Description

The present invention relates to an endoscope treatment tool.

Endoscopic treatment tools such as grasping forceps used together with a flexible endoscope have a coil sheath wound with strands and are inserted into a body cavity via a treatment tool insertion channel of the endoscope. . In order to rotate the distal end portion of the endoscope treatment tool around the axis in this state, the operation portion on the proximal side of the endoscope treatment tool is usually rotated. Therefore, in order to improve the rotational followability of the movable tip portion, there is known a coil sheath provided with a multi-strip coil sheath having a high rotational transmission property.

Here, when the forceps piece is opened and closed by pulling the operation wire from the operation portion like a forceps or the like, a compressive force is applied in the axial direction of the coil sheath in accordance with the opening and closing operation. At this time, the multi-strand coil sheath wound with a plurality of strands has higher rotation transmission than the single-strand coil sheath wound with a single strand, but is easy to compress in the axial direction. For this reason, the multi-strand coil sheath is compressed in the axial direction, the axial force to be transmitted to the distal end portion is reduced, and sufficient treatment cannot be performed, and the procedure becomes complicated. Therefore, a structure in which a single coil sheath is arranged over a plurality of layers (for example, see Patent Document 1) has been proposed.
JP 2000-229084 A

Since the endoscopic treatment instrument described in Patent Document 1 includes a single coil sheath, the coil sheath is excellent in compression resistance in the axial direction, but rotation transmission is still insufficient.
The present invention has been made in view of the above circumstances, and provides an endoscopic treatment tool that can improve both the rotational operability and operability of the movable distal end portion and can facilitate the procedure. The purpose is to do.

The present invention employs the following means in order to solve the above problems.
An endoscope treatment tool according to the present invention includes a movable distal end portion that has a movable portion and a fixed portion and performs a treatment on a living body, and a single coil sheath in which one strand is wound spirally. A plurality of fine wires are braided, and the knitted sheath is impregnated with resin, the knitted sheath sheathed on the one-line coil sheath, and an elongated shape, the tip of which is connected to the movable part An operating shaft member; and an operating portion for operating the operating shaft member to advance and retract, wherein a distal end of the knitted sheath is connected to the fixed portion, and a proximal end is connected to the operating portion. Features.

In this invention, in order to operate the movable distal end portion, the distal end and the proximal end are fixed when the operation portion is moved forward and backward to move the operation shaft member in the axial direction with respect to the single coil sheath. Even when a compressive force is applied to the sheath, one strand is wound spirally, and the compression of the knitted sheath can be relaxed by the single coil sheath having high compression resistance. Therefore, the amount of movement of the operation shaft member with respect to the sheath can be suitably ensured, and a sufficient operation force can be transmitted to the movable portion of the movable tip portion. Further, when rotating the operating portion about the axis to rotate the movable tip, the outer diameter of the knitted sheath having high rotation transmission is larger than the outer diameter of the single coil sheath. The transmission can be further increased.

Further, the endoscope treatment tool according to the present invention is the endoscope treatment tool, wherein when the knitted sheath is viewed from above in a direction perpendicular to the central axis of the knitted sheath, the thin wire Are braided in directions crossing each other with respect to the central axis.

In this invention, since the stitch does not collapse even if the knitted sheath is rotated in any direction around the central axis, the loss of rotational torque in the knitted sheath is more preferably reduced and the rotational force is transmitted to the movable tip. Can do.

According to the present invention, both the rotational operability and the operability of the movable tip portion can be improved, and the procedure can be facilitated.

An embodiment according to the present invention will be described with reference to FIGS. 1 to 5.
An endoscopic forceps (endoscopic treatment tool) 1 according to this embodiment includes a pair of forceps pieces (movable parts) 2A and 2B and a tip cover (fixed part) 3 to treat a living body. The movable tip 5 for performing the above, a single coil sheath 6 in which one strand 6a is spirally wound, a plurality of thin wires 7A are braided, and the stitches are impregnated with a resin 7B. A knitted sheath 7 sheathed on the strip coil sheath 6 and an operation wire (an operation shaft) that is formed so as to be elongated and inserted into the single coil sheath 6 so as to be movable forward and backward and connected to the pair of forceps pieces 2A and 2B. Member) 8 and an operation unit 10 for advancing and retracting the operation wire 8.

The tip cover 3 is formed in a substantially cylindrical shape, and a pivot shaft 11 that pivotally supports the pair of forceps pieces 2A and 2B is rotatably provided on the tip side. The proximal end of the distal end cover 3 is fitted to the outer peripheral surface of the distal end of the single coil sheath 6 and is fitted to the inner peripheral surface of the knitted sheath 7.

The operation unit 10 includes a rod-shaped operation unit main body 12 extending in the central axis C direction, and a slider 13 disposed so as to be movable forward and backward in the central axis C direction with respect to the operation unit main body 12. The operation portion main body 12 is provided with a slit 12 </ b> A through which the operation wire 8 penetrates in the central axis C-axis direction. Further, a protruding portion 12 </ b> B to which the single coil sheath 6 and the knitted sheath 7 are connected is provided on the distal end side of the operation portion main body 12. The projecting portion 12B is provided with a through hole 12a communicating with the slit 12A. The outside of the protruding portion 12B is covered with a bend preventing portion 15 for protecting the connecting portion between the single coil sheath 6 and the knitted sheath 7.

