관절접합용 기구

본 발명의 관절, 특히 손목관절의 접합용 기구 및 방법에 관한 것이다.
지금까지 알려진 손목접합 기구는 거의 없고 효율적이지 못하다. 이러한 관점에서 스완슨 보철(Swanson prosthesis)이 가장 보편적이다. 그것은 각 골수강에 구비된 공간에 고정되도록 스파이크가 달린 탄성중합체의 안내체(guide body)로 되어 있다.
이러한 고정 스파이크와 인접부 근육섬유와의 끊임없는 마찰로 인하여 손상 및 염증의 염려가 있으며, 헐겁게 되는 경향이 있다. 더구나, 사용되는 탄성중합체는 충분한 내마모성이 있는지도 의문시 되어 왔다. 그러나, 상기 공지의 관절보철이 갖고 있는 가장 심각한 결점은, 문제점을 단지 부분적으로만 해결하는데 그친것이다. :상기의 관절보철은 손목의 정상적인 굽힘은 가능하지만, 그와 동시에 손목의 회전은 불가능하다.
원위의 요골의 척골관절(distal radial ulan joint) 접합용 기구의 최근의 실예는 손의 각 부분의 뼈를 대신하지 못한다. 미국 특허 출원 제630,518호(1990. 12. 20 출원)을 참조하기 바란다.
기존의 관절접합용 기구의 결점을 해소한 본 발명에 따르면, 정상적인 손목의 굽힙과 아울러, 필요한 경우에 회전이 가능하며, 따라서, 정상적인 관절과 실질적으로 동일한 기능을 갖는 관절대용 기구를 제공한다.
본 발명의 다른 목적 및 특징은 첨부도면에 나타난 실시예에 근거하여 아래에 기술한다.
본 발명에 따른 기구를 사용하여 손목을 접합할 경우에, 제1도의 빗금친 부분(A)으로 나타난 부분을 외과적으로 제거하고, 아래에 설명하는 인공관절로 대치한다.
제2도에는 본 발명에 따른 기구를 도식적으로 나타내었다. 여기서, 제1주요부를 (1)로 명명하였다. 상기 제1주요부는 정면판(2)을 포함한다. 상기 판의 감춰진 부분(도면에는 나타나지 않음)은, 손목에 부착될 때, 손바닥으로 향하게 된다. 상기 판(2)에는 2개의 적어도 부분적으로 움푹한(hollow) 핀(5)이 구비되어 있다. 상기 핀(5)은 서로 떨어져서 위치하며, 손바닥(3)쪽으로 돌출된다. 상기 핀(5)의 단부는 폐쇄되어 있다. 상기 핀(5) 사이에는 나사형 고정부재(8)를 수용하기 위한 구멍(7)이 있다. 상기 고정부재는, 공지의 방법과 같이 부분적으로 움푹하고, 개구단부(10)에는 단부를 자르는 형태로 구성된 슬릿(9)을 구비하고 있다.
제1주요부에 대한 보다 상세한 것은 제3 및 4도에 나타나 있다.
제1주요부는 전체가 생체와 양립할 수 있는 재질(biocompatible material) 또는 표면을 적어도 상기의 재질, 예를들면 티타늄으로 도금하여 제조된다. 정면판(2)에는, 그 대향하는 장축단을 따라서 홈(11)이 적합하게 구비되어 있으며, 상기 홈은 삽입방향으로 뻗어 있어, 정면판이 골섬유내로 융합이 잘 되도록 해준다. 정면판(2)의 상부면(4)도 마찬가지로 적당히 불규칙적이다. 상기면(4)에는 예를들면, 홈(12)이 구비되어 있다. 정면판(2)에 구비된 구멍(7)은 고정부재(8)의 상부플랜지(13)를 지지하기 위하여 오목하게 되어 있다.
제2주요부(14)는 제5,6 및 7도에 상세히 나타나 있다. 이것은 팔쪽으로 뻗어나 있다.
