1. Мочеточниковый катетер, содержащий:
дренажный просвет, содержащий проксимальную часть, выполненную с возможностью продолжения за пределы тела пациента за счет перехода через мочеиспускательный канал пациента, и дистальную часть, выполненную с возможностью расположения в почке, почечной лоханке и/или в мочеточнике пациента рядом с почечной лоханкой, причем дистальная часть содержит спиральную удерживающую часть, при этом спиральная удерживающая часть содержит по меньшей мере первый виток, имеющий первый диаметр,
по меньшей мере второй виток, имеющий второй диаметр, причем первый диаметр меньше, чем второй диаметр, при этом второй виток находится ближе к концу дистальной части дренажного просвета, чем первый виток; и
одно или более отверстий в боковой стенке спиральной удерживающей части дистальной части дренажного просвета для обеспечения прохождения жидкости в дренажный просвет,
при этом проксимальная часть дренажного просвета по существу не содержит или не содержит отверстий,
причем перед введением в мочевыводящие пути пациента часть дренажного просвета проксимальнее удерживающей части образует прямую или криволинейную центральную ось, и при этом при развертывании первый виток и второй виток удерживающей части проходят вокруг оси удерживающей части, которая по меньшей мере частично совпадает с прямой или криволинейной центральной осью части дренажного просвета.
2. Мочеточниковый катетер по п. 1, в котором ось удерживающей части изогнута относительно центральной оси дренажного просвета.
3. Мочеточниковый катетер по любому из пп. 1-2, в котором катетер выполнен с возможностью перехода между сложенной конфигурацией для введения в мочеточник пациента и развернутой конфигурацией для развертывания внутри мочеточника.
4. Мочеточниковый катетер по любому из пп. 1-3, в котором удерживающая часть дополнительно содержит по меньшей мере третий виток, проходящий вокруг оси удерживающей части, при этом третий виток имеет диаметр, больший или равный либо диаметру первого витка, либо диаметру второго витка, при этом третий виток находится ближе к концу дистальной части дренажного просвета, чем второй виток.
5. Мочеточниковый катетер по пп. 1-4, в котором удерживающая часть дренажного просвета содержит боковую стенку, содержащую обращенную радиально внутрь сторону и обращенную радиально наружу сторону, и при этом общая площадь поверхности для отверстий на обращенной радиально внутрь стороне больше, чем общая площадь поверхности отверстий на обращенной радиально наружу стороне.
6. Мочеточниковый катетер по пп. 1-5, в котором удерживающая часть дренажного просвета содержит боковую стенку, содержащую обращенную радиально внутрь сторону и обращенную радиально наружу сторону, и при этом отверстия расположены на обращенной радиально внутрь стороне, и при этом обращенная радиально наружу сторона трубки по существу не содержит отверстий.
7. Мочеточниковый катетер по пп. 1-6, в котором каждое из одного или более отверстий имеет диаметр от приблизительно 0,7 до приблизительно 0,9 мм.
8. Мочеточниковый катетер по пп. 1-7, в котором удерживающая часть дренажного просвета дополнительно содержит открытый дистальный конец, обеспечивающий протекание жидкости в дренажный просвет.
9. Мочеточниковый катетер по пп. 1-8, в котором первый диаметр первого витка составляет приблизительно от 8 мм до 10 мм, а второй диаметр второго витка составляет приблизительно от 16 мм до 20 мм.
10. Мочеточниковый катетер по пп. 1-9, в котором первый виток содержит боковую стенку, содержащую обращенную радиально внутрь сторону и обращенную радиально наружу сторону, и при этом обращенная радиально внутрь сторона первого витка содержит одно или более отверстий, обеспечивающих протекание жидкости в дренажный просвет.
11. Мочеточниковый катетер по пп. 1-10, в котором первый виток содержит боковую стенку, содержащую обращенную радиально внутрь сторону и обращенную радиально наружу сторону, и при этом обращенная радиально внутрь сторона первого витка содержит по меньшей мере два отверстия, обеспечивающих протекание жидкости в дренажный просвет.
12. Мочеточниковый катетер по пп. 1-11, в котором первый виток содержит боковую стенку, содержащую обращенную радиально внутрь сторону и обращенную радиально наружу сторону, и при этом обращенная радиально наружу сторона первого витка по существу не содержит или не содержит одно или более отверстий.
13. Мочеточниковый катетер по пп. 1-12, в котором первый виток содержит боковую стенку, содержащую обращенную радиально внутрь сторону и обращенную радиально наружу сторону, и при этом обращенная радиально внутрь сторона первого витка содержит одно или более отверстий, обеспечивающих протекание жидкости в дренажный просвет, а обращенная радиально наружу сторона по существу не содержит или не содержит одно или более отверстий.
