- 【特許請求の範囲】 1.先端及び基端を有し、一時的アクティブ固定式心内膜ペーシングリードにお いて、 (a)最大直径が約1.155mm(約3.5フレンチ)以下の外面を備え、先端 と基端と、前記外面の少なくとも部分上に亘って延在し且つこれを覆う生体親和 性で電気絶縁性の材料で形成された外シースと、第1及び第2の電気導体と、こ れらの導体間に配置された電気絶縁性材料とを有する、展性で弾性のリード本体 と、 (b)前記第1及び第2の電気導体の基端に夫々取り付けられた第1及び第2の 電気コネクタと、 (c)心内膜又は心筋組織にアクティブ固定するための生体親和性材料製アクテ ィブ固定装置であって、このアクティブ固定装置の少なくとも一部がチップ電極 を構成し、このチップ電極は、前記第1及び第2の電気導体のうちの一方の先端 に電気的に接続され且つ機械的に連結され、前記アクティブ固定装置の直径は、 約1.32mm(約4フレンチ)以下であり、前記チップ電極の表面積は10mm2 と等しいか、或いはそれ以下である、前記アクティブ固定装置と、 (d)前記アクティブ固定装置と前記リード本体の先端との間に配置された生体 親和性材料製リング電極であって、前記アクティブ固定装置に取り付けられてい ない方の前記電気導体の前記先端に電気的に接続され、前記リング電極の表面積 の前記チップ電極の表面積に対する比が2:1に等しいか、或いはそれ以上であ る、リング電極と、 (e)先端と基端と、前記リード本体を内部に受け入れるのに十分な直径を有す る内面と、外面とを備え、前記リード本体の少なくとも一部に亘って延在し且つ これを覆う、最大外径が約1.32mm(約4フレンチ)以下の展性で弾性の生体 親和性材料製チューブ状ガイドカテーテルと、 を有することを特徴とする、アクティブ固定式心内膜ペーシングリード。 2.更に、先端と基端とを有し、第1端部が前記ガイドカテーテルの基端に取り 付けられ、前記リード本体を内部に摺動自在に受け入れるのに十分な直径を有す るボアが、前記先端と前記基端との間に延在するように構成されたハブ手段を有 する、請求項51に記載の心内膜リード。 3.更に、前記ハブ手段の前記基端に取り付けられ、前記リード本体を内部に摺 動自在に受け入れるのに十分な直径を有する第1ボアが内部に配置された、止血 弁手段を有する、請求項52に記載の心内膜リード。 4.前記止血弁手段内には、第2ボアが更に配置され、前記第2ボアの第1端部 は、前記第1ボアに隣接し且つ少なくとも斜めの角度でこれと交差し、前記第2 ボアの第2端部は、前記弁手段の外面に取り付けられ且つここから外方に延在す るネックで終端する、請求項53に記載の心内膜リード。 5.前記ネックは、シーリングキャップ手段をその外端に受け入れる、請求項5 4に記載の心内膜リード。 6.前記コネクタはピンコネクタである、請求項51に記載の心内膜リード。 7.前記ピンコネクタは、前記リード本体に関してオフセットされて配置されて いる、請求項56に記載の心内膜リード。 8.前記ガイドカテーテルの25.4cm(10インチ)の長さの試料は、その基 端を固定位置に保持し、先端を回転させることによって30°角変位させた場合 、その基端と先端との間で少なくとも約0.115kgcm(約0.10オンス・イ ンチ)のトルクを伝達できる、請求項51に記載の心内膜リード。 9.前記ガイドカテーテルの25.4cm(10インチ)の長さの試料は、その基 端を固定位置に保持し、先端を回転させることによって30°角変位させた場合 、その基端と先端との間で少なくとも約0.230kgcm(約0.20オンス・イ ンチ)のトルクを伝達できる、請求項51に記載の心内膜リード。 10.前記ガイドカテーテルの25.4cm(10インチ)の長さの試料は、その 基端を固定位置に保持し、先端を回転させることによって30°角変位させた場 合、その基端と先端との間で少なくとも約0.346kgcm(約0.30オンス・ インチ)のトルクを伝達できる、請求項51に記載の心内膜リード。 11.前記ガイドカテーテルの25.4cm(10インチ)の長さの試料は、その 基端を固定位置に保持し、先端を回転させることによって30°角変位させた場 合、その基端と先端との間で少なくとも約0.461kgcm(約0.40オンス・ インチ)のトルクを伝達できる、請求項51に記載の心内膜リード。 12.前記ガイドカテーテルの25.4cm(10インチ)の長さの試料は、その 基端を固定位置に保持し、先端を回転させることによって60°角変位させた場 合、その基端と先端との間で少なくとも約0.230kgcm(約0.20オンス・ インチ)のトルクを伝達できる、請求項51に記載の心内膜リード。 13.前記ガイドカテーテルの25.4cm(10インチ)の長さの試料は、その 基端を固定位置に保持し、先端を回転させることによって60°角変位させた場 合、その基端と先端との間で少なくとも約0.461kgcm(約0.40オンス・ インチ)のトルクを伝達できる、請求項51に記載の心内膜リード。 14.前記ガイドカテーテルの25.4cm(10インチ)の長さの試料は、その 基端を固定位置に保持し、先端を回転させることによって60°角変位させた場 合、その基端と先端との間で少なくとも約0.691kgcm(約0.60オンス・ インチ)のトルクを伝達できる、請求項51に記載の心内膜リード。 15.前記ガイドカテーテルの25.4cm(10インチ)の長さの試料は、その 基端を固定位置に保持し、先端を回転させることによって60°角変位させた場 合、その基端と先端との間で少なくとも約0.922kgcm(約0.80オンス・ インチ)のトルクを伝達できる、請求項51に記載の心内膜リード。 16.前記リング電極の表面積の前記チップ電極の表面積に対する比が約2.5 :1と等しいか或いはそれ以上である、請求項51に記載の心内膜リード。 17.前記リング電極の表面積の前記チップ電極の表面積に対する比が約3:1 と等しいか或いはそれ以上である、請求項51に記載の心内膜リード。 18.前記リング電極の表面積の前記チップ電極の表面積に対する比が約3.5 :1と等しいか或いはそれ以上である、請求項51に記載の心内膜リード。 19.前記リング電極の表面積の前記チップ電極の表面積に対する比が約4:1 と等しいか或いはそれ以上である、請求項51に記載の心内膜リード。 20.前記リング電極の表面積の前記チップ電極の表面積に対する比が約4.5 :1と等しいか或いはそれ以上である、請求項51に記載の心内膜リード。 21.前記リング電極の表面積の前記チップ電極の表面積に対する比が約5:1 と等しいか或いはそれ以上である、請求項51に記載の心内膜リード。 22.前記チップ電極の表面積は、約9mm2と等しいか或いはそれ以下である、 請求項51に記載の心内膜リード。 23.前記チップ電極の表面積は、約8mm2と等しいか或いはそれ以下である、 請求項51に記載の心内膜リード。 24.前記チップ電極の表面積は、約7mm2と等しいか或いはそれ以下である、 請求項51に記載の心内膜リード。 25.前記チップ電極の表面積は、約5mm2と等しいか或いはそれ以下である、 請求項51に記載の心内膜リード。 26.前記チップ電極の表面積は、約3mm2と等しいか或いはそれ以下である、 請求項51に記載の心内膜リード。
