Abstract
本發明公開了一種用於治療PIK3CA突變癌症的組合物療法,包括向有需要的物件施用PI3K(磷酸肌醇3-激酶,Phosphoinositide 3-kinase)抑制劑、前列腺素受體(prostaglandin E2,PGE2)抑制劑和PD-1抑制劑。本發明的三種聯合用藥方法所帶來的抗腫瘤活性顯著優於PI3K抑制劑和PD-1抑制劑以及前列腺素受體(PGE2)與PD-1抑制劑兩種聯合用藥的活性。
Description
本申請要求於2020年11月30日提交中國專利局、申請號為202011379205.7、發明名稱為“用於治療PIK3CA突變癌症的組合療法”的中國專利申請的優先權,其全部內容通過引用結合在本申請中。
本發明涉及一種組合治療方法及藥物組合物和/藥物組合,包括向有需要的物件施用PI3K抑制劑、前列腺素受體(PGE2)抑制劑和PD-1(programmed cell death 1)抑制劑。
使用抗CTLA4抗體和抗PD(L)1抗體的免疫檢查點抑制劑(Immune checkpoint inhibitor,ICI)給癌症免疫治療帶來了深刻的變革。自從2001年第一個免疫檢查點抑制劑(ICI)獲批以來,這一系列的藥物由於在複雜癌症上持久的臨床效果引起了廣泛的關注。然而,仍然有大量的癌症患者對於ICI治療不回應。到目前為止經ICI治療的癌症患者的響應率約為20%,所以找到一種更加有效的免疫治療方式來進一步利用免疫系統治療癌症是非常有必要的。
泛I型P13K抑制劑
PI3K-AKT-mTOR信號通路失調在癌症患者中的發生率很高,其主要調控細胞生長,增殖,代謝等關鍵的細胞過程,在癌症患者中能夠促進癌症細胞的存活,擴張和傳播。由PIK3CA基因編碼的PI3K異構體p110α是人類癌症中排名第二的最容易突變的
原癌基因。
PI3K家族中,PI3Kβ同樣也參與了腫瘤發生,但PI3Kα起著主要作用,除此之外,另外兩個異構體PI3Kγ和PI3Kδ則對免疫系統起著重要調控作用。PI3Kδ能夠調控並維持調節性T細胞(Regulatory T cell,Treg)的功能,PI3Kγ不僅能夠募集抑制性的髓系細胞到癌症微環境中,而且能夠增強它們對於T細胞抗癌症作用的抑制能力。
式I結構的化合物(AN2025)及其藥學上可接受的鹽是口服泛I型PI3K抑制劑,目前已經在臨床三期研發階段,其適應症為頭頸鱗癌。它不僅能夠抑制野生型的PI3α,而且能夠抑制突變型的P13Kα,突變位點包括(H1047R,E542K and E545K)。同時,其對於其他PI3K異構體(PI3Kβ,PI3Kγ,PI3Kδ)也具有抑制活性。AN2025已經在臨床上證實了其對於PI3K/PIK3CA突變導致乳腺癌的發生是有效的,其回應率最高的患者都具有PIK3CA突變(Campone M et al.Eur J Cancer.2018;103:147-154)。AN2025同樣可以通過抑制PI3Kδ和PI3Kγ來下調Tregs和抑制性髓系細胞功能以及其進入腫瘤微環境的能力(O'Donnell JS et al.Semin Cancer Biol.2018;48:91-103.Borazanci et al.The Oncol.2020)。
