I just printed several different different sizes back to back, so there doesn't appear to be anything wrong with gutenprint itself here, instead the problem appears to lie somewhere higher up. You didn't mention what client you are using to print. I suspect it is either not specifying the size properly or it is not doing so at all, so gutenprint ends up falling back to the default (ie 4x4).
