Abstract

Description
本案基於美國專利法第119條(35 U.S.C.§ 119)主張於2012年8月3日申請之美國臨時專利申請案第61/679,345號之優先權,其全部之內容皆被引用入本文中做為參考。
本發明係關於一種體外膜氧合(extracorporeal membrane oxygenation;ECMO)之方法。
體外膜氧合(ECMO)提供心臟及呼吸支援性氧氣給心臟及肺臟無法執行功能之病患。ECMO需要血流及抗凝血作用的悉心管理。亟需一種在體外膜氧合期間可提供較佳之調控血流、預防凝血且改善止血的新穎方法。
在一方面,本發明提供一種體外膜氧合之方法。在一具體實施例中,本發明提供一種於具有體外膜氧合之個體中抑制血栓及/或預防出血之方法,該方法包含施予具有體外膜氧合之個體治療有效量之抗凝血酶,以抑制血栓及/或預防出血。在一具體實施例中,該治療有效量係可有
效抑制血栓及預防出血。在另一具體實施例中,該治療有效量亦有效提供抗發炎效用,例如,抑制全身性發炎。
在另一具體實施例中,該抗凝血酶具有高度的甘露糖化模式。在另一具體實施例中,該抗凝血酶具有高度的海藻糖化模式。在另一具體實施例中,該抗凝血酶包含GalNac(N-乙醯半乳醣胺)。在另一具體實施例中,該抗凝血酶為以基因轉殖製造之抗凝血酶。在另一具體實施例中,該抗凝血酶係以基因轉殖於山羊製造。在另一具體實施例中,該抗凝血酶為ATryn®。
在另一具體實施例中,該抗凝血酶係以每公斤6單位之劑量施予。在另一具體實施例中,該抗凝血酶係以每公斤80單位之劑量施予。在另一具體實施例中,該抗凝血酶係以每公斤295單位之劑量施予。

在一方面,本發明提供一種具有體外膜氧合之個體抑制血栓及/或預防出血之方法,其包含施予具有體外膜氧合之個體治療有效量之抗凝血酶,以抑制血栓及/或預防出血。在部分具體實施例中,用於本文方法中之該抗凝血酶為ATryn®。
體外膜氧合(ECMO)
體外膜氧合係為一種手術,可提供心臟及呼吸支援性氧氣給該等心臟及肺臟無法執行功能之病患。在ECMO手術中,將導管放置於病患之血管中,接著病患血液經由ECOM機器抽吸。在該ECMO機器中,血液流經膜氧合器,其可將血液中之二氧化碳移除並加入氧氣。從靜脈系統來的血液可再回流至靜脈系統(靜脈至靜脈ECMO)或至動脈系統(靜脈至動脈ECMO)。
ECMO典型地被用於肺臟及心臟喪失功能之病患上,該功能喪失可能由許多原因所造成,包括病毒感染(如H1N1及hRSV),及創傷(如,溺水)。ECMO亦可被用於帶有呼吸系統發育不完全之新生兒。
如同其他體外手術一樣,使用ECMO的主要困難在於需要預防感染及維持止血,以預防血栓(凝血)及出血。血栓及出血之預防通常係藉由施予肝素及/或其他抗凝血劑以進行。血栓可藉由於不同時間點取出血液樣本及測量活化凝血時間(通常介於140-200秒之間)、aPTT(活化部分血栓時間)或測量抗Xa因子之量予以監控。其他可用於在血液樣本中測
定以評估止血之參數為作用於凝血連鎖反應之酵素的量與活性(請參閱Niebler et al.,2011,Artificial Organs 35:1024-1028及Agati et al.,2006,ASAIO Journal 52:513-516)。
抗凝血酶及ATryn®
