- FIG. 6B shows an example where the range of the degree of recovery on the discharge date (expected discharge date) is small.
- a rehabilitation facility or a rehabilitation hospital may be a candidate for the receiving facility in the candidate specifying unit 15 described later.
- the recovery degree prediction unit 14 predicts the degree of recovery of the patient, so that it is possible to select the receiving facility according to the degree of recovery of the patient and its range.
- the recovery degree prediction unit 14 determines whether the range of the recovery degree on the calculated discharge date (scheduled discharge date) is large or not, and when the recovery degree range is equal to or larger than the predetermined threshold, the receiving facility It may be determined that the processing after the candidate identification processing is stopped.
- the candidate specifying unit 15 receives an instruction from the control unit 11 to start the candidate specifying process. In response to this, the candidate specifying unit 15 starts a process of specifying a candidate for a facility for receiving the patient indicated by the patient ID. Specifically, the candidate specifying unit 15 records each facility recorded in the database 104 in association with the recovery degree acquired from the recovery degree prediction unit 14 and one or more facility categories specified by the range of the recovery degree. Is specified as a candidate for the receiving destination (step S104). It should be noted that the number of facilities specified as the accepting destination candidates is one if the facility is recorded in the database 104 in association with one or a plurality of facility categories specified by the degree of recovery or the range of the degree of recovery. There may be more than one. That is, the matching support apparatus 1 enables selection of a receiving facility from a plurality of hospitals and facilities according to the degree of recovery of the patient.

It may be.
- the acceptance condition is, more specifically, information indicating whether or not the patient can be accepted at the receiving facility according to the degree of the acceptance judgment index of the patient at the receiving facility on each day.
- the acceptance determination index is, for example, the vacancy rate of each receiving facility.
- the degree of the acceptance determination index is the magnitude of the vacancy rate.
- the acceptance condition indicates whether or not the patient can be accepted according to the vacancy rate of the receiving facility.
- the acceptance conditions are determined according to the vacancy rate, for example, whether the facility has a low vacancy rate, if the vacancy rate is medium, if the vacancy rate is medium, and if the vacancy rate is high, the facility is acceptable. Shown in multiple stages. In this example, the acceptability is described in three stages, but the number of stages may be determined according to the facility.
- the acceptance condition may be set for each patient nursing type.
- the acceptance condition may further include an acceptance change enable / disable flag indicating whether the acceptability according to the degree of the acceptance determination index can be changed.
- the acceptance change availability flag indicates “impossible”
- the facility is determined to be a facility that does not change the acceptance status of the patient according to the degree of the acceptance determination index.
- Such a facility is, for example, a facility that does not change the acceptability of a patient belonging to a nursing type requiring special treatment or care according to the vacancy rate.
- the acceptance change availability flag indicates “allowed” (not “impossible”), the facility is determined to be a facility that can change the acceptance status of a patient according to the degree of the acceptance determination index.
- Such a facility is, for example, a facility that changes the acceptance or non-acceptance of a patient belonging to a nursing type requiring special treatment or care according to the vacancy rate. In other words, such facilities do not accept patients belonging to nursing types requiring special treatment or care when the vacancy rate is low, but may accept these patients when the vacancy rate is high.
- the acceptance determination index of the receiving facility may be information other than the vacancy rate, such as the number of staff members, the available business hours of the staff members, and the ratio of severely ill patients among hospitalized patients. In the following description, it is assumed that the acceptance condition includes an acceptance changeability flag.
- FIG. 7 is a diagram showing the acceptance conditions.
- the acceptance condition is information indicating whether or not acceptance is possible according to the vacancy rate (ratio (%)) in a certain facility (facility A in FIG. 7).
- “ ⁇ ” indicates acceptability.
- “ ⁇ ” indicates rejection (impossible).
- FIG. 7 further shows acceptance conditions including acceptance availability according to the vacancy rate for each nursing type.
- the column described as nasal nutrition indicates the acceptability of patients requiring nasal nutrition according to the vacancy rate.
- the column labeled tracheostomy indicates the acceptance of patients undergoing tracheostomy according to the vacancy rate.
- the column described as infectious disease indicates the acceptability of patients suffering from infectious disease according to the vacancy rate.
- the column described as artificial respiration indicates the acceptability of patients who need to wear artificial respirators according to the vacancy rate.
- the candidate specifying unit 15 includes a facility ID indicating one or a plurality of reception destination candidates identified based on the degree of recovery of the patient and a range of the degree, and a list of the one or more reception destination candidates.
- the combination of the information indicating the acceptance condition and the current degree of the acceptance judgment index is output to the facility judgment unit 16 and the acceptance condition output unit 17.
- the facility determination unit 16 acquires the reception condition of each facility that is one or a plurality of reception destination candidates, and determines whether or not the reception change permission / prohibition flag indicates the impossibility (step S106).
- the facility determination unit 16 generates a determination result indicating whether or not the acceptance change availability flag indicates “impossible” for each facility ID, and outputs the determination result to the acceptance condition output unit 17.
- the accepting condition output unit 17 specifies the facility ID for which the accepting change permission / inhibition flag by the facility determining unit 16 has become a determination result indicating impossibility. If the acceptance change availability flag for a certain facility indicates that the facility is unacceptable, the facility is a facility that does not change the acceptance availability according to the degree of the acceptance determination index. Therefore, the receiving condition output unit 17 determines that the information on the receiving condition of the facility is output immediately. In this case, the acceptance condition output unit 17 acquires the facility attribute information (facility name, address, telephone number, etc.) from the database 104 based on the facility ID for which the acceptance change availability flag indicates a disapproval.
- the acceptance condition output unit 17 outputs the facility ID, the facility attribute information, and the current acceptance decision index of the facility (the size of the vacancy rate) for the facility that does not change the acceptability information according to the acceptance decision index. (Numerical value) is generated (step S107).
- the accepting condition output unit 17 transmits the patient ID, the generated first accepting candidate list, and information indicating the accepting condition of each accepting candidate to the terminal 2 or the server 3 that has output the matching request (step S108). ).

