【発明が属する技術分野】
本発明は、医療診断記録帳に関し、更に詳しくは個人の医療診断記録、病歴、薬歴など、症状快復・健康管理に必要な事項を診断を受けた医療機関または科ごとに経時的に記録可能なページと、体温測定手段を備えるページを有する医療診断記録帳に関する。

【従来の技術】
個人が病気などになった場合における自己の医療診断記録を保有することは、病気の治療や健康維持の観点から極めて重要である。また、旅先や出張先などで急に医療機関にかかるような事態が生じた場合、あるいは引っ越しなどで診断を受ける医療機関が替わった場合などにおいて、それまで受けていた診断内容や、以前に服用していた、または現在服用を続けている薬などを医師に知らせることも、適切な診断や処方を受ける上で極めて重要である。

通常、個人の病歴、医療診断記録及び薬歴などは、診断した医療機関がカルテや薬歴簿として保有し、その個人（患者）の治療などに用いている。また、これに類するもので個人が所有するものであって公のものとしては健康手帳や母子手帳などがある。

この他、自己の体調データや診療・検診・服薬などの記録をまとめてシステム管理できる携行型健康管理手帳として、登録実用新案公報第３００８１５２号に記載のものなどが開発されている。

登実第３００８１５２号公報に記載の考案は、自分の体調・健康状態のバロメータとなる複数項目のデータ、例えば毎日の血圧、脈拍、体温、体重、歩いた歩数、服薬量などの記録グラフを、手軽に携行しながら都度その測定データを記録したり経過を観察チェックできるようにすることを目的として、血圧測定値を経時的にプロットする血圧管理グラフと、体温測定値を経時的にプロットする体温管理グラフと、脈拍測定値を経時的にプロットする脈拍管理グラフと、体重測定値を経時的にプロットする体重管理グラフと、歩行数を経時的にプロットする歩行管理グラフと、服薬内容を経時的に記録する服薬管理表と、診察・検診の結果を記録する検診結果管理表のいずれか３つ以上のグラフ（表）をルーズリーフ式に差し替え自在に１冊の手帳にまとめて帳合いしたことを特徴とするものである。

【発明が解決しようとする課題】
しかしながら、一般にカルテは医師が各医療機関において使用するものであって、患者個人に開示される性質のものではなく、患者個人の病気の内容や程度、状態などは診断・診療の際において担当する医師が口頭で告知するものである。また、薬歴簿も各医療機関において当該各個人に対し同一薬の重複投与や副作用を生じる組み合わせを回避するなどの目的で使用されるものである。

また健康手帳は、個人が所有し自らの健康維持・管理や適切な医療の確保などを目的として、各種ガン検診の記録、血圧、体重など、健康検査の結果や自己の健康状態などを記録するものであって地方自治体が配布するものであるが、配布の対象は老人保健法に基づき原則として７０歳以上の高齢者または６５歳以上で老人医療の給付者などに限られている。同様に母子健康手帳も、妊婦を対象として妊婦の健康状態や子供の発育・発達の状態、病気、予防接種などの記録をするもので地方自治体から配布されるものである。すなわち誰でもが所有して自己の適切な医療の確保に役立てようとするものではない。

更に前記登実第３００８１５２号公報に記載の発明は、使用者の身体データを使用者自身が経時的に記録し確認して使用者の日常の健康管理に役立てるものであって、疾病時における病状、通院記録、服薬記録、快復状態を記入するものではなく、症状快復のために用いられるものではない。

このように、自己の診断・医療記録を記入して自己の健康維持・管理や適切な医療を確保するために役立てるものとして、誰でもが所持することができる有効な手段は、公のものも含めて存在していなかった。

【課題を解決するための手段】
前記課題を解決するため、請求項１に記載の発明は、表裏一対の表紙と、該表裏一対の表紙に綴り込んで固着される複数の綴じ込みページから構成される手帳であって、前記綴り込みページは、少なくともそれぞれ１ページ以上の医療診断記録のページと、該医療診断記録のページの束の前に綴り込まれる中間表紙とから構成されるとともに、前記医療診断記録のページと該医療診断記録のページの束の前に綴り込まれる中間表紙の組み合わせを少なくとも２組以上備え、更に体温測定手段を備えたページを備えることを要旨とするものである。