EP-4抑制劑
在癌症中,前列腺素E2(PGE2)和其亞型4受體(EP-4)組成的信號通路,不僅能夠促進癌症的發生,而且能夠形成促進癌症發展,免疫抑制的癌症微環境。T細胞中PGE2/EP-4信號通路啟動產生的cAMP能夠啟動cAMP-PKA信號通路,這個過程能夠有
效地抑制T細胞的功能。PGE2/EP-4信號通路也能夠負調控DC的成熟,誘導包括M2巨噬細胞以及髓源抑制性細胞(myeloid derived suppressor cells,MDSC)在內的多種免疫抑制細胞的形成(Tomić S et al.Front Immunol.2019;10:475)。
式(II)結構的化合物(AN0025)及其藥學上可接受的鹽是一種具有高度選擇性的,針對E型前列腺素受體4的小分子抑制劑。在臨床前研究中,AN0025通過抑制PGE2/EP-4信號通路,能夠在腫瘤微環境中有效的減少免疫抑制細胞。AN0025已經運用到多個臨床策略中。在針對多種晚期實體瘤FIH(first in human)I期臨床實驗中,AN0025已經展現出了一定的單藥藥效;並且在結直腸癌的新輔助治療中,AN0025與放化療聯用表現出很強的臨床藥效(Wyrwicz et.al,Poster # 540,ESMO2019)。目前,AN0025與抗PD-1抗體(pembrolizumab)已經在多個實體瘤中進入臨床1b/2階段(NCT04432857)。臨床前以及早期臨床資料支援AN0025作為一個免疫調節劑,通過調控M2巨噬細胞,MDSC以及T細胞來對抗免疫抑制的癌症微環境。
PD-(L)1(細胞程式死亡-配體1,Programmed cell death 1 ligand 1)抗體
PD-1和PD-L1信號通路在調節T細胞啟動過程中起到關鍵作用。PD-L1主要在免疫細胞以及大多數的人癌症細胞中表達。在腫瘤微環境中,癌症PD-L1與T細胞上的PD-1相互作用,最終抑制T細胞的癌症細胞殺傷活性。大量抗PD-(L)1治療的成功案例,使得它成為現代腫瘤免疫治療的基石。
AN2025和AN0025能夠系統性地調節免疫抑制的腫瘤微環境,這與抗PD(L)1治療殺傷癌症的機制是互補的。通過AN2025靶向PIK3CA突變能夠增加額外的作用機制,使得病人能夠獲益于這種三聯組合帶來的免疫調節活性。這種三聯組合為大多數晚期實體瘤患者,特別是PIK3CA突變的晚期實體瘤患者提供了理想的治療選擇。
本發明所要解決的技術問題在於,解決現有技術中經ICI治療的癌症患者的回應率低的問題。本發明的目的在於提供了一種更加有效的免疫治療方式來進一步利用免疫系統治療癌症。
鑒於此,本發明提供了一種PI3K抑制劑、前列腺素受體(PGE2)抑制劑以及PD-1抑制劑三者聯用來治療癌症的方法,所述方法的抗腫瘤活性顯著優於任意兩兩組合。
具體而言,本發明提供了如下技術方案:
一種在有此需要的物件治療腫瘤/癌症和/或產生針對腫瘤/癌症的記憶免疫應答的方法,包括向所述物件施用有效量的PI3K抑制劑、前列腺素受體(PGE2)抑制劑以及PD-1抑制劑,其中,所述PI3K抑制劑、前列腺素受體(PGE2)抑制劑和所述PD-1抑制劑可以作為單一制劑(Dosage form)同時、分開或者順序施用。

