Abstract

Description
本揭露係關於醫療器械,更具體地,係關於用於動脈瘤治療裝置的遞送系統。
動脈瘤可係複雜且難以治療的。例如,當動脈瘤位於關鍵組織附近時,治療通路可能受到限制或不可用。此類因素與顱內動脈瘤有關,因為顱內血管周圍存在腦組織。

利用栓塞線圈的先前方法可藉由治療動脈瘤入口及/或用栓塞編織植入物填充動脈瘤來改善。例如,參見以引用方式併入本文的美國專利申請案第15/903,860號。用編織植入物治療動脈瘤可具有優於利用栓塞線圈治療的潛在優點,諸如較高的填充密度、能在植入手術期間回縮及重 新定位植入物、能在沒有輔助裝置(諸如支架或球囊)的情況下進行植入、降低重新分析或壓緊的風險,以及改善整個動脈瘤頸部的覆蓋範圍。
在編織植入物遞送系統中,可能有利的是在植入物與遞送系統之間保持附接,直到植入物在治療部位處就位,然後分離植入物以便能夠將遞送系統取出。本揭露描述了可用於至少這一目的的各種例示性系統、裝置、及方法。
圖1a至圖1c繪示被構形成用於通過微導管遞送至治療部位的一例示性遞送系統。圖1a為描繪例示性遞送系統的部件的分解圖。圖1b為描繪包括圖1a所示部件的經組裝例示性遞送系統的剖視圖。圖1c為圖1b的經組裝系統的橫截面俯視圖。如圖所示,系統可包括一編織植入物300、一釋放部件110、一附連部件150、一遞送管500、及一拉線200。
編織植入物300可具有實質上管狀的結構,該結構具有將植入物300的一外折疊部302與植入物300的一內折疊部304分開的一折疊部303。植入物300可被折疊,使得內折疊部304的尺寸被設計為適配在遞送管500的一內腔504內,折疊邊緣303在遞送管500的一遠端506上捲起,並且外折疊部302適配在遞送管500上方而朝近側延伸並附接至釋放部件110。
釋放部件110可附接至編織植入物300的外折疊部分302,並且可用於在遞送至治療部位期間及植入期間將編織植入物300附接至遞送管500。植入完成後,釋放部件110可釋放遞送管,使得其可從患者體內取出。
圖1a至圖1c顯示處於一壓縮構形的一實例釋放部件110,該壓縮構形可使得釋放部件110能夠將編織植入物300附接至遞送管500。如圖1a至圖1c所示般配置,編織植入物300的外折疊部分302可折疊在釋放部件110上並附接至釋放部件110的一帶112。帶112可具有包括一外表面118及一內腔116的一管狀結構,然而可設想其他形狀。釋放部件110可包括從帶 延伸並且抵靠遞送管500的一外表面508壓制的一或多個彈簧構件120,從而在釋放部件110與遞送管500之間提供摩擦配合。彈簧構件120可從帶的一遠側表面114朝遠側延伸並且具有抵靠遞送管500壓制的遠端122。
一附連部件150可用於將釋放部件110附接至編織植入物300。附連部件150可具有一外表面158及一內腔156。附連部件150可被放置在釋放部件110的帶112上,從而將編織植入物300的外折疊部302的一部分固定在附連部件150與釋放部件110之間。
遞送管500可具有穿過其中的一內腔504。內腔504可含有編織植入物300的內折疊部304。內腔亦可含有額外元件以有利於植入編織植入物300(未圖示)。
遞送管500可具有在外表面508上縱向行進的一通道502,並且拉線200可定位在通道502內。拉線200可具有可接合釋放部件110的一延伸部分202(諸如鉤)及可定位在通道502內並朝近側延伸的一細長部分204。如圖所示,延伸部分202可跨帶112的遠側表面114延伸。延伸部分202與釋放部件的接合可防止釋放部件110相對於遞送管500的遠側移動,並且藉此可保持編織植入物300與釋放部件110之間的附接,直到編織植入物300的植入完成。
