Description
- Die vorliegende Erfindung betrifft Einrichtungen zum Anbringen chirurgischer Befestiger und insbesondere einen Sperrmechanismus zum Verhindern eines verfrühten oder wiederholten Abschießens der chirurgischen Befestigungsvorrichtung.
- Chirurgische Befestigungseinrichtungen zum gleichzeitigen Anbringen einer Reihe von chirurgischen Klammern oder anderen Typen von Befestigern sind in der Technik bekannt. Solche Einrichtungen werden zum Verheften von Körpergewebe, wie z. B. Darm und Magenwände mit beabstandeten, parallelen Reihen von längsausgerichteten Klammern verwendet. Diese chirurgischen Klammereinrichtungen verringern die Zeit für das Schließen einer Wunde bei einem chirurgischen Verfahren.
- Typischerweise weisen diese Einrichtungen ein Befestigermagazin auf, das auf einer Seite des zu befestigenden Gewebes angeordnet ist, und einen Anschlagaufbau parallel zum Befestigerhalter auf der anderen Seite des zu befestigenden Gewebes. Das Befestigermagazin wird in linearer Richtung in Richtung des Anschlagaufbaus bewegt, so dass das Gewebe zwischen diese eingeklemmt wird. Die Befestiger werden vom Befestigermagazin so ausgetrieben, dass die Enden der Befestiger durch das Gewebe hindurchtreten und endgültig geformt werden, wenn sie in Kontakt zum Anschlagaufbau treten, wobei sie eine Reihe von fertiggestellten Befestigern im Gewebe erzeugen. Optional kann die Befestigervorrichtung einen Messermechanismus aufweisen, um einen Einschnitt zwischen Reihen von Befestigern zu erzeugen. Die Befestiger können aus Metall, nichtabsorbierbaren Polymeren oder bioabsorbierbaren Polymeren, wie Polyglycolid, Polylactid und Copolymeren derselben hergestellt sein. Zusätzlich kann die Anschlagoberfläche eine Mehrzahl von Haltern tragen, um mit den Befestigern zusammenwirkend in Eingriff zu treten, nachdem die Befestiger durch das Gewebe hindurchtreten.
- Es sind Einrichtungen in gebräuchlicher Verwendung, bei denen das Befestigermagazin ein entsorgbares Magazin umfasst, das abnehmbar auf einer Magazinklemmbacke zum Halten und Betätigen des Magazins befestigt ist. Das Magazin wird nach einer Einmalverwendung, d. h. nachdem die Befestiger abgeschossen sind, entsorgt. Die Befestigereinrichtung ist nach dem erneuten Laden mit einem frischen Magazin wiederverwendbar und im Allgemeinen bei einem nachfolgenden chirurgischen Verfahren nach dem Reinigen, Sterilisieren und erneuten Laden wiederverwendbar. Es sind auch entsorgbare chirurgische Befestigereinrichtungen in der Technik bekannt, bei denen die gesamte Vorrichtung nach einer Einmalverwendung entsorgt wird. Beispiele solcher chirurgischer Befestigereinrichtungen können unter anderem in Green (US- Patent Nr. 4,354,628), Green (US-Patent Nr. 4,665,916) aufgefunden werden. Insbesondere stellt das Green US-Patent Nr. 4,568,009 eine Grundlage für den Oberbegriff von Anspruch 1 unten dar.
- Bei der Verwendung der chirurgischen Befestigereinrichtungen entsteht die Möglichkeit, dass die Befestigereinrichtung betätigt werden kann, wenn das Magazin von Klammern oder Befestigern entleert ist. Dies kann auftreten, wenn die Vorrichtung einmal abgeschossen worden ist, das Magazin aber nicht erneut geladen oder entsorgt worden ist, und kann auch auftreten, wenn die Vorrichtung unbeabsichtigterweise mit einem verbrauchten Magazin erneut geladen wird. Unter solchen Umständen wird die Befestigungseinrichtung nicht das Körpergewebe zusammenheften können, was dem Patienten aufgrund des Verlustes des Chirurgen an wertvoller Zeit. Das Schadensrisiko erhöht sich sehr stark, wenn die Vorrichtung einen Messermechanismus enthält, da dieser einen nicht geschlossenen Einschnitt erzeugen wird.
