具有至少4碳原子的飽和或未飽和碳氫基或羧酸醯基自由基。羧酸醯基自由基可為己醯基、月桂酸基、棕櫚酸基、硬酯酸基、油酸基、花生酸基及二十二酸基。碳氫基自由基可為己基、十二烷基及十八烷基。
在另一實施例中,配方效能促進化合物包括任何兩性分子。”兩性”一詞代表任何同時具有親脂及親水特性的分子,均可溶於水及親脂性溶劑。本發明所使用的兩性分子包括親脂及親水基團。親脂基團較佳為如上所述的天然脂肪酸或烷鏈。親水基團則可選自由聚乙二醇、聚丙二醇、聚乙二醇-聚丙二醇共聚物、聚乙烯吡咯烷酮(polyvinylpyrrolidone)、多醣、糖與其類似物所組成的族群。親水基團較佳為具有至少1,000個乙二醇單元的聚乙二醇。本發明可藉由調整親脂基團及親水基團的大小、組成,得到一預期的兩性特性。
一配方效能促進化合物與一蛋白質或胜肽的共價結合與原蛋白質或胜肽相較可提升治療效果。一般來說,上述的結合係藉由一生物活性分子的一例如一胺基的官能基與一配方效能促進化合物的一酸基或其他反應基團經反應而形成。或者一蛋白質或胜肽與一配方效能促進化合物之間的結合係藉由一例如一架橋、聯接或鍵結基團的外加基團而形成,該等基團可為可降解或不可降解,其典型代表例揭露於日本專利1,254,699、美國專利5,693,609、世界專利WO95/07931、美國專利5750497及世界專利WO96/29342,以及Hashimoto,M.於Pharmaceutical Research

本發明一實施例中,以N-羥基琥珀醯亞胺(N-hydroxysuccinimide)活化棕櫚酸,之後,與octreotide的胺基進行反應,以藉由棕櫚酸親脂基團與胜肽之間的一醯胺鍵結形成octapeptide結合物。octreotide具有兩個一級胺基,可藉由調整反應條件使兩個胺基同時反應或僅有一個胺基進行選擇性反應。

在另一實施例中,lysozyme藉由本身6個胺基以不同比例與棕櫚酸結合。當棕櫚酸與lysozyme的比例小於6時,根據每一胺基的反應性,lysozyme上與棕櫚酸的結合位置會呈隨機態樣。

一”藥學上可接受鹽類”係指一蛋白質或胜肽其任何一或多個帶電基團與任何一或多個藥學上可接受且為無毒性的陽離子或陰離子之間所形成的一鹽類。有機與無機鹽類包括,例如由酸所製備的鹽類,包括例如由鹽酸、硫酸、磺酸、酒石酸、反丁烯二酸(fumaric)、氫溴酸、乙醇酸(glycolic)、檸檬酸、馬來酸、磷酸、琥珀酸、乙酸、硝酸、苯甲酸、抗壞血酸、對-甲苯磺酸、苯磺酸、萘磺酸、丙酸、碳酸與其類似物所製備的鹽類,或例如由胺、鈉、鉀、鈣或鎂所製備的鹽類。
根據本發明,組合物可選擇性地包含添加劑,以修飾組合物,達到蛋白質或胜肽預期的釋放效果。添加劑可調節釋放速率,穩定蛋白質或胜肽。適合的添加劑可為任何聚合物或非聚合物材料,包括生物可降解或非降解聚合物、碳水化合物或其衍生物、有機或無機化合物。添加劑可作為,例如抗氧化劑、酸鹼穩定劑、抗敏劑、分散劑、填充劑、結合劑及其類似物。

根據本發明,組合物可選擇性地包括還原劑、抗氧化劑及自由基清除劑,以穩定組合物。其包括,但不限定於,半胱胺酸、甲硫胺酸、d-α-生育酚醋酸鹽(d-α-tocopherol acetate)、dl-α-生育酚、抗壞血酸棕櫚酸酯(ascorbyl palmitate)、丁基羥基苯甲醚(butylated hydroxyanisole)、丁基羥基醌(butylated hydroxyquinone)、hydroxycomarin、丁基羥基甲苯、腦磷脂(cephalm)、五倍子酸乙酯(ethyl gallate)、五倍子酸丙酯、五倍子酸辛酯、五倍子酸月桂酯(lauryl gallate)、丙基羥基苯甲酸酯、三羥基苯丁酮(trihydroxybutyrophenone)、二甲基酚、二叔丁基酚(ditertbutylphenol)、維生素E與卵磷脂。