It is determined to generate and output.
- the acceptance condition output unit 17 selects one facility from one or a plurality of facilities whose acceptance change availability flag indicates yes, and determines whether the current acceptance determination index of the facility is equal to or greater than the first threshold.
- the first threshold value is set when the acceptance of the selected facility is under the most relaxed condition (for example, when the acceptance of patients belonging to the nursing type requiring special treatment or care is maximized). This is a threshold for determining whether or not there is.
- the acceptance condition output unit 17 determines whether or not the acceptance according to the degree of the acceptance determination index included in the acceptance condition of the facility is a maximum. The limit is changed from unacceptable to acceptable (step S110).
- the acceptance condition output unit 17 outputs, for example, a vacancy rate of 30%.
- acceptability information ( ⁇ or ⁇ )
- all information that is not acceptable is changed to acceptability ( ⁇ ).
- the acceptance condition output unit 17 determines whether the degree of the current acceptance determination index of the selected facility is equal to or greater than the second threshold value smaller than the first threshold value and less than the first threshold value (step S111).
- the second threshold value is set when the acceptance of the selected facility is partially relaxed (for example, when the acceptance of patients belonging to the nursing type requiring special treatment or care is partially disabled) ). If the degree of the current acceptance judgment index of the selected facility is equal to or greater than the second threshold and less than the first threshold, the acceptance condition output unit 17 responds to the degree of the acceptance judgment index included in the acceptance condition of the facility. Of the acceptability, the acceptability of a predetermined nursing type is changed from unacceptable to acceptable (step S112).
- the acceptance condition output unit 17 Nasal nutrition and tracheostomy are acceptable among the information that is not acceptable among the information ( ⁇ or ⁇ ) of the acceptability of the item of artificial respiration when the vacancy rate is 15%. ).
- the acceptance condition output unit 17 determines the degree of the current acceptance judgment index of a facility (a facility whose acceptance change enable / disable flag indicates “acceptable”) in which the information of acceptance is changeable according to the degree of the acceptance judgment index, and changes the acceptance condition. It is determined that has been completed. Then, the acceptance condition output unit 17 outputs the facility attribute information (facility name, address) associated with each facility ID from the database 104, based on each facility ID of the facility whose acceptance information can be changed according to the degree of the acceptance determination index. , Phone numbers, etc.).
- the acceptance condition output unit 17 outputs a facility ID, facility attribute information, and a current acceptance decision index of the facility (the size of the vacancy rate) for a facility in which the information on acceptability according to the degree of the acceptance decision index can be changed. (Numerical value) is generated (step S113).
- the receiving condition output unit 17 outputs the patient ID, the generated second list of receiving destination candidates, and information indicating the changed or unchanged receiving conditions of each receiving destination candidate to the terminal 2 or the server that has output the matching request. 3 (step S114).
- the terminal 2 or the server 3 transmits the second reception destination candidate list and the information indicating the changed or unchanged reception conditions of each reception destination candidate to a predetermined monitor or other output as appropriate according to the request of the person in charge. Output to the means.
- the accepting condition output unit 17 transmits the generated second accepting candidate list and the information indicating the changed or unmodified accepting condition to the terminal 2 or the server 3 that has output the matching request.
- the terminal 2 or the server 3 outputs the second candidate list of receiving destinations and the changed or unchanged receiving conditions to a predetermined monitor or other output means as needed in response to a request from a person in charge.
- the person in charge such as a nurse, the patient himself, and the patient's family can change the information on the acceptability according to the degree of the acceptability determination index by using the facility ID, the facility attribute information (facility name, address, telephone number). Number, etc.), a list of first destination candidates including the degree of the current acceptance judgment index of the facility (such as the size of the vacancy rate (numerical value)), and the changed or unchanged acceptance conditions for each destination candidate And can be viewed.