- Um diese Gefahren für den Patienten zu beseitigen, wäre es nützlich, eine Einrichtung vorzusehen, die den Chirurgen darauf aufmerksam macht, dass ein neues Magazin benötigt wird. Es wäre weiterhin nützlich, wenn solch eine Einrichtung einen Sperrmechanismus vorsieht, um tatsächlich den Chirurgen daran zu hindern, das Abschießen eines Magazins zu versuchen, das bereits abgeschossen worden ist, und eine Annäherung der Klemmbacken verhindert, wenn ein verbrauchtes Magazin in die Magazinklemmbacke geladen werden sollte. Dies würde wertvolle Zeit sparen und das Risiko für den Patienten verringern.
- Die EP-A-373 762 und EP-A399 700 sind zwei Beispiele von Instrumenten, die Einrichtungen aufweisen, um das Abschießen eines verbrauchten Magazins zu verhindern. Das Sicherheitsmerkmal hindert jedoch den Chirurgen nicht daran, die Klemmbacken des Instrumentes erneut anzunähern.
- Die EP-A-489 436 offenbart einen Mechanismus zum Anbringen einer Mehrzahl von chirurgischen Befestigern an Körpergewebe. Es weist einen Sperrmechanismus auf, um das erneute Annähern der Klemmbackenelemente und somit das Abschießen der Vorrichtung zu verhindern, nachdem ein Magazin verbraucht ist (d. h. die Befestiger wurden abgeschossen) und die Magazinklemmbacke von der Anschlagklemmbacke zurückgezogen worden ist. Der darin beschriebene Sperrmechanismus ist als Teil des Magazins aufgebaut und darüber hinaus ist der Sperrmechanismus zumindest teilweise innerhalb des Magazins angeordnet. Die EP-A 489 436 wurde erst nach dem hierin erklärten Prioritätsdatum veröffentlicht.
- Während diese Entwicklung einen Fortschritt in der Technik chirurgischer Befestiger darstellt, ist es weiter wünschenswert, eine Sperreinrichtung vorzusehen, die Teil des Klemmbackenmechanismus einer chirurgischen Befestigungseinrichtung im Gegensatz zu einem Teil eines Magazins ist, die das erneute Annähern eines verbrauchten Magazins verhindert. Eine Sperreinrichtung, die Teil eines Rahmens oder Klemmbackenmechanismus ist, erleichtert das Befestigermagazin von umständlichen Sperrmechanismen und darüber hinaus besteht eine entsprechende Verringerung der Kosten bei der Montage und Herstellung, da der Sperrmechanismus nicht mit dem Magazin entsorgt werden wird, wenn das Magazin verbraucht ist.
- Die vorliegende Erfindung ist in Anspruch 1 definiert. Die Erfindung stellt eine chirurgische Befestigungsvorrichtung bereit, um eine Mehrzahl von chirurgischen Befestigern an Körpergewebe anzubringen, umfassend eine Einrichtung, um ein erstes Klemmbackenelement in Richtung eines zweiten Klemmbackenelementes vorzurücken, um Gewebe dazwischen vor dem Eintreiben der Befestiger in das Gewebe zu greifen. Die Vorrichtung umfasst weiterhin eine Einrichtung, die schwenkbar auf einer der Klemmbacken montiert ist, um zu verhindern, dass die Vorrückeinrichtung das erste Klemmbackenelement in Richtung des zweiten Klemmbackenelementes in dem Falle bewegt, dass ein verbrauchtes Magazin in der Magazinklemmbacke gehalten wird. Wenn die Befestiger abgeschossen werden, verschwenkt sich die Einrichtung zum Verhindern gemäß einer Ausführungsform nach oben in einen Bereich, der durch das Klemmbackenelement begrenzt und durch die Befestigereintreiber freigemacht ist. Wenn der Griffmechanismus gelöst wird, bewegt sich die Magazinklemmbacke in ihre nicht vorgerückte (zurückgezogene) Position und verschwenkt die Einrichtung zeitweilig nach unten, wenn sie über die Einrichtung hinwegfährt. Die Einrichtung verschwenkt sich dann zurück nach oben, um in Eingriff zu treten mit einer Oberfläche der Magazinklemmbacke, so dass das erste Klemmbackenelement an einem Vorrücken, gehindert wird, bis ein neues Magazin zur Verwendung in der Vorrichtung positioniert ist. Das Positionieren der Einrichtung zum Verhindern neben dem zweiten Klemmbackenelement des Rahmens stellt eine Einrichtung bereit, um ein frühzeitiges Vorrücken des ersten Klemmbackenelementes zu verhindern und erlaubt die Verwendung in einem Magazin mit einer Mehrzahl von Befestigern ohne eine einstückig mit dem Magazin ausgebildete Sperreinrichtung.
