Definitions
- the present invention relates to a process for the manufacture of a pharmaceutical composition in the form of extended-release tablets containing Pirfenidone (PFD), which offers advantages and a better therapeutic efficacy over other pharmaceutical forms of Pirfenidone for oral administration and therapeutic application. in the regression of chronic renal failure, breast capsular contracture and human liver fibrosis.
- PFD Pirfenidone
- Pirfenidone is a drug made up of a small molecule whose chemical name is 5-methyl-1-phenyl-2- (1 H) -pyridone. It is a synthetic non-peptide molecule with a molecular weight of 185.23 daltons. Its chemical formula is C12H-11 NO, and its structure is known.
- Pirfenidone is being clinically evaluated as a broad-spectrum anti-fibrotic drug. Pirfenidone has anti-fibrotic and anti-inflammatory properties that are reflected in its activity of decreasing the expression of TGF- ⁇ , the expression of TNF- ⁇ , PDGF and very importantly, the expression of different types of collagen.
- Phase III human studies in pulmonary fibrosis, ' chronic renal failure secondary to renal glomerulosclerosis, liver cirrhosis and breast capsular contracture are being conducted.
- Pirfenidone decreases the progressive progression of lesions in processes that occur with fibrosis. More importantly, Pirfenidone performs these actions safely and non-toxicly. In addition, it is known that pirfenidone prevents the formation of fibrotic lesions after damage to a certain organ, for example, liver, skin, kidney, etc. It is known that one of the mechanisms by which Pirfenidone exerts its therapeutic effects is by modulating the actions of various cytokines. Pirfenidone is a potent inhibitor of fibrogenic cytokines and TNF- ⁇ . It is well documented that pirfenidone inhibits excessive biosynthesis and release of several fibrogenic cytokines such as TGF- ⁇ , bFGF, PDGF, and EGF. Zhang S et al.,
- Pirfenidone is a drug that has been applied in the restoration of tissues with fibrotic lesions and for the prevention of fibrotic lesions.
- This compound is itself a known compound and its pharmacological effects have been described in, for example, Japanese applications KOKAI Nos. 87677/1974 and 1284338/1976, as an anti-inflammatory agent that includes anti-pyretic and analgesic effects .
- United States patents
- Pirfenidone New applications of Pirfenidone have been discovered, which are the subject of the present invention, demonstrating that the compound has activity in reducing the harmful effects (breast capsular contracture) observed after surgical implantation of breast implants in humans.
- Pirfenidone is also effective in the regression of chronic renal failure secondary to human primary glomerulosclerosis and liver fibrosis.
- the causes of these complications may be fluid accumulation in tissue in the implant pocket, intense inflammatory response, sub-clinical infection, patient age, foreign materials and alteration of cellular and molecular mechanisms in the area of the implantation.
- the body reacts by encapsulating it and initiating a rejection reaction (3.4) with the formation of a hypertrophic scar (5.6).
- This immune response produces cytokines and growth factors such as IL-1, IL-6, TNF-a, PDGF and TGF-b1 (7, 8).
- myofibroblasts in the capsule structure has been reported with the production of alpha-SMA (alpha smooth muscle actin), where the most seriously deformed capsules have a higher production of alpha-SMA that suggests a direct role of the Myofibroblasts activated in the development of contraction (8). It has also been shown that the number of myofibroblasts present in the tissue is proportional to the thickness of the contraction (9). To reduce the activation of fibroblasts and the contraction of the wound, infiltration of spheroids in the wound and inside the implants with minor complications has been used. These complications occur as thin skin, atrophy of tissue, stratification, blue skin and implant exposure. However, the side effects of the spheroids and other medications used are quite important and avoid their continued and prolonged use.
- alpha-SMA alpha smooth muscle actin
- Pirfenidone (5-methyl-1-phenyl-2- (1 H) -pyridone) (PFD) has proven effective in preventing fibrosis formation, both in vitro and in vivo. It inhibits lung fibrosis (10), peritoneal adhesion (1), liver cirrhosis (12, 13), uterine fibromiomas (14), kidney fibrosis (15), keloid scars (16) and retards development of tumors in the central nervous system. Pirfenidone can also inhibit, in a highly specific way, cytokines such as TNFalfa, FGF, PGDF and TGF-beta in human fibroblast by blocking the G1 phase of the cell cycle.
- cytokines such as TNFalfa, FGF, PGDF and TGF-beta
- TNF-a Tumor Necrosis Factor alpha
- the TNF family of proteins includes TNF-a, TNF- ⁇ , Fas ligand, CD40 ligand, OX-40, RANK-L (NF-kB ligand activating receptor) and TRAIL (apoptosis-inducing ligand related to TNF).