제2주요부도 나사형 고정부재(17)를 위한 개구부(16)를 갖춘 정면판(15)을 포함하며, 개구단부(19)에는 단부를 자르는 형태의 슬릿(18)을 구비하고 있다.
폐단부(21)를 가지는 함몰핀(20)은 상기판(15)에서 돌출되어 있고, 개구부(16)의 한쪽 측면에 위치한다. 개구부(16)이 다른 측면에는, 정면판(15)이, 핀(20)과 같은 방향으로 돌출된 소매형 안내체(22)가 구비되어 있다. 상기 안내체(22)에는 실질적으로 타원형의 구멍(23)이 구비되어 실질적으로 타원형의 삽입체(24)를 수용하도록 설계되어 있다. 삽입체(24)의 저면(25)은 적어도 부분적으로, 안내체(22)의 내향 돌출단 플랜지(26)와 접촉하여 있다. 삽입체(24)에는 슬롯(slot)형의 종방향 안내자(26')가 구비되어 있고 헤드(28)를 가진 조절부재(27)를 수용할 수 있도록 설계되어 있다. 조절부재(27)는 슬롯의 상한(29)에 접촉하도록 하여 헤드(28)에 인접한 평탄부(30)는 실질적으로 삽입체(24)내에 위치되도록 한다.
조절부재(27)에는 또한, 관련 골섬유(척골)에 미리 배열된 고정체(32)와 영구나사 접합되는 하부접속부(31)가 구비되어 있다.
삽입체(24)를 제외하고는, 제2주요부의 모든 구성요소는 생체와 양립할 수 있는 재질, 바람직하게는 티타늄으로 제조된다. 한편, 삽입체(24)는 플라스틱제, 고밀도 폴리에틸렌이 적합하다.
본 발명에 따른 기구는 또한, 예를들면 실리콘플라스틱의 탄력체로 구성되는 관절기구인 제3주요부(33)을 포함한다. 상기 탄력체는 제1주요부 및 제2주요부(1,14)의 각 정면판(2,15)사이에 위치하게 된다.
상기 탄력체는 정면판(2)의 함몰핀(5), 제2주요부의 핀(20) 및 부착나사(17)의 상부함몰부에 삽입되는 탄력체 돌출핀에 의해 고정된다.
본 발명에 따른 기구는 다음과 같이 응용된다. 첫째, 손상되거나 결함이 있는 제1도에서 A로 표시된 본래의 손목부를 제거한다. 다음에, 척골에 통상의 방법으로 티타늄 고정체(32)를 부여하여 조절부재(27)상의 나사선을 수용할 수 있도록 한다.
필요한 준비를 한 후, 제1주요부의 정면판(2)을 부착하여 상기판의 실질적으로 납작한 정면판(C)이 제1도의 선(D)로 표시된 면에 위치시킨다.
중수골(Ⅲ)내로 부착나사(8)를 나사결합하여 정면판(2)을 고정시킨다(제7도).
다음으로는, 제2주요부의 정면판(15)를 부여하여 그 평평면(E)을 제1도의 선(F)와 실질적으로 동일한 평면에 위치시킨다. 그런후에, 요골안으로 부착나사(17)를 나사결합하여 정면판(15)을 제위치에 고정시킨다.
계속해서, 삽입체(24)를 안내체(22)에 위치시키고, 그 위에 조절부재(27)가 삽입체(24)를 상부에서 하부로 관통하도록 한다. 그리고, 하부접속부(31)를 고정체(32) 내부로 나사결합한다. 접속부(31)가 나사결합되는 정도는 너트 등의 정지부재를 이용하여 다양하게 할 수 있다. 마지막으로, 관절기구(33)를, 정면판 사이에 배치하여 상술한 바와같이 고정시킨다.