拉線200的細長部分204可朝近側延伸超過遞送管500的近端505,從而使系統的操作者能夠觸及拉線200。如圖1b所示,拉線200可用黏著劑250可拆卸地附接至遞送管500。黏著劑250可抑制拉線200的移動,直到向拉線200施加足夠的力以破壞黏著劑250。
圖1c繪示靠近遞送管的遠端506的系統的截面圖,近側折疊部303朝近側觀察,如圖1b所示。編織植入物300的內折疊部304可定位在遞送管500的內腔504的內部;釋放部件110可定位在遞送管500的外表面508之上;彈簧構件120可從釋放部件110的帶112的一遠側表面114延伸;外折疊部分302可適配於釋放部件110上,從而附接至釋放部件的帶112;並且 一附連部件150可適配在釋放部件的帶112及編織植入物300的外折疊部302的一部分上,從而將編織植入物300附連至釋放部件110。
圖2a至圖2g繪示用根據本發明的一系統或裝置部署一編織植入物300的一方法。圖2a繪示可提供的一編織植入物遞送系統或裝置。系統可包括一編織植入物300、一遞送管500、一釋放部件110、及一拉線200。該系統可如參考圖1a至圖1c所述進行組裝,使得拉線200與釋放部件110接合,並且釋放部件110接合至遞送管500。可經由一遞送導管600(諸如微導管)將經組裝的系統遞送至動脈瘤治療部位。
一旦被遞送,就可以例如如美國專利申請案第15/903,860號中所述部署編織植入物300。圖2b繪示具有從遞送導管600的遠端602射出的一彈出部分306的編織植入物300。如圖所示,編織植入物300可保持附接至遞送系統,使得外折疊部302的至少一部分保持在導管600內。只要釋放部件110保持附接至遞送管500,編織植入物300就可保持附接至遞送管500。在完成編織植入物的植入之前,可回縮並重新定位編織植入物300的彈出部分306。
圖2c繪示在植入完成後可採取的步驟,以將釋放部件110從遞送管500釋放,從而將編織植入物300從遞送管500釋放。圖2c顯示從釋放部件110脫離的拉線200及開始移動至一釋放構形的釋放部件110。
拉線200的延伸部分202可為可撓的,並且在將拉線200的細長部分204朝近側拉動時,延伸部分202可與釋放部件110脫離。一旦脫離,拉線200就能夠移動,以便不抑制釋放部件110(相對於遞送管)的遠側移動。
彈簧構件120的遠端122可遠離遞送管500延伸,從一壓縮構形移動至一延伸構形。當彈簧構件120處於壓縮構形時,彈簧構件120可防止釋放部件110相對於遞送管500移動。彈簧構件120可由鎳鈦諾(Nitinol)或其他記憶形狀材料製成,使得在接觸體液諸如血液時,彈簧構件120可移動 至遠離遞送管500延伸的預定形狀。圖2c顯示遞送管500相對於導管朝遠側移動,使得遞送系統開始退出導管600,從而暴露彈簧構件120的遠端122。
圖2d繪示彈簧構件120在退出導管600後繼續移動至一釋放構形。彈簧構件120可壓抵編織植入物300,從而延伸植入物300的部分,例如以使治療部位開口閉塞,諸如動脈瘤的頸部。
圖2e繪示從遞送管500的遠端506滑出的釋放部件110。圖2f為如圖2e所示朝近側觀察的截面圖。當彈簧構件120向外延伸時,釋放部件110的帶112顯示為包圍遞送管500的遠端506。
圖2g繪示與遞送管500分開的釋放部件110。一旦脫離,可在編織植入物300及釋放部件110保持植入的同時取出遞送管500。