因此,本發明的組合物包括一疏水性、非聚合物載體材料,一可選擇的藥學上可接受有機溶劑,一蛋白質或胜肽,與一配方效能促進化合物共價結合,以及可選擇的一添加劑。
本發明施予體內的組合物提供一黏性膠體或固體儲庫,以用於蛋白質或胜肽的控制及持續釋放。根據本發明製備的組合物可控制持續釋放一預期時間周期。依據適當選擇的非聚合物載體材料及其他成分,可控制持續釋放的時間從數天至數周的預期時間周期。
在一較佳製備及施予本發明組合物的方法中,蛋白質或胜肽以一配方效能促進化合物進行共價修飾。結合物中蛋白質或胜肽與配方效能促進化合物的莫耳比會有所變動,例如根據蛋白質或胜肽的特性其變動範圍從1:1至1:10。經共價結合的蛋白質或胜肽可與疏水性、非聚合物載體材料、可選擇的藥學上可接受溶劑及其他可選擇的添加劑結合,以形成一單相配方,適合一合理時間周期的貨架儲存。
在另一較佳製備本發明組合物的方法中,可將組合物中共價結合的蛋白質或胜肽與剩餘成分分開包裝置於不同容器(注射器)。在施予一生物體之前,可將容器內成分與組合物的剩餘成分結合。
根據本發明,組合物較佳為一均勻溶液或均質懸浮液。於施予前,維持配方的均勻性是獲得一致性儲庫系統以控制生物活性物質釋放傳輸的關鍵。配方的均勻性必須維持至少1小時,以允許配方的重現性製備(reproducible preparation)及形成原位儲庫系統。此處的均勻性係藉由測量蛋白質或胜肽於配方上部及底部的比例或分布而決定,此配方置於一5毫升玻璃測試管中。若比例為1.0,此配方具有一理想均勻性。若比例低於1.0,表示發生相分離。較佳來說,組合物維持一0.9的均勻性至少1小時,更佳來說,組合物維持一0.9的均勻性至少24小時,最佳來說,組合物維持一0.9的均勻性至少7天。
根據本發明,組合物包括10~99.5wt%的疏水性、非聚合物載體材料,較佳介於25~95wt%(相對於組合物的總重量)。組合物亦包括一0~50wt%的藥學上可接受溶劑與一0.1~40wt%的蛋白質或胜肽。組合物更包括一1~25wt%的添加劑。
在一較佳實施例中,使用蔗糖乙酸異丁酯(SAIB)作為疏水性、非聚合物載體材料,選擇甲基吡咯烷酮(NMP)作為溶劑,以及選擇由octreotide與第一型類升糖素胜肽(GLP-1)所組成族群的胜肽或蛋白質。蛋白質或胜肽較佳與一兩性分子結合。而此得到的結合物可與蔗糖乙酸異丁酯(SAIB)/甲基吡咯烷酮(NMP)溶液結合,以形成一持續傳輸配方。
根據本發明,組合物可施予一生物體,其蛋白質或胜肽的持續釋放傳輸是符合預期的。此處,”生物體”一詞意指包括溫體動物,較佳為哺乳類,更佳為人類。
此處,”施予”一詞意指藉由任何適當途徑分配、傳輸或給予一組合物(藥物配方)至一生物體,以傳輸組合物至生物體內的預期位置。組合物可局部施予一生物體,藉由皮下、肌肉內、腹腔內或皮膚內注射,及藉由口服、經直腸、陰道或鼻腔施予,以根據蛋白質或胜肽治療不同疾病的已知參數,提供一蛋白質或胜肽的預期劑量。
此處,”控制、持續釋放傳輸”一詞包括,例如施予後一蛋白質或胜肽於體內經一時間周期的持續傳輸,較佳至少數天至數周或數月。蛋白質或胜肽的控制、持續釋放傳輸可加以驗證,例如藉由試劑隨時間的持續治療效果而驗證(以octreotide舉例,胜肽的持續傳輸可藉由對類胰島素生長因子(IGF-I)隨時間的持續抑制加以驗證),或者octreotide的持續傳輸可藉由偵測胜肽於體內隨時間的存在而驗證。
首先,溶解50毫克的octreotide醋酸鹽於1毫升包含100μL三乙基胺的無水二甲基亞碸(DMSO)。另溶解40.2毫克的棕櫚酸N-羥基琥珀醯亞胺酯(分子量353.50)於3毫升的無水二甲基亞碸(DMSO)並加入上述胜肽溶液,於室溫反應3小時。之後,將混合物倒入二乙基醚,以沈澱出棕櫚酸化的octreotide。沈澱物以二乙基醚清洗2次並於真空條件下進行烘乾,即可獲得一白色粉末的醯化胜肽。
