Beschreibungdescription
Gelenk-Pfanne für Hüftgelenk-EndoprothesenJoint socket for hip joint endoprostheses
The invention relates to a pressfit cup for hip joint endoprostheses according to the preamble of claim 1.
In hip joint endoprostheses, the hip joint socket of the pelvic bone is replaced by an artificial socket implanted in the bone. It is known to use either a screw pan or a Pfessfit pan.
Screw pans have an external thread on the outer surface, which forms the contact surface with the bone, with which they can be screwed, preferably self-tapping, into a corresponding milling of the pelvic bone.
Pressfit pans are pressed into a corresponding cutout in the pelvic bone and held in a snug fit. In addition, the surface can have a structure to ensure that the press-fit socket is anchored in the cutout. The press fit cup can exert pressure on the surrounding bone through the fit, which can lead to damage to the bone tissue, which can impair the growth of the bone to the pressfit cup or even cause the pressfit cup to loosen.
The elasticity of the pelvic bone also leads to forces acting radially on the press-fit socket in the pelvic bone plane when the load changes. These forces can also cause the pressfit pan to loosen. From WO 00/74604 AI acetabular cups are known which have a conical outer contour. Furthermore, this document discloses an articular socket which has a conical outer contour in the equatorial region, to which a spherically formed pole region adjoins.
Die EP 0 381 351 Bl offenbart eine Hüftgelenkspfanne mit einer Außenflächenkontur, welche sich in zwei im wesentlichen sphärische Oberflächenbereiche mit zwei unterschiedlichen Radien unterteilen lässt.EP 0 381 351 B1 discloses an acetabular cup with an outer surface contour which can be subdivided into two essentially spherical surface areas with two different radii.
A hip joint socket is known from EP 0 732 903 B1, the convex outer surface of which corresponds to the surface of a half ellipsoid of revolution.
A disadvantage of such curved or conical press-fit pans is that they partially convert a force caused by the elastic pelvic movement in the plane of the pelvic bone through the outer surface of the press-fit pan inclined towards the force into an axial force which is in the direction of pressing out the Pressfit pan from the pelvic bone works.
The object of the invention is to provide a pressfit cup for hip joint endoprostheses, which ensures improved anchoring of the pressfit cup in the pelvic bone.
This object is achieved according to the invention by a pressfit cup for hip joint endoprostheses with the features of claim 1. According to the invention, the pressfit pan has an outer surface which is cylindrical in shape in the equatorial edge region of the pressfit pan, at least one conical section adjoining this region.
Advantageously, a conical section, which is flattened in the pole area, is sufficient to achieve the desired effect.
The elastic deformation of the pelvic bone essentially causes radially acting forces in the plane of the pelvic bone. In the case of a conical pressfit socket, the outer surface of the pressfit socket, which is inclined against the force, partially converts this into an axial force, which can result in the pressfit socket being pressed out of the pelvic bone and thus loosened. In order to reduce this effect, the equatorial edge region of the pressfit cup is cylindrical in accordance with the invention, since in this case a force acting in the plane of the pelvic bone on the surface of the pressfit cup consists exclusively of a radial force component that comes from the pressfit cup can be taken up without straining the anchoring of the Pressfit cup in the cutout of the pelvic bone.
In an advantageous embodiment of the invention, the outer surface of the pressfit cup is structured, preferably roughened or has projections in order to additionally secure the pressfit cup against loosening in and pushing it out of the pelvic bone. The pressfit pan is preferably made of metal, particularly preferably the pressfit pan is made of a biocompatible metal such as titanium or a titanium alloy.
The inner recess of the press-fit socket, into which the bearing shell for the joint head is inserted, is advantageously conical, so that the bearing shell can be held in the socket by means of a self-locking cone effect.
An embodiment of the invention is explained with reference to the following figure.
Es zeigtIt shows
Figur 1 einen halben Axialschnitt durch eine erfindungsgemäße Pressfit-Pfanne .Figure 1 shows a half axial section through a pressfit pan according to the invention.
The equatorial edge region 30 is adjoined by two regions 20 and 25, in which the outer surfaces of the pressfit socket are conically shaped. The conical region 20 directly adjoining the equatorial edge region 30 has a cone angle of approximately 20 °. Advantageous values for the cone angle of the first conical region 20 are between 10 ° and 40 °. The first conical region 20 is adjoined by the second conical region 25, which has a cone angle of approximately 75 °. The cone angle of the second conical region is advantageously between 50 ° and 85 °. A flattened pole region 50 of the press-fit socket adjoins the second conical region 25. The pressfit socket 10 has an inner recess 40 for an insert, not shown, which receives the joint head. The inner recess 40 is formed in a manner known per se with a conical surface, the conical angle of which can be selected in accordance with the material pairing of press-fit socket 10 and insert in such a way that a self-locking conical effect holds the insert in press-fit socket 10. It is also possible that the inner recess 40 has a surface consisting of several conical sections.
