Abstract

A system for monitoring health outcomes and outcomes including: a user database for retaining information relevant to a plurality of users; a patient record database operatively associated with the user database for retaining information relevant to a plurality of patients, the operative association including an association of each patent with one or more users and each user with one or more patients; a data analysis module operatively associated with the user database and patient record database for generating one or more reports; a user interface operatively associated with the user database, patient record database and data analysis module for entering user and patient information and generating the or each report, at least one of the reports containing survival information of patients selected according to predetermined criteria; and wherein the survival information is used to produce a survival curve using a statistical estimator operable on a survival function using survival analysis according to inferential statistics and/or hypothesis testing.

Description

METHOD OF AND SYSTEM FOR MONITORING HEALTH OUTCOMES
FIELD OF INVENTION
[0001] THIS INVENTION relates to a method of and system for monitoring health outcomes. The invention has particular application to a web-based system which provides for surgeons to assess their individual performance against the average performance of their peers. However, the invention also has application to generally to monitoring or other medical praxes.
BACKGROUND ART
[0002] It can be difficult for surgeons to know whether their individual performance is effective or ineffective, improving or deteriorating. There are systems which provide surgeons and other medical practitioners with feedback based on morbidity and mortality. But these measures are related to the community, not the individual surgeon, and therefore provide at best only an indirect indication of performance. There are systems which provide feedback to medical practitioners on performance of procedural tasks, but these are limited to feedback on each task in and of itself, and do not provide practitioners with feedback on the outcomes of the health of their patients which are expected to emerge as a result of their medical interventions. Moreover, such systems do not accommodate such non-procedural or ancillary information pertinent to a patient's healthcare outcomes, such as age.
[0003] The present invention aims to provide a method of and system for monitoring health outcomes which alleviate one or more of the shortcomings of the prior art. Other aims and advantages of the invention may become apparent from the following description.
DISCLOSURE OF THE INVENTION
[0004] With the foregoing in view, this invention in one aspect resides broadly a system for monitoring health outcomes including:
a user database for retaining information relevant to a plurality of users;
a patient record database operatively associated with the user database for retaining information relevant to a plurality of patients, the operative association including an association of each patent with one or more users and each user with one or more patients;
a data analysis module operatively associated with the user database and patient record database for generating one or more reports; a user interface operatively associated with the user database, patient record database and data analysis module for entering user and patient information and generating the or each report, at least one of the reports containing survival information of patients selected according to predetermined criteria; and wherein the survival information is used to produce a survival curve using a statistical estimator operable on a survival function using survival analysis according to inferential statistics and/or hypothesis testing.
[0005] In another aspect, the present invention resides broadly in a method of monitoring health outcomes including:
entering user information into a user database operable for retaining information relevant to a plurality of users;
entering patient information into a patient record database operatively associated with the user database and operable for retaining information relevant to a plurality of patients;
operatively associating each patent with one or more users and each user with one or more patients;
analysing the user and patient information selected according to predetermined criteria; generating one or more reports, the or at least one of the reports containing survival information of patients selected; and
using the survival information to produce a survival curve by way of a statistical estimator operable on a survival function through survival analysis according to inferential statistics and/or hypothesis testing.
[0006] Preferably, the survival curve is produced according to Kaplan-Meier estimation as disclosed in Kaplan, E. L. & Meier, P. (1958) "Nonparametric estimation from incomplete observations" Journal of the American Statistical Association 53: 457-481.
[0007] The method preferably includes generating the one or more reports from the analysis of the information by way of a data analysis module operatively associated with the user database and patient record database to produce survival data. In such form, the survival data analysis is produced using the Kaplan-Meier estimator whereby Kaplan-Meier survival curves may be produced and rendered into diagrams of a form suitable for display on a computer terminal. It is further preferred that the method further includes monitoring changes in reported information using cumulative summation analysis of the user and patient information.
[0008] Preferably, the system includes a discussion forum database operatively associated with the patient record database. The discussion forum database preferably takes the form of, or includes, a bulletin board, wiki or such like. Preferably, the analysis of the information is performed by way of a data analysis module operatively associated with the user database and patient record database for generating the one or more reports. Preferably, the entering of user and patient information in the method of the invention is performed by way of a user interface operatively associated with the user database, patient record database and data analysis module.
[0009] The user and patient databases may be of a kind found in other information retrieval systems. However, the data analysis module makes use of information from the patient database to produce survival data, preferably using a Kaplan-Meier estimator. In such form, the data analysis module produces Kaplan-Meier survival curves and renders them into diagrams of a form suitable for display on a computer terminal using, for example, hypertext pre-programming (php).
[0010] It is further preferred that the data analysis module includes functionality to calculate follow-up times to assist the user (practitioner) to service their patients. Preferably, the Kaplan- Meier estimator is used to provide this functionality.
[0011] It is also preferred that access to the information in the databases be limited in such a way that individual users may compare the health outcomes of their own patients or clients against the health outcomes of the patients of all of the other users, but without being able to access other users' information, particulars or details of their performance. Moreover, the information reported to individual users relating to the health outcomes the patients or clients of other users is preferably restricted so as to preserve patient confidentiality. The user may thereby analyse the outcomes of their practice against the average performance of all of the other users.
BRIEF DESCRIPTION OF THE DRAWINGS
[0012] In order that the invention may be more readily understood and put into practical effect, a preferred embodiment of the present invention will now be described with reference to the following drawings, and wherein: Fig. 1 is a diagrammatic representation of a system for monitoring health outcomes according to the invention.
DETAILED DESCRIPTION OF THE DRAWINGS
[0013] The system 10 shown in Fig. 1 includes a login module shown generally at 30 which includes a step shown at 11 where the user visits a website, a user login box 12, a registration display box 14, a subscription management display 16, a login validity check 13 and a user subscription check 15. The user logs into the website at 11 whereupon a user login is displayed. The login is check for validity against a user record in a user record date base 17. If the user is subscribed the system permits access to a display menu 18 associated with a selection module 19 which permits a selection of a patient records subprogram 20, a discussion forum subprogram 21 , or a display report menu 24.
[0014] The patient record sub program 20 permits the user to correct or edit records of patient details, the information being stored in a patient record database 22. The discussion forums sub program allows the user to enter discussions, such as by hypertext pre programming, bulletin board type discussions forums. Information relating to the forums is stored in a discussion forum database 23. It can be seen that the patient records sub program is operatively associated with patient record database. It can also be seen that the discussion forum sub program is operatively associated with the discussion forum database. There is also operative interaction between the patient record database and the discussion forum database, as well as an operative association between the user record database and the patient record database.
[0015] If the user selects the report menu option at 24, then the system permits the user to select a report at a report selection query at 25. This then permits that system to proceed to a statistical reports sub program 26. The statistical reports sub program is operatively associated with the patient record database, and is operable to generate reports from the patient data, user record data and the like as required, depending on the kind of report request by the user. The user makes a selection of the report at a user selection point 27 in order to print patients' statistical graphical reports, an example of which is shown typically at 28. The reports according to the invention include information concerning survival of patients in order to monitor the performance of surgery, the reports including Kaplan-Meier curves calculated from the survival data. A surgeon his or her own health outcomes according to the present invention and make a comparison with stochastic information concerning the general performance of surgery in a predetermined area or according to predetermined selection criteria.
[0016] The discussion forum can be used to initiate enquires, enter comments or answers to queries and generally elevate the understanding in the selected fields of endeavor.
[0017] Although the invention has been described with reference to a specific example, it will be appreciated by those skilled in the art that the invention may be embodied in other forms within the broad scope and ambit of the invention as herein set forth and provisionally defined by the following claims.

