Abstract
Description
本願は、米国仮特許出願第12/348,722号(2009年1月5日出願)の優先権を主張し、この出願の開示は、その全体が本明細書に参考として援用される。
当技術分野は、電気手術、具体的には、扁桃摘出術およびアデノイド切除術である。
鼻/咽頭の後部に位置するアデノイド組織は、口腔または鼻を通して除去され得るが、一般的には、全身麻酔下で口腔を通して除去され得る。したがって、アデノイドに到達するのが困難であるだけでなく、気管内チューブの存在が、すでに制限されている手術野をさらに制約する。本明細書に記載するアデノイド用先端は、アデノイド切除術のこれらの問題の多くに対処するように特に構成される交換可能な先端である。
扁桃腺は、咽頭の両側に位置し、全身麻酔下で除去される。この手技において、扁桃腺の結合されていない端部は、通常、鉗子で把持され、同時に、咽頭から扁桃腺を除去するために、もう一方の端部で切断が行われる。したがって、アデノイド切除術と同様に、アクセスが困難であり、手術野は制約される。本明細書に記載する扁桃摘出術用先端は、扁桃摘出術のこれらの問題の多くに対処するように特に構成される、交換可能な先端である。
切除したヒト口蓋扁桃に、30Wの切断および30Wの凝固の設定で、従来の「ボビー」針先を使用した一連の外科的切開を行った。切断に対して切断5(20Wの出力)および凝固に対して凝固6(30Wの出力)の設定で、本明細書に記載する電気手術扁桃摘出術デバイス(「扁桃腺用ブレード」)を使用して、追加の切開を行った。切開の直後に組織学的試料を採取し、顕微鏡を使用して残りの熱傷に関して評価した。熱傷深度(mm)および熱傷の減少の割合の比較を表1に提供する。
Claims (29)
- 電気手術デバイスであって、
a)近位端、遠位端を有する細長い本体であって、自身を通って延在する吸入を印加するための管腔を画定する本体と、
b)近位端、遠位端を有する先端であって、該細長い本体の該遠位端において該吸入管腔に流体的に接続される先端管腔を画定する先端と、
c)該先端の該遠位端に固定される筐体であって、該先端管腔に流体的に接続される開口を画定する筐体と、
d)該筐体の中にあり、電極を備えるブレードアセンブリと
を備え、
該ブレードアセンブリまたはそれの一部は、該筐体の該開口を有する吸入ポートを形成する、デバイス。 - 前記先端は、可鍛性である、請求項1に記載の電気手術デバイス。

- 請求項1に記載のデバイスを使用して、扁桃腺またはアデノイド組織を切除する方法であって、
a)前記電極にパルス電気信号を印加することにより、それの上にプラズマを形成することと、
b)該プラズマによって該組織を切断するか、または凝固させることと
を含む、方法。 - 前記吸入ポートを介して前記組織に吸引を印加することをさらに含む、請求項24に記載の方法。
- 吸引は、前記電極の近位移動と同時に印加されることにより、前記組織を吸引切断する、請求項25に記載の方法。
- 外科手術中に前記先端を取り外し、それを第2の先端と交換することをさらに含む、請求項24に記載の方法。
- a)1つ以上の先端であって、該先端は筐体を備えており、該筐体は、開口を画定し、電極を備えるブレードアセンブリを自身の中に有しており、該ブレードアセンブリまたはその一部は、該筐体の該開口を有する吸入ポートを形成する、先端と、