首先,溶解50毫克的octreotide醋酸鹽於1,000μL包含100μL三乙基胺的無水二甲基亞碸(DMSO)。另溶解17.1毫克的棕櫚酸N-羥基琥珀醯亞胺酯(分子量353.50)於3毫升的無水二甲基亞碸(DMSO)並以直接注射法加入上述胜肽溶液,於室溫反應過夜。之後,將混合物倒入二乙基醚,以沈澱出棕櫚酸化的octreotide。沈澱物以二乙基醚清洗2次並於真空條件下進行烘乾,即可獲得一白色粉末的醯化胜肽。
首先,溶解50毫克的octreotide於2毫升、20mM以0.1M醋酸緩衝液(pH5)配製的氰基硼氫化鈉(NaCNBH3
,分子量62.84,2.51毫克)溶液。接著,以直接注射法加入13.7毫克的癸醛(分子量156.27)(OCT與DCL的比例為1:2)至上述胜肽溶液,於4℃反應過夜。混合物以離心法進行分離。之後,將沈澱的DCL-OCT進行冷凍乾燥。
首先,溶解302毫克的lysozyme(分子量14,500)於1,000μL包含200μL三乙基胺的無水二甲基亞碸(DMSO)。另溶解18.25毫克的棕櫚酸N-羥基琥珀醯亞胺酯(分子量353.50)於3毫升的無水二甲基亞碸(DMSO)並以直接注射法加入上述蛋白質溶液,於室溫反應過夜。之後,將混合物倒入二乙基醚,以沈澱出PAL-LYZ。待移除有機溶劑後,對最終產物進行冷凍乾燥。
首先,溶解50毫克的lysozyme(分子量14,500)於水中,將pH值調整為9.58,並對溶液進行冷凍乾燥。之後,將乾燥粉末溶於3毫升的二甲基亞碸(DMSO)。以直接注射法加入322μL、20mg/mL以無水二甲基亞碸(DMSO)配製的棕櫚酸N-羥基琥珀醯亞胺酯(分子量353.50)溶液至上述蛋白質溶液,於4℃反應過夜。之後,將混合物倒入二乙基醚,以沈澱出PAL-LYZ。待移除有機溶劑後,對最終產物進行冷凍乾燥。
首先,溶解50毫克的lysozyme(分子量14,500)於水中,將pH值調整為9.58,並對溶液進行冷凍乾燥。之後,將乾燥粉末溶於3毫升的二甲基亞碸(DMSO)。以直接注射法加入799μL、20mg/mL以無水二甲基亞碸(DMSO)配製的棕櫚酸N-羥基琥珀醯亞胺酯(分子量353.50)溶液至上述蛋白質溶液,於4℃反應過夜。之後,將混合物倒入二乙基醚,以沈澱出PAL-LYZ。待移除有機溶劑後,對最終產物進行冷凍乾燥。
將lysozyme加入以PBS(pH8)配製包含2%去氧膽酸(deoxycholate,DOC)的PAL-NHS溶液。之後,將混合物於37℃培養6小時。接著,藉由離心法移除混合物中未反應的PAL-NHS。產物以包含0.15%去氧膽酸(DOC)的PBS透析48小時(PAL-NHS與lysozyme的比例為15:1)。
首先,在減壓條件下,藉由蒸餾移除甲苯進行單棕櫚酸-聚乙二醇(平均分子量1124,5.0克,4.45mmole)與甲苯(75毫升)混合物的共沸乾燥。接著,將乾燥的單棕櫚酸-聚乙二醇溶於50毫升的無水甲苯,並加入一20wt%以THF(4.0毫升,6.6mmole)配製的第三丁氧基鉀溶液及0.96克的二乙基4-氯縮丁醛(5.3mmole,分子量180.67)。之後,將混合物置於一氫氣環境,於100~105℃攪拌過夜。待冷卻至室溫後,將混合物過濾並於0~5℃加入150毫升的乙醚。將所得的沈澱產物進行過濾並於減壓環境下乾燥之。
在一典型製備方法中,首先,溶解201.6毫克的單棕櫚酸-聚乙二醇-二乙基縮丁醛(PAL-PEG-BADA)於10毫升、0.1M的磷酸(pH2.1)。接著,將上述溶液於50℃加熱1小時後,冷卻至室溫。以1N的氫氧化鈉調整溶液的pH值至5.5。之後,將上述溶液加入一以3.5毫升、0.1M磷酸鈉緩衝液(pH5.5)配製含有195.3毫克octreotide的溶液。待1小時後,加入18.9毫克的氰基硼氫化鈉(NaCNBH3
