Abstract

发明背景
1.发明领域。结疤是对患者皮肤中伤口愈合的自然反应。伤口愈合过程可分为三个连续的阶段。第一炎症阶段在造成伤口的损伤之后立即开始。所述炎症阶段持续长达一周,在这段时间里,细胞过程从伤口清除受损组织和异物。炎症阶段之后是增殖阶段,其特征在于成纤维细胞增殖以及胶原蛋白和蛋白聚糖的产生。增殖阶段可持续数天到数周,且通常在此阶段内开始形成增生性疤痕,这通常是为了愈合伤口而合成的细胞外基质的过量产生所造成的结果。此类疤痕称为“增生性疤痕”。在增殖阶段之后开始重塑期,其中产生于增殖阶段中的基质被重塑为有组织的交联结构,这增加了愈合组织的机械强度。
在愈合反应期间形成的疤痕经常在愈合之后通过美容品或手术治疗而得到治疗。美容治疗最多只是临时解决方案,而需要进行后续手术对患者而言既不方便又有风险。因此,期望提供可以在愈合过程本身期间减少疤痕形成的方法和装置,以使得此类后续程序不再必要或者至少在范围上得到减小。
背景技术。WO/2011/019859描述了用于抑制皮肤伤口中结疤的设备,其中该设备固定于皮肤并对间隔开的皮肤部位施加向外的力。US2008/0069855描述了将会防止疤痕和组织粘连的材料。美国专利号7,511,185、4,702,251、4,539,990和4,535,772描述了对伤口的任一侧组织横向向内施加反向力的伤口愈合设备。
发明内容
本发明提供了用于在皮肤表面上伤口的愈合期间抑制疤痕形成的方法和设备。术语“疤痕”意在不仅包括由于切伤、擦伤等而发生的普通增生性疤痕,而且还包括疤痕疙瘩——其为增生性包块,通常在创伤之后发生在某些个体的真皮和相邻的皮下组织中。
本发明的方法和装置通过如下方式来治疗正在愈合的伤口:其通常在伤口愈合过程的增殖阶段和/或重塑阶段期间在伤口上施加向下的力并于此同时施加压迫力,以便减轻横跨伤口或在伤口周围的组织中的张力。在炎症阶段的后期部分中的治疗也可能是有用的,并且在一些情况下,还可在重塑之后执行治疗。例如,可以在炎症阶段结束时或将近结束时,通常是在伤口发生并最初开始愈合之后的1天到30天范围内的某一时间,对伤口同时施加下向压迫和侧向压迫。在开始治疗之后,这些力可持续施加范围从1天到21天或更长的一段时间,并且在治疗期间这些力可以增大、减小或保持恒定。
根据本发明的装置特别适合于同时将此类下向压迫力和侧向压迫力提供至皮肤中的伤口区域,该装置通常包含组织移位器件,该组织移位器件可放置在皮肤表面上以覆盖伤口本身以及伤口侧面的一定距离,该距离通常在伤口的每一侧从几毫米到1厘米或2厘米或更大。可以以如下所述的多种方式促动组织移位器件以施加所期望的垂直向下的力。
本发明的装置通常还将包括衬垫,该衬垫可固着于正在愈合的伤口的任一侧皮肤,其中该衬垫通常适合用于将皮肤表面和下面的组织层侧向向内牵拉以提供所期望的侧向压迫,并转而减轻组织中的张力。在所述的实施方式中,将通过(在衬垫已施加于皮肤表面之后)升高衬垫的中心(通常作为促动组织移位器件的结果)以侧向向内地牵拉衬垫的侧面,来实现皮肤和下面的组织的向内侧向移动。
衬垫可包含可附接至组织移位器件上的皮肤的材料条或材料层,其中衬垫的侧向区域粘附至移位器件的相对两侧上的皮肤表面。通常,至少衬垫的侧面部分将会是不可扩大的(抗拉伸或者无弹性),并且更通常情况下整个衬垫将具有有限的拉伸性或弹性,常常是完全不可扩大的。“不可扩大”意味着衬垫材料将具有小于20%的断裂伸长率,通常小于15%,且常常小于10%。备选地,衬垫可包含束带或其他较长的结构,该束带或其他较长结构可缠绕在患者的肢体周围,以便将组织移位器件上下约束在皮肤表面上。
