Description

本開示は、一般に、人工弁、より具体的には、マルチパートフレーム人工弁デバイス、システム及び方法に関する。
天然弁の機能及び性能を模倣しようとする生体弁が開発されている。生体弁は、合成材料、生体組織などの天然組織、又は、合成材料と天然組織の組み合わせから形成することができる。
弁尖フレームサブコンポーネント及びアンカーフレームサブコンポーネントを含むマルチフレーム人工弁に関連する様々な本発明の概念は開示され、弁尖フレームサブコンポーネントは、患者の解剖学的構造と直接結合せず、ギャップ又は空間によってアンカーフレームサブコンポーネントから分離されている。任意選択的な接続シースを提供して、アンカーフレームサブコンポーネント及び弁尖フレームサブコンポーネントを相互接続し、非ネスト化薄型プロファイル構成での人工弁のデリバリー及びネスト化展開構成への移行を支援することができる。アンカーフレームサブコンポーネントと弁尖フレームサブコンポーネントとの間のギャップ又は空間をサブコンポーネント流入端及び/又は流出端などで、橋渡しし及び覆うために、1つ以上ブリッジング部材又は環状溝カバーが提供される。このようなブリッジング/カバー特徴部は、人工弁に出入りするスムーズなフロープロファイルを提供し、塞栓に関連する合併症の発生率を減らし、人工弁デリバリー中の灌流を促進し、デリバリー中のサブコンポーネントのテレスコープ式ネスト化を支援し、デリバリー後のサブコンポーネントの相対的な配置を支援し、又は、あらゆる様々な追加の機能及び利点を提供することができる。関連する人工弁デバイス、デリバリーシステム、デリバリー方法及び組み立て方法はすべて、本明細書で考えられ、扱われる。

定義及び用語

様々な例において、弁尖サブコンポーネント1228の1つ以上の弁尖1230は、生体適合性の合成材料(例えば、ePTFE及びePTFE複合材料、又は所望に応じて他の材料を含む)から形成される。他の例において、弁尖1230は、ウシ組織、ブタ組織などを含む再利用組織などの天然材料から形成されている。
図1及び図2A~2Dは、アンカーフレームサブコンポーネント1100と弁尖フレームサブコンポーネント1200との間に延在する接続シース1300を示し、弁尖フレームサブコンポーネントはネスト化されておらず、又は、アンカーフレームサブコンポーネント1100からオフセットされている。様々な例において、接続シース1300は、弁尖フレームサブコンポーネント流入端1202で弁尖フレームサブコンポーネント1200の周囲に、そして、アンカーフレームサブコンポーネント流出端1104でアンカーフレームサブコンポーネント1100の周囲に結合された可撓性管状膜であり、弁尖フレームサブコンポーネント1200をアンカーフレームサブコンポーネント1100に結合するように動作可能である。接続シース1300は、薄くて可撓性であり、デリバリー構成において、より小さな直径に折り畳むか、又は弾性的に収縮するように動作可能である。

図12は、デリバリーデバイス1600のデリバリーカテーテル1604に搭載された人工弁1000を示している。示されているように、デリバリーカテーテルは、人工弁1000の様々なサブコンポーネントを所望の比較的長手方向の位置及び所望の直径で維持するための複数の閉じ込め要素又は拘束要素1716(例えば、繊維又はテザー)を含む。デリバリーカテーテル1604は、本明細書に記載のデリバリーシーケンスを容易にするように構成されることができ、拘束要素1716のうちの1つ以上は、所望のシーケンスで解放可能である。デリバリーデバイス1600は、後続のセクションでさらに詳細に記載されるように、追加の特徴部(例えば、デリバリーシース)を含むことができる。

- 前記1つ以上のブリッジング部材の少なくとも一部は組織の内部成長のために構成され、及び/又は、 前記1つ以上のブリッジング部材の少なくとも一部は組織の内部成長に抵抗するように構成されている、請求項39~52のいずれか1項記載の人工弁。
- 前記弁尖フレームサブコンポーネントを前記アンカーフレームサブコンポーネントに結合する接続シースをさらに含み、その結果、前記弁尖フレームサブコンポーネントが前記アンカーフレームサブコンポーネントと非ネスト構成からネスト化構成へ移行する際に、前記接続シースは裏返される、請求項39~53のいずれか1項記載の人工弁。

- 前記接続シースは、前記接続シースの壁において流れ可能特徴部を含み、前記壁は、接続シース流入端と接続シース流出端との間に延在しており、前記流れ可能特徴部は、人工弁のコンパクト化デリバリー構成から拡張された完全展開構成への移行の開始後であるが、移行の完了の前に、前記接続シース壁を通る順行性流体流を可能にし、前記接続シース壁を通る逆行流を防ぐように動作可能である、請求項54又は55記載の人工弁。
- 前記弁尖フレームサブコンポーネントがネスト化構成であるときに、前記弁尖フレームサブコンポーネントと前記アンカーフレームサブコンポーネントとの間の環状ギャップにデリバリーされるように動作可能な充填剤をさらに含み、前記人工弁は治療部位で完全に展開される、請求項39~56のいずれか1項記載の人工弁。