在一方面,本發明提供一種具有體外膜氧合之個體抑制血栓及/或預防出血之方法,其包含施予具有體外膜氧合之個體治療有效量之抗凝血酶,以抑制血栓及/或預防出血。在部分具體實施例中,該抗凝血酶具有高度的甘露糖化模式。在部分具體實施例中,該抗凝血酶具有高度的海藻糖化模式。在部分具體實施例中,該抗凝血酶包含GalNac(N-乙醯半乳醣胺)。在部分具體實施例中,該抗凝血酶為以基因轉殖所製造之抗凝血酶。在部分具體實施例中,該抗凝血酶係以基因轉殖於山羊中所製造。在部分具體實施例中,該抗凝血酶為ATryn®。
傳統上,「抗凝血酶」乙詞是指一群緊密相關蛋白之家族,包括抗凝血酶I、抗凝血酶II、抗凝血酶III及抗凝血酶IV。然而,抗凝血酶III為該抗凝血酶家族中唯一被發現與重要的生理功能有關係的成員,以及目前文獻中常將抗凝血酶及抗凝血酶III替換使用。除非特別指出不同處,於本文所使用之抗凝血酶係指抗凝血酶III。然而,應可理解的是抗凝血酶III之外的抗凝血酶家族中之成員,應與抗凝血酶III具有相似之活性,亦可被用於本文所述之方法中。
通常,抗凝血酶為432個胺基酸之蛋白,其分子量約為58kDA。然而,部分非人類之抗凝血酶具433個胺基酸之長度。抗凝血酶為絲胺酸蛋白酶抑制劑,其可抑制凝血酶及Xa因子。抗凝血酶係天然發現於哺
乳動物(包含人類)之血清中。來自健康個體之人類血清的抗凝血酶之生理量約為14-20mg/dL。
抗凝血酶為一醣蛋白,其包括四個醣化位置:Asn96、Asn135、Asn155及Asn192。抗凝血酶係以α形式(α抗凝血酶)及β形式(β凝血酶)存在,其中以α形式為主。β形式之抗凝血酶可與α形式區別,係因β形式於Asn135沒有醣化。在部分具體實施例中,用於本文所述方法之抗凝血酶,包括主要的α形式抗凝血酶,及次要之β形式抗凝血酶。在部分具體實施例中,用於本文所述方法之抗凝血酶,為α形式抗凝血酶。在其他具體實施例中,該抗凝血酶為β形式抗凝血酶。
抗凝血酶於哺乳動物物種間是保守的,僅於胺基酸序列上有細微之差異。在部分具體實施例中,該用於治療個體之抗凝血酶物種,根據本文所述之方法,係與該治療個體為相同之物種。因此,例如,人類抗凝血酶(根據其胺基酸序列)係用於治療人類之方法中,而牛的抗凝血酶(根據其胺基酸序列)係用於治療牛之方法中。在其他具體實施例中,該用於治療個體之抗凝血酶之物種可來自與治療個體不同之物種。
應可進一步了解的是,除了胺基酸序列外,該抗凝血酶之醣化作用亦具物種特異性。因此,例如,分離自人類血漿之人類抗凝血酶(血漿衍生的人類抗凝血酶)具有與分離自山羊血漿之山羊抗凝血酶不同之醣化模式。然而,如下述說明,例如人類抗凝血酶可於山羊中製造以提供帶有模擬山羊抗凝血酶之醣化模式之人類抗凝血酶(帶有人類胺基酸序列之抗凝血酶)。
在部分具體實施例中,用於本文所述之方法中之抗凝血酶具
有高度的甘露糖化模式。本文所用之具有高度的甘露糖化模式之抗凝血酶係指一抗凝血酶,其中一個或多個醣化側鏈包含寡甘露糖或雜合型式之寡甘露糖(而非側鏈包含雙端複合寡醣類,其為可自自血漿衍生的之人類抗凝血酶中發現之主要側鏈結構)。在部分具體實施例中,該用於本文方法中之抗凝血酶具有高度海藻糖化模式。本文所述之高度海藻糖化模式,係指一抗凝血酶,其於具有複合寡醣之多數醣化位置上之近端GlcNac上具有海藻醣。在部分具體實施例中,用於本文所述方法之抗凝血酶包含GalNac(N-乙醯半乳醣胺)。在部分具體實施例中,用於本文所述方法之抗凝血酶具有高度的甘露糖程度、高度的海藻糖程度及包括GalNac。應知道的是人類抗凝血酶(具有人類胺基酸序列之抗凝血酶)係於山羊中以基因轉殖製造者,具有高度的甘露糖程度、高度的海藻醣程度及包括GalNac,而血漿衍生的人類抗凝血酶一般不具有該等醣化模式(請參閱美國專利第5,843,705號、美國專利第6,441,145號及美國專利第7,019,193號)。
