本発明は概して、薬物のエアロゾル化送達のための製剤、ならびに製剤のpHを中性から離れる方向に変え、投与後に製剤がより中性となることを可能にすることによって特性を得るための、上記製剤の使用に関する。
概して、いくつかのタイプの注射によって投与される薬物が多く存在する。薬物の注入は多くの利点を提供するが、時折、何人かの患者にとっては不都合であり、痛みを伴う場合があり、かつ感染症の伝播の原因となり得る。代わりに、注射の恐怖および痛みならびに感染症による合併症の可能性を避けるために、そのような薬物を体循環中へ、肺を介して投与することが可能である。吸入によって薬物を投与する別の理由は作用の目標部位が気道であるということであり、気道内へ薬物が沈着することによって、所望の器官では高濃度となり、気道外では比較的低濃度となる。これは、吸入以外の経路による気道の処置のための薬物の投与と比較して、効力および安全性を向上させ得るであろう。
Cmaxは、投薬後の身体における薬物の最高濃度である。
吸入による患者の気道への送達のための製剤について開示するが、この場合、製剤は、薬学的に活性な薬物、薬学的に許容される担体、およびpHに影響を与える薬剤で構成され、このpHに影響を与える薬剤は、担体中での薬物の溶解性を高め、製剤のpHを7.4から少なくとも0.5log単位離れさせ、かつ5.4log単位を超えて離れさせないようなモル濃度において存在する。
ガリウムは、周期表の第13族(IIIa)の半金属元素である。ガリウムは、水溶液中で三価である(Ga3+)。遊離水和イオンGa3+は、ほぼ中性のpH値においてほとんど完全に加水分解し、高度に不溶性のアモルファスGa(OH)3を容易に形成する。Gaは、水酸化物およびオキシ水酸化物として沈殿することに加え、ほぼ中性のpH値において、高不溶性のリン酸塩も形成するであろう。LR Bernstein (1998)により、ガリウムの溶液科学の短い解説が提供されている。pH7.4および25℃において、ガリウムの総水溶解度は約1μMに過ぎず、最低溶解度はpH5.2において10-7.2Mである。ガリウムは、低いpH値および高いpH値において、桁が異なる高い溶解度を有する。例えば、pH2での溶解度は約10-2Mであり、これはpH7.4での溶解度より約10,000倍高い値である。さらに、pH10での溶解度は約10-3.3Mであり、これは、pH7.4での溶解度の約500倍高い値である。この溶解度の違いを、ガリウムまたはその塩(例えば、硝酸ガリウム)を非常に低いpHまたは非常に高いpHにおいて製剤化することにより、吸入製品に利用することができる。
第二の実施例は、肺感染症または肺疾患をより効果的に処置するための、抗感染症薬または抗生物質の吸入送達である。抗生物質は、細菌源に由来し細菌感染症を処置するために使用される抗感染症薬の部分群として非公式に定義され得る。他のクラスの薬物、とりわけスルホンアミドは、有効な抗菌薬であり得る。同様にいくつかの抗生物質には、二次的用途、例えば、抗利尿ホルモン不適合分泌症候群(SIADH)を処置するためのデメクロサイクリン(デクロマイシン、テトラサイクリン誘導体)の使用などがあり得る。他の抗生物質は、原虫感染の処置において有用であり得る。
Claims (15)
- 吸入による患者の気道への送達用製剤であって、該製剤が、薬学的に活性な薬物、薬学的に許容される担体、およびpHに影響を与える薬剤で構成され、該pHに影響を与える薬剤が、担体中での薬物の溶解性を高め、ならびに製剤のpHを7.4から少なくとも0.5log単位離れさせ、かつ5.4log単位を超えて離れさせないようなモル濃度で存在する、製剤。
- 製剤がヒトの肺中の条件下で患者の気道液にしばらくの間接触した場合に、該製剤のpHが投与前の製剤のpHに比べて7.4に0.5log単位以上近づくような特徴をさらに有する、請求項1記載の製剤。
- ヒトの肺中にある間に薬物の溶解度が投与前の製剤の溶解度と比べて小さくなるような特徴をさらに有する、請求項2記載の製剤。
- 薬物がガリウム塩である、請求項1〜3のいずれか1項記載の製剤。
- ガリウム塩が硝酸ガリウムである、請求項1〜3のいずれか1項記載の製剤。
- pHに作用する薬剤が、製剤のpHを7.4から0.75〜4.15log単位離れさせる、請求項1〜3のいずれか1項記載の製剤。
- pHに作用する薬剤が、製剤のpHを7.4から1.0〜2.0log単位以上離れさせる、請求項6記載の製剤。
- 薬物が抗生物質である、請求項1〜3のいずれか1項記載の製剤。
- 抗生物質が、ペニシリン、セファロスポリン、フルオロキノロン、テトラサイクリン、またはマクロライドからなる群より選択される、請求項8記載の製剤。
- 2.0ミクロン〜12.0ミクロンの範囲の空気力学的直径を有する粒子へとエアロゾル化された、請求項1〜3のいずれか1項記載の製剤。
- 4.0ミクロン〜10.0ミクロンの範囲の空気力学的直径を有する粒子へとエアロゾル化された、請求項1〜3のいずれか1項記載の製剤。
- 混合された粒子が0.05mL〜5.0mLの範囲の総体積を有する、請求項10記載の製剤。
- 混合された粒子が0.1mL〜3.0mLの範囲の総体積を有する、請求項10記載の製剤。
- 高カルシウム血症の処置のための、請求項1〜7のいずれか1項記載の製剤。
- 抗生物質がシプロフロキサシンである、請求項8記載の製剤。
