Background
The keratinized gum width has an impact on the long-term success of dental implants. It is believed that both peri-implant plaque accumulation and gingival inflammation increase when the keratinized gum width is <2 mm. However, it is also considered that under conditions of good oral hygiene, the width of the cornified gum has little effect on soft tissue inflammation, but there is a risk of an increase in the gum index, plaque index, and probe bleeding index.
Periodontal disease is one of two most common oral infectious diseases in China, and is also the leading cause of tooth loss of adults. Investigation shows that the middle-aged and elderly people in China have poor periodontal health and oral hygiene conditions and are not optimistic in condition. Therefore, from the current situation of oral hygiene in China, if the implant in the mouth of a patient needs to be maintained to be stable and effective for a long time, the cornified gum around the implant is important.
Therefore, the measured value of the pre-operative keratinized gum width can help us to predict the prognosis risk of peri-implant gingivitis of a patient, and to judge the necessity of keratinized gum transplantation and the like in advance, guide the design of surgical soft tissue incisions and the like. It is currently generally accepted that a keratinized gum width of 2mm is necessary for a chinese implant site, both for long-term implant maintenance stability and for aesthetic reasons.
In the presence of natural teeth, the keratinized gingiva is measured from the gingival margin to the membrane-gingival junction, divided into the bucco-lingual sides. Currently, periodontal probes and calipers are used clinically for the measurement of keratinized gingiva. However, the alveolar ridge anatomy in the edentulous area is complex and the cornified gum is not a flat regular structure.
