Description
(a)OSDI問診の完了;
(b)日誌の確認;
(c)TearLab(商標)を用いた涙液浸透圧の測定;
(d)視力測定(BCVA、矯正視力);
(e)眼の前眼部のスリットランプ評価(生体顕微鏡);
(f)シルマー‐1試験による涙液分泌の測定;
(g)眼内圧の測定;
(h)角膜感受性の測定;
(i)10秒未満の値の場合に3回連続して実施するフルオレセインでの涙膜破壊時間試験(FBUT)による涙膜安定性の測定;;
(j)オックスフォード比色パターンに従った眼表面の評価。
試験終了処理により観察期間を終了した。
1.吸収紙を下眼瞼の縁上で、初めはの右眼、次に左眼の外眼角に挿入する。
2.患者に眼を閉じるよう求める。
3.5分後、折りたたみ部分を除いた、紙の湿った部分の長さを測定する。
1.右眼の球結膜嚢内に防腐剤を含まない2%のフルオレセインナトリウム2μLを、反射的な流涙を誘導しないように注入する(滅菌チップを備えたマイクロピペットの使用を推奨)。
2.患者に通常4〜5回瞬きすることを求め、次いで瞬きせずにフルオレセイン注入から10〜30秒後に、患者に眼を開いたまま真っ直ぐ前方を見るように求める。
3.12倍率のスリットランプを観察し、一定の強度のバックライト(コバルトブルー光)を維持し、角膜全体にわたる涙膜の観察を容易にする(黄色フィルターを用いる)。
4.最後の瞬きと、最初の乾燥スポットの発生又は最初の乾燥領域の出現との間の時間を記録する。
5.FBUTを観察した後、オックスフォードスキームに従って階級付けする。
6.左眼に対して工程1〜5を繰り返す。
1.反射性涙液を刺激することを避けながら、外眼角中の涙液メニスカスに試験カードを当て;涙液試料を回収する;
2.装置ディスプレイ上の浸透圧値を読み取る。
表1に試験スキームをまとめる。
Claims (10)
- 1つ以上の電極を備えるドライアイを治療するための電極システム(2;100)であって、(a)眼(10)、眼窩、側頭、眼瞼領域又はこれらの一部と接触させて当接するように意図した前記1つ以上の電極の一部はそれぞれ、眼、眼窩、側頭、眼瞼領域又はこれらの一部の表面形状を模倣しているか、あるいは(b)眼(10)、眼窩、側頭、眼瞼領域又はこれらの一部と接触させて当接するように意図した前記1つ以上の電極の一部は可撓性でり、形状適合性であり、それにより(a)及び(b)の場合、前記1つ以上の電極の一部と、眼(10)、眼窩、側頭、眼瞼領域又はこれらの一部の表面形状との形態適合した装着が達成可能になり、眼、眼窩、側頭、眼瞼領域又はこれらの一部と接触させて当接するように意図した前記1つ以上の電極の一部には、関連する高調波、好ましくは少なくとも第2及び第3高調波を有するある共振周波数、好ましくは4MHzの共振周波数の歪んだ正弦波電流の電磁波を供給するか、又は供給可能であることを特徴とする電極システム(2;100)。
- 前記1つ以上の電極の一部は可撓性であり、形状適合性であり、前記一部には、その表面延在を画定する二次元の異なる伸展性を示す金属織布(16;118)が導電性かつ放射性要素として備えられることを特徴とする請求項1に記載の電極システム(2;100)。
- 前記1つ以上の電極の一部は可撓性であり、形状適合性であり、また前記一部には、患者と接触するように意図した面に、100kHzを超える周波数のみを通過させることに適した絶縁層(12;108)が備えられることを特徴とする請求項1又は2に記載の電極システム(2;100)。
- 前記1つ以上の電極の一部は可撓性であり、形状適合性であり、前記一部はサンドイッチ構造内において、以下の順序で、緩衝クッション材(26;114、116)、導電性かつ放射性要素(16;118)及び絶縁層(12;108)を備え、前記導電性かつ放射性要素(118)はフィルム(132)の開口部に窓として一体化され、前記フィルムは前記導電性かつ放射性要素(118)に電磁波を供給するための電気回路を有し、電極の一部であり、緩衝クッション材(114、116)、導電性かつ放射性要素(118)、絶縁層(108)及び回路を有するフィルム(132)は全て可撓性であり、眼、眼窩、側頭及び眼瞼領域、又はこれらの一部に対してその形状を適合できることを特徴とする請求項1〜3のいずれか1項に記載の電極システム(100)。
- 前記電気回路が開放され、電磁波が伝達される負荷インピーダンスとしての外部物体によってのみ閉鎖可能であるように構成した電磁波伝達用電気回路が備えられることを特徴とする請求項1〜4のいずれか1項に記載の電極システム(2;100)。
- 前記電極システムはマスク(2)又は一対のゴーグル(100)であり、前記1つ以上の電極の一部は、眼、眼窩、側頭領域及び/もしくは眼瞼領域又はこれらの一部を覆うことを意図したマスク(2)又はゴーグル(100)の全領域に行き渡るように延在し、特にその延在領域の前記一部はマイボーム腺及び涙腺の大部分を包含できることを特徴とする請求項1〜5のいずれか1項に記載の電極システム(2;100)。
- ドライアイを治療するための電気刺激装置であって:
(i)請求項1〜6のいずれか1項に記載の電極システム(2;100);
(ii)関連する高調波、好ましくは少なくとも第2及び第3高調波を有するある共振周波数、好ましくは4MHzの共振周波数の歪んだ正弦波電流の電磁波を放出することに適した無線周波数回路
が備えられ、歪んだ正弦波電流の電磁波が、前記1つ以上の電極、特に眼、眼窩、側頭、眼瞼領域又はこれらの一部と接触させて当接するように意図した前記1つ以上の電極の一部に供給されることを特徴とする電気刺激装置。 - ドライアイを治療する方法であって、関連する高調波、好ましくは少なくとも第2及び第3高調波、より好ましくは4MHz共振周波数の場合は最大64MHzの高調波を有するある共振周波数、好ましくは4MHzの共振周波数の歪んだ正弦波電流の電磁波を、眼、眼窩、側頭、眼瞼領域又はこれらの一部に印加することが含まれることを特徴とする方法。
- 前記印加は、請求項1〜6のいずれか1項に記載の電極システム(2;100)により、又は請求項7に記載の電気刺激装置により行うことを特徴とする請求項8に記載のドライアイを治療する方法。
- 角膜に関する眼疾患、特に眼瞼炎もしくは上皮創傷、及び/又は例えば網膜色素変性症もしくは加齢性変性性黄斑症などの網膜疾患を治療するための、請求項1〜6に記載の電極システム(2;100)、請求項7に記載の装置、及び請求項8及び9に記載の方法の使用。