在一些實施方案中,抗體以有效量施予受試物件。有效量通常是每天0.01mg/kg體重至500mg/kg體重。在一些實施方案中,藥學上可接受的組合物可經過配製,使得可以向接受這些組合物的患者施用每天0.01mg/kg體重至200mg/kg體重或0.01mg/kg體重至100mg/kg體重的化合物的劑量(例如基於75kg的人,劑量為0.75mg至7.5g或15g)。在某些實施方案中,本發明的組合物經過配製,以提供0.01mg/kg至70mg/kg的劑量(例如基於75kg的人,劑量為0.75mg至5.25g)。

抗體的劑量可以在治療過程中每週一次、兩次、三次、四次、五次或更多次、每週一次、每兩週一次、或甚至每三週一次施用。給藥時間可以是每天一次、每兩天一次、每三天一次、每四天
一次、每五天一次、每週一次、每兩週一次或每三週一次。可以製備包含抗體的製劑,使得可以容易地且便利地分配任何劑量。
術語“物件”是指哺乳動物物件,並且尤其是人類物件,包括雄性或雌性物件,並且包括新生兒、嬰兒、幼兒、青少年、成人或老年人物件,並且進一步包括各種人種和種族,例如高加索人種、非洲人種和亞洲人種。本文中,所述對象患有腫瘤/癌症。在一個實施方案中,不考慮所述腫瘤患者是否表達PD-L1。在另一個實施方案中,所述腫瘤患者是PD-L1表達陽性的腫瘤患者。