在示例性实施方式中,组织移位器件包含球囊或其他可扩张器件,所述球囊或其他可扩张器件具有相对不可扩大的底面和相对可扩大的顶面,其中所述底面贴靠伤口安置,而所述顶面向上膨胀以升高衬垫的中心区域,这转而导致衬垫的侧面部分侧向向内牵拉从而提供所期望的向内的组织压迫。
为了治疗覆盖5厘米、10厘米、20厘米或更长的长伤口,本发明的装置可形成为多个节段,每一节段通常包含如先前所描述的单独的衬垫和单独的组织压迫器件。所述节段将通过柔性接头、链节或其他组件而连结在一起,这样允许所产生的组装件符合于皮肤表面,并遵循伤口和皮肤的不规则拓扑结构。所产生的“柔韧性”对于允许组装件在患者移动或由其他人移动时与皮肤表面相符合也可以起到作用。在一些实施方式中,接头将形成短管或其他结构,其能够从组装件的任一端向每个连续节段递送膨胀介质。
根据本发明的原理还可构建其他装置。例如,对于局部伤口,可提供具有粘结面的圆形可拉伸弹性衬垫,用于粘附至皮肤。通过首先例如使用约束框架或其他组件径向向外拉伸弹性衬垫,可将弹性衬垫放置在伤口上并将其从约束中释放出来,以对伤口施加侧向向内的力。在衬垫下或衬垫上可放置各种组织压迫器件,以便提供所期望的垂直压迫。
备选地,还可提供这样的装置:其包括更坚硬的盘片,该盘片在外侧环形表面上具有粘合剂以及在中心部分无粘合剂,该中心部分可以外翻或以其他方式变形以向内挤压组织并向内牵拉盘片的环形圆周,从而同时提供径向压迫和垂直力。例如,中心部分可以从外翻配置“卡扣”到翻转配置,这很像是具有对瓶盖先前已被开启过的指示的瓶盖。
在其他实施方式中,可用药物包覆组织移位器件的其他下表面,以便在治疗期间将该药物递送到皮肤。

具体实施方式
图1是图示皮肤表面S中的皮下伤口W的示意图,其中该伤口在所述表面之下延伸一定距离。根据本发明的原理,为了在伤口愈合时减少结疤,将在跨越伤口并从伤口的每一侧向皮肤表面上延伸几毫米到几厘米或更长的区域上施加大体上由垂直箭头所指示的向下或垂直的力。压力优选地沿着细长伤口的大部分或全部长度施加。通过本发明治疗的切伤、切口和其他细长伤口通常将延伸至真皮中几毫米或更多。除了垂直力之外,本发明将提供由箭头12所指示的压迫力或压力,其优选地从表面向下延伸至接近或等于伤口深度的深度。
用于施加图1中所示之力的本发明的方法可使用各种装置来执行。一个示例性装置在图2中示出,包括衬垫14和组织移位器件16,它们可组装起来并联合用于施加所期望的力。衬垫16在其从末端18到末端20的大部分或全部长度上通常将是不可扩大的。长度通常将在从5厘米到10厘米、20厘米或更长的范围内。宽度通常将在从1厘米到5厘米的范围内。衬垫将分成多个区域,包括第一侧面区域22和第二侧面区域24。这些侧面区域中的每一个的至少一部分将是基本上不可扩大的,并且它们将具有粘结面26和28,所述粘结面26和28可施加于皮肤以将衬垫固定就位。在侧面区域22与24之间的衬垫中心区域14可具有一些拉伸能力(可扩大性),但是该中心区域通常在其全部或一部分长度上也将是不可扩大的。然而,衬垫的这个中心区域的底面通常将没有任何粘合剂,以便使中心区域可以放置在组织移位器件16上,并且当该移位器件如图2中虚线所示扩张时中心区域能够在移位器件的顶部上移动。移位器件16通常将由在其上部的弹性材料所形成,以在该器件16的底部或基部30抵抗扩张的同时允许所期望的垂直扩张。因此,当促动该器件时,弹性的上表面32可扩张到虚线所示的程度。通常,组织移位器件16将会是可膨胀结构,诸如球囊,其中该结构的底部的厚度和/或底部内的加固会抵抗拉伸,而上部则是弹性的,以允许在该结构膨胀时扩张。在其他情况下,可使用具有圆形或椭圆形轮廓的球囊或其他可膨胀的可扩张结构。
