Abstract
Способ обезболивания после грыжесечения паховым доступом при паховых грыжах, включающий введение местного анестетика через предварительно установленный мультиперфорированный катетер в область хирургического вмешательства, отличающийся тем, что введение анестетика осуществляют подапоневротически в паховый канал, для чего после основного этапа операции - герниопластики, перед ушиванием апоневроза наружной косой мышцы живота, выполняют прокол на 1 см медиальнее и на 1 см выше передне-верхней подвздошной ости через подкожно-жировую клетчатку и апоневроз наружной косой мышцы живота в паховый канал, через прокол под пальцевым и визуальным контролем вводят катетер так, чтобы его дистальный конец находился у лонного бугорка, катетер располагают под апоневрозом наружной косой мышцы живота в паховом канале выше и параллельно семенному канатику у мужчин или круглой связке матки у женщин так, чтобы все боковые отверстия катетера находились в подапоневротическом пространстве, местный анестетик вводят через катетер в первые 48 ч послеоперационного периода непрерывно капельно со скоростью 2-4 мл/ч или болюсно по 10 мл каждые 4-6 ч, по истечению 48 ч катетер удаляют.Method of anesthesia after hernia by inguinal access with inguinal hernias, including the introduction of local anesthetic through a pre-installed multiperforated catheter into the surgical area, characterized in that the anesthetic is administered subaponeurotically into the inguinal canal, for which, after the main stage of the operation, hernioplasty is performed before suturing the external braid aponeurosis abdominal muscles, perform a puncture 1 cm medial and 1 cm higher than the anteroposterior iliac spine through subcutaneous fat glue the hand tissue and aponeurosis of the external oblique muscle of the abdomen in the inguinal canal, a catheter is inserted through the puncture under the finger and visual control so that its distal end is located at the pubic tubercle, the catheter is placed under the aponeurosis of the external oblique muscle of the abdomen in the inguinal canal above and parallel to the spermatic cord in men or round ligament of the uterus in women so that all the lateral openings of the catheter are in the subaponeurotic space, the local anesthetic is introduced through the catheter in the first 48 hours of the postoperative period, continuously drip at a rate of 2-4 ml / h or bolus of 10 ml every 4-6 hours, after 48 hours the catheter is removed.
