Description
Claims (21)
- 1.カニューレを有しており、該カニューレが、これを通して患者の総胆管内に 輸液すべきX線不透過性造影剤の源に連結される手元側端部と、前記X線不透過 性造影剤を輸液できるように患者の縦胆管内に導入される先端側端部とを備えて おり、 前記カニューレが、胆管内への前記先端側端部の挿入を促進するための、ばねワ イヤガイドをカテーテルを通して導入及び挿通する第1管腔手段と、前記手元側 端部と先端側端部との間で造影剤を輸液するための第2管腔手段とを備えている ことを特徴とする胆管カテーテル。
- 2.前記カテーテルの先端側端部を内視鏡により視ることができるようにするた めの、前記先端側端部に設けられた手段を更に有していることを特徴とする請求 の範囲第1項に記載の胆管カテーテル。
- 3.前記先端側端部に設けられた手段が、カテーテルの先端側端部の外面に付さ れた複数のストライプからなることを特徴とする請求の範囲第2項に記載の胆管 カテーテル。
- 4.前記ストライプがデジタル指標からなることを特徴とする請求の範囲第3項 に記載の胆管カテーテル。
- 5.前記カテーテルが約200cmの長さを有していることを特徴とする請求の 範囲第1項に記載の胆管カテーテル。
- 6.前記カテーテルがポリウレタンで形成されていることを特徴とする請求の範 囲第1項に記載の胆管カテーテル。
- 7.前記カテーテルが、ナイロン、ペバックス及び硫酸バリウムからなる材料で 形成されていることを特徴とする請求の範囲第1項に記載の胆管カテーテル。
- 8.前記先端側端部がテーパ伏であることを特徴とする請求の範囲第1項に記載 の胆管カテーテル。
- 9.前記ばねワイヤガイドが、カテーテルの少なくとも2倍の長さを有している ことを特徴とする請求の範囲第1項に記載の胆管カテーテル。
- 10.前記ポリウレタンが少なくとも約60Dのデュロメータ硬度を有している ことを特徴とする請求の範囲第6項に記載の胆管カテーテル。
- 11.前記ポリウレタンが親水性コーティングで処理されており、該親水性コー ティングが胆汁の存在により活性化され、カテーテルの外面に潤滑性を付与し且 つカテーテルの柔軟性及びキンク抵抗性を増大させることを特徴とする請求の範 囲第6項に記載の二重管腔胆管カテーテル。
- 12.実質的に円筒状の側壁と、造影剤の源に連結される手元側端部と、患者の 胆管内に導入される先端側端部とを備えたチューブを有しており、該チューブが 前記手元側端部と先端側端部との間に延びた第1管腔チャンネルを備えており、 該第1管腔チャンネルが前記造影剤の源から胆管まで造影剤を輸液でき、 前記チューブが更に、前記ばねワイヤガイド上での前記胆管カテーテルの挿入及 び挿通を容易にするための、前記手元側端部と先端側端部との間に延びた第2管 腔チャンネルを備えていることを特徴とする胆管カテーテル。
- 13.前記ポリウレタンが、前記カテーテルの外面に潤滑性を付与し且つカテー テルの柔軟性及びキンク抵抗性を増大させる親水性コーティングで処理されてい ることを特徴とする請求の範囲第12項に記載の二重管腔胆管カテーテル。
- 14.実質的に円筒状の側壁をもつチューブを有しており、該チューブが手元側 端部と患者の総胆管内に導入される先端側端部とを備えており、前記チューブが 前記手元側端部と先端側端部との間に延びた三日月状の第1管腔チャンネルを備 えており、該第1管腔チャンネルが前記手元側端部から先端側端部までX線不透 過性造影剤を輸液し、 前記チューブが更に、前記胆管カテーテル内へのばねワイヤガイドの挿入及び挿 通を容易にするための、前記手元側端部と先端側端部との間に延びた円形の第2 管腔チャンネルを備えていることを特徴とする二重管腔胆管カテーテル。