用於本文所述方法之抗凝血酶可藉由不同方法所製造。在部分具體實施例中,該抗凝血酶係藉由從血漿中分離來製造。在部分具體實施例中,該抗凝血酶係以重組方式製造。在部分具體實施例中,該抗凝血酶係以基因轉殖方式製造。

抗凝血酶可為來自第二物種之抗凝血酶。因此,例如,人類抗凝血酶可於小鼠及山羊中以基因轉殖方式製造。相似的,牛的抗凝血酶亦可以基因轉殖方式於小鼠及山羊中製造。此外,抗凝血酶亦可以基因轉殖方式於來源物種中製造。因此,山羊之抗凝血酶可以基因轉殖方式於山羊製造。
在部分具體實施例中,用於本文所述方法之抗凝血酶係以基因轉殖所製造。在部分具體實施例中,該以基因轉殖所製造之抗凝血酶具有不同於來自血漿衍生之抗凝血酶之醣化模式。一般而言,該抗凝血酶之醣化模式取決於製造該抗凝血酶之動物物種。因此,例如,於小鼠中以基因轉殖製造之抗凝血酶可預期地具有與於山羊中所製造之抗凝血酶不同的醣化模式。
在部分具體實施例中,用於本文所述方法之抗凝血酶具有於山羊中以基因轉殖製造之抗凝血酶之醣化模式。
應可了解的是,以基因轉殖製備之抗凝血酶之醣化模式亦可取決於製造該蛋白之轉殖基因動物之器官或其身體部分的本質。因此,於乳腺中所製造之抗凝血酶之醣化模式,可預期地在不同於血液中所製造之抗凝血酶,即便是從相同物種中所製造出者。在部分具體實施例中,用於本文所述方法之抗凝血酶係於非人類之哺乳動物之乳腺中所製造。在部分具體實施例中,用於本文所述方法之抗凝血酶係於山羊之乳腺中所製造。
應可進一步了解的是,具有山羊所製造的抗凝血酶之醣化模式的抗凝血酶,可藉由從例如非山羊之其他物種中製造或獲得,且修飾其醣化模式如於後續修飾作用中而提供。例如,醣化的抗凝血酶可於小鼠中製備,且由小鼠產生之抗凝血酶之醣化模式可藉由活體外修飾進行改變,
以生成山羊抗凝血酶之醣化模式。例如,小鼠生成之抗凝血酶可透過醣解酶或轉移酶之作用而改變。此外,該醣化模式可藉由非酵素方法(如合成)來修飾。
具有山羊所製造的抗凝血酶之醣化模式之抗凝血酶亦可藉由於細胞(如昆蟲細胞、細菌細胞)中製造以及於後續修飾過程中加入或修改其醣化模式以提供。亦或是,具有山羊所製造的抗凝血酶之醣化模式之抗凝血酶亦可藉由從非山羊物種(或山羊之不同組織)之血漿中分離,以及於後續修飾過程接續地修飾醣化模式而提供。
在部分具體實施例中,用於本文所述方法之抗凝血酶為ATryn®。ATryn®為基因轉殖之人類α抗凝血酶,係於山羊之乳腺中所製造者(請參閱美國專利第5,843,705號、美國專利第6,441,145號及美國專利第7,019,193號)。ATryn®係由FDA所核准可供天生抗凝血酶缺失之病患中,用於預防手術期間的及圍產期間的血栓栓塞症。於歐洲,ATryn®被允許可於手術中具有先天性抗凝血酶缺失之病患,於其臨床危險情況時用來預防深層靜脈栓塞及血栓栓塞。
山羊製造之(人類)抗凝血酶ATryn®的醣化模式係與血漿衍生之人類抗凝血酶之醣化模式不同。因為該醣化模式之不同,ATryn®具有與血漿衍生的人類抗凝血酶不同的生理特性。例如,ATryn®之清除速度高於血漿分離的之抗凝血酶之清除速度(請參閱US 7,019,193)。
個體
在一方面,本發明提供一種於具有體外膜氧合之個體中抑制血栓及/或預防出血之方法,該方法包含施予具有體外膜氧合之個體治療有
效量之抗凝血酶,以抑制血栓及/或預防出血。本文所使用的「個體」乙詞是指人類或其他脊椎哺乳動物,包括但不限於,老鼠、大鼠、犬、貓、馬、牛、豬、綿羊、山羊、或非人類之靈長類。在部分具體實施例中,該個體為人類。在部分具體實施例中,該個體為兒童。