- Die vorstehenden Merkmale der vorliegenden Erfindung werden schneller offensichtlich und verständlich durch Bezugnahme auf die folgende detaillierte Beschreibung einer darstellenden Ausführungsform der Sperreinrichtung zur Verwendung mit einer chirurgischen Befestigungseinrichtung der vorliegenden Erfindung, in Verbindung mit den begleitende Zeichnungen, in denen:
- Fig. 1 eine perspektivische Explosionsansicht mit einer teilweisen Schnittansicht ist, die ein Magazinende eines Klemmbackenmechanismus einer chirurgischen Befestigungsvorrichtung mit der Sperreinrichtung gemäß einer ersten Ausführungsform der vorliegenden Erfindung darstellt;
- Fig. 2 eine perspektivische Ansicht ist, welche die Sperreinrichtung darstellt, die in der ersten Ausführungsform enthalten ist.
- Fig. 3 ist eine Seitenrißansicht im teilweisen Aufbruch, welche die erste Ausführungsform der vorliegenden Erfindung darstellt mit der Sperreinrichtung in einer nicht eingerückten Position vor dem Abschießen der Befestiger;
- Fig. 4 ist eine Seitenrissansicht im teilweisen Aufbruch, welche die erste Ausführungsform während des Abschießens der Befestiger darstellt;
- Fig. 5 ist eine Seitenrissansicht der Ausführungsform in Fig. 1 im teilweisen Aufbruch, welche die im Eingriff befindliche Sperreinrichtung darstellt, die das Vorrücken der Magazinklemmbacke verhindert;
- Fig. 6 ist eine Schnittansicht des in Fig. 5 gezeigten Klemmbackenmechanismus entlang der Linie 6-6;
- Fig. 7 ist eine perspektivische Ansicht, die den Führungsschienenbereich des Klemmbackenmechanismus der ersten Ausführungsform dastellt;

- Fig. 10 ist eine Seitenrissansicht im teilweisen Aufbruch, welche die erste alternative Sperreinrichtung darstellt, die auf einem Klemmbackenmechanismus in einer nicht eingerückten Position einer chirurgischen Befestigungseinrichtung gemäß einer zweiten Ausführungsform der vorliegenden Erfindung positioniert ist;
- Fig. 11 ist eine Seitenrissansicht in teilweisem Aufbruch, welche die Sperreinrichtungen in Fig. 10 während des Abschießens der Befestiger darstellt; und
- Fig. 12 ist eine Seitenrissansicht in teilweisem Aufbruch, welche die in Eingriff befindliche Sperreinrichtung der Fig. 10 und 11 darstellt, welche das Vorrücken der Magazinklemmbacke verhindert.
- Bezugnehmend nun im speziellen Detail auf die Zeichnungen, in denen gleiche Referenzziffern ähnliche oder gleiche Elemente in den verschiedenen Ansichten bezeichnen, ist ein Klemmbackenmechanismus 10 dargestellt mit einer Sperreinrichtung 27, die es erschwert oder verhindert, dass der Klemmbackenmechanismus 10 in eine Abschießeposition angenähert wird, wenn keine Befestiger in einem Magazin 14 vorhanden sind.