- TNF- ⁇ For historical reasons it is called TNF- ⁇ to distinguish it from TNF- ⁇ or lymphotoxin.
- TNF TNF-induced monocytes
- ⁇ TNF is the main mediator of the acute inflammatory response to gram negative bacteria and other infectious microorganisms.
- TNF membrane-associated metalloprotease
- TNF-RI TNF-RI
- TNFRII TNFRI
- the type I receptor can stimulate the gene expression of inflammatory mediators or induce apoptosis.
- TRADD Dead Domain Associated to the TNF Receptor
- TRAF-2 TNF Receptor Associated Factors
- RIP-1 Interaction Protein with the Receptor
- FADD Dead Domain Associated with FAS
- TRAF TRAF
- TNF receptors • The genes induced by TNF receptors primarily encode inflammation mediators and anti-apoptotic proteins.
- Pirfenidone As described herein and Pirfenidone has been shown to have an extremely strong and selective action to inhibit the production of TNF- ⁇ . This information is shown in Figure 6.
- TGF- ⁇ Transforming Growth Factor beta
- TGF- ⁇ was isolated from human platelets in the 1980s and was identified as a product of cells transformed by the murine sarcoma virus. It was subsequently named transforming growth factor beta due to its ability to cause a Phenotypic transformation of a culture of epithelial cells since it reversibly induced the transformation of fibroblasts.
- TGF- ⁇ The main action of TGF- ⁇ in the immune system is to inhibit lymphocyte proliferation and activation.
- TGF- ⁇ Outside the immune system TGF- ⁇ is considered the dominant stimulus in the production of extracellular matrix components, inducing fibrosis by stimulating the production of type I, III and IV collagen, fibronectin, laminin and proteoglycans. It has been increased in hepatic, pulmonary and renal fibrosis in both experimental and human models, ⁇ It is synthesized as an inactive dimeric precursor.
- Active TGF- ⁇ binds to the extracellular portion of the type II receptor.
- the ligand binding promotes intracytoplasmic autophosphorylation of ⁇ by its serine / threonine kinase activity, and in turn TpRIl phosphorylates the type I receptor, which triggers the activation of Smads (intracellular signal transducer molecules) capable of translocating to the nucleus and regulate the transcription of white genes such as Smad 7, PAI-I, collagen I, PDGF and the same TGF- ⁇ .
- Smads intracellular signal transducer molecules
- Smad2 / 3-Smad4 complexes in the nucleus can be associated with coactivators and co-repressors of transcription.
- Pirfenidone has an extremely strong and selective action to inhibit the production of TGF- ⁇ . This information is shown in Figure 5.
- One of the objectives of the present invention is to describe a process for the manufacture of a pharmaceutical composition in the form of extended-release tablets comprising from 600 milligrams to 2400 milligrams of Pirfenidone, so that the drug that contains it is bioavailable for a prolonged period of 12 hours from its administration. In this way the anti-fibrotic and anti-inflammatory action of the drug Pirfenidone is optimized.
- another objective of the present invention is that the application of extended-release tablets containing Pirfenidone is effective in the regression of chronic renal failure secondary to primary glomerulosclerosis.
- Another objective of the present invention is the anti-TNF- ⁇ and anti-TGF- ⁇ 1 action of Pirfenidone in extended-release tablets for the treatment of liver fibrosis.
- Figure 1 shows a graph of the evolution of the Breast Capsular Contracture during the study considering the percentage according to Baker's scale with respect to the number of cases. It also shows a comparison between cases treated with PFD. All cases showed significant improvement at the end of the administration of PFD. The six-month follow-up showed only one case of recurrence.
- Figure 2 shows the photograph of a patient diagnosed with bilateral Breast Capsular Contracture.
- the image on the left shows the right breast with a grade IV contracture and left breast with a grade III contracture;

- Figure 5 shows a graph with the determination of serum levels of TGF- ⁇ in female rats that were subjected to the placement of silicone implants in mammary glands in order to induce contracture development.
- Figure 6 shows a graph with the determination of serum levels of TNF- ⁇ in female rats that were subjected to the placement of silicone implants in mammary glands in order to induce the development of contracture.
- Pirfenidone and silicon dioxide are screened by mesh no. 30 in the oscillating granulator and placed in the lath mixer and mixed for 5 minutes. This mixture is made to obtain a better flow of the asset since pirfenidone does not flow very well and silicon dioxide due to its non-stick properties gives us that desired flow.
