Claims (36)
- 2개의 관절면과 그 사이의 간극을 포함하는 비인간 동물체의 활막성 관절 내 유합 장치를 이식함으로써 상기 관절을 유합시키는 방법으로서,
액화성 재료를 포함하고 상기 액화성 재료에 에너지를 전달하여 액화시킬 수 있는 상기 유합 장치를 제공하는 단계, 및
상기 간극에 또는 상기 간극을 가로질러 유합 장치를 이식하고, 이식 중에 액화성 재료의 적어도 일부를 액화시키기에 충분한 시간 동안 에너지를 액화성 재료에 전달하고, 상기 간극의 영역에 있는 골 조직으로 액화성 재료를 침투시켜 재고화시 골 조직과 돌출 끼움 연결구조를 형성하는 단계를 포함하는 방법. - 제1항에 있어서, 상기 에너지가 액화성 재료에서 또는 근처에서 마찰열로 변형되는 진동 또는 회전 에너지이거나, 상기 에너지가 흡수되어 액화성 재료에서 또는 근처에서 열에너지를 생성하는 가시광선 또는 적외선 주파수 범위의 전자기 방사선 에너지인 방법.
- 제1항에 있어서, 상기 활막성 관절이 후관절, 손, 손가락, 발 또는 발가락의 관절, 천장관절, 흉쇄관절, 흉골늑골관절 및 늑골척추관절 중 하나인 방법.
- 제1항에 있어서, 상기 이식 단계 전에 관절면을 소정의 위치에 상호 고정시키는 단계와 이식 단계 후에 관절면의 고정을 해제하는 단계를 더 포함하는 방법.
- 제1항에 있어서, 상기 이식 단계 전에 또는 고정 단계 후에 관절면으로부터 조직의 제거 및 관절면에 인접한 조직의 제거 중 적어도 하나를 수행함에 의해 관절을 준비하는 단계를 더 포함하는 방법.
- 제5항에 있어서, 상기 조직 제거는 관절면의 연골층 및 연골층 바로 아래 또는 연골층으로부터 일정 거리에 있는 골 조직 중 적어도 하나와 관련이 있는 방법.
- 제5항에 있어서, 상기 유합 장치가 상기 간극에 이식되고 상기 준비 단계는 관절면 중의 적어도 하나에 홈(11)을 제공하거나 관절면의 각각에 한 쌍의 대향 홈(11)을 제공하는 것을 포함하거나, 또는 상기 유합 장치가 상기 간극을 가로질러 이식되고 상기 준비 단계는 상기 간극의 어느 한 면 위에 있는 골 조직에 개구(13)를 제공하는 것을 포함하는 방법.
- 제7항에 있어서, 상기 유합 장치가 표면 위에 액화성 재료가 배치되는 적어도 하나 이상의 고착부(1)를 포함하고, 상기 이식 단계가 고착부(1)의 상기 표면이 홈(11) 또는 개구(13)에서 골 조직과 접촉하도록 유합 장치를 위치시키고 에너지를 고착부(1)의 인접 단부에 결합시키는 것을 포함하는 방법.
- 제8항에 있어서, 상기 에너지가 진동에너지이고 이 진동에너지가 상기 표면과 골 조직 사이에 마찰을 일으키도록 상기 장치를 위치시키는 방법.
- 제8항에 있어서, 상기 에너지가 가시광선 또는 적외선 주파수 범위의 전자기 방사선 에너지이고, 이 전자기 방사선 에너지를 흡수하고 흡수에 의해 생성되는 열에너지를 상기 표면의 액화성 재료로 전달하기 위해 상기 장치를 구성하는 방법.
- 제7항에 있어서, 상기 유합 장치가 적어도 하나 이상의 고착부(1)를 포함하고 고착부(1)의 일부를 구성하는 구멍이 형성된 외피(52) 내부에 액화성 재료가 배치되어 있으며, 상기 이식 단계는 외피(52)의 상기 구멍이 홈(11) 또는 개구(13)에 있는 골 조직과 접촉하도록 유합 장치를 위치시키고 에너지를 구멍이 형성된 외피(52) 내부에 위치되어 있는 액화성 재료의 인접면에 결합시키는 것을 포함하는 방법.
- 제11항에 있어서, 상기 에너지가 구멍이 형성된 외피와 액화성 재료 사이에 마찰을 일으키는 진동 또는 회전에너지인 방법.
- 제11항에 있어서, 상기 에너지가 가시광선 또는 적외선 주파수 범위의 전자기 방사선 에너지이고, 이 전자기 방사선 에너지를 흡수하고 흡수에 의해 생성되는 열에너지를 액화성 재료로 전달하기 위해 상기 장치를 구성하는 방법.
- 제8항 내지 제13항 중 어느 한 항에 있어서, 상기 유합 장치가 적어도 하나 이상의 안정화부(2)를 더 포함하고, 상기 고착부와 안정화부가 서로 고정되어 상기 이식 단계가 1단계 공정이거나, 상기 고착부와 안정화부가 별도의 장치 구성부를 구성하고 상기 이식 단계가 2- 또는 다-단계 공정이고, 상기 적어도 하나 이상의 안정화부(2)가 관절면 사이의 간극에 위치되기 전 또는 후에 적어도 하나 이상의 고착부(1)가 골 조직에 고착되는 방법.
- 제1항 내지 제13항 중 어느 한 항에 있어서, 상기 이식 단계를 위해 유합 장치가 진동 또는 푸싱 도구(27)의 말단부에 고정되는 방법.