術語“腫瘤”和“癌症”在本文中可互換使用,表示機體局部組織的失控的異常增生。如果不及時進行醫療介入,腫瘤/癌症可以不受控制地生長,並有可能轉移到機體其他位置,最終造成機體死亡。

“轉移性癌症”是指其中來自一個器官或身體部分的癌細胞已經(通過“轉移”)擴散到另一個非相鄰器官或身體部分的癌症。非相鄰器官或身體部分處的癌症(“繼發性腫瘤”或“轉移性腫瘤”)包括來源於癌症或癌細胞已經從該器官或身體部分擴散的器官或身體部分的癌細胞。可以發生繼發性腫瘤的部位包括(但不限於)淋巴結、肺、肝、腦和/或骨頭。
“PIK3CA突變癌症”是指癌症細胞中PIK3CA基因由於核酸堿基突變導致的氨基酸變化,這一變化最終導致PI3K細胞傳導信號的異常,突變位元點包括但不限於H1047R,E542K and E545K。
本文中,術語“可藥用”和“藥學上可接受”可互換使用,是指製藥領域技術人員一般接受的類型。例如可藥用鹽、可藥用載體等。

本發明所述的式I或式II化合物作為口服製劑施用時,其優選地被膜包被。適宜膜為本領域已知且可自市場購得或可根據已知方法來製造。通常,膜衣材料是親水聚合物,例如聚乙二醇、聚乙烯吡咯啶酮、聚乙烯醇、羥丙基纖維素、羥甲基纖維素和羥丙基甲基纖維素等。膜衣組合物成份可包括常用量的增塑劑,例如聚乙二醇、檸檬酸三乙酯、鄰苯二甲酸二乙酯、丙二醇、甘油;以及遮光劑,例如二氧化鈦和著色劑,例如氧化鐵,鋁色澱等。通常,膜衣材
料是以例如可提供介於固體口服劑劑型的1%至6%範圍內的膜衣的量來施加。
本發明式I或式II化合物作為口服製劑施用時,可任選地進一步包含至少一種用於藥物的藥學上可接受的載體。“藥學上可接受的載體”是指實質上不具有生物活性且構成製劑的實質性部分的任何藥學上的惰性材料。這些載體的實例包括但不限於稀釋劑和填充劑、崩解劑、助流劑、黏合劑、穩定劑、著色劑、增味劑和防腐劑。
作為稀釋劑,可以選擇的是例如但不限於:微晶纖維素、甘露醇、粉糖、可壓制糖、葡聚糖、糊精、石旋糖、乳糖、纖維素粉末、山梨醇、蔗糖和滑石粉或其組合。其中,稀釋劑可以基於口服製劑總重量的5%至90%,優選為10%至80%,20%-70%,30%-60%,40%-50%。
作為崩解劑,可以選擇的是例如但不限於:纖維素、藻酸鹽、樹膠、交聯聚合物,例如交聯聚乙烯吡咯啶酮或交聚維酮、交聯羧甲基纖維素鈉、交聯羧甲基纖維素鈣、大豆多糖、澱粉羥乙酸鈉、瓜爾膠或其任意組合。
其中,稀釋劑可以基於口服製劑總重量約1%至15%,優選為2%至10%的量存在。
作為黏合劑,可以選擇的是例如但不限於澱粉、纖維素或其衍生物,例如微晶纖維素、羥丙基纖維素、羥乙基纖維素和羥丙基甲基纖維素、蔗糖、右旋糖、玉米糖漿、多糖、明膠或其任意組合。其中,黏合劑可以基於製劑總重量的0.01至10%,優選為1%-10%的量存在。
作為助流劑,可以選擇的是例如但不限於膠體二氧化矽、三矽酸鎂、纖維素粉末、滑石粉或其組合。助流劑可以基於組合物的總重量的0.1%至10%,優選為0.1%-0.5%的量存在。
除非另有說明,否則如本文所用術語“包含”和“包括”具有開放性和非限制性含義。例如,除本發明的PI3K抑制劑、PGE2受體抑制劑與PD-1抑制劑所形成的藥物組合之外,還可以包含其它治療劑,例如“生物治療劑”、“化學治療劑”。

