- 複数の不整頻脈心臓事象間を鑑別する装置であって、
前記不整頻脈を示す信号を受信する入力回路と、
検出された前記不整頻脈に応答して一連のペーシングパルスを生成する第1の出力回路と、を含み、
以下の特徴、即ち、
前記一連のペーシングパルスの後の、前記入力回路によって受信された内因性心拍を示す信号が、
前記一連のペーシングパルスの最後のものと、前記入力回路による内因性心拍の検出との間の経過期間(T3)が、所定の鑑別閾値(T4)より小さい場合に、第1のタイプの不整頻脈に対応し、
又は、
前記一連のペーシングパルスの最後のものと、前記内因性心拍の検出との間の前記経過期間が、前記所定の鑑別閾値より大きい場合に、第2のタイプの不整頻脈に対応すること、
をマイクロプロセッサが決定すること、を有する、
複数の不整頻脈心臓事象間を鑑別する装置。 - 前記生成されたペーシングパルスに引き続いて前記入力回路によって受信された信号が、前記第1のタイプの不整頻脈に相当することに応答して、前記マイクロプロセッサが、引き続き予定されていた治療の実施を許可する、請求項1に記載の装置。
- 前記マイクロプロセッサが、前記生成されたペーシングパルスに引き続いて前記入力回路によって受信された信号が、前記第1のタイプの不整頻脈に相当しないことに応答して、引き続き予定されていた治療の実施を抑止する、
請求項1又は2に記載の装置。 - ショック治療を行う第2の出力回路を更に含み、
前記第1の出力回路は、前記第2の出力回路の充電中に前記パルスを生成する、
請求項1、2又は3に記載の装置。 - ショック治療を行う第2の出力回路をさらに含み、前記第1の出力回路は、前記第2の出力回路の充電より先に前記パルスを生成する、
請求項1、2又は3に記載の装置。 - 前記第1の出力回路は、前記入力回路が、所定期間(T1)を隔てた、前記第1のタイプの不整頻脈のレートに相当する連続信号を受信したことに応答して、前記パルスを生成する、
請求項1ないし5のいずれかに記載の装置。 - 前記第1の出力回路は、前記入力回路が、所定期間(T1)を隔てた、かつ前記第1のタイプの不整頻脈のレートに相当する連続信号を受信したことに応答して、前記パルスを生成し、
前記経過期間が、前記所定期間に対して正規化される、
請求項1ないし5のいずれかに記載の装置。 - 前記第1の出力回路は、前記入力回路が、所定期間(T1)を隔てた、かつ前記第1のタイプの不整頻脈のレートに相当する連続信号を受信したことに応答して、前記パルスを生成し、
前記所定の鑑別閾値が、前記所定期間に対する一定の割合である、
請求項1ないし5のいずれかに記載の装置。 - 前記所定の鑑別閾値は、前記所定期間のおよそ190%に相当する、請求項8に記載の装置。
- 前記第1の出力回路によって生成された前記パルスは、抗頻脈ペーシングパルスに相当する、請求項1ないし9のいずれかに記載の装置。
- 前記マイクロプロセッサが、前記生成されたペーシングパルスに引き続いて前記入力回路によって受信された信号が、前記第2のタイプの不整頻脈に相当することに応答して、第1のタイプの不整頻脈のための、引き続き予定されていた治療の実施を抑止する、
請求項1に記載の装置。 - 前記マイクロプロセッサは、前記生成されたペーシングパルスに引き続いて前記入力回路で受信された前記信号が前記第2のタイプの不整頻脈に相当すると決定されたことに応答して、前記第2のタイプの不整頻脈に対応する治療を開始する、
請求項11に記載の装置。 - 前記第1のタイプの不整頻脈は心室性頻脈であり、前記第2のタイプの不整頻脈は上室性頻脈である、請求項1ないし12のいずれかに記載の装置。
- 前記マイクロプロセッサは、前記引き続き受信された信号が上室性頻脈に相当すると決定されたことに応答して、上室性頻脈に対する治療を開始する、請求項13に記載の装置。
- 前記第1の出力回路は、前記入力回路が、所定期間(T1)を隔てた、かつ前記第1のタイプの不整頻脈のレートに相当する連続信号を受信したことに応答して、前記パルスを生成し、前記出力回路により生成された該パルスは、前記所定期間(T1)の関数である所定間隔(T2)を隔てて出力される、
請求項1に記載の装置。 - 前記第1の出力回路により生成された前記パルスは、バーストパルス、ランプパルス、およびランププラスペーシングパルスのうちの1つである、請求項1ないし15のいずれかに記載の装置。
- 前記経過期間が、前記所定の鑑別閾値以上であることに応答して、前記マイクロプロセッサが、前記引き続き受信された信号が、前記第2のタイプの不整頻脈に相当すると判断する、請求項1ないし16のいずれかに記載の装置。
- 前記マイクロプロセッサは、前記引き続き受信された信号が前記上室性頻脈に相当すると決定されたことに応答して、SVT治療を開始する、請求項17に記載の装置。