圖3a及圖3b是繪示在動脈瘤10中植入的不同階段的一編織植入物300的剖視圖。圖3a繪示被部分地部署至血管20的動脈瘤10中但尚未從其遞送系統釋放的一編織植入物300。導管600已被遞送至動脈瘤10的部位。如圖所示,編織植入物300可形成經定尺寸以延伸至動脈瘤10的壁14的一閉塞囊袋308。編織植入物300的未形成閉塞囊袋308的部分可保持管狀形狀,該管狀形狀可被填充至動脈瘤囊12中,從而形成一栓塞填料編織物310。圖3a繪示部分地部署的栓塞填料編織物310,使得編織物的一部分保留在遞送系統中。在一部分部署狀態中,編織物310可被回縮回遞送管500中,重新定位並重新部署。在回縮、重新定位、及重新部署期間,釋放部件110可保持附接至遞送管500或與該遞送管接合,從而保持編織植入物300與遞送系統之間的附接。
圖3b繪示所部署的植入物300。釋放部件110可保持附接至編織植入物300,並且一旦植入就可駐留在動脈瘤10的頸部處。釋放部件110可具有彈簧構件120或向外延伸的其他結構,從而在動脈瘤10的頸部展開閉塞囊袋308。
圖4a至圖4c繪示一釋放部件110的另一個實例。如圖所示,釋放部件110可具有閉塞構件130。圖4a繪示處於經定尺寸以適配在一遞送管500上並行進穿過一微導管600的一構形的釋放部件110。圖4b為圖4a中以一例示性部署構形示出的釋放部件110的側視圖。圖4c為處於部署構形的釋放部件110的俯視圖。處於部署構形時,閉塞構件130可向內彎曲,至少部分地閉塞釋放部件110的內腔116。如關於彈簧構件120所述,閉塞構件130可由可撓性材料構成及/或可由記憶金屬諸如鎳鈦諾構成。閉塞構件130可在處於遞送構形時變形,並且在處於部署構形時移動至一預定的形狀。當閉塞構件130接觸血液或其他體液時,可藉由溫度變化促使從該變形移動至該預定的形狀。
釋放部件110可被導向成使得由閉塞構件130使釋放部件110的內腔116閉塞不會導致閉塞構件130阻止釋放部件110與遞送管500脫離及分開。如圖所示,閉塞構件130可附接至帶112的近側,並且彈簧構件120可附接至遠側。正因為如此導向,在離開微導管600時,彈簧構件120可接觸血液或體液,並且因此使遞送管500脫離。同時,導管600可在閉塞構件130與血液或流體之間提供屏障。然後可在閉塞構件130接觸血液或體液之前取出遞送管500。一旦遞送管500被取出,則可取出遞送導管600,從而將閉塞構件130暴露於血液或流體,從而使閉塞構件130回縮以使帶112的內腔116閉塞。
圖5a顯示在導管600的剖面前方的裝置的外部。一拉線200定位在一遞送管500的一外表面508上的一通道502內。拉線200在釋放部件110的下方延伸以接合釋放部件110。一編織植入物300用一附連部件150附接至釋放部件110,並且被折疊成使得一外折疊部分302定位在遞送管500上方且一折疊部303定位在遞送管500的一遠端上方。
圖5b顯示如圖5a所示的系統的截面圖。圖5b顯示遞送管500、遞送管通道502、拉線200、釋放部件110的帶112、編織植入物300的外折疊部分302、附連部件150、及導管600的可能定位。
圖6是處於圖5a及圖5b所示構形的裝置及導管600的截面圖。在截面圖中,可看見遞送管500的內部或內腔504、編織植入物300的近端312、以及拉線200、遞送管通道502、釋放部件110、附連部件150、編織植入物內折疊部304、遞送管500、以及編織植入物外折疊部302的橫截面。拉線200的延伸部分202顯示為接合釋放部件110的帶112,並且彈簧構件120顯示為從釋放部件110的帶112朝遠側延伸。拉線200可另外延伸以接合編織植入物300。
圖7a至圖7c為在植入編織植入物300之後的各個階段的例示性裝置的剖視圖,繪示從一遞送系統釋放編織植入物300。圖7a至圖7c顯示一遞送管500、遞送管通道502、拉線200、釋放部件110、附連部件150、編織植入物近端312、及閉塞囊袋308的橫截面。在圖7a所示的階段,編織植入物300已從遞送管500射出,並且編織植入物300開始與遞送系統分離。遞送管500的遠端506朝遠側延伸出遞送導管600,並且彈簧構件120延伸而遠離遞送管。拉線200可利用一延伸部分202接合釋放部件110,從而抑制釋放部件110的遠側移動。