),濃度為20mM。於室溫下,持續進行反應過夜。接著,將反應所得的混合物以一具有篩選2,000道耳吞(分子量)能力的膜進行透析或以一碳18管柱進行HPLC分析。純化的已結合octreotide為一單一化合物,其具有一一級胺(離胺酸)與一二級胺(N端)。
首先,溶解octreotide醋酸鹽(OCT-Ac)與octreotide結合物(PAL-PEG-BA-OCT)粉末於甲基吡咯烷酮(NMP)。之後,將上述包含各種octreotide型式的溶液與蔗糖乙酸異丁酯(SAIB)溶液(90wt%甲基吡咯烷酮(NMP))充分混合。所有配方中octreotide含量約為6%,如表一所示。
將上述製備的配方置於室溫(約22℃),並於預定時間點紀錄配方的外觀及收集表層與底部的組合物,對octreotide進行HPLC分析。藉由直接觀察或利用octreotide於表層及底部組合物中的比例可決定配方的物理穩定性。若比例為1.0,配方為均勻且穩定。若比例低於1.0,則表示發生相分離。
當混合octreotide醋酸鹽(OCT-Ac)與蔗糖乙酸異丁酯(SAIB)/甲基吡咯烷酮(NMP)溶液時,相分離立即發生,可觀察到厚實的固體沈澱物,得到一非均質配方。此厚實的凝集物隨時間沈澱,於底部形成一黃色黏著相。此非均質配方將阻塞注射針頭,不適合用於注射給藥。當混合PAL-PEG-BA-OCT與蔗糖乙酸異丁酯(SAIB)/甲基吡咯烷酮(NMP)溶液時,則得到一透明溶液,適合用於注射給藥。
第1圖為不同配方置於室溫24小時後的照片。包含octreotide醋酸鹽(OCT-Ac)的配方明顯產生相分離,而包含PAL-PEG-BA-OCT的配方則否。如表二所示,2天後,octreotide於底部相的濃度大約為上部相濃度的20倍,且5天後,其比例降至0.03以下。出人意料地,包含PAL-PEG-BA-OCT的配方,並沒有觀察到相分離。此外,亦發現包含octreotide醋酸鹽(OCT-Ac)的配方於底部相有較多的甲基吡咯烷酮(NMP)。
此外,亦發現octreotide在包含octreotide醋酸鹽(OCT-Ac)的配方中並不穩定。如表三所示,當成分混合時,octreotide的不純物立刻生成。於2小時後,大約4%的octreotide分解或反應,而表層中超過一半的octreotide及底部相中超過20%的octreotide於7天後分解。此即表示此系統不通合用於胜肽的持續傳輸。然而,不預期地發現,即使於室溫下放置3個月後,從包含PAL-PEG-BA-OCT的配方中僅偵測到極少量或甚至偵測不到octreotide的分解物。包含PAL-PEG-BA-OCT的配方不會產生相分離。
混合octreotide醋酸鹽(OCT-Ac)與octreotide結合物(PAL-PEG-BA-OCT)粉末與蔗糖乙酸異丁酯(SAIB)溶液(90wt%甲基吡咯烷酮(NMP)),以製備不同配方。每一配方中octreotide含量約為6%,如表四所示。
由於包含octreotide醋酸鹽(OCT-Ac)配方的物理不穩定性,於配方製備後,須立即進行體外釋放的試驗。此處,選擇懸浮液進行體外釋放試驗。將每一大約0.1毫升包含octreotide的配方注入置有3毫升釋放緩衝液(磷酸鹽緩衝液(PBS),pH7.4,包含0.1%疊氮鈉(sodium azide))的4毫升玻璃瓶中。將玻璃瓶培養於37℃並於不同時間點取樣。於每一時間點,移除2毫升的釋放培養基並加入2毫升的新鮮釋放培養基。將收集的樣品進行胜肽濃度的分析並利用YMC-Pack ODS-120A管柱進行高效液相層析(HPLC)。每一配方使用三份樣品。