- Step 2 Microcrystalline cellulose, low viscosity hydroxypropyl methylcellulose (HPMC) and high viscosity hydroxypropyl methylcellulose (HPMC) are screened by mesh 30 in the oscillating granulator and placed in the lath mixer of step 1 and mixed for 15 minutes .
- HPMC low viscosity hydroxypropyl methylcellulose
- HPMC high viscosity hydroxypropyl methylcellulose
- Microcrystalline cellulose is a diluent that allows the tablet to obtain a good hardness despite using low compression force, in addition to producing low tablet friability.
- the mixture of low and high viscosity HPMC is the release matrix; It is in a proportion the mixture that is gradually releasing us and the asset steadily until we achieve total release at twelve hours and with this we manage to maintain a bioavailability of the asset for longer.
- the sodium stearyl fumarate is screened by mesh 30 in the oscillating granulator and placed in the lath mixer of step 1 and mixed for 3 minutes.
- Sodium stearyl fumarate is a lubricant and fulfills the function of preventing tablets from sticking to the surface of the punches, as well as reducing friction between the particles.
- the granulate obtained is tabletted in the stokes tableting machine provided with punches oblongly with the following compression parameters: Average weight: 850 mg + 5%; Hardness: maximum 10 kg F; Friability: less than 1%.
- PROLONGED RELEASE TABLETS comprising 600 milligrams of Pirfenidone
- diabetic nephropathy is the main cause of end-stage renal disease after the fourth decade of life, in the young adult there are other causes that in some way are the continuity of pathologies of childhood and adolescence, so that the population under 40 years of age that merits functional replacement by dialysis and transplantation more frequently due to primary or secondary glomerular diseases.
- the primary cause of end-stage renal disease below 16 years of age is primary glomerular diseases, and many of them are diagnosed late when their impact on conditioned damage Severe renal structural, that is, has damaged more than 60% of its nephron population and unfortunately manifests itself in advanced stages of renal failure.
- the earliest manifestation of a primary glomerular disease is proteinuria, which in most cases is documented in the range of "nephrotic syndrome". Following this generally progressive finding, we usually observe systemic arterial hypertension and finally progressive functional failure.
- intraglomerular hypertension and hyperfiltration There are several mechanisms responsible for the progression of damage, one of them is intraglomerular hypertension and hyperfiltration, they initiate a disorderly response that induces glomerulosclerosis and destruction of nephrons.
- the increase in intraglomerular hydrostatic pressure can cause an increase in mesangial cell contraction and an increase in TGF- ⁇ production.
- cytokines and growth factors include vasoactive agents such as angiotensin II, thromboxanes, endothelin- and cytokines of type IL- and TNF-cc.
- vasoactive agents such as angiotensin II, thromboxanes, endothelin- and cytokines of type IL- and TNF-cc.
- massive and persistent proteinuria is considered to be an independent factor that contributes to progression in all forms of chronic kidney disease.
- Excess protein filtration within the proximal tubule and its subsequent endocytosis may represent a pathological mechanism in the development of renal fibrosis.
- a possible scenario to explain the relationship between the degree of proteinuria and the progression to renal insufficiency may be related to the cells of the tubular epithelium, which, when endocytizing the proteins, chemotactic factors such as monocyte (MIP-) are activated and overexpressed.
- MIP- monocyte
- an antifibrotic especially if it reduces the expression of TGF- ⁇ and TNF-, would be useful for treating patients who are shown to have early stages of glomerular nephropathy. This would result in the arrest of the progressive and gradual deterioration observed in these patients and avoid reaching the transplant early.
- Pirfenidone has been shown to:
- Reduce proteinuria levels Reduces the levels of urea nitrogen and serum creatinine.
- TGF- ⁇ expression was also reduced.
- Pirfenidone is an anti-fibrotic agent, which has proven effective in vitro and in vivo for the prevention and resolution of fibrous tissue accumulation in experimental models of:
- Liver fibrosis The use of the drug in humans has been with the same objective, aimed at different devices and systems. There are two reports in 2003 and 2006 by Dr. Armendariz and collaborators with the results obtained from Pirfenidone in patients with liver fibrosis, a study that currently has increased its population and has shown beneficial effects translated into liver biopsy.
- Pirfenidone was also evaluated by Cho Me and collaborators in 2007 in patients with nephrotic syndrome and focal and segmental glomerulosclerosis with the aim of delaying the fall in renal function, obtaining favorable results.