この場合において、前記医療診断記録のページは、少なくとも診察日、担当医師名、診察内容、医師からのアドバイス、次回の診断予定日の各項目名が印刷されるとともに、前記各項目名に対応する事項を記入可能な空白箇所を備え、前記医療診断記録のページの束の前に綴り込まれる中間表紙は、診断を受けた医療機関名及び科名、その連絡先、主治医を含む担当医師の氏名、その当番日の各項目名が印刷されるとともに、前記各項目名に対応する事項を記入可能な空白箇所を備えることが望ましく、前記体温測定手段を備えるページは、ページの表面に温度に応じて色が変化する示温材料を備えるか、ページの一部または全部が温度に応じて色が変化する示温材料から構成されてなることが望ましい。また、前記温度に応じて色が変化する示温材料は、温度に応じて色が変化する示温塗料を塗布または染み込ませた薄膜を適用することが可能である。

かかる構成とすることにより本医療診断記録帳の使用者は、自身が受けた診断・診療・治療の内容を診断などを受けた医療機関または科ごとに経時的に記録、確認することができて、使用者自身の病状の変化を経時的に知ることが可能となり、症状快復・健康維持に役立たせることができる。また、旅先や出張先などで急に医療機関にかかることになった場合においても、過去または現在の医療診断記録を正確に医師に提示することができ、その結果、初めてかかる医療機関においても適切な処置を受けることが可能となるなど、適切な診断の確保する上で極めて有効である。また、体温測定手段を備えることにより、旅先や出張先においても簡便に体温を測定することが可能である。体温は体調のバロメータとして広く利用されており、体に不調を感じた場合などに直ちに体温を測定して医療機関にかかるか否かなどの判断の材料とすることができるなど、使用者自身の健康管理に有用である。

【発明の実施の形態】
以下、本発明の一実施の形態を図面を参照して詳細に説明する。

図１は本実施の形態における医療診断記録帳の斜視図を示したものである。図１に示されるように、医療診断記録帳１は表裏一対の表紙２０と、該表紙２０に綴り込まれて固着される複数枚の綴り込みページから構成される。該綴り込みページは、複数枚からなる医療診断記録のページ６０の束とその直前に綴り込まれる医療診断記録のページ６０の中間表紙５０の組み合わせから構成され、該組み合わせが少なくとも２以上繰り返されるよう構成されている。この他に、当該医療診断記録帳の意義や健康維持管理の心得などが印刷されたページ３０と、医療機関にかかる前などに予め問診形式で体調などを記入可能なページ４０とを備える。本実施の形態において前記体温測定手段を備えるページは、独立したページではなく裏表紙の内側に体温測定手段を備える構成としている。

図２は、本実施の形態における医療診断記録帳の表紙２０の一例を示した図である。表紙の外側２１には、該医療診断記録帳の名称が印刷された欄２２と、使用開始日及び該医療診断記録帳が何冊目であるかを示す各項目名が印刷されるとともに、該各項目名に対応して使用開始日及び何冊目であるかを記入可能な空白箇所からなる欄２３、２４と、使用者についての情報の各項目名が印刷されるとともに、該各項目名に対応する事項を記入可能な空白箇所とからなる欄２５とを備える。使用者についての情報の項目は、少なくとも使用者の氏名、連絡先（住所、電話番号など）、保険の種類及び保険証番号が含まれることが望ましい。

表紙の外側２１には前記項目の他、使用者の血液型や緊急時の連絡先などを記入可能な欄を含んでいても良い。また使用者に関する情報を記入可能な欄については、前記全ての項目を表紙の外側に記入可能な構成とするのではなく、例えば表紙の外側には、例えば使用者の氏名の項目名が印刷されるとともに使用者の氏名を記入可能な空白箇所とを備えるにとどめ、表紙の内側に使用者についての詳細な情報、すなわち連絡先、保険証番号など記入可能な欄を備える構成であっても良い。かかる構成とすれば、医療機関にかかる際に必要な情報を簡単且つ確実に提示することが可能となって極めて便利であり、医療機関側からすると患者本人の情報の確実な確認が可能となる。また、使用者の情報を表紙の内側に記入する構成とすることにより不用意に使用者の詳細な情報が第三者に知られることを防止できる。

図３は、本実施の形態における医療診断記録帳の第１ページ目の一例を示した図である。第１ページ目３０は、当該医療診断記録帳の使用意義や健康維持管理のために必要な心がけなどが印刷されるページである。該ページに記載される事項は図３に示されている事項に限られるものではなく、必要に応じて適宜増減・変更しても良い。

図４は、本実施の形態における医療診断記録帳の第２ページ目を示した図である。該第２ページ目４０は問診のページであり、図４に示されるように、該ページが問診のページであることが印刷される欄４１と、使用者の体調に関する質問が印刷されるとともに、該質問に対応する使用者の体調などを記入可能な空白箇所とからなる欄４２と、を備える。体調不良などにより医療機関にかかる場合に使用者が予め症状を記入しておくことにより、客観的に症状を把握することができ、医師に対しては自身の症状を的確に伝えることができる。該問診のページ４０には、この他記入日時を記入可能な欄を備えることが望ましい。また、家族からの連絡欄の項目名が印刷されるとともに、該項目名に対応する事項が記入可能な空白箇所からなる欄４３を備えており、症状を使用者本人が記入可能な場合であっても可能であれば家族から気がついた点などを記入してもらうことにより、より客観的に症状を医師に伝達することが可能となり、適切な診断を受ける一助となるものである。

