Add source and line info to `<admonition>` and `<definition_list>` nodes.

Based on [patch:#202] by Hood Chatham.

Update one more test sample to the changes in the last commit [r9473].

Fix option_marker parsing (bugs:#474).

Don't split at `", "` inside angle brackets.

Do not use HTML5 text-level tags inside ``<code>`` and ``<code-block>``.

Add CSS rules to align SVG objects.

In HTML5, SVG images can be directly embedded with the "raw"
directive. Image alignment can be specified with the "align-left" and
"align-right" class values.

Update functional tests for HTML

Extend SVG tests and bring them up to date.
Test with both, HTML4 and HTML5.

Minor corrections and edits elsewhere.

Small documentation fixes.

Correct CSS color name.