Hereinafter, a matching support device according to an embodiment of the present invention will be described with reference to the drawings.

The

The recovery
The
In the following description, the above-mentioned predetermined time is referred to as “discharge expected time”. The expected discharge time may be indicated on a specific day, or may be indicated for a period in an appropriately determined range. The expected discharge time may be determined based on, for example, the average number of hospital days in the hospital and other conditions. The average hospital stay in a hospital may be determined for each diagnosis of a patient.
The

The matching
FIG. 5 is a diagram illustrating a processing flow of the matching support apparatus.
A person in charge, such as a nurse at the facility where the patient is hospitalized, instructs the
It is assumed that the matching request includes the patient's medical record information indicating the ID of the facility that made the request, the ID of the patient, and physical attributes such as the patient's age, weight, and gender. The medical record information further includes a history of treatment and care for the patient. The treatment / care history includes a disease type that identifies the patient's disease, a nursing type that identifies the type of treatment and care required for the patient based on the patient's condition including the disease, etc., for each day of the hospital stay, The state of the doctor's operation based on the disease, the type of the disease that occurred concurrently with the hospitalization period and its concurrent date, the body temperature of each day during the hospitalization period of the patient, the state information such as the number of urination and defecation, etc. are included. For example, information other than these may be included. The
回復度合は、一例として、回復の程度をいくつかの段階に区分したカテゴリ変数やFIM(Functional Independence Measure)のスコアなどの数値で出力される。
The recovery
The recovery degree prediction model may be generated by the matching
The recovery
The 例 recovery degree indicates, for example, a degree of daily activity (ADL: Activities of Daily Living) of the patient. Reception facilities vary depending on the degree of recovery. For example, if the degree of recovery is low, it is necessary to search for a facility for receiving patients with a lower degree of recovery, such as a nursing home. When the degree of recovery is high, it is necessary to search for a facility for receiving a patient with a higher degree of recovery, such as a recovery facility.
FIGS. 6A and 6B are diagrams conceptually showing the degree of recovery over time. 6A and 6B, the horizontal axis represents the passage of time, and the vertical axis represents the degree of patient recovery. The degree of recovery of the patient is represented by the ADL described above as an example. 6A and 6B, the solid line indicates the state of change of the recovery degree predicted by the recovery
FIG. 6A shows an example where the range of the degree of recovery is large. In such a case, a rehabilitation facility, a rehabilitation hospital, or a normal facility may be a candidate for a receiving facility in the
The recovery
The
(4) The
The acceptance condition is, more specifically, information indicating whether or not the patient can be accepted at the receiving facility according to the degree of the acceptance judgment index of the patient at the receiving facility on each day. The acceptance determination index is, for example, the vacancy rate of each receiving facility. The degree of the acceptance determination index is the magnitude of the vacancy rate. In other words, the acceptance condition indicates whether or not the patient can be accepted according to the vacancy rate of the receiving facility. The acceptance conditions are determined according to the vacancy rate, for example, whether the facility has a low vacancy rate, if the vacancy rate is medium, if the vacancy rate is medium, and if the vacancy rate is high, the facility is acceptable. Shown in multiple stages. In this example, the acceptability is described in three stages, but the number of stages may be determined according to the facility. The acceptance condition may be set for each patient nursing type.
The acceptance condition may further include an acceptance change enable / disable flag indicating whether the acceptability according to the degree of the acceptance determination index can be changed. When the acceptance change availability flag indicates “impossible”, the facility is determined to be a facility that does not change the acceptance status of the patient according to the degree of the acceptance determination index. Such a facility is, for example, a facility that does not change the acceptability of a patient belonging to a nursing type requiring special treatment or care according to the vacancy rate. When the acceptance change availability flag indicates “allowed” (not “impossible”), the facility is determined to be a facility that can change the acceptance status of a patient according to the degree of the acceptance determination index. Such a facility is, for example, a facility that changes the acceptance or non-acceptance of a patient belonging to a nursing type requiring special treatment or care according to the vacancy rate. In other words, such facilities do not accept patients belonging to nursing types requiring special treatment or care when the vacancy rate is low, but may accept these patients when the vacancy rate is high. There is a facility. The acceptance determination index of the receiving facility may be information other than the vacancy rate, such as the number of staff members, the available business hours of the staff members, and the ratio of severely ill patients among hospitalized patients. In the following description, it is assumed that the acceptance condition includes an acceptance changeability flag.
