Claims (11)
- 瘻孔拡張デバイスであって、
当該デバイスを貫通する連続的通路を画定するチューブ状支持体と、
前記チューブ状支持体上に配置され、かつ当該デバイスの第1の部分を形成する拡張領域及び、当該デバイスの第2の部分を形成する留置領域を有する膨張式拡張バルーンと、
バルーン膨張ルーメンとを含み、
前記留置領域が、その完全自由膨張時に、前記拡張領域の膨張時の直径よりも大きい直径を有するように構成したことを特徴とするデバイス。 - 請求項1に記載の瘻孔拡張デバイスであって、
前記膨張式拡張バルーンが、柔軟性材料、非柔軟性材料、半柔軟性材料、またはそれらの組み合わせから作製されることを特徴とするデバイス。 - 請求項2に記載の瘻孔拡張デバイスであって、
前記膨張式拡張バルーンの前記拡張領域及び前記留置領域が、互いに異なる材料から作製されることを特徴とするデバイス。 - 瘻孔拡張デバイスであって、
長さ、幅及び長手方向軸を有し、当該デバイスを貫通する連続的通路を画定するチューブ状支持体と、
前記チューブ状支持体上に軸方向に配向して配置され、かつ当該デバイスの第1の部分を形成する拡張領域及び、当該デバイスの第2の部分を形成する留置領域を有する膨張式拡張バルーンと、
バルーン膨張ルーメンとを含み、
前記留置領域が、その完全自由膨張時に、前記拡張領域の膨張時の直径よりも大きい直径を有するように構成したことを特徴とするデバイス。 - 請求項4に記載の瘻孔拡張デバイスであって、
前記膨張式拡張バルーンが、柔軟性材料、非柔軟性材料、半柔軟性材料、またはそれらの組み合わせから作製されることを特徴とするデバイス。 - 請求項5に記載の瘻孔拡張デバイスであって、
前記膨張式拡張バルーンの前記拡張領域及び前記留置領域が、互いに異なる材料から作製されることを特徴とするデバイス。 - 瘻孔を拡張し、非血管系カテーテルチューブを挿管するためのシステムであって、
瘻孔拡張デバイス及び非血管系カテーテルチューブを備えており、
前記瘻孔拡張デバイスが、
長さ、幅及び長手方向軸を有し、当該デバイスを貫通する連続的通路を画定するチューブ状支持体と、
前記チューブ状支持体上に軸方向に配向して配置され、かつ当該デバイスの第1の部分を形成する拡張領域及び、当該デバイスの第2の部分を形成する留置領域を有する膨張式拡張バルーンと、
拡張バルーン膨張ルーメンとを含み、
前記留置領域が、その完全自由膨張時に、前記拡張領域の膨張時の直径よりも大きい直径を有するように構成されており、
前記非血管系カテーテルチューブが、完全にまたは部分的に膨張させた拡張バルーン上に適合し、かつ、拡張された前記瘻孔経路を介して、前記留置領域によって安定化された前記非血管系管腔内の所定の位置に挿入することができるように構成されており、
当該瘻孔拡張デバイスが、収縮させることができ、かつ当該デバイスの少なくとも一部を前記非血管系カテーテルチューブを介して引き出すことができるように構成されていることを特徴とするシステム。 - 請求項7に記載のシステムであって、
前記膨張式拡張バルーンが、柔軟性材料、非柔軟性材料、半柔軟性材料、またはそれらの組み合わせから作製されることを特徴とするシステム。 - 請求項8に記載のシステムであって、
前記膨張式拡張バルーンの前記拡張領域及び前記留置領域が、互いに異なる材料から作製されることを特徴とするシステム。 - PEGデバイスを配置する方法であって、
内視鏡を非血管系管腔内に挿入した後、PEGチューブの配置部位を触診により確認することを可能にするために空気を注入して胃を膨らませるステップと、
ニードルを腹部を介して前記非血管系管腔内に挿入した後、前記ニードルを介してガイドワイヤを導入するステップと、
標準的な内視鏡鉗子、内視鏡スネアまたはバルーン接続具を、前記内視鏡の作業チャンネルを介して挿入するステップと、
前記ガイドワイヤを把持し、該ガイドワイヤを前記内視鏡の前記作業チャンネルを介して患者の口から引き出すステップと、
患者の口から引き出した前記ガイドワイヤの一方の端部に膨張ルーメンを含む拡張デバイスを接続した後、前記ガイドワイヤの他方の端部を引っ張ることによって前記拡張デバイスを前記内視鏡の前記作業チャンネルを介して前記非血管系管腔内に引き入れるステップと、
前記ガイドワイヤをニードル経路内に残して、前記ニードルを前記非血管系管腔から取り出すステップと、
前記拡張デバイスを前記ニードル経路から部分的に引き出すステップと、
制御された量の流体を徐々に導入し前記拡張デバイスの前記バルーンの内圧を増加させることにより前記バルーンを膨張させ、それにより、前記非血管系管腔を持ちあげて腹壁に密着させながら、前記ニードル経路をスムーズにかつ段階的に拡張し瘻孔を形成するステップと、
前記拡張デバイス上にピールアウェイシースを配置した後、該シースを前記拡張デバイスの遠位端を通過させ、前記瘻孔を通過させ、前記非血管系管腔内に挿入することができるように、前記バルーンを少しだけ収縮させるステップと、
PEGデバイスを前記ガイドワイヤに沿わせてかつ前記ピールアウェイシースを介して挿入するステップと、
前記バルーンを完全に収縮させ、前記ピールアウェイシースを介して取り出すステップと、
前記ピールアウェイシースを分解して取り出すステップとを含むことを特徴とする方法。 - 請求項10に記載の方法であって、
前記拡張デバイスが、
長さ、幅及び長手方向軸を有し、当該デバイスを貫通する連続的通路を画定するチューブ状支持体と、
前記チューブ状支持体上に軸方向に配向して配置され、かつ当該デバイスの第1の部分を形成する拡張領域及び、当該デバイスの第2の部分を形成する留置領域を有する膨張式拡張バルーンと、
拡張バルーン膨張ルーメンとを含み、
前記留置領域が、その完全自由膨張時に、前記拡張領域の膨張時の直径よりも大きい直径を有するように構成されていることを特徴とする方法。
