Abstract
本发明提供了一种膀胱穿刺装置,属于医疗器械领域,用于对膀胱内尿潴留进行穿刺造瘘。它包括中空式穿刺针、扩张套管、双套管式导尿管和导尿管固定架。穿刺针的前端带有防脱出装置,扩张套管带有自攻式无损伤安全螺纹,双套管式导尿管头端带有被动扩张式伞状防脱出装置。穿刺使用时,穿刺针头端释放防脱出装置,扩张套管扩皮后,将导尿管套入穿刺针,并释放尿管前端防脱出伞,最后妥善固定导尿管。本发明的防脱出穿刺针可减少对周围组织的副损伤,且不会脱出造成穿刺失败;本发明的双套管式导尿管,结构简单、使用方便、易于固定,对膀胱粘膜刺激小,可以大大提高膀胱穿刺的成功率,减少给病员带来的痛苦,同时后期更换导尿管方便。
Description
技术领域
本发明专利提供了一种膀胱穿刺造瘘装置,属于医疗器械技术领域,用于对膀胱内尿潴留进行穿刺造瘘引流。
背景技术
膀胱穿刺造瘘常用于各种原因导致的膀胱内尿潴留、排尿困难,传统膀胱穿刺造瘘采用手术方式,病人需要全身或椎管麻醉,创伤大,费用高。近年来,各种一次性膀胱穿刺造瘘装置也应用于临床,但操作相对麻烦,不利于临床医生实际操作,应用范围比较狭窄。
发明内容
本发明专利包括穿刺针、扩张套管、双套管式导尿管和导尿管固定架。
1、穿刺针:本发明专利提供一种中空式防脱出穿刺针,由针体、尾帽和防脱出钢丝组成。整体设计长度200mm,头端直径2mm,尾端直径3mm。尾端为帽状结构,设计长度10mm,直径3mm,尾帽为中空式设计,尾端连接针芯内两根细钢丝,向下压尾帽可使钢丝头端露出针尖,两根细钢丝头端为皱褶状,皱褶段垂直长度也为10mm,置于针芯内,露出针尖后皱褶展开,达到防止穿刺针脱出的目的。尾帽头端与穿刺针体尾端连接处各设计一个环状关卡,尾帽内凹,针体外凸,使尾帽和针体尾端相对固定,并易于用力下压。另外单独设计一个连接装置,可连接尾帽与普通针筒。再单独设计一个帽塞,防止尿液外溢,造成操作困难。
2、扩张套管:本发明专利提供一种带有自攻式无损伤安全螺纹的扩张套管,设计长度150mm,头端直径4mm,尾端直径6-8mm,类似于临床常用的扩皮器,头端带有自攻式无损伤安全螺纹,在下压扩张扩过程中可减轻腹壁塌陷,造成膀胱深部组织血管损伤。扩张套管为硬质塑料结构,尾端为双耳状结构,易于手握及用力。
3、双套管式导尿管:本发明专利提供一种双套管式导尿管,由外层套管、环状旋钮、挤压开花式装置和内层套管组成。设计长度300mm,为双层套管模式,外层套管直径和普通导尿管相同,根据不同患者提供不同直径的导尿管。具体规格如下表:

本发明专利提供的导尿管采用比普通导尿管稍硬的塑料结构,为双层套管模式,外层套管主要为固定装置,头端(入膀胱端)带有被动扩张式伞状防脱出装置,为独立挤压开花式伞状结构,伞状结构的伞翼设计长度如上表(表1),伞状结构尾端与外层套管头端采用环状挂钩式结构相连,便于日后取出。外层套管尾端为一环状旋钮,与内层套管尾端形成螺纹,旋钮也采用独立环状挂钩式结构与外层套管相连,利用旋钮螺纹下压,使外层套管头端伞状结构撑开。独立旋钮在旋转下压过程中,不会使整个外层套管一起旋转。内层套管主要为引流装置,头端为环柱状膨出结构,设计长度8mm,环柱头端直径小于外层套管直径1mm,尾端直径和外层套管直径相同,这样头端较细便于插入膀胱,尾端则达到撑住伞状结构的目的。内层套管尾端可连接普通集尿袋,尾端下部与外层套管旋钮连接处为螺纹状结构,螺纹长度根据伞翼长度确定,以使伞翼达到最大面积。螺纹底部带有锁定螺纹,防止自行滑脱。取出时,反向旋转外层套管尾端环状旋钮,利用螺纹拉力收起伞状结构,以便安全取出,减少损伤。
4、导尿管固定架:本发明专利提供一个导尿管固定装置,采用硬塑料材质,设计高度20mm,为上下两个环状结构,中间用四根柱状结构连接固定,上层环设计直径和导尿管直径相同,边缘开口,以使导尿管卡入其中,并牢固固定;下层环设计直径30-40mm,边缘开口,开口位置和上层环相同。上层环卡住导尿管,下层环贴于腹壁,以减少牵拉。

