Fixier-Knochenschraube
Die Neuerung betrifft eine Fixier-Knochenschraube für den Bereich der Osteosynthese, wie auch bei der endoskopischen Rekonstruktion von Sehnen und/oder Bändern, wobei die Knochenschraube mit einem den Schaft umgebenden Gewinde versehen ist und an einer Seite eine Einschraubhilfe und an der gegenüber liegenden Seite eine Ausschraubhilfe besitzt.
Fixier-Knochenschrauben sind auch als Interferenz- Schrauben bekannt, beispielsweise durch den Prospekt der Fa. Arthrotek Inc. Ontario, Kanada, oder durch den Prospekt der Fa. ORTHOMED, Frankreich. Die hier gezeigten Knochenschrauben sind in der Art von Madenschrauben ohne Kopf und mit einem Innensechskant oder Schlitz als Einschraubhilfe ausgestattet. Bekannt sind ferner verschiedene Arten von Knochenschrauben anderer Hersteller, wobei alle bisher bekannten Knochenschrauben lediglich auf einer Schraubenseite eine Einschraubhilfe, wie Sechskant, Schlitz oder Kreuzschlitz aufweisen.
Bei Frakturoperation, sei es an der Schulter, Hüfte, Knie oder anderer Bereiche werden eine Vielzahl von Knochenschrauben verwendet, die später nach einer Heilungsphase wieder entfernt werden müssen oder entfernt werden sollen. Beispielsweise bei der endoskopischen Rekonstruktion von Bändern im Kniegelenk wird ein Band mit zwei Knochenstücken am jeweiligen Ende zwischen Femur und Tibiae gesetzt und das jeweilige Knochenstück mit einer Interferenzschraube befestigt, wie dies beispielsweise in der EP 556 571 beschrieben ist. Diese Operation ist ein relativ großer Eingriff in das Kniegelenk, wobei bei der späteren Entfernung der Schraube wiederum und unnötigerweise der Entfernungseingriff groß ist um die Schraube wieder zu entfernen. Hierbei ist die Gefahr einer Beschädigung des Knochenteiles und sogar des Bandes gegeben.
Die Neuerung hat sich daher die Aufgabe gestellt, derartige Schrauben so auszugestalten, dass eine Entfernung dieser Fixier- Knochenschraube auch von einer anderen Seite als von der Einschraubseite her möglich ist.
Zur Lösung dieser Aufgabe wird vorgeschlagen, Knochenschraube mit beidseitigen für Schraubwerkzeuge geeigneten Aufnahmen auszugestalten, so dass auch bekannte Schraubwerkzeuge, wie Innensechskant, Kreuzschlitz, Schraubenzieher oder andere Werkzeugdreher verwendet werden können.
In einer bevorzugten Ausfuhrungsform ist eine InfeffererßSchfaube*als*trine Art* *·· Madenschraube gestaltet, an beiden Enden mit einer Einschraub- bzw. Ausschraubhilfe versehen, vorzugsweise ein Innensechskant, wobei die Schraube eine bereits optische von der Einschraubseite zur Ausschraubseite verjüngende Tendenz aufweist, um eindeutig die Einschraubseite zu kennzeichnen. Die Schraube trägt ein den Schaft umgebendes Gewinde, welches auch ein Spongiosa- oder Kortikalisgewinde sein kann.
Dies eröffnet nunmehr den Operateur die Möglichkeit, bei der Entfernung dieser Schrauben von einer anderen Seite als von der Einschraubseite her zu der Schraube zu gelangen insbesondere, wenn dies ein bedeutend kleinerer Eingriff und damit geringere Belastung für den Patienten bedeutet. Die Ausschraubdrehrichtung ist dabei die gleiche wie beim Einschrauben, so dass keine Veränderung oder Zerstörung der die Schraube umgebenden Knochensubstanz erfolgt.
Weitere Vorteile ergeben sich aus der nachfolgenden Beschreibung eines Ausfuhrungsbeispiels und anhand der Zeichnungen. Es zeigt:
Fig. 1 Eine Fixier-Knochenschraube in Seitenansicht, Fig. 2 die Fixier- Knochenschraube im Längsschnitt, Fig. 3 eine Schraube mit verschiedenen Ein-Ausschraubhilfen in den Enden.
