Description

(技術分野)
本開示は、手術手順の間に器官を引き込むために使用されるデバイスおよび方法、より具体的には、最小限に侵襲性の手術の間に器官を引き込むかまたは位置づけるための内視鏡装置に関する。
手術用器機における最近の技術的進歩の結果として、最小限に侵襲性の技術を使用する手術手順は、慣用的に行われており、その手術手順は、あまり患者に外傷を生じさせない。いくつかの最小限に侵襲性の手術手順には、多くの一般に使用されるカニューレシステムおよびトロカールシステムのその小さな内径に起因して、制限がある。結果として、器機製造業者は、変形可能なフレーム開口部を利用して、トロカールを通って、患者に適合した多くの一般的器機(例えば、器官開創器および組織回収バッグ)を使用するための革新的かつ新規の解決法を見つけ出さねばならない。実際に、いくつかの器官開創器および組織回収バッグは、膨張可能なフレームを使用し、この膨張可能なフレームは、組織を操作するために使用される構成要素を形成する。例示的な膨張可能器官引き込みデバイスは、Mollらの米国特許第5,823,945号に開示され、その内容全体は、本明細書中に参考として援用される。
本開示は、最小限に侵襲性の手術の間に器官を引き込むための内視鏡開創器装置に関する。この装置は、近位端部および遠位端部、ならびにこの近位端部と遠位端部との間に延びる管腔を有する細長シャフト、この細長シャフトと操作可能に結合された開創スリーブ(retracting sleeve)を備える。この開創スリーブは、上記細長シャフトの管腔中に各々配置可能な第1スピンドルおよび第2スピンドルであって、各スピンドルは、この細長シャフトの管腔の少なくとも一部を通って選択的に並進可能である、第1スピンドルおよび第2スピンドル;ならびにこの第1スピンドルの遠位端部と第2スピンドルの遠位端部との間に延びる吊り下げ部分(sling portion)であって、この吊り下げ部分は、その中に器官を受容するためのクレードル(cradle)を規定し、この吊り下げ部分は、この第1スピンドルおよび第2スピンドルのうちの少なくとも一方の並進の際に選択的に移動可能である、吊り下げ部分、を備える。
本明細書中に開示される器官開創器の好ましい実施形態は、ここで図面を参照して詳細に記載され、類似の参照番号は、類似または同一の要素と同一視する。図面および以下の説明において、用語「近位」とは、従来通り、術者に最も近い、本開示の手術デバイスまたは器機の端部をいう一方、用語「遠位」とは、術者から最も遠いそのデバイスまたは器機の端部をいう。
Claims (18)
- 最小限に侵襲性の手術の間に器官を引き込むための内視鏡開創器装置であって、
近位端部、遠位端部、および該近位端部と前記遠位端部との間に延びる管腔を有する細長シャフトと、
該細長シャフトと操作可能に結合された開創スリーブと、を備え、
該開創スリーブは、
該細長シャフトの前記管腔中に各々配置可能な第1スピンドルおよび第2スピンドルであって、前記細長シャフトの管腔の少なくとも一部を通って選択的に並進可能である第1スピンドルおよび第2スピンドルと、
前記第1スピンドルの遠位端部と該第2スピンドルの遠位端部との間に延び、メッシュ、リボンおよび布のうちの1つから製作された吊り下げ部分であって、その中に器官を支持するためのクレードルを規定し、前記第1スピンドルおよび第2スピンドルのうちの少なくとも一方の並進の際に選択的に移動可能である吊り下げ部分とを備え、
前記開創スリーブは、前記第1および第2のスピンドルが前記管腔内に配置される第1引込み位置と、前記第1および第2のスピンドルの遠位端が前記管腔から展開される第2展開位置とをとる、
ことを特徴とする内視鏡開創器装置。 - 前記細長シャフト、前記第1スピンドルおよび前記第2スピンドルの各々の近位端部に操作可能に接続された制御部分をさらに備え、
該制御部分は、該第1スピンドルおよび第2スピンドルと協働して、ユーザーが、前記器官と係合するように前記吊り下げ部分を遠隔的に動かすことを可能にする、
請求項1に記載の内視鏡開創器装置。 - 前記制御部分は、前記開創スリーブに対して、所望の部位に前記吊り下げ部分を維持するためのロッキング機構を備える、
請求項2に記載の内視鏡開創器装置。 - 前記ロッキング機構は、展開位置および引込位置のうちの少なくとも一方で前記吊り下げ部分を維持する、
請求項3に記載の内視鏡開創器装置。 - 前記開創スリーブの前記吊り下げ部分が前記細長シャフトの前記管腔に少なくとも部分的に保持された第1位置と、該開創スリーブの該吊り下げ部分が該細長シャフトの該管腔に保持されない第2位置とをとる、
請求項1に記載の内視鏡開創器装置。 - 前記内視鏡開創器装置が第2位置にある場合、前記吊り下げ部分は、前記器官を包み込む支持領域を規定する、
請求項5に記載の内視鏡開創器装置。 - 前記吊り下げ部分は、該吊り下げ部分の組織接触表面に沿って設けられた、前記吊り下げ部分と前記器官との間の把持力を増強するリブを備えている、
請求項1に記載の内視鏡開創器装置。 - 前記吊り下げ部分は、該吊り下げ部分の組織接触表面に沿って形成された複数のポートを備え、
少なくとも前記ポートの少なくとも1部材が真空源と流体連通している、
請求項1に記載の内視鏡開創器装置。 - 前記吊り下げ部分は、制御部分に操作可能に接続されており、該制御部分は、該吊り下げ部分に形成された前記複数のポートの前記一部を減圧するように構成された、
請求項8に記載の内視鏡開創器装置。 - 前記複数のポートの前記一部は、前記第1スピンドルおよび前記第2スピンドルのうちの少なくとも一方を介して、減圧される、
請求項9に記載の内視鏡開創器装置。 - 該装置は、前記器官の周りに前記吊り下げ部分の少なくとも一部を締め付けるように構成および適合されたシンチをさらに備える、

該細長管状本体の遠位端部から延びるループとを備え、
該ループは開口部を規定し、該ループの開口部は、前記細長管状本体からループを解放することにより拡大可能であり、該ループの開口部は、前記細長管状本体にループを引っ込めることによって縮小可能である、