在使用中,将图2的组装件放置在伤口上,以使得衬垫14的侧面区域22和24粘附至伤口W的相对两侧上的皮肤表面。如图2A中所示,组织移位器件16由衬垫的中心部分直接保持在伤口W之上,并且继而如图2B中所示那样膨胀组织移位器件16以升高中心部分。通过升高中心部分25,将侧面部分22和24侧向向内牵拉以压迫组织的上表面,组织的上表面转而如图2B中的箭头所示那样压迫伤口任一侧上处在较低深度的组织。同时地,组织移位器件的底部在伤口上施加如图2B中的垂直箭头所指示的向下压力。组织移位器件的不对称结构允许这种对力的选择性施加,以便实现本发明的目标。侧面部分22和24中的每一个将附接在皮肤表面上直至点P。衬垫将不附接至点P与由组织移位器件16所覆盖的区之间的皮肤。如图2B中所示,正是衬垫的这一短的非附接区域允许了向内牵拉侧面部分22和24而不会显著地向上拉拽皮肤。
为了处理很长的伤口,可将如在图2中大体示出的装置形成为如图3中所示的多个节段40。每个节段40将大体上具有在图2中所示的结构,且相邻的节段将由链节的短管状连接件42联结起来,该连接件42将节段连接在一起并且允许每个组织移位器件16同时膨胀。图3的分段结构是有利的,这是因为其允许组装件遵循非直线伤口,并且还允许组装件在疤痕治疗期间当患者移动或被移动时与组织表面相符。
图4中图示了备选治疗器件50。该器件包含具有外侧环形部分54的衬垫52,该外侧环形部分54具有用于附接至患者皮肤的粘结面。器件50还包括中心部分54,该中心部分54可外翻或压下以便如图5A和5B中所示那样对伤口施加垂直力并于同时施加径向向内的力。这样的可外翻的中心部分还可与先前描述的细长的或条状器件一起使用。
如图5A和5B中所示,局部(非细长的)伤口W可由器件50的中心部分54所覆盖,同时环形部分52粘附至伤口周围的皮肤。通过继而如图5B中所示那样变形和压下中心部分54,在伤口上施加垂直力,而同时将环形部分52(其通常将是弹性的)向内牵拉以径向地压迫伤口周围的组织。
Claims (11)
1.用于抑制皮肤表面上正在愈合的伤口中的疤痕形成的装置,所述装置包括:
可扩张结构,其具有相对不可扩大的底面和相对可扩大的顶面,其中所述底面用于接合所述伤口,所述顶面当被促动时向上远离所述底面;以及
衬垫,其具有适合粘附于所述伤口的相对两侧的皮肤上的两个侧面区域,以及当放置在所述伤口上时适合覆盖所述可扩张结构的中心部分;
其中所述可扩张结构的扩张导致所述底面对所述伤口和相邻组织施加向下的压力,并且同时导致所述顶面升高并向内牵拉所述衬垫的所述侧面区域,以便侧向地压迫邻近所述伤口的组织。
2.根据权利要求1所述的装置,其中所述可扩张结构是可膨胀的。
3.根据权利要求2所述的装置,其中所述可膨胀结构具有柔性下表面和弹性上表面,以便在所述结构膨胀时所述上表面在所述下表面保持尺寸稳定的同时变形和远离所述下表面。
4.根据权利要求3所述的装置,其中所述可膨胀结构在膨胀时具有D形轮廓。
5.根据权利要求1所述的装置,其中至少所述衬垫的所述侧面区域是不可扩大的。
6.根据权利要求1所述的装置,其中整个所述衬垫是不可扩大的。
7.根据权利要求1所述的装置,其中每个所述侧面区域具有粘附于所述皮肤的粘结面。
8.根据权利要求1所述的装置,其中所述扩张的结构包含沿一条线连结起来的多个节段,其中所述节段可相对于彼此移位,以便允许所述结构符合于长伤口。
9.根据权利要求8所述的装置,其中每个节段由单独的衬垫保持在所述皮肤上。
10.根据权利要求1所述的装置,其中所述可扩张结构包含弹簧元件,所述弹簧元件可被压下以提供接合所述伤口并向其施加力的凸面。
11.根据权利要求1所述的装置,其中所述可扩张结构的底面包覆有药物。