상술한 방법으로 적용된 기구는 정상적인 관절과 양립할 수 있는 정도의 관절기능을 가진다. 관절기구(33)에 의해서 손목의 주요 굽힘운동이 달성되며, 조절부재(27)의 배치에 의해서 회전각도 ±90°의 소망의 손목회전(즉, 척골을 중심으로 하는 요골의 회전)을 달성할 수 있다.
본 발명은, 물론 도면에 나타낸 실시예에 국한되지 않으며 여러가지 방법으로 변화가 가능하다. 따라서, 본 발명은 명세서에 개시된 내용에 의해서 제한받지 않으며, 단지 청구항에 의해 제한받을 뿐이다.

Claims (9)

  1. 관절, 특히 손목관절 접합용 기구로서, 손의 방향으로 향하는 외향대면과 그 반대편의 내향대면을 가지는 제1정면판, 제1정면판으로 지지되고 정면판의 대면의 외향에서 돌출되어 제1정면판의 대면의 외향에 있는 골질방향으로 뻗어 있는 제1고정핀을 포함하는 제1주요부(1) : 손의 반대방향으로 향하는 제2내향대면과 그 반대편의 제2외향대면을 구비한 제2정면판, 제2정면판상에 지지되고 제2판의 제2내향대면에서 골질내부로 뻗어 있는 제2고정핀을 포함하는 제2주요부(14) : 제1정면판의 내향대면과 제2정면판의 외향대면 사이에 위치하며, 제1, 제2정면판 및 제3주요부상에 위치하고 상기 제1 및 제2정면판을 함께 고정하기 위한 수단 및 손목 부위에서 손의 동작이 용이하도록 변형 가능한 재질의 탄성중합체로 구성되는 제3주요부를 포함하는 것을 특징으로 하는 관절접합용 기구.
  2. 제1항에 있어서, 상기 제1 및 제2주요부에 대한 제3주요부의 부착물은 제3주요부에서 제1 및 제2주요부로 돌출된 핀을 포함하는 것을 특징으로 하는 관절접합용 기구.
  3. 제1항에 있어서, 제2주요부에 제한되는 안내수단 및 상기 안내수단으로부터 손목척골 쪽으로 뻗어나 있는 조절부재를 더 포함하며, 상기 안내수단은 제2정면판에 연결되고 접합된 손목의 회전운동이 가능하도록 형태와 위치가 정해지는 것을 특징으로 하는 관절접합용 기구.
  4. 제3항에 있어서, 상기 제2고정핀이 손목에서 요골쪽으로 뻗어 나가는 것을 특징으로 하는 관절접합용 기구.
  5. 제3항에 있어서, 상기 안내수단은 슬롯형 안내개구부가 설치된 안내체를 가지는 제2정면판으로 구성되며, 상기 조절부재는 척골안으로 뻗어나가서 그곳에 고정되며, 상기 조절부재는 상기 안내개구부 및 안내체의 외부에 위치하며, 상기 조절부재와 상기 안내체의 조합에 의해서 손목의 동작이 가능하도록 하는 것을 특징으로 하는 관절접합용 기구.
  6. 제5항에 있어서, 상기 조절부재는 상기 슬롯형 안내개구부에 대해서 측방향 및 수직방향으로 회전 및 직선이동 가능하도록 상기 조절부재 및 상기 슬롯형 안내개구부의 형태가 갖추어진 것을 특징으로 하는 관절접합용 기구.
  7. 제6항에 있어서, 안내체에 삽입되는 중합체 재질의 소매형 부분 및 상기 소매형 부분에 한정되는 슬롯형 안내 개구부를 더 포함하는 것을 특징으로 하는 관절접합용 기구.
  8. 제7항에 있어서, 상기 소매형 부분은 중합체 재질로 구성되는 것을 특징으로 하는 관절접합용 기구.
  9. 제8항에 있어서, 삽입체 및 관절기구를 제외하고는, 부품들이 티타늄으로 구성되는 것을 특징으로 하는 관절접합용 기구.
