Abstract
Description
本願は、米国特許法§119に基づき、米国仮出願第61/824,471号(2013年5月17日出願)の利益を主張する。上記出願は、その全体が参照により本明細書に援用される。
本発明は、概して、主幹動脈等の脈管を通して行われる医療的介入に関し、より具体的には、経皮的弁置換術等の経皮的手技を行うためのアクセスおよび展開構成に関し、導入器シースが、器具、プロテーゼ、および他の構造物を通すための低侵襲的血管アクセスを提供するために利用され得る。
例えば、本願は以下の項目を提供する。
(項目1)
脈管を横断して遠位場所にデバイスを展開するためのシステムであって、
a.開放セル繊維壁材料を備えている細長い導入器シースチューブ部材であって、前記開放セル繊維壁材料は、前記細長い導入器シースチューブ部材を通る管腔を画定し、縮小構成において、前記シースは、第1の断面外径および第1の管腔内径を有し、拡張構成において、前記シースは、第2の断面外径および第2の管腔内径を有する、細長い導入器シースチューブ部材と、
b.シースの近位部分に結合され、前記管腔内に存在する流体が前記繊維壁材料を横切ることを防止するように構成されている実質的に非多孔性の拡張可能層と
を備えている、システム。
(項目2)
前記縮小構成において、前記シースは、前記シースの第1の断面外径と前記脈管の管腔の内径プロファイルとの間の実質的サイズ干渉を伴わずに、前記脈管の少なくとも一部を横断して、前記遠位場所に隣接する位置まで前進させられるように構成されている、項目1に記載のシステム。
(項目3)
前記縮小構成を前記遠位場所に対する所望の位置に位置付けると、前記シースは、前記管腔を通した1つ以上の比較的大きい直径の構造物の通過を促進するために前記拡張構成に拡張されることが可能であり、前記管腔は、直径において前記第1の断面外径より大きい、項目1に記載のシステム。
(項目4)
前記1つ以上の比較的大きい直径の構造物の通過が完了すると、前記シースは、前記縮小構成に戻るように縮小されることが可能である、項目3に記載のシステム。
(項目5)
前記第1の管腔内径は、約0mm〜約4mmに等しい、項目1に記載のシステム。
(項目6)
前記第2の管腔内径は、約4mm〜約7mmに等しい、項目1に記載のシステム。
(項目7)
前記シースに結合されている1つ以上の放射線不透過性マーカーをさらに備え、前記1つ以上の放射線不透過性マーカーは、蛍光透視下で観察しているオペレータが前記シースを前記脈管に対して位置付けることを補助するように構成されている、項目1に記載のシステム。
(項目8)
前記開放セル繊維壁材料は、繊維のマトリクスを備えている、項目1に記載のシステム。
(項目9)
前記繊維のマトリクスは、編組パターンで配列されている、項目8に記載のシステム。
(項目10)
前記繊維は、ポリマー材料を備えている、項目8に記載のシステム。
(項目11)
前記ポリマー材料は、ポリエステル、ポリアミド、ポリプロピレン、およびそれらのコポリマーから成る群から選択される、項目10に記載のシステム。
(項目12)
前記繊維は、各々、約0.003インチ〜約0.015インチの直径を有する、項目8に記載のシステム。
(項目13)
前記繊維のマトリクスは、前記第2の断面外径を越える前記シースの拡張を防止するように機能する、項目8に記載のシステム。
(項目14)
前記繊維のマトリクスは、前記シースが前記管腔を通る構造物の通過によって前記拡張構成にさせられるまで、前記シースを前記縮小構成のままであるように付勢するように構成されている、項目8に記載のシステム。
(項目15)
前記繊維のマトリクスは、前記管腔に通される前記構造物の周囲に局所的に拡張し、次いで、前記構造物が前記管腔の隣接部分へ通過するにつれて、局所的に再縮小するように構成されている、項目14に記載のシステム。
(項目16)
前記実質的に非多孔性の拡張可能層は、シリコーンゴム、オレフィンブロックコポリマー、およびそれらのコポリマーから成る群から選択される可撓性ポリマー材料を備えている、項目1に記載のシステム。
(項目17)
前記繊維のマトリクスは、約0.002インチ〜約0.20インチの直径を有する、前記壁材料を横断する細孔を画定する、項目8に記載のシステム。

内側作業管腔を画定するために、前記細長い導入器シースチューブ部材の管腔を通して動作可能に結合されている内側ライナ部材をさらに備え、前記内側ライナ部材は、前記チューブ部材を構造的に補強し、前記内側作業管腔に通され得る構造物の間の相対運動を促進するように構成されている、項目1に記載のシステム。

前記実質的に非多孔性の拡張可能層は、約10センチメートルの長さにわたって、前記細長い導入器シースチューブ部材の近位端から遠位に延びている、項目1に記載のシステム。
(項目20)
前記デバイスは、前記脈管を横断して前記遠位場所まで、前記拡張可能シースに通されるように選択される埋込型プロテーゼを備えている、項目1に記載のシステム。
(項目21)
前記埋込型プロテーゼは、心臓弁プロテーゼを備えている、項目20に記載のシステム。
(項目22)
前記繊維のマトリクスは、メッシュパターンを備えている、項目8に記載のシステム。
(項目23)
前記繊維のマトリクスの少なくとも一部に動作可能に結合されている張力部材をさらに備え、前記張力部材は、そのような部分を弛緩構成に維持するように構成され、前記張力部材は、オペレータによって手動で引っ張られるか、または緩められるように構成されている近位部分を備えている、項目1に記載のシステム。
図2Aを参照すると、これらの課題に対処する拡張可能導入器シースの一実施形態が、描写され、導入器シースチューブ材またはアセンブリ(14)は、編組またはメッシュパターンに配列される複数の編組繊維を備え、複数の編組繊維は、導入器作業管腔(56)を画定するシースチューブ材を構成する開放セル繊維壁材料を形成する。一実施形態では、導入器シースチューブ材(14)の遠位部分(88)は、非多孔性コーティングを伴わないその多孔性形態の開放セル繊維壁材料を備えているが、導入器シースチューブ材(14)の約近位10センチメートル等の近位部分(86)の一部は、患者内に据え付けられるときの出血を防止することを補助するために、実質的に非多孔性の拡張可能層でコーティングされる。(近位部分は、外科的に生成された創傷(動脈切開等)から外へ経皮的に延びる)。