- the Institute of Renal Digestive Diseases and Diabetes as well as the Health Institute of the American Union provide information regarding the use of pirfenidone in two studies authorized by the FDA, one of them in patients with focal segmental glomerulosclerosis at various stages of renal damage and the second in patients with diabetic nephropathy. JUSTIFICATION
- CRF chronic renal failure
- the renal damage and its progression can be studied by laboratory methods aimed at measuring the magnitude of proteinuria, renal function evaluated by creatinine clearance, reciprocal creatinine (1 / creatinine) and cystatin C and with the comparative findings of Doppler ultrasound in which we observe anatomical, echo-sonographic, corticomedular changes, flow velocity to different renal areas and resistance indices;
- the golden rule to evaluate the magnitude of the damage is the histological study which expresses the affected structures and the extent of the fibrosis. Not all of these studies can be carried out routinely, since renal biopsy being an invasive procedure would only be justified repeatedly in those who have a first procedure to compare the findings.
- laboratory and cabinet methods can be carried out with a defined schedule of activities.
- Pirfenidone positively influenced the rate of progression of renal damage (phases 1-4 according to the KDIGO Classification) in patients with chronic renal failure secondary to primary nephropathy.
- Pirfenidone in renal function was monitored by cystatin C, reciprocal plasma creatinine and creatinine clearance in 24-hour collection.
- Pirfenidone stops histological damage in the affected kidneys by analyzing comparative ultrasonic changes at the beginning and end of treatment.
- capsular contracture has not been fully understood.
- Different publications mention a variable incidence of capsules ranging from 0% to 74% (1), depending on the type of implant, the surface (textured or smooth), and the implantation or anatomical site (subglandular or subpectoral) (2).
- the causes of these capsules could be the accumulation of fluids in the tissues of breast pocket implants, intense inflammatory response, subclinical infection, the patient's age, materials, and the alteration of cells and molecular mechanisms in the area of implantation.
- IL-1 interleukin-1
- TGF- ⁇ tumor necrosis factor- ⁇
- TGF- ⁇ transformation factor Growth- ⁇
- Pirfenidone (5-methyl-1-phenyl-2- [1 H] pyridone) (PFD), a new anti-fibrotic agent, has proven effective both in vitro and in vivo, preventing and eliminating fibrous tissue in experimental models of pulmonary fibrosis (10), peritoneal adhesions (1 1), experimental and human liver cirrhosis (12, 13), uterine fibroids (14), renal fibrosis (15), and scars (16).
- PFD is able to inhibit fibroblast growth factor and TGF- ⁇ produced by human fibroblasts.
- Pirfenidone administered orally in female rats with silicone implants in the mammary gland prevents the development of characteristic fibrosis in the mammary capsule that ultimately causes the painful contracture (Mat ⁇ as Gancedo, Aesthetic Plastic) Surgery 2008). Since breast implants induce fibrosis and inflammation, and PFD has been shown to have antifibrotic and anti-inflammatory properties in different Pathologies and organs, one of the central objectives of this invention is to know the pharmacological properties of oral Pirfenidone in the post-surgical evolution of patients after the placement of breast implants.
- CC capsular contracture
- SNPs single nucleotide polymorphisms
- TGF-p1 Growth Transformation Factor- ⁇
- This study aims to provide another therapeutic approach of the Breast Capsular Contracture, in order to implement a treatment that reduces or eliminates the use of medications or surgical procedures (capsulotomy, capsulectomy) described so far for its management, achieving a lower recurrence rate and thus improving the patient's quality of life.
- TGF- ⁇ Genetic Markers
- the statistical analysis included 20 cases, taking into account that 3 patients presented bilateral capsular contracture with different grade in each breast; analyzing a total of 20 breasts.
- Fibrous implant capsule Capsul fibrous capsule.
- Capsular Contracture Capsule Thickness> _ 1.5 mm. According to Stavros, and 1.42 mm. According to Alón Zahavi.
- Pirfenidone was prepared according to the quantitative Formula for prolonged release described previously and before being converted into tablets, it was used to carry out the experiments described below in order to demonstrate its anti-TNF- ⁇ and anti-action TGF- ⁇ .
- Pirfenidone selectively inhibits NF-kfi by dramatically decreasing the activation of genes that code to produce pro-inflammatory proteins (TNF-a and TGF- ⁇ ).
- TGF- ⁇ serum levels of TGF- ⁇ in female rats that were subjected to the placement of silicone implants in mammary glands in order to induce the development of contracture is shown in Figure 5.
- the animals were subjected to a regimen Pirfenidone daily orally for 21 days.
- the same graph shows that animals with chronic liver damage due to the hepatotoxic CCI4 (carbon tetrachloride) have a lower serum level of TGF- ⁇ when treated with Pirfenidone.
- TGF- ⁇ levels were determined by a commercial ELISA Kit.