肝素及其他療法
在一方面,本發明提供一種於具有體外膜氧合之個體中抑制血栓及/或預防出血之方法,該方法包含施予具有體外膜氧合之個體治療有效量之抗凝血酶,以抑制血栓及/或預防出血。在部分具體實施例中,抗凝血酶係與肝素合併施予。肝素與抗凝血酶結合,且增加抗凝血酶之蛋白酶抑制劑活性。在本發明之具體實施例中,其中抗凝血酶係與肝素合併施予,可為任何合併抗凝血酶及肝素之施予方式。因此,在部分具體實施例中,肝素係與抗凝血酶同時施予(如同時輸注)。在部分具體實施例中,肝素係於抗凝血酶投用之前或之後給予。在部分具體實施例中,該抗凝血酶及肝素具有部分重疊之施予療程(如,單獨施予抗凝血酶一段時間後,再同時施予抗凝血酶及肝素)。

在部分具體實施例中,該於具有體外膜氧合之個體中抑制血栓及/或預防出血之方法包含施予抗凝血酶並搭配其他療法。在部分具體實施例中,其他療法為可用於抑制血栓及/或降低出血之療法。該等其他療法包括施予抗凝血劑(除抗凝血酶或抗凝血酶連同肝素以外者)。該領域中已知之抗凝血劑包括可邁丁、華法林、醋硝香豆素片、苯丙香豆素、裂盒蕈色素、溴鼠靈、苯茚二酮、磺達肝癸鈉、艾卓肝素、利伐沙班、阿呱沙
班、水蛭素、來匹盧定、比伐盧定、阿加曲班、達比加群、希美加群、巴曲酶及裂纖酶。
在部分具體實施例中,抗凝血酶(如ATryn®)亦可與其他血液製品一起給予,例如,全血、紅血球濃厚液、FFP(新鮮冷凍血漿(fresh frozen plasma))、或血小板,以於ECMO過程中用於協助其他不足處。
治療有效量
在一方面,本發明於提供一種具有體外膜氧合之個體中抑制血栓及/或預防出血之方法,該方法包含施予抗凝血酶。在部分具體實施例中,抗凝血酶係以治療有效量以施予,藉以於體外膜氧合中抑制血栓及/或預防出血。所使用的「治療有效量」及「有效量」乙詞係可轉換使用,係指該劑量必須或足以實現所欲之療效,例如,抑制血栓及/或預防出血。結合與此提供之技術,藉由從眾多活性化合物及加權因子(例如,效力、相對生體可用率、個體體重、副作用之嚴重程度、及較佳之施予方式),可選擇一有效預防或治療之處理療程,其不會造成實質的毒性且仍對特定個體有治療效用。
對於各種特定應用之有效量可能因不同因子而有所不同,如治療之疾病或情況、施予之特定治療劑、個體大小、或疾病或失調之嚴重度。該領域具有通常技藝者可熟稔地判定抗凝血酶之有效量而不需額外之試驗。一般較佳地,使用最大劑量係為根據部分醫療診斷之最高安全劑量。可考慮每天、每周、或每月多劑量以達到適當的抗凝血酶之全身性程度。在部分具體實施例中,全身性程度可藉由於ECMO中大量注射後連續輸注抗凝血酶而達成(如,重組的人類抗凝血酶,ATryn®,等等)。在部分具體實
施例中,相較於藉由斷斷續續地單獨給予抗凝血酶之輸注,該施予方式可提供更好的控制及精確之凝血反應。該大量注射可經,如靜脈注射、或肌肉注射、脊椎內注射、或皮下注射給予。適當的全身性程度可藉由如測量病患之抗凝血酶之高點或於血漿中維持程度來判定。任何施予方法可包括監控抗凝血酶程度及/或活性,且如需要之劑量調整。

在部分具體實施例中,抗凝血酶係以為每天10單位或以上、每天50單位或以上、每天100單位或以上、每天200單位或以上、每天500單位或以上、每天1000單位或以上、每天1500單位或以上、每天2000單位或以上、每天2500單位或以上、每天3000單位或以上、每天3500單位或以上、每天4000單位或以上、每天4500單位或以上、每天5000單位或以上、每天