14. Мочеточниковый катетер по пп. 1-13, в котором одно или более отверстий на боковой стенке дренажного просвета обеспечивают протекание жидкости за счет отрицательного давления в дренажный просвет.
15. Мочеточниковый катетер по пп. 1-14, в котором диаметр витков увеличивается к дистальному концу дренажного просвета, приводя к спиральной конструкции, имеющей сужающуюся или частично сужающуюся конфигурацию.
16. Мочеточниковый катетер, содержащий:
часть с дренажным просветом, содержащую проксимальный конец, дистальный конец, выполненный с возможностью продолжения за пределы тела пациента за счет прохождения через мочеиспускательный канал пациента, и дистальную часть, выполненную с возможностью расположения в мочеточнике и/или почке пациента, и удерживающую часть, продолжающуюся радиально наружу от участка дистального конца части с дренажным просветом, при этом удерживающая часть содержит проксимальный конец, имеющий первый диаметр, дистальный конец, имеющий второй диаметр, и стенку и/или поверхность, продолжающуюся между ними, при этом удерживающая часть выполнена с возможностью развертывания в мочеточнике и/или почке пациента таким образом, чтобы второй диаметр был больше, чем первый диаметр.
17. Мочеточниковый катетер по п.16, в котором удерживающая часть содержит:
расширяемый элемент или баллон в жидкостном сообщении с просветом для раздувания, продолжающимся вдоль части с дренажным просветом; или
спиральную трубку, продолжающуюся от дистального конца части с дренажным просветом, причем трубка образует просвет в жидкостном сообщении с дренажным просветом, образованным частью с дренажным просветом; или
клиновидное или воронкообразное расширение, образованное из сжимаемого и/или пористого материала; или
сужающуюся внутреннюю поверхность, выполненную с возможностью направления жидкости к дренажному просвету, образованному частью с дренажным просветом; или
при этом удерживающая часть выполнена за одно целое с частью с дренажным просветом; или
при этом спиральная трубка содержит отверстия, проходящие через боковую стенку трубки, обеспечивая прием жидкости внутри просвета, или при этом отверстия расположены на обращенной радиально внутрь части трубки, и при этом противоположная обращенная радиально наружу часть трубки по существу не содержит отверстий или не содержит отверстий; или
при этом часть с дренажным просветом и удерживающая часть по меньшей мере частично образованы из одного или более из меди, серебра, золота, никель-титанового сплава, нержавеющей стали, титана, полиуретана, поливинилхлорида, политетрафторэтилена (PTFE), латекса и силикона; или
при этом дренажный просвет катетера выполнен с возможностью нагнетания отрицательного давления для сбора жидкости из мочеточника и/или почки.
18. Система создания отрицательного давления в части мочевыводящих путей пациента, содержащая:
мочеточниковый катетер, содержащий:
часть с дренажным просветом, содержащую проксимальный конец, дистальный конец, выполненный с возможностью расположения в мочеточнике и/или почке пациента, и удерживающую часть, продолжающуюся радиально наружу от участка дистального конца части с дренажным просветом и выполненную с возможностью перехода в развернутое положение, в котором диаметр удерживающей части больше, чем диаметр части с дренажным просветом, при этом по меньшей мере одна из части с дренажным просветом или удерживающей части содержит по меньшей мере один дренажный порт, обеспечивающий протекание жидкости в дренажный просвет; и
насос в жидкостном сообщении с дренажным просветом, образованным частью с дренажным просветом мочеточникового катетера, при этом насос выполнен с возможностью создания отрицательного давления в части мочевыводящих путей пациента для вытягивания жидкости через дренажный просвет мочеточникового катетера.
19. Система по п. 18, в которой удерживающая часть содержит проксимальный конец, имеющий первый диаметр, дистальный конец, имеющий второй диаметр, и стенку и/или поверхность, продолжающуюся между ними, при этом удерживающая часть выполнена с возможностью развертывания в мочеточнике или почке пациента таким образом, чтобы второй диаметр был больше, чем первый диаметр, или
при этом насос обеспечивает чувствительность 10 мм рт.ст. или менее, или насос выполнен с возможностью обеспечения прерывистого отрицательного давления; или
при этом система дополнительно содержит:
один или более датчиков в жидкостном сообщении с дренажным просветом, причем один или более датчиков выполнены с возможностью определения информации, включая по меньшей мере одно из емкостного сопротивления, концентрации аналитов и температуры мочи внутри соответствующего дренажного просвета; и
контроллер, содержащий машиночитаемую память, содержащую команды программы, которые при выполнении заставляют контроллер:
получать информацию от одного или более датчиков и регулировать рабочий параметр насоса по меньшей мере частично на основании информации, полученной от одного или более датчиков, для увеличения или уменьшения вакууметрического давления в дренажном просвете по меньшей мере одного мочеточникового катетера для регулирования протекания мочи через дренажный просвет.
