【発明の属する技術分野】
本発明は、椎間板を除去した後の上下の椎体間に前側から挿入配置される椎間ケージに係り、さらに詳細には、前側の左右斜め方向から椎体間に挿入配置することのできる椎間ケージに関する。

前記特許文献１に係る椎間ケージ５０は、図５に示すように、半円形状の左右一対の横スペーサ５１Ａ，５１Ｂと前後の中央スペーサ５３Ａ，５３Ｂとの４つの構成要素を組合せ、左右の固定ネジ５５によって上記４個の構成要素を一体的に固定することにより、前記左右の横スペーサー５１Ａ，５１Ｂ及び前後の中央スペーサ５３Ａ，５３Ｂとの間に、ほぼ四角形状のキャビティ５７を形成した構成である。

【発明が解決しようとする課題】
前述のごとき従来の構成においては、構成部品が多く複雑であると共に、挿入配置後に位置ずれを生じ易いという問題がある。

また前記従来技術においては、椎間ケージを上下の椎体の間に前側から挿入配置することを前提とするものであって、左右の斜め前方から椎体間に挿入するものではなく、さらなる改良が望まれるものである。

【課題を解決するための手段】
本発明は、前述したごとき従来の問題に鑑みてなされたもので、脊椎の椎体間に挿入配置される椎間ケージであって、前端側の厚さ（Ｈ１）よりも後端側の厚さ（Ｈ２）を厚く形成したケージ本体（３）の上下両面に、前記先端側の一方の角部（３Ｃ）の角度をほぼ二等分する二等分線に対してほぼ直交する方向に稜線（１７）を形成した複数の抜け止め防止用の爪状部（１９）を備え、前記ケージ本体（３）の後端面に形成した左右方向のＶ字形状の溝部（１３）に形成した斜め上下方向のスクリュー貫通孔（１５Ｕ，１５Ｌ）を左右方向に長い長孔に形成して備え、前記ケージ本体（３）の前側，中央部，後側に左右方向に長い前側上下孔（５），中央部上下孔（７）及び後側上下孔（９）を上下に貫通して備えると共に、前記各上下孔（５，７，９）と左右方向の長さがほぼ等しい前後方向の穴（１１）を前記ケージ本体（３）の前面（３Ｆ）から前記後側上下孔（９）に亘って形成してケージ本体（３）を中空状に形成し、かつ前記ケージ本体（３）の左右の両側面の前記前側上下孔（５），中央部上下孔（７）、後側上下孔（９）に対応した位置にそれぞれ横貫通孔（５Ａ，７Ａ，９Ａ）を備えていることを特徴とするものである。

【発明の実施の形態】
図１〜図４を参照するに、本発明の実施の形態に係る椎間ケージ１は、中空状のケージ本体３を備えている。すなわち、ケージ本体３は、全体が一体物であって、図２に示すように、平面図の外形は、概略的には正方形状の後辺に台形状の底辺部を合せて変形六角形状に形成し、かつ各角部を円弧状に形成した形状である。そして、上記ケージ本体３は、前端側（先端側）の厚さＨ１よりも後端側の厚さＨ２を厚く形成してある。

上記ケージ本体３の前側，中央部，後側には左右方向に長い前側上下孔５，中央部上下孔７及び後側上下孔９が上下に貫通して形成してある。さらに、ケージ本体３には、前記各上下孔５，７，９と左右方向の長さがほぼ等しい前後方向の穴１１（図３参照）が前面３Ｆから前記後側上下孔９に至って形成してある。

前記前側上下孔５，中央部上下孔７，後側上下孔９及び穴１１を形成したことにより前記ケージ本体３は中空状に形成してある。そして、前記ケージ本体３の左右の両側面の、前記前側上下孔５、中央部上下孔７，後側上下孔９に対応した位置には横貫通孔５Ａ，７Ａ，９Ａがそれぞれ形成してある。

さらに前記ケージ本体３の後端面には、後端側が広くなるようにＶ字形状に開口した溝部１３が形成してあり、この溝部１３には、上面及び下面に貫通した上下のスクリュー貫通孔１５Ｕ，１５Ｌが斜め上下方向に形成してある。このスクリュー貫通孔１５Ｕ，１５Ｌは、左右方向に長い長孔に形成してある。