具体实施方式
穿刺使用时,找到膀胱位置,定位合适穿刺点,局麻成功后,穿刺出尿液后去除针筒,下压穿刺针尾端帽状结构,使针芯内两根细钢丝头端褶皱释放于膀胱内,用帽塞塞住针孔,防止尿液外溢;用手术刀片切开皮肤长度约8mm,将带有自攻式无损伤安全螺纹的扩张管套套入穿刺针,扩开肌肉筋膜组织;扩张完成后,将双套管式导尿管套入穿刺针,利用螺纹挤压释放尿管前端防脱出伞状结构,最后拔出穿刺针,用导尿管固定架固定导尿管于腹壁,操作完毕。本发明专利提供的防脱出穿刺针可以减少对周围组织特别是肠管和血管的副损伤,且在膀胱尿液流出后不会脱出造成穿刺失败,减少并发症的发生,提高治疗效果。本发明专利提供的双套管式伞状无气囊尿管,结构简单、使用方便、易于固定,对膀胱粘膜刺激小,可以大大提高膀胱穿刺的成功率,同时减少给病员带来的痛苦。使用该造瘘装置患者的创伤小,恢复快,同时操作简单、容易掌握。同时后期更换导尿管方便。
Claims (14)
1.一种中空式膀胱穿刺针,专门用于膀胱穿刺造瘘,其特征在于由针体、尾帽和针芯内两根细钢丝组成,穿刺针总长度200mm,头端直径2mm,尾端直径3mm;尾帽套于针体尾端,为中空结构,长度10mm,直径3mm,可借助于连接头连接普通针筒,也可用帽塞封闭;尾帽连接针芯内两根细钢丝;尾帽可下压使钢丝露出头端。
2.根据权利要求1中所述的膀胱穿刺针,其尾帽与针体尾端连接部采用两个环形关卡,尾帽内凹,针体外凸,使尾帽和针体尾端相对固定。
3.根据权利要求1中所述的膀胱穿刺针,其针芯内有两根细钢丝,钢丝尾端与尾帽相连,钢丝头端为皱褶状,皱褶段垂直高度和尾帽长度相同,尾帽下压可使头端皱褶全部露出针尖,并完全释放。
4.一种扩张套管,专门用于膀胱穿刺造瘘,其特征在于由管体、尾端双耳状结构和头端无损伤自攻式安全螺纹组成,扩张套管长度150mm,头端直径4mm,尾端直径6-8mm,采用硬质塑料结构。
5.根据权利要求4中所述的扩张套管,其尾端采用双耳状结构,易于手握及用力。
6.根据权利要求4中所述的扩张套管,其头端采用自攻式无损伤安全螺纹,在下压扩张扩过程中可减轻腹壁塌陷。
7.一种双套管式导尿管,专门用于膀胱穿刺造瘘,其特征在于由内外两层套管、环状旋钮和挤压开花式伞状结构组成,导尿管长度300mm,采用双层套管模式,外层套管主要为固定装置,内层套管主要为引流装置,外层套管直径和普通导尿管相同,采用比普通导尿管稍硬的塑料材质。
8.根据权利要求7中所述的双套管式导尿管,其外层套管尾端为一环状旋钮,与内层套管尾端形成螺纹,旋钮采用独立环状挂钩式结构与外层套管相连,独立旋钮在旋转下压过程中,不会使整个外层套管一起旋转。
9.根据权利要求7中所述的双套管式导尿管,其内层套管尾端可连接普通集尿袋,尾端下部与外层套管旋钮连接处为螺纹状结构,螺纹长度根据伞翼长度确定,以使伞翼达到最大面积;螺纹底部带有锁定螺纹,防止自行滑脱。
10.根据权利要求7中所述的双套管式导尿管,其外层套管头端为被动扩张式伞状防脱出装置,为独立挤压开花式伞状结构,伞状结构尾端与外层套管头端采用环状挂钩式结构相连,便于日后取出;利用权利要求8中所述的环状旋钮螺纹下压,可使外层套管头端伞状结构完全撑开。
11.根据权利要求7中所述的双套管式导尿管,其内层套管头端为环柱状膨出结构,环柱长度8mm,头端直径小于外层套管直径1mm,尾端直径和外层套管直径相同。
12.一种导尿管固定架,专门用于固定导尿管于腹壁,其特征在于由上下两个环状结构连接中间四根柱状结构组成,导尿管固定架高度20mm,采用硬塑料材质。
13.根据权利要求12中所述的导尿管固定架,其上层环直径和导尿管直径相同,边缘开口,以使导尿管卡入其中,并牢固固定。
14.根据权利要求12中所述的导尿管固定架,其下层环直径30-40mm,边缘开口,开口位置和上层环相同。