The single coil sheath 6 is fixed with the distal end side connected to the distal end cover 3 in a state where a part of the strand 6a is cut, and the proximal end is inside the through hole 12a of the projecting portion 12B of the operation portion main body 12. It is fitted and fixed to the peripheral surface. Since the single coil sheath 6 is formed by spirally winding the single strand 6a, it is difficult to be deformed even if it is compressed in the direction of the central axis C, and has a compression resistance.

The knitted sheath 7 is braided in a direction in which the thin wire 7A intersects the central axis C when the knitted sheath 7 is viewed in a plan view from a direction orthogonal to the central axis C of the knitted sheath 7. Therefore, for example, when the base end is rotated around the central axis C, the twist of the knitted sheath 7 is suppressed, and the rotational torque is suitably transmitted to the distal end. The distal end of the knitted sheath 7 is connected and fixed to the proximal end of the distal end cover 3, and the proximal end is fitted and fixed to the outer peripheral surface of the protruding portion 12 </ b> B of the operation unit main body 12.

The operation wire 8 includes a first wire 8A that is rotatably connected to the proximal end of the forceps piece 2A, and a second wire 8B that is rotatably connected to the proximal end of the forceps piece 2B. Each of the wires 8A and 8B is arranged to pass through the through hole 12a of the operation unit main body 12, and is inserted through the slit 12A to be connected to the slider 13 at the base end. The distal ends of the first wire 8A and the second wire 8B are bent so as to be separated from the central axis C.

Next, the operation of the endoscopic forceps 1 according to this embodiment will be described.
The endoscope forceps 1 is inserted into a treatment instrument insertion channel of an endoscope (not shown) that has been inserted into the body cavity in advance, and the movable distal end portion 5 is projected from the distal end of the endoscope to perform a predetermined treatment.

When the opening / closing direction of the pair of forceps pieces 2A and 2B is confirmed, and the state is different from the direction in which the affected part (not shown) is to be gripped, it is necessary to adjust the endoscopic forceps 1 so that both directions coincide with each other. There is. Therefore, the braided sheath 7 is gripped and rotated around the central axis C together with the operation unit 10 so that the opening / closing direction of the pair of forceps pieces 2A and 2B matches the direction in which the affected part should be gripped.
At this time, since the knitted sheath 7 is configured as described above, the movable tip 5 also rotates around the central axis C in a state of following the rotated angle when rotated at a predetermined angle on the hand side. .

After making the opening / closing direction of the pair of forceps pieces 2A and 2B coincide with the direction to be gripped, the slider 13 is moved to the proximal end side with respect to the operation section main body 12, and the operation wire 8 is connected to the single coil sheath 6 And it moves to the front end side with respect to the knitted sheath 7. At this time, since the distal ends of the first wire 8A and the second wire 8B are separated from the central axis C, the pair of forceps pieces 2A and 2B rotate around the pivot shaft 11, and the pair of forceps pieces 2A and 2B. Will be open.

In this state, the slider 13 is moved to the proximal end side with respect to the operation portion main body 12, and the operation wire 8 is moved to the proximal end side with respect to the single coil sheath 6 and the knitted sheath 7. At this time, since the distal ends and proximal ends of the single coil sheath 6 and the knitted sheath 7 are fixed, the single coil sheath 6 and the knitted sheath 7 move in the direction of the central axis C along with the movement of the operation wire 8. Compressed.

Here, since the single coil sheath 6 has the above-described configuration, even if the knitted sheath 7 is compressed more than necessary, the single coil sheath 6 has high compression resistance, and the knitted sheath 7 is more than necessary. Not compressed. Therefore, the axial force associated with the movement of the operation wire 8 relative to the single coil sheath 6 and the knitted sheath 7 is suppressed from being used for compressing the single coil sheath 6 and the knitted sheath 7, and the pair of forceps pieces 2A and 2B are restrained. Communicated. In this way, the pair of forceps pieces 2A and 2B are rotated and closed around the pivot shaft 11, respectively, and the affected part is gripped with a necessary gripping force.

According to the endoscopic forceps 1, even if a compressive force is applied to the knitted sheath 7 with the distal end and the proximal end fixed to each other in the direction of the central axis C, one strand 6a is spirally wound. In addition, since the single coil sheath having high compression resistance is disposed, the compression of the knitted sheath 7 can be relaxed. Therefore, the amount of movement of the operation wire 8 with respect to the single coil sheath 6 and the knitted sheath 7 can be suitably secured, and a sufficient operation force can be transmitted to the pair of forceps pieces 2A and 2B of the movable tip portion 5. .

In addition, when the operation portion 10 is rotated about the central axis C to rotate the movable tip portion 5, the outer diameter of the knitted sheath 7 having high rotation transmission is larger than the outer diameter of the single coil sheath 6. The transmission performance of the rotational torque in the sheath 7 can be further enhanced.
Therefore, both the rotational operability of the movable tip 5 and the operability associated with opening and closing can be improved, and the procedure can be facilitated.

Further, since the thin wire 7A of the knitted sheath 7 is braided in a direction intersecting with the central axis C, the knitting of the knitted sheath 7 is not broken even if the knitted sheath 7 is rotated in any direction around the central axis C. It is possible to suppress the loss of the rotational torque in the knitted sheath 7 and transmit the rotational force to the movable tip portion more suitably.