於本文中所述方法之抗凝血酶(如ATryn®)之劑量取決於所欲之治療目的而有所不同。在部分具體實施例中,該抗凝血酶係於體外膜氧合起始時,,以一可足以維持存在於血液中抗凝血酶活性程度之劑量施予。在部分具體實施例中,該抗凝血酶係於體外膜氧合起始時,以一足以超過血液中抗凝血酶活性之劑量施予。在部分具體實施例中,該抗凝血酶係於體外膜氧合起始時,以一足以得到血液中至少50%、至少60%、至少70%、至少80%、至少90%、至少100%、至少110%、至少120%、至少130%、至少140%、至少200%、至少300%、或更多之抗凝血酶活性之劑量施予。
在部分具體實施例中,該治療有效量係以一劑量施予。在部分具體實施例中,該治療有效量係以多劑量施予。根據施予方式,可適當地調控劑量,以達到所欲之抗凝血酶程度(局部的或系統性的)。若一個體於此劑量之反應不足時,可藉由給予較高劑量(或藉由不同的、更局部的給藥方式以給予有效的較高劑量)至該個體可容許之耐受程度,以補足之。亦可考量每天給予多劑量以達到適當的化合物全身性濃度。
施藥方式
在一方面,本發明提供一種於具有體外膜氧合之個體中抑制血栓及/或預防出血之方法,該方法包含施予抗凝血酶。在部分具體實施例中,該抗凝血酶係經由連續輸注方式施予。在部分具體實施例中,該抗凝血酶係以一劑量方式施予。
抗凝血酶典型地係以醫藥組合物方式施予個體,其可能常規地含有醫藥上可接受濃度之鹽類、緩衝劑、保存劑、相容載劑、佐劑、及可視需要的其他醫療成分。該醫藥載劑及該醫藥組合物之其他組成之本質將取決於施予之方式。
本發明之醫藥組合物可以任何該領域具有通常技藝者所熟
知之方法及路徑施予以施行本文所述之治療方法。較佳施予路徑包括但不限於口服、靜脈、皮下、腸外、瘤內、肌肉、鼻內、顱內、舌下腺、氣管內、吸入、經眼的、經陰道的、及經直腸的。
當欲將抗凝血酶遞送至全身,則其可能被調配以供腸外方式注射施予,如藉由大量注射或連續輸注。注射配方可以單位劑量形式呈現,如以安瓶、或多劑量容器伴隨著添加的保存劑。該組合物可以懸浮液、溶液、或於油質或水質載劑中之乳液形式、及可含有配方劑,像是懸浮劑、穩定劑及/或分散劑。用於腸外施予之醫藥配方包括該活性化合物於水可溶性形式之水溶液。此外,該活性化合物之懸浮液可製備為適當的油質注射懸浮液。適用的親油性溶劑或載體包括脂肪油,像是芝麻油、或合成脂肪酸酯,像是油酸乙酯或甘油三酸酯、或微脂體。水溶液注射懸浮液可含有物質,其可增加懸浮液之黏性,像是羧甲基纖維素鈉、山梨醇、葡聚糖。可視需要地,該懸浮液亦可含有適用的穩定物或穩定劑,其可增加化合物之可溶性,以製備高濃度的溶液。

抗凝血酶可被包括於細末的多顆粒配方中,以顆粒或藥丸形式呈現。用以膠囊施予之物質配方,亦可為粉末、輕壓片劑、或甚至為錠劑。該醫藥組合物可藉由壓縮以製備。可藉由惰性物質以稀釋或增加該醫藥組合物之量。該等稀釋劑可包括碳水化合物,特別是,甘露醇、a-乳糖、無水乳糖、纖維素、蔗糖、修飾葡聚醣及澱粉。特定無機鹽類亦可被作為填充劑般使用,包括三磷酸钙、碳酸鎂及氯化鈉。部分市售可得之稀釋劑為Fast-Flo、Emdex、STA-Rx 1500、Emcompress及Avicell。
