- a first hip replacement may involve fitting a prosthetic cup socket to the acetabulum and resurfacing the femoral head before fitting a femoral head prosthesis.
- Resurfacing of the femoral head typically involves the insertion of a guide pin through the femoral head. The guide pin is then used to align cutting equipment during resurfacing of the head of the femur.
- a surgeon may use a jig which is placed onto the femur and adjusted by means of screws until the jig is attached to and aligned upon the femur. The surgeon then applies the drill to the jig which guides the drill bit into the head of the femur at the selected site.
- a jig is typically constructed so as to direct the drill to contact the femoral head at a specific angle of attack, which is appropriate for a femoral head and neck of 'normal' shape. However, this does not easily allow a surgeon to alter the angle of attack which may be appropriate where the head and/or neck are of atypical shape or slight valgus.
- Jigs may include a plethora of components such as complicated moving platforms and outriggers making them expensive to produce, difficult to assemble and also complicated to use.
- jigs can therefore make it difficult for the surgeon to maintain alignment of the jig before and during drilling. In some cases, the surgeon may require two hands to hold the jig in alignment while also attempting to operate the drill.
- the present invention provides an apparatus for alignment of a drill bit for drilling of a femoral head said apparatus comprising at least one guide hole for passage of a drill bit and one or more visual indicators to assist in alignment of the apparatus and the drill bit with respect to the femoral head.
- the at least one guide hole may be surrounded by a tubular projection.
- the one or more visual indicators may include at least one arm.
- the visual indicators may include two or more offset arms.
- the arms may be radially offset by about 90° from each other.

- the cap is curved having a convex outer surface 16 and a concave bone-contacting inner surface 18 beneath, which is conformed so as to fit around at least part of a femoral head as shown in Figures 3, 4 and 5.
- At the center of the cap is located a central hole 20 which is surrounded by four offset holes 22.
- Each hole is surrounded by a tubular projection 24 which extends outwardly from the outer surface of the cap.
- the four offset holes are each directed at an angle which is offset by approximately 10 degrees with respect to the angle of the central hole.
- Each hole is of a width appropriate to allow passage of a drill bit but may alternatively receive a fixation pin or screw.
- Visual indicators in the form of two legs 26, 27 extend outwardly and downwardly from the cap. These legs provide a visual indication of the alignment of the apparatus when the apparatus is located on a femoral head as shown in Figures 3, 4 and 5. Each leg is of approximately 40-45 mm in length is shaped to conform with at least part of a femoral head as best shown in Figures 3, 4 and 5. The illustrated legs are radially offset by approximately 90° to each other as best shown in Figure 2. The legs facilitate visual assessment of an appropriate alignment of the apparatus on a femoral head as will be described.
- the legs of the apparatus may also include slots or other shaped apertures (not shown) to allow the surgeon to see alignment markings on the bone while the apparatus is placed onto the femur.
- the surgeon has exposed the femoral head 12 for resurfacing and applied the apparatus 10 to the femoral head.
- the surgeon views the femoral head from with apparatus 10 applied from the side, and adjusts the position of the apparatus 10 to align leg 27 generally along the mid line of the femoral neck.
- the surgeon changes their viewing position to view the apparatus from the direction of leg 26 (see figure 4).
- the surgeon now adjusts the attitude of the apparatus 10 to align leg 26 generally along the mid line of the femoral head (see figure 4).
- the surgeon then again goes back to view the apparatus as shown in figure 3 to ensure that the leg 27 is still aligned, and adjusts the position of apparatus accordingly.
- both of legs 26 and 27 are aligned, then this means that hole 20 is aligned centrally with the femoral head.
- the surgeon will make markings on the bone prior to fitting the apparatus, and will line up the apparatus with the markings by aligning the apparatus so that the markings are aligned with the apertures.
- the surgeon then chooses which hole will be used for passage of the drill bit and may use fixing pins or screws to attach the apparatus to the femur using any of the other holes before drilling commences.
- fixing pins or screws is optional such that the surgeon may hold the apparatus on the head of the femur using a single hand while operating the drill with the other hand.
- the surgeon may choose one of the five holes 20, 22 with respect to the positioning of the apparatus and any valgus present.
- the drill is applied to the femoral head through the selected hole on the apparatus.
- the apparatus is maintained in position until the drilling is completed and a guide wire is inserted into the bore hole.
- the apparatus can be used in relation to the femoral head in a variety of angular orientations. That is to say, either of the legs 26, 27 do not have to be aligned with specific surfaces of the femur.
- the device may be used in various orientations to suit the particular circumstance. For instance, if the femur is accessed from the front, or from the rear, then alternative orientations may be used in either case. Referring to figure 5, the device is shown in an alternative orientation to that shown in figures 3 and 4.
- Apparatus may be easily held in alignment by a surgeon using a single hand during drilling operation.
- Integrally formed apparatus does not require assembly by the end user and is easy to sterilise.
- Integrally formed apparatus may be produced at a lower cost than multipart systems

Abstract
La présente invention concerne un appareil pour l'alignement d'un foret destiné à percer une tête fémorale, ledit appareil comprenant au moins un trou de guide pour le passage d'un foret et un ou plusieurs indicateurs visuels pour aider à l'alignement de l'appareil et du foret par rapport à la tête fémorale. L'invention porte en outre sur des méthodes d'utilisation du dispositif.
