Description
したがって、本発明は、以下の項目を提供する:
(項目1)
個体の線維症性障害または線維増殖性障害を治療する方法であって、この障害に罹患している、またはこの障害を発症するリスクに瀕しているヒトまたは獣医の患者に1つ以上の線維細胞抑制薬および1つ以上の線維化誘導因子拮抗薬または抗線維症剤の組み合せを投与するステップを含む、方法。
(項目2)
個体の線維症性障害または線維増殖性障害の治療を提供するキットであって:

上記患者が腎臓に関連する線維化性疾患から生じる状態を有し、この線維化性疾患が増殖性糸球体腎炎、硬化性糸球体腎炎、腎性線維化性皮膚症、糖尿病性腎症、腎尿細管間質性線維症、および巣状分節性糸球体硬化症から成る群より選択される、項目1に記載の方法。

線維症を引き起こすイベントの制御には、少なくとも2つの主要なイベントが含まれる。1つは線維細胞の増殖および分化である。線維細胞は、末梢血単球に由来する線維芽細胞様細胞の独特な集団であり、通常、組織損傷部位に進入して血管新生や創傷治癒を促進する。線維細胞はCD14+末梢血単球から分化して、他のPBMC細胞から分化することができる。SAP、IL−12、ラミニン−1、抗FcγR抗体、架橋IgGおよび/または凝集IgGの存在は、本プロセスを阻害する、または少なくとも部分的に遅延させることがある。
本明細書で使用するように、「治療」、「治療すること」などは所望の薬理学的および/または生理学的効果を得ることを指す。効果は、障害またはその症状を完全または部分的に予防するという点で予防的でありうる、および/または線維性または線維増殖性障害および/または該障害に起因する悪影響の完全または部分的な治癒という点で治療的でありうる。「治療」は本明細書で使用するように、哺乳類、特にヒトでの疾患のいずれの治療も対象として、(a)生存期間を延長することと;(b)疾患による死のリスクを低下させることと;(c)疾患にかかりやすいかもしれないが、疾患を有するとしてまだ診断されていない対象において、疾患が発生するのを防止することと;(d)疾患を抑制すること、すなわちその進展を停止させること(たとえば疾患進行速度を低下させること)と;(e)疾患を軽減すること、すなわち疾患の退行を引き起こすことと;を含む。
A.線維細胞の増殖および分化の抑制薬
本発明の併用療法の1つの成分は、線維細胞形成を抑制する薬剤である。これらの線維細胞抑制薬は、それらが本明細書で総称的に呼ばれるように、線維細胞の形成を抑制するか(たとえば分化または増殖を阻害する)、または他の実施形態において、単球の除去(細胞死)を引き起こすかのどちらかの方式で、CD14[+]末梢血単球に作用しうる薬剤である。
(i)血清アミロイドP
線維細胞がCD14+末梢血単球から分化することが可能であり、ヒト血清の存在が本過程を劇的に遅延させることが先に確認されている。線維細胞分化を阻害するヒト血清中の因子は、血清アミロイドP(SAP)である。SAPは、C反応性タンパク質(CRP)を含むタンパク質のペントラキシンファミリーのメンバーであり、肝臓によって産生されて、血中に分泌され、安定な五量体として血中を循環する。SAPは1分子当たり5つの考えられるFcγR結合部位を持つ五量体タンパク質であるため、SAPは、各種の細胞でIgG抗体のFc部分の受容体(FcγR)に結合して、さらにタンパク質を用いずにFcγRを効果的に架橋することができる。SAPがFcγRに結合すると、FcγR活性化と整合性のある細胞内シグナル伝達イベントが開始される。
本発明の詳細な実施形態において、SAPを含有する組成物は、標的位置でのSAP濃度を少なくとも約0.5μg/mlまで上昇させるように操作可能である。ヒトでは、先にI125放射性標識SAPを投与して、アミロイドーシス患者を調査した。治療では、SAP約600μgをヒト成人に投与した。したがって、ヒト成人へのSAP約600μgの全身投与は安全である。より多い投薬量も適切な条件下では安全であるかもしれない。
IL−12は、先に線維症および線維化性疾患に関与してきたが、大半の研究は、Th1免疫応答を促進することにおける、またはインターフェロン−γの産生を誘発することによる、IL−12の役割に焦点を合わせてきた。