如第2圖所示,從octreotide自包含octreotide醋酸鹽(OCT-Ac)的配方中釋放的情形顯示其具有一非常高的最初爆炸釋放。超過60%的octreotide於24小時內釋放,而超過90%的octreotide於2周後釋放。然而,從octreotide自包含PAL-PEG-BA-OCT的配方中釋放的情形顯示其並無大量的最初爆炸釋放。低於5%的octreotide於24小時內從包含PAL-PEG-BA-OCT的配方中釋放,之後,隨時間逐漸釋放。
首先,在室溫下,溶解100毫克的顆粒細胞-巨噬細胞群落刺激因子(GM-CSF)於10毫升的磷酸緩衝液(pH7.5)。之後,加入100毫克的三氟代乙烷磺醯(tresyl)-單棕櫚酸-聚乙二醇(分子量5,000道耳吞),攪拌1小時。接著,利用凝膠層析法將未反應的顆粒細胞-巨噬細胞群落刺激因子(GM-CSF)、聚乙二醇化的顆粒細胞-巨噬細胞群落刺激因子(GM-CSF)片段與未反應的三氟代乙烷磺醯-單棕櫚酸-聚乙二醇予以分離。之後,以100mM的Tris緩衝液對聚乙二醇化的顆粒細胞-巨噬細胞群落刺激因子(GM-CSF)進行透析,並將其濃度調整至50mg/mL。
首先,溶解116毫克的人類重组胰島素於4毫升包含200μL三乙基胺的無水二甲基亞碸(DMSO)。另溶解1克的mPEG(5000)-SPA於10毫升的無水二甲基亞碸(DMSO)並以直接注射法加入上述胰島素溶液,於室溫下反應過夜(超過10小時)或待90%以上的蛋白質完成聚乙二醇化。以乙醚沈澱兩次,以分離未反應的聚乙二醇及聚乙二醇化的胰島素。產物以反相HPLC進行分析。
首先,將5毫升以20mM硼酸鹽緩衝液(pH9.0)製備的lysozyme溶液(0.4%)冷卻至4℃。之後,將209毫克的mPEG-SS(分子量2,000,11mole以上)緩慢加入上述蛋白質溶液,並以一端對端旋轉(end to end rotation)方式於4℃反應過夜。加入10莫耳以上的甘胺酸,以終止反應。反應所得的混合物利用3,500道耳吞孔洞大小的膜與去離子水進行透析。透析樣品在不添加任何添加劑的情況下於水中進行凍乾(lyophilized)並保存於-20℃。
雖然本發明已以較佳實施例揭露如上,然其並非用以限定本發明,任何熟習此項技藝者,在不脫離本發明之精神和範圍內,當可作更動與潤飾,因此本發明之保護範圍當視後附之申請專利範圍所界定者為準。
第1圖為不同配方置於室溫24小時後之照片。
第2圖為octreotide自包含octreotide醋酸鹽(OCT-Ac)與PAL-PEG-BA-OCT的蔗糖乙酸異丁酯(SAIB)/甲基吡咯烷酮(NMP)配方中體外釋放的情形。
Claims (15)
- 一種用於蛋白質或胜肽持續釋放之組合物,包括:一疏水性、非聚合物載體材料;一藥學上可接受溶劑,該溶劑與水互溶;以及一蛋白質或胜肽,與一或多個兩性分子共價結合,該兩性分子具有下列一般結構:L-S-(OC2 H4 )m OH 式1其中L為親脂基團,係選自由C4~36烷基、C4~36烯基、C4~36烷二烯基、生育醇(tocopherol)與固醇殘基所組成之族群,其中S為一鍵結,係選自由一酯基、醯胺基、二級或三級胺基、胺基甲酸酯基(carbamate group)、磺酸酯基、硫酸酯基、磷酸酯基、亞磷酸酯基與乙醚基所組成之族群,以及其中m介於1~1,000,其中該蛋白質或胜肽與該一或多個兩性分子共價結合以避免相分離,維持該組合物之穩定性。
- 如申請專利範圍第1項所述之用於蛋白質或胜肽持續釋放之組合物,其中該疏水性、非聚合物載體材料具有低結晶性與非極性特性。
- 如申請專利範圍第1項所述之用於蛋白質或胜肽持續釋放之組合物,其中該疏水性、非聚合物載體材料為一液體,其黏度於37℃至少達5,000cp且於環境或生理條件下不產生均勻結晶。