- 15.約60Dのデュロメータ硬度をもつポリウレタンで形成され且つ胆汁に曝 されると活性化されてキンク抵抗性、潤滑性及び柔軟性を付与する親水性コーテ ィングが被覆されたチューブを有しており、該チューブが、実質的に円筒状の側 壁をもつチューブと、造影剤の源に連結される手元側端部と、患者の胆管内に導 入される先端側端部とを備えており、前記チューブが前記手元側端部と先端側端 部との間に延びた三日月状の第1管腔を備えており、前記第1管腔チャンネルが 前記造影剤の源から胆管まで造影剤を輸液し、 前記チューブが更に、前記二重管腔胆管カテーテル内へのばねワイヤガイドの挿 入及び挿通を容易にするための、前記手元側端部と先端側端部との間に延びた円 形の第2管腔チャンネルを備えていることを特徴とする胆管カテーテル。
- 16.前記円形の第2管腔チャンネルが前記親水性コーティングで被覆されてい ることを特徴とする請求の範囲第15項に記載の胆管カテーテル。
- 17.カテーテル及びX線不透過性媒体を胆管内に入れるための方法であって、 ばねワイヤを内視鏡を通して胆管内に挿入し、前記ばねワイヤガイド上の胆管カ テーテルを前記胆管内に押通し、胆管をX線透視できるように、X線不透過性媒 体を、前記胆管カテーテルを通して胆管内に輸液することからなることを特徴と する方法。
- 18.前記ばねワイヤガイドが前記胆管カテーテルの2倍よりも大きな長さを有 することを特徴とする請求の範囲第17項に記載の方法。
- 19.胆石の周囲に胆管ステントを配置するための方法であって、ばねワイヤを 、内視鏡を通して、胆石を含んだ胆管内に挿入し、前記ばねワイヤガイド上の胆 管カテーテルを、前記胆石を含む胆管内に挿通し、前記ばねワイヤガイドが前記 胆管カテーテルの2倍より大きい長さを有し、胆石をX線透視できるように、X 線不透過性媒体を、前記胆管カテーテルを通して胆石に近い胆管内に輸液し、 前記胆管カテーテルを内視鏡から取り出し且つばねワイヤガイドから胆管カテー テルを引き抜き、前記ばねワイヤガイドの長さは、該ばねワイヤガイドの位置を 胆管内に維持したまま前記胆管カテーテルをばねワイヤガイドから完全に引き抜 くことができる長さであり、 前記ステントが前記X線透視可能な胆石の周囲に配置されるように、ガイディン グカテーテルを用いて、ばねワイヤガイド上の胆管ステントを胆管内に挿通する ことからなることを特徴とする方法。
- 20.膵管閉塞部の周囲にステントを配置するための方法であって、ばねワイヤ を、内視鏡を通して、膵管閉塞部を含んだ膵管内に挿入し、前記ばねワイヤガイ ド上のカテーテルを、前記閉塞部を含む膵管内に挿通し、前記ばねワイヤガイド が前記カテーテルの2倍よりも大きな長さを有し、閉塞部をX線透視できるよう に、X線不透過性媒体を、前記カテーテルを通して閉塞部に近い弊管内に輸液し 、 前記カテーテルを内視鏡から取り出し且つばねワイヤガイドからカテーテルを引 き抜き、前記ばねワイヤガイドの長さは、該ばねワイヤガイドの位置を膵管内に 維持したまま前記カテーテルをばねワイヤガイドから完全に引き抜くことができ る長さであり、 前記ステントが前記X線透視可能な閉塞部の周囲に配置されるように、ガイディ ングカテーテルを用いて、ばねワイヤガイド上のステントを膵管内に挿通するこ とからなることを特徴とする方法。
- 21.胆嚢管閉塞部の周囲にステントを配電するための方法であって、ばねワイ ヤを、内視鏡を通して、胆嚢管閉塞部を含んだ胆嚢管内に挿入し、前記ばねワイ ヤガイド上のカテーテルを、前記閉塞部を含む胆嚢管内に挿通し、前記ばねワイ ヤガイドが前記カテーテルの2倍よりも大きな長さを有し、閉塞部をX線透視で きるように、X線不透過性媒体を、前記カテーテルを通して閉塞部に近い胆嚢管 内に輸液し、 前記カテーテルを内視鏡から取り出し且つばねワイヤガイドからカテーテルを引 き抜き、前記ばねワイヤガイドの長さは、該ばねワイヤガイドの位置を胆嚢管内 に維持したまま前記カテーテルをばねワイヤガイドから完全に引き抜くことがで きる長さであり、 前記ステントが前記X線透視可能な閉塞部の周囲に配置されるように、ガイディ ングカテーテルを用いて、ばねワイヤガイド上のステントを胆嚢管内に挿通する ことからなることを特徴とする方法。