Entsprechend der Figuren 1 und 2 ist eine Fixier-Knochenschraube 10 dargestellt, mit einem Schaft 1, einen den Außendurchmesser bildenden Kopf 2, einem Gewinde 3 und einem sich verjüngenden Ende 4.
Im Kopf 2 ist gerundet und trägt eine Einschraubhilfe 5, hier vorzugsweise als Innensechskant 7 ausgebildet. In das andere Ende 4 der Fixier-Knochenschraube ist ebenfalls ein Innensechskant eingearbeitet der als Ausschraubhilfe 6 dient.
Weiterhin ist die Fixier-Knochenschraube mit einer Durchgangsbohrung 8 versehen, welche nicht zwingend notwendig ist.
Das Gewinde 3 kann je nach Einsatzort ein Spezialgewinde oder ein bekanntes Kortikalis- oder Spongiosagewinde sein.
Es für diese Art der Fixier- Knochenschraube unerheblich, welche Art von Einschraub- bzw Ausschraubhilfe im Kopf 2 und Ende 4 eingelassen ist, da hier ebenso gut andere Werkmittel wie Schlitz, Kreuzschlitz, Vierkant, Dreikant usw. als Ein-und/oder Ausschraubhilfe verwendet werden können.
In einer weiteren Ausgestaltung der Neuerung ist dargestellt, das eine Fixier-Knochenschraube 20 eine Einschraubhilfe als Innensechskant 7 und auf der gegenüberliegenden Seite eine Ausschraubhilfe als Schlitz 21 tragen kann.
Nicht näher dargestellt ist auch die Möglichkeit, dass anstelle der Durchgangsbohrung 8 die jeweilig verwendete Art der Einschraubhilfe,
beispielsweise ein Innensechskant, vollständig bis* ziim*Brtde*4 der fixier- *··* *..* Knochenschraube durchfuhrt.
Andererseits liegt im Bereich dieser Neuerung, dass anstelle der beschriebenen und dargestellten Formen der Einschraubhilfe 7 und Ausschraubhilfe 6,21 auch Aussenformen als Sechskant Vierkant oder Dreikant verwendezt werden können.
Derartige Fixier-Knochenschrauben 10,20 werden aus einem entsprechenden Metall beispielsweise Titan, Metalllegierung, aus einem verträglichen Kunststoff oder aber auch aus einem Knochenersatzmaterial hergestellt.
Claims (8)
1. Fixier- Knochenschraube mit einem Schaft mit Gewinde und einer Einschraubhilfe in einem Ende der Schraube, dadurch gekennzeichnet, dass die Schraube (10, 20) in ihren beiden Enden (2, 4) eine Einschraubhilfe (7) und eine Ausschraubhilfe (6) trägt.
2. Fixier- Knochenschraube nach Anspruch 1, dadurch gekennzeichnet, dass die Einschraubhilfe (7) und die Ausschraubhilfe (6, 21) gleiche für Werkzeugmittel zu verwendende Formen tragen.
3. Fixier- Knochenschraube nach Anspruch 1, dadurch gekennzeichnet, dass die Einschraubhilfe (7) und die Ausschraubhilfe (6, 21) als Innensechskant, Kreuzschlitz, Schlitz oder vierkantartig ausgebildet sind.
4. Fixier- Knochenschraube nach Anspruch 1, dadurch gekennzeichnet, dass die Schraube (20) als Einschraubhilfe (7) und die Ausschraubhilfe (21) ungleiche für Werkzeugmittel zu verwendende Formen tragen.
5. Fixier- Knochenschraube nach Anspruch 1, dadurch gekennzeichnet, dass eine Durchgangsbohrung (8) vorhanden ist.
6. Fixier- Knochenschraube nach einem der vorherigen Ansprüche, dadurch gekennzeichnet, dass anstelle der Durchgangsbohrung (8) die jeweilige Form der Einschraubhilfe (7) bis zum Ende (4) durchgehend ist.
7. Fixier- Knochenschraube nach Anspruch 1, dadurch gekennzeichnet, dass die Einschraubhilfe (7) und/oder die Ausschraubhilfe (6, 21) als Aussenformteil in Sechskant, Vierkant oder Dreikantform ausgebildet sind.
8. Fixier- Knochenschraube nach Anspruch 1, dadurch gekennzeichnet, dass die Fixier- Knochenschraube (10, 20) aus Metall, Kunststoff oder knochenersatzmaterial besteht.