- Eine erste Ausführungsform der Sperreinrichtung 27, die in einem Klemmbackenmechanismus 10 einer chirurgischen Befestigungseinrichtung eingesetzt ist, ist in Fig. 1 und 2 der Zeichnungen dargestellt. Der Klemmbackenmechanismus 10 der chirurgischen Befestigungsvorrichtung umfasst ein Magazinklemmbackenelement 12, das gestaltet und ausgebildet ist, um ein Befestigermagazin 14 zwischen Klemmbackenarme 12a und 12b aufzunehmen. Das Magazin 14 weist eine Mehrzahl von Befestigern auf und ist mit Führungspfeilern 24a und 24b versehen, die in Eingriff treten mit einer Führungsschiene 26 auf den Rahmenbereich 18. Das proximale Ende des ersten Klemmbackenelementes 12 ist mit einem Vorrückelement 22 zum Annähern gekoppelt. Ein Anschlagklemmbackenelement 16 weist einen Anschlagbereich einstückig mit einem U-förmigen Rahmenbereich 18 auf. Das proximale Ende des U-förmigen Rahmenbereichs 18 ist mit einem Körperbereich 19 der chirurgischen Befestigungsvorrichtung gekoppelt.
- Eine Sperreinrichtung 27 ist schwenkbar am U-förmigen Bereich 18, wie gezeigt, angebracht und weist ein Sperrelement 28 und ein Halteelement 30 auf. Das Halteelement 30 umfasst einen Federarm 32, der das Sperrelement 28 nach oben in eine Eingriffsposition vorspannt. Das Sperrelement 28 ist ein im allgemeinen H-förmiges Element, wie in Fig. 2 gezeigt ist, mit einem sich seitlich erstreckenden Anschlagbereich 34. Das Sperrelement 28 weist weiterhin einen sich in Längsrichtung erstreckenden Körperbereich 38 mit einem Gelenk oder einer Umbiegung 40 und einem abgestuften, sich seitlich erstreckenden distalen Schwenkpfeiler 36 auf. Der abgestufte distale Pfeiler 36 ist mit einem U-förmigen Rahmenbereich 18 am Schwenkpunkt 44 (Fig. 3) gekoppelt. Die Sperreinrichtung ist aus einem elastischen Material aufgebaut und wird durch den Federarm 32 des Halteelementes 30 nach oben verschwenkt, wenn dieser mit dem U-förmigen Rahmenbereich 18 gekoppelt ist, um mit der Unterseite des Magazins 14 mit einer Mehrzahl von darin befindlichen Befestigern in Eingriff zu treten.
- Bei der Verwendung ist der Klemmbackenmechanismus 10 nachfolgend dem Positionieren des Körpergewebes zwischen das Befestigermagazin 14 und die Anschlagklemmbacke 16 betätigbar, indem ein Griffmechanismus und Annäherungsmechanismus betätigt werden, um das Annäherungselement 20 in distaler Richtung zu drücken, so dass die Magazinklemmbacke 12 in Richtung der Anschlagklemmbacke 16 gedrückt wird, um das Körpergewebe zwischen die Magazinklemmbacke 12 und die Anschlagklemmbacke 16 zu positionieren. Nachdem die chirurgischen Befestiger in das Gewebe eingetrieben sind und die Klammerbeine durch die Anschlagklemmbacke 16 umgebogen sind, wird die Vorrichtung in ihre Ruheposition durch das Lösen des Griffmechanismus zurückgeführt und das Annäherungselement 20 wird in proximaler Richtung mit der Magazinklemmbacke 12 von der Anschlagklemmbacke 16 wegbewegt, wobei das Körpergewebe losgelassen wird.
- Wie in Fig. 3 gezeigt ist, ist der Klemmbackenmechanismus 10 in einer Ruheposition. Das Befestigermagazin 14, das eine Mehrzahl von Befestigern hält, ist in der Magazinklemmbacke 12 positioniert. Das Sperrelement 28 stößt gegen die Unterseite des Magazins 14 an den Befestigereintreibern 42 an und wird nach unten aus dem Eingriff durch die Eintreiber 42 gegen den vorspannenden Federarm 32 des Halters 30 verschwenkt.