- 如申請專利範圍第1項所述之用於蛋白質或胜肽持續釋放之組合物,其中該疏水性、非聚合物載體材料包括 一或多個非聚合物酯類或混合酯類。
- 如申請專利範圍第1項所述之用於蛋白質或胜肽持續釋放之組合物,其中該疏水性、非聚合物載體材料係形成自具有至少20個羥基之聚醇,其與羧酸進行酯化。
- 如申請專利範圍第5項所述之用於蛋白質或胜肽持續釋放之組合物,其中該羧酸包括具有2個碳以上之有機酸。
- 如申請專利範圍第1項所述之用於蛋白質或胜肽持續釋放之組合物,其中該疏水性、非聚合物載體材料為蔗糖乙酸異丁酯(sucrose acetate isobutyrate,SAIB)。
- 如申請專利範圍第1項所述之用於蛋白質或胜肽持續釋放之組合物,其中該藥學上可接受溶劑於25℃與水之互溶性至少為1wt%。
- 如申請專利範圍第1項所述之用於蛋白質或胜肽持續釋放之組合物,其中該藥學上可接受溶劑係選自由苯甲醇、己內醯胺(caprolactam)、己內酯(caprolactone)、二甲基亞碸(DMSO)、乙醇、乳酸乙酯、丙三醇(甘油)、甘油縮甲醛(glycerol formal)、四氫呋喃聚乙二醇醚(glycofurol)、甲基吡咯烷酮(N-methyl-2-pyrrolidone,NMP)、聚乙二醇、PEG-300、PEG-400、烷氧基聚乙二醇、mPEG-350、碳酸丙烯酯(propylene carbonate)、甘油三乙酸酯(triacetin)、檸檬酸三乙酯(triethyl citrate)與其混合物所組成之族群。
- 如申請專利範圍第1項所述之用於蛋白質或胜肽 持續釋放之組合物,其中該蛋白質或胜肽係選自由催產素(oxytocin)、血管升壓素(vasopressin)、促腎上腺皮質激素(adrenocorticotropic hormone,ACTH)、表皮生長因子(epidermal growth factor,EGF)、血小板衍生性生長因子(platelet-derived growth factor,PDGF)、泌乳激素(prolactin)、促黄體素(luteinising hormone)、促黄體素釋放素(luteinising hormone releasing hormone,LHRH)、促黄體素釋放素促進劑、促黄體素釋放素拮抗劑、生長激素(包括人、豬、牛)、生長激素釋放因子、胰島素、類胰島素生長因子(IGF-I,IGF-II)、紅血球生成素(包括具有紅血球生成活性之蛋白質)、體抑素(somatostatin)、升糖素(glucagon)、介白素(interleukin)、干擾素-α、干擾素-β、干擾素-γ、胃泌素(gastrin)、四肽胃泌素(tetragastrin)、五肽胃泌素(pentagastrin)、尿抑胃素(urogastrone)、胰泌素(secretin)、抑鈣激素(calcitonin)、腦啡肽(enkephalins)、腦內啡(endorphins)、血管收縮素(angiotensins)、促甲狀腺釋放素(thyrotropin releasing hormone,TRH)、腫瘤壞死因子(tumor necrosis factor,TNF)、副甲狀腺素(parathyroid hormone)、神經生長因子、顆粒細胞群落刺激因子(granulocyte-colony stimulating factor,G-CSF)、顆粒細胞-巨噬細胞群落刺激因子(granulocyte macrophage-colony stimulating factor,GM-CSF)、巨噬細胞群落刺激因子(macrophage-colony stimulating