在圖7b所示的階段,拉線200已與釋放部件110脫離。延伸部分202可由可撓性材料製成,該可撓性材料可回應於力而拉直,使得朝近側拉動拉線200的細長部分204導致延伸部分202從釋放部件110的帶112的遠側表面114回縮並拉直以適配在釋放部件110的帶112下方的遞送管的通道502內。一旦拉線200與釋放部件110如圖7b所示脫離並且彈簧構件120延伸而遠離遞送管500,釋放部件110就能夠自由地從遞送管500的遠端506滑出。
在圖7c所示的階段,編織植入物300已與遞送管500分離。如圖所示,彈簧構件120可向外延伸以與編織植入物300並置。一旦分開,就能夠從患者體內取出導管600、拉線200、及遞送管500。
圖8及圖9為概述使用根據本發明的裝置的例示性方法步驟的流程圖。方法步驟可以藉由本文描述的例示性方式或藉由所屬技術領域中具有通常知識者已知的任何方式來實施。
參見圖8中概述的方法700,在步驟710中,可提供具有一編織植入物、一遞送管、一釋放部件、及一拉線200的一編織植入遞送系統。該編織植入物遞送系統可以是本文所述的任何遞送系統,其具有本文描述的特徵的任何組合、以及所屬技術領域中具有通常知識者已知的任何特徵。在步驟720中,可將拉線接合至釋放部件。在步驟730中,可將釋放部件接合至遞送管。在步驟740中,可將編織植入物植入治療部位處的動脈瘤中。在步驟750中,可拉動拉線以使拉線與釋放部件脫離。在步驟760中,釋放部件可自遞送管脫離。在步驟770中,釋放部件可從遞送管釋放,從而從遞送管釋放編織植入物。
參見圖9中概述的方法800,在步驟810中,可提供具有一編織植入物、一遞送管、一釋放部件、及一拉線的一編織植入物遞送系統。該編織植入物遞送系統可以是本文所述的任何遞送系統,其具有本文描述的特徵的任何組合、以及所屬技術領域中具有通常知識者已知的任何特徵。在步驟815中,拉線可利用黏著劑緊固至遞送管。在步驟820中,拉線可接合釋放部件。在步驟830中,釋放部件可接合遞送管。在步驟840中,可將編織植入物植入治療部位處的動脈瘤中,藉由反轉編織植入物的一部分來形成一閉塞囊袋。在步驟850中,黏著劑能夠被破壞,並且可藉由拉動拉線而使拉線自釋放部件脫離。在步驟860中,可藉由將一彈簧構件從接合遞送管的一外表面的一接合構形移動至延伸而遠離遞送管的外表面的一釋 放構形而使釋放部件自遞送管脫離。在步驟870中,釋放部件可從遞送管釋放,從而從遞送管釋放編織植入物。

Claims (19)
- 一種用於在動脈瘤中釋放一植入物的系統,該系統包含:一編織植入物;一遞送管,該遞送管包含穿過其中的一內腔、一遠端、一外表面、及設置在該外表面上的一通道;一釋放部件,該釋放部件包含:一帶,該帶包含一外表面及一內腔,該外表面附接至該編織植入物的一第一部分,該內腔包圍該遞送管而鄰近該遞送管的該遠端,及一彈簧構件,該彈簧構件附接至該帶,該彈簧構件可從一壓縮構形移動至一延伸構形,該壓縮構形壓制該遞送管的該外表面,且該延伸構形延伸而遠離該遞送管的該外表面;及一拉線,該拉線定位在該遞送管的該通道內,該拉線可從一接合構形移動至一釋放構形,該接合構形接合該釋放部件,且該釋放構形允許該彈簧構件展開至其延伸構形。
- 如請求項1所述之系統,其中該編織植入物的該第一部分藉由一折疊部而與該編織植入物的一第二部分分開,其中該第一部分包圍該遞送管的該遠端的該外表面的一部分,且其中該第二部分包含一細長管狀結構且定位在該遞送管的該內腔內。