Claims

1. A system for monitoring health outcomes and outcomes including:
a user database for retaining information relevant to a plurality of users;
a patient record database operatively associated with the user database for retaining information relevant to a plurality of patients, the operative association including an association of each patent with one or more users and each user with one or more patients;
a data analysis module operatively associated with the user database and patient record database for generating one or more reports;
a user interface operatively associated with the user database, patient record database and data analysis module for entering user and patient information and generating the or each report, at least one of the reports containing survival information of patients selected according to predetermined criteria; and
wherein the survival information is used to produce a survival curve using a statistical estimator operable on a survival function using survival analysis according to inferential statistics and/or hypothesis testing.
2. The system according to Claim 1 , wherein a discussion forum database is operatively associated with the patient record database.
3. The system according to Claim 1 , including a user interface operatively, patient record database and data analysis module each associated with the user database for entering of user and patient information.
4. A method of monitoring health outcomes and procedural outcomes including:
entering user information into a user database operable for retaining information relevant to a plurality of users;
entering patient information into a patient record database operatively associated with the user database and operable for retaining information relevant to a plurality of patients; operatively associating each patent with one or more users and each user with one or more patients; analysing the user and patient information selected according to predetermined criteria; generating one or more reports, the or at least one of the reports containing survival information of patients selected; and using the survival information to produce a survival curve by way of a statistical estimator operable on a survival function through survival analysis according to inferential statistics and/or hypothesis testing.
5. The method according to Claim 4, including generating the one or more reports from the analysis of the information by way of a data analysis module operatively associated with the user database and patient record database to produce survival data.
6. The method according to Claim 5 , wherein the survival data analysis is produced using a Kaplan-Meier estimator whereby Kaplan-Meier survival curves may be produced and rendered into diagrams of a form suitable for display on a computer terminal.
7. The method according to any one of Claims 4 to 6, and including monitoring changes in reported information using cumulative summation analysis of the user and patient information.
8. The method according to and one of Claims 4 to 7, including limiting access to the information in the databases in such a way that a user may compare his health outcomes against all of the other users, but without being able to access other users' information, particulars or details of their performance.
9. The method according to and one of Claims 4 to 8, wherein the monitoring of the health outcomes and procedural outcomes is provided in real time.
10. The method according to and one of Claims 4 to 9, wherein the monitoring of the health outcomes and procedural outcomes is provided over a network of computers.
11. The method according to Claim 10, wherein the network of computers is web-based.