図５は、本実施の形態における医療診断記録帳の医療診断記録のページの中間表紙の一例を示した図である。該中間表紙５０は後述の医療診断記録のページ６０の束の直前に綴り込まれるものである。該中間表紙５０には、医療機関名、科名とその連絡先、主治医の氏名の各項目名が印刷されるとともに、該各項目名に対応する事項を記入可能な空白箇所からなる欄５１を備える。また、主治医を含む担当医師とその当番日を記入可能な表５２を備える。医師の当番日を記入可能な表５２は、左側に医師の氏名を記入可能な欄５４と、中央に当番日を記入可能な欄５５と、右側および最下部に空白欄５６、５３と、を備える。当番日を記入可能な欄５５は、最上部に曜日が印刷された欄を備えており、曜日ごとの各医師の担当する時間帯その他の必要な事項を記入可能な構成である。また、右側の空白欄５６には各医師ごとの、最下部の空白欄５３にはその医療機関またはその科ごとの診断時間などに関する特記事項またはその他の必要な事項を記入可能な構成としている。かかる構成とすることにより、診断を受ける医療機関または科ごとに、診断を受けるために必要な事項を漏らさず記入することができ、かつ見やすい配列に構成されているため、主治医の当番日などを確認しやすく便利である。

図６は、本実施の形態における医療診断記録帳の医療診断記録のページの一例を示した図である。該医療診断記録帳の医療診断記録のページ６０には、診察内容、処置、薬、医師からのアドバイスの各項目名が印刷されるとともに、該各項目名に対応する事項を記入可能な空白箇所からなる欄６１を備える。前記診断内容などを記入可能な欄６１の上方には診断日及び担当医師名の各項目名が印刷されるとともに、該各項目名に対応して診断日及び担当医師名を記入可能な空白箇所からなる欄６２を備える。前記診断内容などを記入可能な欄６１の下方には少なくとも次回の診断予定日、医療機関及への支払額及び薬局への支払額の各項目名と該各項目名に対応する事項を記入可能な空白箇所からなる欄６３とを備える。かかる構成によれば、診断を受けた際において、使用者の医療診断記録や症状快復のために必要な記入すべき事項、その他必要な事項が網羅されており、且つ見やすい配列で構成されているため、構成されている欄に従って記入すると、使用者自身の医療診断記録の必要事項をもれなく記入できて使い勝手が良いものである。更に診断を受けるごとに記入を繰り返すことにより、使用者自身の症状の経時変化を的確に把握することができて、症状快復に役立てることができる。なお、本実施の形態においては、診断内容、処置、薬（処方）などを１つの欄に記入する構成としているが、別々の欄を設ける構成としても良い。

また、本実施の形態にかかる医療診断記録帳においては、医療診断記録帳１に複数枚の医療診断記録のページ６０の束と、該医療診断記録のページ６０の束の直前に中間表紙５０とが綴られる組み合わせが複数繰り返される構成とされている。すなわち当該各組み合わせを、例えば、医療機関Ａの中間表紙とその医療診断記録のページの束、医療機関Ｂの中間表紙とその医療診断記録のページの束、・・・、あるいは内科の中間表紙とその医療診断記録のページの束、外科の中間表紙とその医療診断記録のページの束、胃腸科の中間表紙とその医療診断記録を記入するページの束、・・・、などとすることにより、診断を受けた医療機関ごとに、あるいは科ごとに分けて医療診断記録などを記録可能となる。かかる構成とすることにより、複数の医療機関または科において診断を受けた場合においても１冊の手帳に全て記入することができるため、同時並行的に複数の医療機関または科において診断を受けるような場合であっても、医療診断記録が錯綜することなくそれぞれの医療機関または科において医師は患者の病状や医療診断記録を知ることができ、投薬においても副作用が生じるような組み合わせを避けることができる。更に異なる医療機関の医師どうしの間においても、使用者の医療診断記録を共有することができるのみならず、本医療診断記録帳を連絡帳として意志の疎通を図ることができ、各医師は各医療機関において適切な診断を施すことが可能となる。このため、使用者は安心して診断を受けることが可能となる。この場合、各組み合せにおける医療診断記録のページ６０は５〜１０枚程度であることが望ましい。この場合、各組み合わせごとの医療診断記録のページ７０の枚数を異ならせても良い。また、組によりページの色を異ならせるなどして、一見して見分けがつく構成とすると、より使い勝手が良くなるものである。