ラミニンは、単球の循環から組織中への移動に関与する細胞外基質タンパク質である。白血球は、組織に進入するために、内皮細胞と、血管壁の周囲基底膜を通過しなければならない。本過程は、白血球の内皮細胞での係留、回転および停止を含む。内皮細胞への付着後に、白血球は次に内皮細胞の間を横切って、血管壁を介して、組織中へ通過する。血管壁を通じた細胞の血管外遊出過程は、その表現型と機能を改変する。
抗FcγR抗体が末梢血単球の線維細胞への分化を防止できることも確認されている。抗FcγR抗体は、IgG抗体のFc部分の受容体(FcγR)に結合するIgG抗体である。抗FcγR抗体は、その定常(Fc)領域を介してではなく、その可変領域を介して結合する。しかし適切な源からのIgG(たとえばヒト受容体へのヒトIgG)は通常、そのFc領域を介してFcγRに結合できる。FcγRは各種の造血細胞表面に見出される。FcγRには4つの別個のクラスがある。FcγRI(CD64)は、末梢血単球によって発現され、モノマーIgGに高い親和性で結合する。FcγRII(CD32)およびFcγRIII(CD16)は、IgGの低親和性受容体であり、凝集IgGのみに効率的に結合する。FcγRIIは末梢血B細胞および単球によって発現されるが、FcγRIIIはNK細胞および単球亜群によって発現される。FcγRIVは、最近、マウスで同定され、マウス末梢血単球および好中球、マクロファージおよび樹状細胞に存在して、マウスIgG2aおよびIgG2b抗体に効率的に結合する。推定ヒトFcγRIV遺伝子があるが、そのタンパク質の生物学的機能、たとえばリガンド特異性および細胞発現は今のところ知られていない。
架橋または凝集IgGは、少なくとも2つのこのようなIgG抗体が相互に物理的に結合されているという条件で、そのFc領域を介して標的FcγRに結合可能であるいずれのIgGも含みうる。
本発明に含まれるIL−15拮抗薬としては、これに限定されるわけではないが、抗IL−15抗体、抗IL−i5R抗体、可溶性IL−15R、IL−15ムテイン、抗IL−15小型分子および抗IL−15R小型分子を含む、IL−15活性を拮抗または阻害する(すなわちIL−15媒介抗アポトーシス)多種多様の分子が挙げられる。IL−15活性を阻害することができる他の拮抗薬、たとえば結合タンパク質およびペプチドミメティックスも含まれる。詳細な実施形態において、拮抗薬はIL−15Rα、β、およびγサブユニットの構築を妨害することが可能であり、たとえば拮抗薬は、IL−15のβまたはγ鎖相互作用ドメインに位置するエピトープに結合する。別の実施形態において、拮抗薬はIL−15ムテイン、たとえばIL−15Rαへの結合は可能であるが、IL−15Rのβ−および/またはγ−サブユニットの片方または両方に結合することはできず、したがってシグナル伝達を引き起こすことができないIL−15変異体である。
主題の併用療法の別の成分は、線維化誘導因子を阻害または拮抗する薬剤、たとえば線維症性組織の形成および維持に関与する1つ以上の成長因子またはサイトカインを拮抗する薬剤である。この方法で、本発明の組成物および方法は、併用治療の一部として線維細胞分化ならびに線維症性組織の形成および維持の両方を目的としている。
形質転換成長因子(TGF)ベータファミリのサイトカインは、創傷治癒や組織修復で中心的な役割を果たし、すべての組織で見出される。TGF−βは、多くの実質細胞タイプはもちろんのこと、リンパ球、単球/マクロファージ、および血小板などの浸潤細胞によっても産生される。創傷形成または炎症の後に、このような細胞は潜在的なTGF−β源である。一般に、TGF−βは各種の細胞外基質タンパク質の産生を刺激して、これらの基質タンパク質の分解を阻害し、組織線維症を促進し、このことすべてが罹患組織の修復と再建に寄与する。多くの疾患において、過剰なTGF−βは、正常な臓器機能を損なうことがある病的に過剰な線維症の原因となる。
ある実施形態において、線維化誘導因子拮抗薬は、抗線維化効果を有することが公知なサイトカイン、たとえばIFN−γ、BMP−7、HGFまたはIL−10によって置き換えまたは増強することが可能である。