FIG. 7 is a diagram showing the acceptance conditions.
As shown in FIG. 7, the acceptance condition is information indicating whether or not acceptance is possible according to the vacancy rate (ratio (%)) in a certain facility (facility A in FIG. 7). In FIG. 7, “〇” indicates acceptability. “×” indicates rejection (impossible).
FIG. 7 further shows acceptance conditions including acceptance availability according to the vacancy rate for each nursing type. The column described as nasal nutrition indicates the acceptability of patients requiring nasal nutrition according to the vacancy rate. The column labeled tracheostomy indicates the acceptance of patients undergoing tracheostomy according to the vacancy rate. The column described as infectious disease indicates the acceptability of patients suffering from infectious disease according to the vacancy rate. The column described as artificial respiration indicates the acceptability of patients who need to wear artificial respirators according to the vacancy rate.
The
(4) The accepting
Accordingly, the staff such as a nurse, the patient himself, and the patient's family can change the facility ID, facility attribute information (facility name, address, telephone Number, etc.), a list of first destination candidates including the degree of the current acceptance judgment index of the facility (the size of the vacancy rate (numerical value), etc.), and the acceptance conditions of each destination candidate can be viewed. . This list may include information indicating whether or not acceptance is possible according to the degree of the current acceptance determination index. For example, based on a comparison between the degree of the acceptance determination index and the threshold, it is set to be acceptable if the degree of the acceptance determination index is equal to or greater than the threshold, and to be unacceptable if the degree of the acceptance determination index is less than the threshold.
When the acceptance change availability flag indicates “acceptable,” the facility is a facility that may change the acceptance according to the degree of the acceptance determination index. Such a facility is, for example, a facility in which the acceptability of a patient belonging to a nursing type requiring special treatment or care may be changed depending on the vacancy rate. For this reason, when the acceptance change availability flag indicates “acceptable”, the acceptance
The acceptance
The acceptance
Thereby, the person in charge such as a nurse, the patient himself, and the patient's family can change the information on the acceptability according to the degree of the acceptability determination index by using the facility ID, the facility attribute information (facility name, address, telephone number). Number, etc.), a list of first destination candidates including the degree of the current acceptance judgment index of the facility (such as the size of the vacancy rate (numerical value)), and the changed or unchanged acceptance conditions for each destination candidate And can be viewed.
The accepting
The person in charge, such as a nurse, the patient himself, and the patient's family, enter the desired acceptance list in the first candidate list, the second candidate list, and the combined candidate list output from the monitor. A candidate can be selected. In response to the selection of the destination candidate, the
The receiving
As a result, the desired reception information is transmitted to the facilities desired by the patient or the like among the facilities of the candidate reception destinations.
In the above-described processing, an example has been described in which the staff such as a nurse, the patient himself, and the family of the patient use the
The matching
(Modification)
A modified example is assumed for the above-described
FIG. 8 is a functional block diagram of a matching support device according to a modification of the embodiment.
The matching
The discharge
The discharge

An operation example in the case where the receiving facility is specified by the above-described matching support device will be described.
First, it is assumed that the recovery
FIG. 9 is a diagram showing acceptance conditions for each nursing type of a patient in a rehabilitation hospital one week after the request for matching is made. In the example of FIG. 9, the acceptance condition is indicated by the presence or absence of an empty floor. That is, the AA hospital, the BB hospital, and the CC hospital have vacant floors, so acceptance is possible (可), and the DD hospital is fully occupied, so acceptance is not acceptable (x). In addition, the AA hospital and the BB hospital can accept or not accept patients suffering from infectious diseases, and the CC hospital can accept or not accept patients who suffer from infectious diseases. It is assumed that the acceptance change availability flag is negative. In this case, the receiving
Further, it is assumed that the patient desires a hospital near his / her home as desired information. The AA hospital is located at a distance of 3 km from the patient's home, and the BB hospital is located at a distance of 10 km from the patient's home. In this case, the receiving