図７は、本実施の形態における体温測定手段を備えるページの一例を示した図である。本実施の形態においては当該体温測定手段を備えるページを独立したページとする構成ではなく、裏表紙の内側に体温測定手段を備える構成としている。該体温測定のページ７０は、該ページが体温測定のページであることを示すページ名などが印刷される欄７１と、体温測定シート７２ａ、７２ｂ、７２ｃと、使用方法が印刷された欄７３とを備える。体温測定シート７２ａ、７２ｂ、７２ｃは、温度により色が変化する薄膜であり、該ページに貼付される。図７の本実施の形態においては、体温測定シートが３枚貼付される構成を示しており、それぞれ７２ａは平熱の温度付近で変色する薄膜、７２ｂは微熱の温度付近で変色する薄膜、７２ｃは高熱の温度付近で変色する薄膜が適用される。前記各体温測定シート７２ａ、７２ｂ、７２ｃは、温度変化により可逆的に色が変化する塗料（示温塗料）や、温度により結晶構造が変化し変色する液晶カプセルインキなどが塗布あるいは染み込ませるなどされた薄膜などを好適に適用することができるほか、該ページの所定の箇所に直接塗布または染み込ませる構成であっても良い。

そして該体温測定シート７２ａ、７２ｂ、７２ｃを印刷された使用方法に従って３枚を同時に例えば額などに所定時間密着させて体温を測定する。そして体温測定シート７２ａが変色した場合には平熱と、体温測定シート７２ｂが変色した場合には「微熱」と、体温測定シート７２ｃが変色した場合には「高熱」と判断するものである。体温は健康状態を把握するバロメータとして一般に用いられているものであり、体温をいつでも即座に測定できることは健康維持のため有用である。また、旅先や出張先などで体調不良を感じた場合、その場で直ちに体温を測定して医療機関にかかるかどうかなどの判断材料とすることができ、医療機関にかかる場合には、併せて前記問診のページ４０に症状を記入すると、体調不良を感じた時の症状を直ちに的確に記入して医師に提示できるため、適切ら診断を受けることができる。なお、該体温測定ページはとっさの時に体温の目安とするためのものであり、正確な体温は医療機関などにおいて測定することが望ましい。

以上本発明の実施形態について説明したが、本発明に係る医療診断記録帳は前記実施の形態に何ら限定されるものではなく、本発明の趣旨を逸脱しない範囲で種々の改変が可能である。

本医療診断記録帳に綴り込まれるページの項目の種類・内容、各項目名・空白箇所の寸法・配列、ページの枚数などは前記実施の形態に限られるものではなく適宜増減・変更可能である。また体温測定手段についても、必ずしも示温塗料あるいは液晶カプセルインキなどを塗布または染み込ませた薄膜を使用する構成である必要はなく、その他の体温測定手段を適宜適用することが可能である。また、薄膜を使用する場合であっても、体温測定シートは３枚である必要はなく、１枚または４枚以上であっても良い。なお、手帳本体も糊などで綴り込まれるページを固着するものではなく、加除式のものであっても良い。

【発明の効果】
以上説明したとおり、本願請求項１に記載の発明によれば、使用者自身の医療診断記録などを経時的に記録、確認できるため、病気などになった際に症状快復の役に立てることができる。また、旅行、出張先などで急に医療機関にかかる場合、引越などで医療機関を替える場合などに過去に受けたあるいは継続中であった診断内容や服用薬を速やかに提示でき、初めてかかる医療機関であっても適切な診断を受けることが可能となる。

医療診断記録は診断を受ける医療機関ごとまたは科ごとに記入することが可能であるため、複数の医療機関または複数の科において同時並行的に診断を受けている場合であっても、各診断内容が錯綜することなく経時的に記録されるため、使用者は自己の医療診断記録を症状快復のために利用しやすい。また、全ての医療機関または科における医療診断記録を１冊の医療診断記録帳にまとめて記入することができるため、それぞれの医療機関または科において医師は他の医療機関または科における該医療診断記録帳の使用者（患者）の病状や診断内容を知ることができ、適切な診断をすることができると同時に、投薬においても副作用などが生じるような組み合わせを避けることができる。更に異なる医療機関の医師どうしにおいても、医療診断記録を共有することができると共に、該医療診断記録帳を連絡帳として意志の疎通を図ることが可能となり、医師は各医療機関において適切な診断を施すことが可能となる。特に服薬記録を提示できることは、特定の組み合わせによって生じる副作用を未然に防止することができるなど、適切な医療を受ける上で極めて有用な効果を奏するものである。