- Bezugnehmend auf Fig. 4 ist der Klemmbackenmechanismus 10 in einer angenäherten Position mit dem Magazin 14 neben der Anschlagklemmbacke 16 dargestellt. Der Klemmbackenmechanismus 10 wird in diese Position bewegt, indem ein Annäherungsmechanismus betätigt wird, um das Annäherungselement 20 so zu bewegen, dass es sich nach vorne der Magazinklemmbacke 12 annähert. Ein Griffmechanismus (nicht dargestellt) wird betätigt, um das Betätigungselement 22 durch die Magazinklemmbacke 12 in Richtung des distalen Endes der Vorrichtung anzutreiben. Das Betätigungselement 22 treibt die Befestigereintreiber 42 in die Befestiger, um die Befestiger in das Gewebe einzudrücken. Nach dem Abschießen verbleiben die Befestigereintreiber 42 in einer distalen Position, so dass es dem Sperrelement 28 gestattet ist, nach oben um den Schwenkpunkt 44 durch den wie gezeigten Federarm 32 in den inneren Raum zwischen die Klemmbackenarme 12a und 12b verschwenkt zu werden, so dass die Biegung 40 des Körperbereichs 38 zwischen den Klemmbackenarmen 12a und 12b des Klemmbackenelementes 12 positioniert ist.
- Wie in Fig. 5 gezeigt ist, wird, nachdem die Befestiger in das Gewebe eingebracht sind, der Klemmbackenmechanismus 10 in die Position der Fig. 3 zurückgeführt, indem die Griffelemente und der Annäherungsmechanismus gelöst werden, so dass das Betätigungselement 22 und das Annäherungselement 20 sich in Richtung des proximalen Endes der Vorrichtung bewegen. Das Sperrelement 28, das sich elastisch zwischen den Klemmbackenarmen 12a und 12b nach oben erstreckt, wird nach unten verschwenkt, um es der Magazinklemmbacke 12 zu gestatten, über ihn hinweg zu verfahren und dann nach oben zurück, um in eine Nut 46 einzurücken, die einstückig mit dem proximalen Ende der Magazinklemmbacke 12 ausgebildet ist. Somit können das Betätigungselement 22 und Annäherungselement 20 nicht in distaler Richtung verfahren werden, um die Magazinklemmbacke 12 in Richtung der Anschlagklemmbacke 16 anzunähern, wenn keine Befestiger in dem Magazin sind.
- Bezugnehmend auf Fig. 6 und 7 ist die Führungsschiene 26 des U-förmigen Bereichs 18 dargestellt, auf der das Magazin 14 verfährt. Die Führungsschiene 26 des U-förmigen Bereichs 18 ist gestaltet, um mit den Führungspfosten 24a und 24b des Magazins 14 zusammenzuwirken, um das Magazin 14 während des Vorrückens und Abschießens ordnungsgemäß auszurichten. Die Führungsschiene 26 besitzt eine im Allgemeinen T-förmige Gestaltung und weist einen abgestuften Bereich 48 auf, um das Laden des Magazins 14 auf dieser zu erleichtern. Die Verriegelungseinrichtung der vorliegenden Erfindung gestattet es der Vorrichtung, nur einmal bei einer Einmalverwendung abgeschossen zu werden, indem eine erneute Annäherung des Magazins verhindert wird, nachdem die Befestiger abgeschossen worden sind und der Klemmbackenmechanismus in seine nicht eingreifende Position zurückgezogen worden ist. Die Vorrichtung kann erneut abgeschossen werden, indem ein neues Magazin ausgetauscht wird, das mit Befestigern geladen ist, um die Sperreinrichtung 28 aus dem Eingriff zu verschwenken. Wenn der Klemmbackenmechanismus versehentlich mit einem verbrauchten (bereits abgeschossenen) Magazin erneut geladen wird, wird die Sperreinrichtung eine Annäherung des Magazins verhindern, da die Befestigereintreiber das Sperrelement nicht aus dem Eingriff mit der Magazinklemmbacke drücken werden und somit das Abschießen des Instrumentes nicht erlauben.