そして、前記ケージ本体３の上下面には、前記先端側の一方の角部３Ｃの角度（前面３Ｆと側面とのなす角度、約９０）をほぼ二等分する二等分線に対してほぼ直交する方向の稜線１７を形成した断面三角形状の抜け止め防止用の複数の爪状部１９が適宜間隔に形成してある。すなわち、前記爪状部１９の断面三角形状の頂角の部分が前記稜線１７をなすものであって、上記爪状部１９は椎体間への挿入時には挿入し易く抜け難い形状に形成してある。

以上のごとき構成において、前述した従来技術と同様に、椎間板を除去した後の上下の椎体間に椎間ケージ１を挿入配置するには、例えば鉗子などのごとき適宜の工具を適宜の横貫通孔５Ａ，７Ａ，９Ａに係合してケージ本体３を保持し、当該ケージ本体３の一方の角部３Ｃが先行するように、脊椎の左前方から椎体間に挿入配置するものである。なお、上下を反転することにより、脊椎の右前方から椎体間にケージ本体３を挿入配置することができるものである。

したがって、脊椎の前側に例えば臓器等が位置する場合であつても、上記臓器等を回避して脊椎の椎体間へケージ本体３を挿入配置することができるものである。この際、ケージ本体３は後端側よりも先端側が薄く形成してあるので、椎体間へのケージ本体３の挿入を容易に行い得るものである。

上述のように、椎体間にケージ本体３を挿入配置した後は、上下両面に備えた抜け止め防止用の複数の爪状部１９における稜線１７が上下の椎体の終板に喰い込む態様となり、椎体間からの抜け止め効果が得られるものである。また、ケージ本体３の後端面に形成したＶ字形状の溝部１３からスクリュー貫通孔１５Ｕ，１５Ｌを貫通して上下の椎体にインプラントスクリューＳを螺入することにより、上下の椎体間にケージ本体３を確実に固定することができるものである。この際、スクリュー貫通孔１５Ｕ，１５Ｌが長孔であることにより、椎体の状態に対応してインプラントスクリューＳの螺入位置を左右にずらすことができるものである。

前述のごとく上下の椎体間にケージ本体３を固定した後、上下面に形成した前側上下孔５，中央部上下孔７及び後側上下孔９に骨が成長して次第に入り込む態様となり、骨癒合を促進できるものである。そして、前記各上下孔５，７，９に対応して設けた横貫通孔５Ａ，７Ａ，９Ａを透過してＸ線撮影を行うことにより、前記骨癒合の判定を行うことができるものである。

【発明の効果】
以上のごとき説明より理解されるように、ケージ本体は全体が一体物であって、構成が簡単になるものである。また先端側の一方の角部の角度をほぼ二等分する二等分線に対してほぼ直交する方向の稜線を備えた抜け止め防止用の複数の爪状部が上下両面に適宜間隔に設けられているので、脊椎の斜め前方から椎体間に挿入配置することができると共に、挿入配置後は抜け止めを効果的に行うことができるものであって、前述したごとき従来の問題を解消し得るものである。

Claims (1)

  1. 脊椎の椎体間に挿入配置される椎間ケージであって、前端側の厚さ（Ｈ１）よりも後端側の厚さ（Ｈ２）を厚く形成したケージ本体（３）の上下両面に、前記先端側の一方の角部（３Ｃ）の角度をほぼ二等分する二等分線に対してほぼ直交する方向に稜線（１７）を形成した複数の抜け止め防止用の爪状部（１９）を備え、前記ケージ本体（３）の後端面に形成した左右方向のＶ字形状の溝部（１３）に形成した斜め上下方向のスクリュー貫通孔（１５Ｕ，１５Ｌ）を左右方向に長い長孔に形成して備え、前記ケージ本体（３）の前側，中央部，後側に左右方向に長い前側上下孔（５），中央部上下孔（７）及び後側上下孔（９）を上下に貫通して備えると共に、前記各上下孔（５，７，９）と左右方向の長さがほぼ等しい前後方向の穴（１１）を前記ケージ本体（３）の前面（３Ｆ）から前記後側上下孔（９）に亘って形成してケージ本体（３）を中空状に形成し、かつ前記ケージ本体（３）の左右の両側面の前記前側上下孔（５），中央部上下孔（７）、後側上下孔（９）に対応した位置にそれぞれ横貫通孔（５Ａ，７Ａ，９Ａ）を備えていることを特徴とする椎間ケージ。