於醫藥組合物配方中所包括之崩解劑,可為固體劑型。可作為崩解劑之物質包括但不限於澱粉,包括市售的以澱粉為基底之崩解劑Explotab。亦可使用羧甲基澱粉鈉、Amberlite、羧甲基纖維素鈉、超支鏈澱粉、海藻酸鈉、明膠、橘皮酸、羧甲基纖維素、天然海綿及膨潤土。可使用黏合劑以使治療劑結合在一起以形成堅硬的錠劑,且可作為材料者包括從天然產品中來源者,像是洋槐、黃耆膠、澱粉及明膠。於該治療配方中,可包括抗磨劑,以預防調配過程中之沾黏。可使用潤滑劑作為介於治療劑及堅硬外殼間之一層,且可作為潤滑劑者包括但不限於,硬脂酸包括其鎂及鈣鹽、聚四氟乙烯(PTFE,polytetrafluoroethylene)、液狀石蠟、植物油及蠟。助流劑可改善於調配過程中藥物的流動特性,且可於製壓過程中添加以幫助重整。助流劑可包括澱粉、滑石、熱解矽及含水矽鋁酸鹽。
用於吸入施予時,抗凝血酶可藉由壓縮管或氣霧器,伴隨適用之推進劑,以氣霧劑噴霧形式呈現以易於遞送;該推進劑如二氟二氯甲烷、三氯氟甲烷、二氯四氟乙烷、二氧化碳或其他適用氣體。

經鼻遞送之醫藥組合物亦包含抗凝血酶。經鼻遞送經直接施予該組合物至鼻子後,可使醫藥組合物進入血流中,而不需使藥物沉積於肺部。
在部分具體實施例中,抗凝血酶係局部輸送。局部施予方法是該領域所熟知者,且將取決於施用部位或施用器官。局部施予路徑包括使用標準的局部施用方法,像是經由表皮(用於皮膚上)、經吸入、直腸(如藉由灌腸劑或塞劑)、藉由點眼(點於結膜上)、點耳、鼻內路徑及陰道。
抗凝血酶亦可製備成大腸或陰道組合物,像是栓塞劑或維持栓塞劑,如含有常規栓塞劑基底者,像是可可脂或其他甘油脂。除了前述
之配方外,該化合物亦可製備如長效製劑。該等長效配方可以適用之聚合物或疏水物質調配(如一種於可接受之油中之乳液)、或離子交換樹脂、或可略溶的類似物,例如,可略溶鹽類。
該醫藥組合物亦包含適用之固體或膠體載劑或賦形劑。該等載劑或賦形劑之例子包括但不限於碳酸钙、磷酸钙、各種不同糖類、澱粉、纖維素類似物、明膠、及聚合物,像是聚乙二醇。
適用的液體或固態醫藥組合物劑型為,例如,供吸入、微膠囊、螺旋化、以微金顆粒包覆、包含於微脂體中、霧化的、噴霧的、植入皮膚之藥錠、或滴於一尖銳物體上,以刮入皮膚中之水狀或鹽水溶液。該醫藥組合物亦包括粒狀、粉末狀、錠劑、包覆錠、(微)膠囊、栓塞劑、糖漿、乳液、懸浮液、乳霜、可緩釋活性化合物之滴狀或製劑、製劑中之賦形劑及添加劑及/或一個或多個輔助劑,像是崩解劑、黏合劑、包覆劑、潤脹劑、潤滑劑、調味劑、甜味劑或增溶劑,上述所用者皆為常用者。該醫藥組合物適用於各種不同之藥物遞送系統。藥物遞送方法之簡要說明請參閱Langer,1990,Science 249,1527-1533,其亦被納入本文中以參閱。
本文所述之藥劑及組合物可以本身(最純淨)形式或醫藥可接受鹽類形式施予。當使用於醫藥時,該鹽類應為醫藥可接受者,但非醫藥可接受鹽類可易被用在製備其之醫藥可接受鹽類。該等鹽類包括但不限於,以下列酸類所製備者,鹽酸、氫溴酸、硫酸、硝酸、磷酸、順丁烯二酸、醋酸、水楊酸、對甲苯磺酸、酒石酸、檸檬酸、甲基磺酸、蟻酸、丙二酸、琥珀酸、2-萘磺酸、及苯磺酸。同時,該等鹽類可被製備如鹼金屬或鹼土金屬鹽,像是羧酸基之鈉、鉀或鈣鹽。
本發明之醫藥組合物含有一有效量之抗凝血酶,內含於一醫藥可接受載劑中。醫藥可接受載劑乙詞係指一個或多個兼容之固體或液體填充劑、稀釋劑或封膠物質,其可適用於人類或其他脊椎動物之施予。載劑乙詞表示一有機或無機成分、天然或合成,其中該活性成分係被添加入以促進應用。該醫藥組合物之成分,亦可與本發明之組合物混合在一起,以一種不會有交互作用之方式而不會影響所欲之醫藥效用之方式混合。