線維症は一般に、コラーゲン性結合組織の病的または過剰蓄積を特徴とする。主題の方法による治療を適用できる線維症性障害としては、これに限定されるわけではないが、膠原病、間質性肺疾患、ヒト線維性肺疾患(たとえば閉塞性細気管支炎、特発性肺線維症、公知の病因による肺線維症、肺疾患における腫瘍間質、肺に影響を及ぼす全身性強皮症、ヘルマンスキー−パドラック症候群、炭坑作業員塵肺症、石綿沈着症、珪肺症、慢性肺高血圧症、AIDS関連肺高血圧症、サルコイドーシスなど)、線維性血管疾患、動脈硬化、アテローム性動脈硬化、拡張蛇行静脈、冠梗塞(coronary infarct)、脳梗塞、心筋線維症、筋骨格線維症、術後癒着、ヒト腎臓疾患(たとえば腎炎症候群、アルポート症候群、HIV関連腎症、多発性嚢胞腎疾患、ファブリー病、糖尿病性腎症、慢性糸球体腎炎、全身性狼瘡に関連する腎炎など)、皮膚ケロイド形成、全身性進行性硬化症(PSS)、原発性硬化性胆管炎(PSC)、肝線維症、肝硬変、腎線維症、肺線維症、嚢胞性線維症、慢性移植片対宿主病、強皮症(局所性および全身性)、グレーヴス眼症、糖尿病性網膜症、緑内障、ペイロニー病、陰茎線維症、膀胱鏡を使用する検査後の尿道狭窄症、術後内部癒着、瘢痕化、骨髄線維症、特発性後腹膜線維症、公知の病因による腹膜線維症、薬物誘発麦角中毒、良性または悪性癌から起こる線維症、微生物感染(たとえばウイルス性、細菌性、寄生虫性、真菌性など)から起こる線維症、アルツハイマー病、炎症性腸疾患から起こる線維症(クローン病および顕微鏡的大腸炎での狭窄形成)、化学的または環境的傷害(たとえば癌化学療法、殺虫剤、放射線(たとえば癌放射線療法)など)に誘発される線維症などが挙げられる。
本発明は、特発性肺線維症(IPF)の治療方法を提供する。該方法は一般に、IPFを有する個体に線維細胞抑制薬の有効量および線維化誘導サイトカイン拮抗薬の有効量の組み合せを投与するステップを含む。
本発明は、臨床肝線維症を減少させることと、肝線維症が発生する可能性を低下させることと、肝線維症に関連するパラメータを低下させることとを含む、肝線維症を治療する方法を提供する。多くの実施形態で特に興味深いのは、ヒトの治療である。
腎線維症は、細胞外基質(ECM)成分の過剰な蓄積を特徴とする。TGF−βの過剰産生は、ECMの過乗な沈着によって引き起こされる組織線維症の根底にあり、疾患を生じると考えられる。TGF−βの線維形成作用は、基質タンパク質合成の同時刺激、基質変性の阻害、およびECM構築を促進する増強されたインテグリン発現から生ずる。
(i)ブレオマイシン誘発肺線維症
体重が200〜250グラムのオスのSprague Dawleyラットに肺線維症を発生させる。第0日に、0.9%塩化ナトリウムに溶解させたブレオマイシン2.5〜6.67U/kgの気管内用量を(経口経路で)1.5mL/kgの体積で投与する。試験の第1、3、5、7および9日に、処置群のラットに尾静脈を介してSAP 1.6mg/kgを1.3mL/kgの用量体積で静脈内に投薬する。未処置ラットには生理食塩水1.3mL/kgを投薬する。第14日に血中酸素飽和度(パルスオキシメトリー)および/またはPO2(血液ガス測定装置)を測定することによって、肺機能を評価する。次に動物を屠殺して、左肺を総コラーゲン含有量(Sircolアッセイ)について処理して、右肺を10%ホルマリンで固定し、切片にして、シリウスレッドならびにヘマトキシリンおよびエオシンで染色して、コラーゲン沈着を評価する。(Cortijoら、Attenuation by oral N−acetylcystein of bleomycin−induced lung injury in rats.Eur Respir J 17:1228−1235,2001を参照されたい)。
体重が200〜225グラムのオスのウイスターラットに肝線維症を発生させる。第0日に、ラットにオリーブ油中のCCl4(0.08mLのCCl4/mLオリーブ油;初回用量412mg CCl4/kg)またはオリーブ油単独(対照)の胃内投薬を与える。ラットに研究期間中にわたって週2回、CCl4を投薬し、体重変化に基づいて毎週投薬量を調整して死亡率を低下させる。処置ラットには、第1日から開始して1日おきに1.6mg/kgのSAPを腹腔内投薬する;対照ラットには同じ体積のビヒクルを投薬する。第24日にラットを屠殺して、体および肝臓の重量を評価し、分析のために肝組織を採取する。総コラーゲン含有量をSircolアッセイで測定して、コラーゲン沈着をMassonトリクロームおよびシリウスレッド染色によって測定する。筋線維芽細胞活性化は、α−SMAの免疫染色によって決定する。(Parsons CJら、Antifibrotic effects of a tissue inhibitor of metalloprotein−ase−1 antibody on established liver fibrosis in rats.Hepatology 40:1106−1115,200およびRivera CAら、Attenuation of CCL4−induced hepatic fibrosis by GdCl3 treatment or dietary glycine.Am J Physiol Gastrointest Liver Physiol 281:G200−G207,2001を参照されたい)。

ラットの一側尿管閉塞(UUO)は、腎線維症の適切なモデルである。体重が200〜250グラムのSprague Dawleyラットで腎線維症を誘発させた。ラットは、ケタミン(100mg/kg)およびキシラジン(5mg/kg)で麻酔した。すべての外科処置は無菌技法を使用して実施した。
Claims (5)
- 血清アミロイドP(SAP)を含む、患者における線維症を治療するための組成物であって、該組成物は、エナラプリルと組み合せて投与されることを特徴とする、組成物。

- 前記血清アミロイドP(SAP)およびエナラプリルが、同時に投与されることを特徴とする請求項1に記載の組成物。
- 前記血清アミロイドP(SAP)およびエナラプリルが、相互に患者内で重複治療濃度を生成する時間内に投与されることを特徴とする請求項1に記載の組成物。
- 前記患者が腎線維症を有する請求項1に記載の組成物。