- Eine andere Ausführungsform der Sperrelemente sind in Fig. 8 gezeigt. Das Sperrelement 50 weist einen im allgemeinen dreieckigen Körperbereich 56 und ein Federelement 60 auf, das z. B. vorzugsweise an den dreieckigen Körperbereich 56 punktgeschweißt ist. Der dreieckige Körperbereich 56 weist eine Anschlagoberfläche 54 an einem Mittelpunkt auf, um in Eingriff zu treten mit der Nut 66, wie am besten in Fig. 12 ersichtlich ist. Ein sich seitlich erstreckender Schwenkpfeiler am distalen Ende ist an einem proximalen Ende des Klemmbackenmechanismus 10 im Schwenkpunkt 64 positionierbar.
- Eine weitere Ausführungsform des Sperrelements ist in Fig. 9 gezeigt. Das Sperrelement 62 ist ähnlich dem Sperrelement 50 oben gemäß des wie gezeigten Aufbaus.
- Bezugnehmend nun auf Fig. 10 bis 12 ist der Klemmbackenmechanismus 10 der chirurgischen Befestigungsvorrichtung dargestellt, der entweder das Sperrelement 50 oder 62 beinhaltet. Der Klemmbackenmechanismus 10 weist eine Magazinklemmbacke 12 und eine Anschlagklemmbacke 16, wie in der in Fig. 1 bis 7 gezeigten vorherigen Ausführungsform, auf. Jedoch ist bei der in Fig. 10 bis 12 gezeigten Ausführungsform das Sperrelement 50 schwenkbar an der proximalen Seite des U-förmigen Bereichs 18 am Schwenkpunkt 64 angebracht. Ein Federbereich 60 drückt das Sperrelement 50 nach oben, um mit der unteren Seite des Magazins 14 in Eingriff zu treten, wie in Fig. 10 gezeigt ist, welche den Klemmbackenmechanismus 10 in einer Ruheposition zeigt. Ähnlich den oben diskutierten Fig. 4 und 5 zeigt Fig. 11 den Klemmbackenmechanismus 10 in einer Eingriffsposition mit der Sperreinrichtung 50, die sich nachfolgend der Annäherung der Klemmbackenelemente 12 und 16 elastisch nach oben erstreckt.
- Nach dem Abschießen der Befestiger wird der Klemmbackenmechanismus 10 in proximaler Richtung über das Sperrelement 50 bewegt, um das Element 50 nach unten und dann nach oben zu verschwenken, um in die Nut 66 einzurücken und es dem Körperbereich des Sperrelementes 50 zu gestatten, innerhalb des Raumes positioniert zu werden, der durch die Klemmbackenarme 12a und 12b der Magazinklemmbacke 12 begrenzt ist.
- Der Begriff "Befestiger" wird hierin als ein allgemeiner Begriff verwendet, der chirurgische Klemmen und den klemmenförmigen Bereich zweiteiliger chirurgische Befestiger und Äquivalente derselben bezeichnet. Es sollte weiterhin deutlich sein, dass die hierin beschriebenen Befestiger für Instrumente zum Anbringen von Metallklammern einsetzbar sind wie auch für Klammern und zweiteilige Befestiger, die aus nicht bioabsorbierbaren oder bioabsorbierbaren Polymeren (z. B. Polyglycolid, Polylactid und Copolymere derselben) hergestellt sind.
Claims (17)
1. Chirurgische Befestigungsvorrichtung mit einer ersten
Klemmbacke (12), die ein entsorgbares Magazin (14) mit
einer Mehrzahl von Befestigern besitzt, enthält, eine
zweite Klemmbacke (16) gegenüberliegend der ersten
Klemmbacke, eine Einrichtung (20), um eine der
Klemmbacken in Richtung der anderen Klemmbacke
vorzurücken und zurückzuziehen, und eine Einrichtung
(22), um die Befestiger von dem Magazin nach dem
Vorrücken einer der Klemmbacken auszutreiben, und
gekennzeichnet durch
eine Einrichtung (28, 54), die schwenkbar auf einer der
Klemmbacken montiert ist, um das Vorrücken der
Klemmbacke in Richtung der gegenüberliegenden Klemmbacke
nach dem Austreiben der Befestiger und dem Zurückziehen
der Klemmbacke und vor dem Ersetzen des verbrauchten
Magazins zu verhindern.
