Abstract
Description
本実施例は、ポリエステルおよび多糖を含む絡み合ったマトリクスを組み立てる方法を説明する。
本実施例においては、1.5×05の分子量を有するポリ(ラクチド−コ−グリコシド)をジクロロメタン(125mg/ml)に溶解し、約1.3×106ダルトンの分子量のヒアルロン酸(HA)を水に溶解した(15mg/ml)。2のポリマー溶液、2部のPLGAおよび1部のHAを1部のMilli Q水と、約5分間高速でボルテックス攪拌することによって混合した。乳化した混合物は、直ちに、イソプロピルアルコール中のドライアイスを含有する浴中で予め−70℃に冷却した型に注いだ。凍結したの地に、型およびその内容物を、ドライアイスで不可し、真空ラインに連結した第2の容器に移した。−70℃ないし−40℃の温度でこのプロセスによって有機溶媒を除去して、湿潤氷相にHAを残した。ついで、真空下にて温度を−10℃まで上昇することによって水を除去した。
本実施例は、膝傷害の治療を説明する。
本実施例においては、外傷膝傷害を有する運動家患者を整形外科手術に付する。診断は、大腿部関節丘の損傷した関節の軟骨からなる。外科手術は、患者の大腿部関節丘に対する鏡視下骨穿孔法を行い、ヒアリン軟骨が存在する骨を通るチャネルを作製した。外科医はヒアルロン酸をからみ合わせたポリエステルを含む膜を選択し、関節丘の輪郭に沿って形成した。ポリエステルは、3:1の重量比の乳酸およびポリグリコール酸のコポリマーであり、100,000の重量平均分子量を有する。ヒアルロン酸は1,700,000の平均分子量を有する。ヒアルロン酸に対するポリエステルの重量比は9:1であり、膜は3mmの厚さを有する。外科医は、膜の1の側をTISSEEL(登録商標)VHフィブリン・シーラントでコートした。ついで、彼女は、穏やかな圧力を用いて膜を損傷した大腿部の関節丘に適用した。患者には、数週間、膝に圧力がかからないように指示した。関節丘は、外科手術介入後6週間までに新たなヒアリン軟骨で修復した。
本実施例は、骨関節症の治療を説明する。
本実施例においては、肘関節に完全な厚さの軟骨の欠損を有する骨関節症患者を付した。患者は外科医によって手術され、上腕骨に存在する関節に鏡視下骨穿孔法を行った。ポリエステルを含む膜をヒアルロン酸とからみ合わせ、上腕骨の関節丘の輪郭に沿って形成し、ついで、それを外科医によって関節丘上に位置決定した。ポリエステルは、4:1の重量比の乳酸およびポリグリコール酸のコポリマーであり、100,000の重量平均分子量を有する。ヒアルロン酸は1,700,000の重量平均分子量を有する。ヒアルロン酸に対するポリエステルの重量比は8:1である。膜は1mmの厚さを有する。外科医は、再吸収性のPLA/PLGポリマーからなる一連のスクリューで膜を適所に固定する。外科手術後に、新たなヒアリン軟骨が6ヶ月にわたって関節丘に沿って沈殿した。新たな軟骨は、正常なヒアリン軟骨から解剖学的に見分けがつかなかった。
本実施例においては、肩の外傷性脱臼を有する中年男性を付した。診断は、その関節の咬合のヘッドをカバーする関節の軟骨を肩甲骨の浅窩受口で破裂することからなる。患者は外科医によって手術され、上腕骨のヘッドに鏡視下骨穿孔法を行った。ポリエステルを含む膜をヒアルロン酸とからみ合わせ、上腕骨の輪郭に近いように形付けし、ついで、上腕骨のヘッドの輪郭に沿ってほぼ形付け、ついで外科医によって上腕骨のヘッド上に位置決定された。ポリエステルは、3:1の重量比の乳酸およびポリグリコール酸のコポリマーであり、200,000の重量平均分子量を有する。ヒアルロン酸は1,700,000の重量平均分子量を有する。ヒアルロン酸に対するポリエステルの重量比は9:1である。膜は1mmの厚さを有する。外科医は、一連の再吸収性のピンで膜を適所に固定した。外科手術後、新たなヒアリン軟骨が6ヶ月にわたって関節丘にそって沈殿した。新たな軟骨は、正常なヒアリン軟骨から解剖学的に見分けがつかなかった。
Claims (34)
- 哺乳動物の関節におけるヒアリン軟骨欠損を修復する方法であって、
関節に自己の間葉系幹細胞を浸潤させ;ついで
関節に、多糖とからみ合ったポリエステルを含む膜を適用する
ことを含む該方法。 - 自己の間葉系幹細胞が、関節に存在する骨の髄腔に起源を有する自己の骨の間葉系幹細胞である請求項1記載のヒアリン軟骨欠損を修復する方法。
- 関節に自己の間葉系幹細胞を浸潤させることが、関節に存在する骨に少なくとも1の開口を導入することを含み、ここに少なくとも1の開口が骨髄腔から関節への自己の骨の間葉系幹細胞の移動を可能にするのに十分に大きい請求項2記載のヒアリン軟骨欠損を修復する方法。
- 哺乳動物が治療を必要とするヒト患者である請求項1記載のヒアリン軟骨欠損を修復する方法。
- ヒアリン軟骨欠損を修復することが軟骨全層欠損を修復することを含む請求項1記載のヒアリン軟骨欠損を修復する方法。
- 関節に存在する骨に少なくとも1の開口を導入することが、骨を摩耗、鏡視下骨穿孔または穿孔することを含む請求項3記載のヒアリン軟骨欠損を修復する方法。