- 제1항 내지 제13항 중 어느 한 항에 있어서, 고착부와 안정화부 중 적어도 하나 이상이 완전히 액화성 재료로 이루어지고 이식 단계 중에 변형되는 방법.
- 2개의 관절면과 그 사이의 간극을 포함하는 인간 또는 동물 환자의 활막성 관절을 유합시키기 위한 유합 장치로서,
적어도 하나 이상의 고착부(1)와 적어도 하나 이상의 안정화부(2)를 포함하되,
상기 유합 장치는 이식 방향(I)에 평행하고 인접면(4)으로부터 말단부로 연장되어 있는 전체 깊이(D)와 상기 이식 방향(I)에 수직인 전체 폭(W)과 두께 형태(T1, T2)를 갖고,

- 제19항에 있어서, 상기 골 성장 촉진 재료가 동종이식 또는 자가이식 골 재료, 골 대체재, 스펀지 및 BMP 운반체 중 적어도 하나 이상인 유합 장치.
- 제17항 내지 제20항 중 어느 한 항에 있어서, 적어도 하나 이상의 고착부(1)가 고착부(1)의 표면 위 또는 고착부(1)의 일부를 구성하는 구멍이 형성된 외피(52) 내에 배치되는 액화성 재료를 포함하고, 고착부(1) 또는 상기 구멍이 형성된 외피(52)의 상기 표면이 유합 장치 위에 배치되어 이식시 골 조직과 접촉하게 되는 유합 장치.
- 제21항에 있어서, 상기 액화성 재료 또는 상기 액화성 재료에 인접한 다른 장치 영역이 가시광선 또는 적외선 주파수 범위의 전자기 방사선 에너지를 흡수할 수 있는 유합 장치.
- 제21항에 있어서, 상기 유합 장치 전체가 액화성 재료로 제조되는 유합 장치.
- 제17항에 있어서, 안정화부(2)가 골 유착을 증진시킬 수 있는 코팅 및 표면 구조 중 적어도 하나가 마련된 표면을 포함함으로써 골 유착을 촉진하도록 구성되는 유합 장치.
- 제17항에 있어서, 고착부(1)의 깊이가 안정화부(2)의 깊이보다 더 큰 유합 장치.
- 제25항에 있어서, 2개의 핀 형상의 고착부(1)와 하나의 안정화부(2)를 포함하고, 상기 안정화부가 고착부(1) 사이에 배치되거나, 상기 고착부와 안정화부가 별도의 장치 구성부를 구성하고 안정화부(2)가 고착부(1)의 인접 단부에 고정되어 그 사이에서 연장되도록 구성되거나, 안정화부(2)가 고착부(1)를 수용하기 위한 관통 개구(55) 또는 홈(60)을 포함하는 유합 장치.
- 제17항에 있어서, 인접 장치면(4)이 볼록한 만곡부 및 하나 이상의 개구(5) 또는 돌출부 중 적어도 하나를 포함하는 유합 장치.
- 제21항에 있어서, 상기 액화성 재료가 열가소성이고 적어도 0.5 GPa 이상의 탄성률과 최대 350 ℃의 융점을 갖는 재료인 유합 장치.
- 2개의 관절면과 그 사이의 간극을 포함하는 비인간 동물체의 활막성 관절을 유합시키는 방법으로서,
제21항에 따른 유합 장치를 제공하는 단계,
상기 관절면 중 적어도 하나 이상을 준비하는 단계, 및
상기 적어도 하나 이상의 고착부가 상기 적어도 하나 이상의 준비된 관절면의 영역에서 골 조직과 접촉하는 배치 구조에 유합 장치를 밀어넣는 단계를 포함하되,
상기 밀어넣는 단계는 밀어넣는 도중 또는 이후에 유합 장치를 진동 또는 회전 구동장치 또는 가시광선 또는 적외선 주파수 범위의 전자기 방사선 에너지 공급원에 결합시키고 상기 구동장치 또는 공급원을 액화성 재료의 적어도 일부를 액화시키기에 충분한 시간 동안 기동시킴으로써 유합 장치의 인접면(4)에 에너지를 결합시켜 액화성 재료가 상기 골 조직으로 침투되도록 하여 재고화 후 상기 골 조직과 돌출 끼움 연결구조를 형성하는 것을 포함하는 방법. - 2개의 관절면과 그 사이의 간극을 포함하는 비인간 동물체의 활막성 관절을 유합시키는 방법으로서,

- 제32항 내지 제34항 중 어느 한 항에 있어서, 축방향 터널(32)을 갖거나 또는 진동 또는 푸싱 도구(27)의 단면에 맞도록 구성된 단면을 가진 가이드 부시(21)를 더 포함하고, 가이드 부시(21)의 말단면은 관절면 사이 간극의 어느 한 면 위의 골 표면으로 강제 도입될 수 있도록 배치된 복수 개의 스파이크를 구비하여 소정의 위치에서 관절면을 상호 고정시키는 도구 세트.
- 제35항에 있어서, 후관절 탐지기(20), 드릴 가이드(22)와 이에 상응하는 드릴(23) 및 절단기 가이드(24)와 이에 상응하는 절단기(25) 중 적어도 하나 이상을 더 포함하고, 후관절 탐지기(20), 드릴 가이드(22) 및 절단기 가이드(24)가 가이드 부시(21)의 축방향 터널(32)의 단면에 맞게 구성된 단면을 갖는 도구 세트.
