I am unable to use xmlto for pdf output, because of unmet dependancies
on my system (passivetex is in debian oldstable, and i use texlive
instead of tetex).

But it seems xmlto do not use db2latex stylesheets at all.
(dpkg -L xmlto | xargs grep db2latex)

Nethertheless, using xsltproc together with the db2latex stylesheets
processes correctly your sample :)