對於本發明的聯合治療的劑量方案的選擇取決於多個因素,例如:受治療的個體的實體血清或組織翻轉率、症狀水準、整體免疫原性和靶細胞、組織或器官的可接近程度。優選地,劑量方案將遞送至患者的每種治療劑的量最大化,符合可接受的副作用水準。因此,聯合治療中每種生物治療劑和化學治療劑的劑量和給藥頻率部分取決於具體的治療劑、受治療的癌症的嚴重程度和患者的表徵。
本發明的聯合治療可以在移除腫瘤的手術前或後施用,也可以在放射治療之前、期間、或之後施用。

在一些實施方案中,PI3K抑制劑、EP-4抑制劑與PD-1抑制劑組合施用導致所述PD-1抑制劑增強,使得例如更小劑量或者更長時間間隔的PD-1抑制劑對於治療可能是有效的。

具體實施例
實施例1:式I化合物(AN2025)、式II化合物(AN0025)與PD-1抗體的組合療法治療小鼠EMT6,CT26腫瘤
材料和方法
試劑和儀器:AN0025購買自上海喀露藍科技有限公司;AN2025購買自CSNpharm。針對小鼠的PD-1的抗體(克隆號:RMP1-14)購買自BioXcell。
細胞系:小鼠結腸癌CT26細胞(CRL-2638)、乳腺癌EMT6細胞(CRL-2755)購自美國組織培養中心。所有細胞都在補充有10%胎牛血清的RPMI-1640(CT26)或者Waymouth’s(1x)MB 752/1(EMT6)培養基中在培養箱中以37℃和5%二氧化碳條件培養,並每週傳代培養兩次,直至獲得必需數量的用於接種小鼠的細胞。
動物:6-8周的BalB/c雌性小鼠購自上海斯萊克實驗動物技術有限公司。動物被圈養在微型隔離籠中,每籠最多5只,執行12h光/暗迴圈。籠每週更換兩次。每週觀察一次動物並記錄臨床徵象。
動物研究:收穫體外培養的癌細胞,分別以2.0×105細胞(CT26)/ml的細胞濃度和2.5×105細胞(EMT6)/ml的細胞濃度懸浮於100μl磷酸鹽緩衝鹽水中,並使用注射器在右側前肢背部皮下(sc)注入小鼠體內。在細胞植入後的6-7天,基於腫瘤大小隨機分配小鼠,對於EMT6模型,採用AN2025給藥:30mg/kg,每天口服(PO)一次。AN0025給藥:150mg/kg,每天口服(PO)一次。PD-1抗體給藥:10mg/kg,每週腹腔注射(IP)兩次的方式進行給藥;對於CT26模型,採用AN2025給藥:30mg/kg,每天口服(PO)一次。AN0025給藥:100mg/kg,每天口服(PO)一次。PD-1抗體給藥:10mg/kg,每週腹腔注射(IP)兩次的方式進行給藥。在各個研究中被分配到治療組的動物在所有組別中都具有接近的平均腫瘤重量。用數顯卡尺每週2-3次測量腫瘤大小,並使用公式(l×w2)/2=mm3計算體積,其中l和w是指在每次測量中收集的較大和較小正交尺寸。使用GraphPad Prism 6軟體繪製群組腫瘤大小(平均±SEM)對比時間的曲線圖。使用Independent Sample t檢驗進行分析進行統計分析。
實驗結果
如圖1,圖2所示,儘管AN0025和PD-1抗體聯用以及AN2025與PD-1抗體聯用在小鼠乳腺癌EMT6模型和小鼠結直腸癌CT26模型上都體現出了一定的抗腫瘤活性;但是三者聯用,即AN2025,AN0025和PD-1抗體三聯所帶來的抗腫瘤活性顯著優於AN0025和PD-1抗體(p<0.05)以及AN2025與PD-1抗體(p<0.05)二聯活性。
實施例2:AN2025、AN0025與PD-1抗體的組合療法治療PBMC免疫系統人源化小鼠HCT116腫瘤
材料和方法
試劑和儀器:AN0025購買自上海喀露藍科技有限公司;AN2025購買自CSNpharm。針對人的PD-1的抗體(Pembrolizumab)由菲諾克生物科技有限公司提供。human PBMC購買自妙順(上海)生物科技有限公司。
細胞系:HCT116人結腸癌細胞購自中科院細胞庫。細胞在補充有10%胎牛血清的McCoy's 5a培養基中在培養箱中以37℃和5%二氧化碳條件培養,並每週傳代培養兩次,直至獲得必需數量的用於接種小鼠的細胞。
動物:6-8周的NPSG雌性小鼠購自PNK。動物被圈養在IVC隔離籠中,每籠5只,執行12h光/暗迴圈。溫度20~26℃,濕度30-70%。
動物研究:NPSG小鼠以human PBMC 5x106/100ul的接種量,IV注射。接種6天后,在小鼠右側肩部皮下接種HCT116細胞,接種量1×106/mice。接種後,每週1次測量腫瘤體積及體重,當平均腫瘤體積達到約80-100mm3時測量小鼠血液中hCD45+細胞比例。依據腫瘤體積和小鼠血液中hCD45+細胞比例,將小鼠分別隨機分為6組,每組10只。分組時腫瘤體積變異係數CV應30%(CV=標準差/平均值)。分組後即開始給藥。AN2025給藥:37.5mg/kg,每天口服(PO)一次。AN0025給藥:30mg/kg,每天口服(PO)一次。PD-1抗體給藥:10mg/kg,每週腹腔注射(IP)兩次。給藥開始後,小鼠每週2次測量體重及腫瘤體積。每週1次流式監控小鼠血液中hCD45+細胞比例。在各個研究中被分配到治療組的動物在所有組別中都具有接近的平均腫瘤重量。腫瘤大小使用公式(l×w2)/2=mm3計算體積,其中l和w是指在每次測量中收集的較
大和較小正交尺寸。對於兩兩兩比較,採用T-Test分析方法,對於三個以上的組相互比較,採用單因素方差分析(One-Way ANOVA)方法進行分析。對於比較潛在的協同效應,採用雙因素方差分析(Two-Way ANOVA)。所有的資料使用SPSS 24.0。p值小於0.05視為有顯著差異。
實驗結果
儘管AN0025和PD-1抗體聯用以及AN2025與PD-1抗體聯用在PBMC免疫系統人源化小鼠HCT116腫瘤體現出了一定的抗腫瘤活性;但是三者聯用,即AN2025、AN0025和PD-1抗體三聯所帶來的抗腫瘤活性顯著優於AN0025和PD-1抗體(p<0.05)以及AN2025與PD-1抗體(p<0.01)二聯活性。