2. Vorrichtung gemäß Anspruch 1, wobei die Einrichtung zum
Verhindern (28, 54) ein schwenkbares Sperrelement (28,
54) umfasst, das zwischen ersten und zweiten Positionen
schwenkbar ist, und wobei das Sperrelement mit der
ersten Klemmbacke in der zweiten Position in Eingriff
tritt.
3. Vorrichtung gemäß Anspruch 1 oder 2, wobei das
Sperrelement in die zweite Position federbelastet (32,
60) ist.
4. Vorrichtung gemäß Anspruch 1, 2 oder 3, wobei die erste
Klemmbacke einen Zahnbereich (46) besitzt, der mit dem
Sperrelement in Eingriff tritt, wenn dieses in der
zweiten Position ist.
5. Chirurgische Befestigungsvorrichtung gemäß einem der
vorhergehenden Ansprüche und umfassend eine
Magazinführungseinrichtung (26), die auf dem zweiten
Klemmbackenelement angeordnet ist und mit dem Magazin in
Eingriff tritt, um das Magazin in Ausrichtung mit einer
Anschlagoberfläche auf dem zweiten Klemmbackenelement zu
führen.
6. Vorrichtung gemäß Anspruch 5, wobei die
Führungseinrichtung ein T-förmiges Schienenelement (26)
umfasst, das am zweiten Klemmbackenelement (16)
befestigt ist und mit einem komplementär geformten
Innenbereich (24a, 24b) im Magazin zusammenzuwirkt,
wobei die Rinne während des Annäherns des ersten
Klemmbackenelementes mit dem Schienenelement in Eingriff
tritt.
7. Vorrichtung gemäß einem der vorhergehenden Ansprüche und
umfassend einen Klemmbackenmechanismus (10) mit einer
ersten Klemmbacke (12) mit einem Magazin (14), das eine
Mehrzahl von Befestigern enthält, und die zweite
Klemmbacke (16), die Einrichtung (20) zum Vorrücken, um
eines der Klemmbackenelemente in Richtung der anderen
Klemmbacke zum Greifen von Gewebe dazwischen anzunähern,
die Einrichtung (22) zum Eintreiben, um die Befestiger
in distaler Richtung von dem Magazin in das Gewebe
einzutreiben, und eine Sperreinrichtung (27, 50, 62),
die von einer Nichteingriffsposition vor dem Eintreiben
der Befestiger von dem Magazin zu einer
Eingriffsposition nachfolgend dem Eintreiben der
Befestiger bewegbar ist, wobei die Sperreinrichtung eine
proximale Bewegung der Einrichtung zum Vorrücken erlaubt
und eine distale Bewegung der Einrichtung zum Vorrücken
behindert, nachdem die Befestiger von dem Magazin
eingetrieben worden sind,
wobei die Sperreinrichtung die Einrichtung zum
Verhindern des Vorrückens (28, 54) umfasst, die auf dem
zweiten Klemmbackenelement positioniert ist und von der
Nichteingriffsposition in die Eingriffsposition zum in
Eingrifftreten mit der Einrichtung zum Vorrichtung
verschwenkbar ist.
8. Vorrichtung gemäß Anspruch 7, wobei die Einrichtung zum
Vorspannen ein Blattfederelement (32, 60) umfasst, das
mit der Eingriffseinrichtung gekoppelt ist, die in
Kontakt mit der zweiten Klemmbacke positioniert ist, um
die Eingriffseinrichtung in die Eingriffsposition zu
verschwenken.
9. Vorrichtung gemäß Anspruch 7 oder 8, wobei die
Sperreinrichtung weiterhin ein Halteelement umfasst, das
an der zweiten Klemmbacke befestigt ist und einen
Federarm (32, 60) aufweist, um die Eingriffseinrichtung
in die Eingriffsposition vorzuspannen.