抗凝血酶可被用於控制釋放系統中。「控制釋放」乙詞是指任何本文所述之含有醫藥配方之藥劑及組合物,其中本文所述之藥劑及組合物從醫藥配方中釋放方式及釋放情形皆被控制。此包含立即地及非立即地釋放配方,其中非立即釋放配方包括但不限於持續釋放及緩釋配方。「持續釋放」(亦稱為延長釋放)乙詞於其常規之用途,係指醫藥物配方可提供隨著時間週期之延長,以逐步釋放化合物,且較佳的,但非必須,可於延
長之時間週期下,實質地維持血液中之常態藥物濃度。「緩釋」乙詞於其常規之用途,係指一藥物配方於施予配方時至從中釋放化合物之間,延遲釋放之用途。「緩釋」於延長時間週期下,可為或可不為逐步釋放化合物,且可為或可不為「持續釋放」。長效持續釋放植入物之用途可特別適用於治療慢性情況。於本文所用之「長效」釋放係指該植入物係被建構及調整以遞送該活性成分之療效程度至少要7天,較佳為30-60天。長效持續釋放植入物皆為該領域具有通常技藝者所熟知,且包括部分上述之釋放系統。

實施例
實施例1:於體外膜氧合過程施予ATryn®
於體外膜氧合時施予病患ATryn®以控制抗凝血及抑制血栓及/或預防出血。肝素亦被以一劑量施予,該劑量係足以維持所需活化之血液凝集時間(一般界於140-200秒間)。ATryn®係於ECMO起始內24小時,以單次輸注劑量方式施予。該施予ATryn®劑量界於2-300units/kg。
ATryn®之效果係由抗凝血酶活性於血液中之程度判定(請參閱Niebler et al.,2011,Artificial Organs 35:1024-1028)。
實施例2:於兒科進行體外膜氧合過程施予ATryn®
於體外膜氧合過程施予兒童病患ATryn®以控制抗凝血及抑制血栓及/或預防出血。連續施予ATryn®係於開始進行ECMO時開始。觀察血液中抗凝血酶活性程度,且該ATryn®的施予量係足以維持至少100%抗凝血酶活性(相較於起始濃度)。施予之肝素劑量,係足以維持所需之活化的凝集時間,至少150秒(請參閱Agati et al.,2006,ASATO Journal 52:513-516)。

Claims (14)
- 一種於具有體外膜氧合之個體中抑制血栓及/或預防出血之方法,該方法包含:施予具有體外膜氧合之個體治療有效量之抗凝血酶,以抑制血栓及/或預防出血。
- 如申請專利範圍第1項之方法,其中該抗凝血酶具有高度的甘露糖化模式。
- 如申請專利範圍第1項或第2項之方法,其中該抗凝血酶具有高度的海藻糖化模式。
- 如申請專利範圍第1-3項中任一項之方法,其中該抗凝血酶包含N-乙醯半乳醣胺(GalNac)。
- 如申請專利範圍第1-4項中任一項之方法,其中該抗凝血酶係為以基因轉殖製造之抗凝血酶。
- 如申請專利範圍第5項之方法,其中該抗凝血酶係於山羊中以基因轉殖製造。
- 如申請專利範圍第1-6項中任一項之方法,其中該抗凝血酶為ATryn®。
- 如申請專利範圍第1-7項中任一項之方法,其中該抗凝血酶係以每公斤6單位之劑量施予。
- 如申請專利範圍第1-8項中任一項之方法,其中該抗凝血酶係以每公斤80單位之劑量施予。
- 如申請專利範圍第1-8項中任一項之方法,其中該抗凝血酶係以每公斤295單位之劑量施予。
- 如申請專利範圍第1-8項中任一項之方法,其中該抗凝血酶,於體外膜 氧合起始時,係以足以維持存在於血液中之抗凝血酶活性程度之劑量施予。
- 如申請專利範圍第1-11項中任一項之方法,其中該抗凝血酶係以一劑量施予。
- 如申請專利範圍第1-11項中任一項之方法,其中該抗凝血酶係以大量注射隨後連續輸注方式施予。
- 如申請專利範圍第1-13項中任一項之方法,進一步包含施予肝素。