factor,M-CSF)、肝素酵素(heparinase)、血管內皮生長因子(vascular endothelial growth factor,VEGF)、骨成型蛋白(bone morphogenetic protein,BMP)、心鈉素(human atrial natriuretic peptide,HANP)、第一型類升糖素胜肽(glucagon-like peptide-1,GLP-1)、exenatide(exendin-3,exendin-4)、胜肽YY(PYY)、腎激素(renin)、缓激肽(bradykinin)、崔西桿菌素(bacitracins)、多黏桿菌素(polymyxins)、黏菌素(colistins)、短桿菌酪素(tyrocidine)、短桿菌素(gramicidins)、環孢菌素(cyclosporins)(包括合成類似物及其藥理活性片段)、酵素、細胞激素、抗體、疫苗、醣蛋白、濾泡刺激素(follicle stimulating hormone)、京都啡肽(kyotorphin)、促吞噬激素(taftsin)、促胸腺生成素(thymopoietin)、胸腺肽(thymosin)、胸腺素(thymostimulin)、胸腺體液因子(thymic humoral factor)、血清胸腺因子(serum thymic factor)、群落刺激因子(colony stimulating factors)、絨毛素(motilin)、蛙皮素(bombesin)、強啡肽(dynorphin)、神經加壓素(neurotensin)、雨蛙素(cerulein)、尿激素(urokinase)、激肽釋放酵素(kallikrein)、P物質類似物及拮抗劑、第二型血管收縮素(angiotensin II)、第七及第九凝血因子(blood coagulation factor VII and IX)、溶菌素(lysozyme)、黑色素細胞刺激素(melanocytes stimulating hormone)、甲狀腺釋放素、甲狀腺刺激素 (thyroid stimulating hormone)、催胰泌素(pancreozymin)、膽囊收縮素(cholecystokinin)、人類胎盤泌乳素(human placenta lactogen)、血小板生成素(thrombopoietin)、人類絨毛膜性腺激素(human chorionic gonadotropin)、蛋白合成刺激肽、抑胃胜肽(gastric inhibitory peptide)與血管活性腸肽(vasoactive intestinal peptide)所組成之族群。
- 如申請專利範圍第1項所述之用於蛋白質或胜肽持續釋放之組合物,其中該兩性分子之親脂基團包括月桂酸基、棕櫚酸基、硬酯酸基、油酸基、花生酸基及二十二酸基。
- 如申請專利範圍第1項所述之用於蛋白質或胜肽持續釋放之組合物,其中該蛋白質或胜肽與一或多個兩性分子共價結合,包括一直鏈聚乙二醇基團與一親脂基團,其中該蛋白質或胜肽、該聚乙二醇與該親脂基團經排列形成該親脂基團位於最外圍之構形,以與親脂環境或細胞膜產生相互作用。
- 如申請專利範圍第1項所述之用於蛋白質或胜肽持續釋放之組合物,其中該蛋白質或胜肽與該兩性分子之莫耳比係根據該蛋白質或胜肽之特性由1:1變動至1:10。
- 如申請專利範圍第1項所述之用於蛋白質或胜肽持續釋放之組合物,其中該疏水性、非聚合物載體材料為蔗糖乙酸異丁酯(SAIB),該藥學上可接受溶劑為甲基吡咯烷酮(NMP),該蛋白質或胜肽與一兩性分子結合。
- 如申請專利範圍第14項所述之用於蛋白質或胜肽持續釋放之組合物,其中該蛋白質或胜肽係選自由octreotide(奧曲肽)與第一型類升糖素胜肽(GLP-1)所組成之族群。