10. Vorrichtung gemäß einem der vorhergehenden Ansprüche,
wobei das Magazin, welches die Mehrzahl von Befestigern
enthält, weiterhin ein Befestigereintreibelement (22)
enthält, um die Befestiger von dem Magazin einzutreiben,
wobei das Eintreibelement die Eingriffseinrichtung (28,
54) berührt, um die Eingriffseinrichtung in die
Nichteingriffsposition zu verschwenken, wenn die
Befestiger in dem Magazin enthalten sind.
11. Vorrichtung gemäß einem der vorhergehenden Ansprüche,
wobei die Einrichtung (28) zum Verhindern des Vorrückens
ein im Wesentlichen H-förmiges Element umfasst, wobei
ein erstes Bein des H-förmigen Elementes in Eingriff
tritt mit der zweiten Klemmbacke, um das H-förmige
Element um eine Öffnung in der zweiten Klemmbacke zu
verschwenken, und ein zweites Bein des H-förmigen
Elementes ein Blockierelement umfasst, um in Eingriff zu
treten mit der Vorrückeinrichtung, um eine distale
Bewegung der Vorrückeinrichtung zu verhindern, nachdem
die Befestiger aus dem Magazin getrieben worden sind.
12. Vorrichtung gemäß einem der vorhergehenden Ansprüche,
wobei die zweite Klemmbacke (16) einen im Wesentlichen
U-förmigen Bereich besitzt, und die Einrichtung zum
Verhindern des Vorrückens auf einem Grundbereich des U-
förmigen Bereichs unter dem Magazin angeordnet ist.
13. Vorrichtung gemäß einem der Ansprüche 1 bis 12, wobei
die zweite Klemmbacke (16) einen im wesentlichen U-
förmigen Bereich umfasst und die Einrichtung zum
Verhindern des Vorrückens auf einer proximalen Seite des
U-förmigen Bereichs positioniert ist.
14. Vorrichtung gemäß einem der vorhergehenden Ansprüche,
wobei die erste Klemmbacke ein Paar von Klemmbackenarmen
umfasst, mit einem Schlitz zwischen diesen, um das
Magazin aufzunehmen, und die Einrichtung zum Verhindern
des Vorrückens in die erste Klemmbacke zwischen den
Armen einrückt, nachdem die Befestiger aus dem Magazin
getrieben worden sind.
15. Vorrichtung gemäß einem der vorhergehenden Ansprüche,
wobei die Einrichtung zum Verhindern des Vorrückens (28,
54) in Eingriff tritt mit der Vorrückeinrichtung (20),
um eine distale Bewegung der Vorrückeinrichtung zu
verhindern, wenn das Magazin von der ersten Klemmbacke
entfernt ist.
16. Vorrichtung gemäß Anspruch 5 oder einem der Ansprüche 6
bis 15, sofern diese von Anspruch 5 abhängig sind, wobei
die Einrichtung (26) zu Führen des Magazins ein T-
förmiges Schienenelement umfasst, das an der zweiten
Klemmbacke befestigt ist und mit einem komplentär
geformten Rinnenbereich in dem Magazin zusammenwirkt,
wobei die Rinne mit dem Schienenelement während des
Annäherns der ersten Klemmbacke in Eingriff tritt.
17. Vorrichtung gemäß einem der vorhergehenden Ansprüche,
wobei die Einrichtung (54) zum Verhindern des Vorrückens
eine Blattfeder umfasst mit einem ersten Bein (58), um
in Eingriff zu treten mit der zweiten Klemmbacke, um die
Eingriffseinrichtung um eine Öffnung der zweiten
Klemmbacke zu verschwenken, und ein zweites Bein (56)
mit einem Anschlagbereich (54), um in Eingriff zu treten
mit der Vorrückeinrichtung, um eine distale Bewegung der
Vorrückeinrichtung und der ersten Klemmbacke zu
behindern, nachdem die Befestiger von dem Magazin
getrieben worden sind.