- 一種藥物組合物和/或藥物組合在製備用於治療腫瘤/癌症和/或產生針對腫瘤/癌症的記憶免疫應答的藥物的用途,所述藥物組合物和/或藥物組合包括PI3K抑制劑、前列腺素受體(PGE2)抑制劑以及PD-1抑制劑,其中,所述PI3K抑制劑、前列腺素受體(PGE2)抑制劑和所述PD-1抑制劑可以作為單一制劑(Dosage form)同時、分開或者順序施用。

- 如請求項1-9任意一項所述的用途,其中所述PI3K抑制劑適於以每天1次或2次施用;或者每2、3、4、5、6、7、8、9或10天或每1、2或3周施用1次所述PI3K抑制劑;或者每週連續5天每天施用1次所述PI3K抑制劑,然後間隔2天。
- 如請求項1-10任意一項所述的用途,其中所述PI3K抑制劑適於在成年人中以約20mg/天-約200mg/天、約30mg/天-約160mg/天、約60mg/天-約120mg/天的劑量範圍施用。
- 如請求項1-11任意一項所述的用途,其中所述PI3K抑制劑適於以約0.5至約250mg/kg、1至約250mg/kg、約2至約200mg/kg、約3至約120mg/kg、約5至約250mg/kg、約10至約200mg/kg、或約20至約120mg/kg的有效劑量施用。
- 如請求項1-12任意一項所述的用途,其中所述前列腺素受體(PGE2)抑制劑適於以經口服、口含或經消化道外途徑的劑型施用,例如所述口服途徑的劑型可以是片劑、膠囊、粉末、丸劑、顆粒、懸浮液、溶液和溶液預濃縮劑、乳液和乳液預濃縮劑,例如所述消化道外途徑的劑型可以是靜脈內、腹膜內、皮內、皮下、肌肉、顱內、鞘內、瘤內、經皮滲透、經粘膜給藥的劑型。
- 如請求項1-13任意一項所述的用途,其中所述前列腺素受體(PGE2)抑制劑適於以每天1次或2次施用;或者每2、3、4、5、6、7、8、9或10天或每1、2或3周施用1次所述前列腺素受體(PGE2)抑制劑;或者每週連續5天每天施用1次所述前列腺素受體(PGE2)抑制劑,然後間隔2天。
- 如請求項1-14任意一項所述的用途,其中所述前列腺素受體(PGE2)適於在成年人中以約20mg/天-約2000mg/天、約30mg/天-約1600mg/天、約60mg/天-約1200mg/天或約100mg/天-約1000mg/天的劑量範圍施用。
- 如請求項1-15任意一項所述的用途,其中所述前列腺素受體(PGE2)抑制劑適於以約0.5至約250mg/kg、1至約250mg/kg、約2至約200mg/kg、約3至約120mg/kg、約5至約250mg/kg、約10至約200mg/kg或約20至約120mg/kg的有效劑量施用。
- 如請求項1-16任意一項所述的用途,其中所述PD-1抑制劑適於以消化道外途徑施用,例如所述消化道外途徑可以是靜脈內、腹膜內、皮內、皮下、肌肉、顱內、鞘內、瘤內、經皮滲透、經粘膜給藥。
- 如請求項1-17任意一項所述的用途,其中所述PD-1抑制劑適於以溶液劑、凍乾劑、粉針劑的形式存在。
- 如請求項1-18任意一項所述的用途,其中所述PD-1抑制劑適於以0.05mg/kg、0.1mg/kg、1mg/kg、2mg/kg、3mg/kg、4mg/kg、5mg/kg、6mg/kg、7mg/kg或8mg/kg的有效劑量施用。
- 如請求項1-19任意一項所述的用途,其中所述腫瘤/癌症選自PIK3CA突變的癌症。
- 如請求項1-20任意一項所述的用途,其中所述腫瘤/癌症選自:頭和頸鱗狀細胞癌、頭和頸癌、腦癌、神經膠質瘤、多形性成膠質細胞瘤、神經母細胞瘤、中樞神經系統癌、神經內分泌腫瘤、咽喉癌、鼻咽癌、食管癌、甲狀腺癌、惡性胸膜間皮瘤、肺癌、乳腺癌、肝癌、肝細胞瘤、肝膽癌、胰腺癌、胃癌、胃腸道癌、腸癌、結腸癌、結腸直腸癌、腎癌、透明細胞腎細胞癌、卵巢癌、子宮內膜癌、子宮頸癌、膀胱癌、前列腺癌、睾丸癌、皮膚癌、黑色素瘤、白血病、淋巴瘤、骨癌、軟骨肉瘤、骨髓瘤、多發性骨髓瘤、骨髓異常增生綜合征、骨髓增生性腫瘤、鱗狀細胞癌、尤因氏肉瘤、全身性輕鏈澱粉樣變性和梅克爾細胞癌;更優選的,所述淋巴瘤選自:霍奇金淋巴瘤、非霍奇金淋巴瘤、彌漫性大B-細胞淋巴瘤、濾泡性淋巴瘤、原發性縱隔大B-細胞淋巴瘤、套細胞淋巴瘤、小淋巴細胞性淋巴瘤、富含T-細胞/組織細胞的大B-細胞淋巴瘤和淋巴漿細胞性淋巴瘤,所述肺癌選自:非小細胞肺癌和小細胞肺癌,所述白血病選自:慢性髓細胞樣白血病、急性髓細胞樣白血病、淋巴細胞白 血病、成淋巴細胞性白血病、急性成淋巴細胞性白血病、慢性淋巴細胞性白血病和髓樣細胞白血病。
- 一種藥物組合物和/或藥物組合,其包含PI3K抑制劑、前列腺素受體(PGE2)抑制劑和PD-1抑制劑,以及可藥用載體。

- 如請求項22-32任意一項所述的藥物組合物和/或藥物組合,其中每單位劑型(unit dosage form)中包含1-1000mg劑量的PI3K抑制劑,例如1、2、3、4、5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、20、25、30、35、40、50、60、70、75、80、90、100、110、120、125、130、140、150、160、170、175、180、190、200、250、300、350、400、450、500、600、700、750、800、900、1000mg或以上任意兩個數值之間的值。
