Abstract

Description
本出願は、2008年10月17日に出願された、米国仮特許出願第61/196,543号に対する優先権を主張する。
本開示はインプラントに関しており、より詳細には、多孔性基材へ塗布される第一ヒドロゲル前駆体および第二ヒドロゲル前駆体を有する多孔性基材を含む止血用インプラントに関する。
インサイチュでの止血治療は、主として、患者の体内での前駆体溶液の固体への変換に集中してきた。変換は、沈降、ポリマー化、架橋、および脱溶媒和を含む様々な手段によって達成されてきた。しかし、インサイチュでの止血治療のために溶液を用いる場合、重大な制限が存在する。低粘度の溶液は流れ去り得、変換および凝固が起こる前に適用部位から取り除かれ得る。さらに、上記溶液の調合は、代表的には、前駆体溶液の調製が上記前駆体の再構成を必要とする場合、または、上記溶液が冷凍して保存され、解凍される場合、複雑であり得る。

本開示は、第一ヒドロゲル前駆体および第二ヒドロゲル前駆体を有する多孔性基材を含む止血用インプラントに関しており、第一ヒドロゲル前駆体および第二ヒドロゲル前駆体は、上記インプラントが移植する部位に配置され、患者の生理学的流体へさらされるまで、互いと反応しないような様式で上記多孔性基材へ塗布される。
本インプラントは、多孔性基材の第一部分へ塗布される第一ヒドロゲル前駆体および上記多孔性基材の第二部分へ塗布される第二ヒドロゲル前駆体を有する多孔性基材を含む。実施形態において、上記第一ヒドロゲル前駆体または上記第二ヒドロゲル前駆体のうちの少なくとも1つは、フィルムとして上記多孔性基材へ塗布される。実施形態において、上記基材に塗布される上記第一ヒドロゲル前駆体を有する上記基材の第一部分は、上記多孔性基材の第二部分から空間的に分離され、上記インプラントが移植される部位に置かれて患者の生理学的流体にさらされるまで、第一ヒドロゲル前駆体と第二ヒドロゲル前駆体とが互いに反応するのを防止する。上記インプラントの生理学的流体への曝露は、上記第一ヒドロゲル前駆体が上記多孔性基材の第一部分から上記多孔性基材の第二部分へ向かって移動し、第二ヒドロゲル前駆体との反応を引き起こす。実施形態において、本インプラントはコーティングされた多孔性基材の部分上で止血特性を示すだけでなく、抗癒着特性もさらに示す。
本開示に従う止血用インプラントは、多孔性基材の第一部分へ塗布される第一ヒドロゲル前駆体および上記多孔性基材の第二部分へ塗布される第二ヒドロゲル前駆体を有する多孔性基材を含む。使用中に上記インプラントは、上記第一ヒドロゲル前駆体が塗布された部分を組織に対してより近くにし、そして塗布された第二ヒドロゲル前駆体を有する部分を上記組織からより遠くにして配置される。実施形態において、上記第一部分および第二部分は、対照的な染料、表面模様付け、着色、または他の視覚的手がかりの付加によって互いに区別可能であり得る。組織、例えば、損傷した組織との接触のとき、上記インプラントは生理学的流体を吸収し、上記第一ヒドロゲルが上記流体によって溶解される。上記流体は上記インプラント中へ毛管作用で運ばれ、インプラントの反対側まで移動するので、インプラントを通って溶解された第一ヒドロゲル前駆体を一緒に運ぶ。ついには、上記流体は、上記インプラントを通って上記第二ヒドロゲル前駆体が塗布された第二部分へ到達するほど十分に移動し、それによって第二ヒドロゲル前駆体を溶解する。上記第一ヒドロゲル前駆体および第二ヒドロゲル前駆体は次いで反応し、生体適合性架橋物質を形成し、それによって止血を助ける。いくつかの実施形態において、上記第一ヒドロゲル前駆体と第二ヒドロゲル前駆体との反応により産生される上記生体適合性架橋物質は、止血特性を提供するだけではなく、上記インプラントに抗癒着特性もまた提供する。
