Description
DESCRIZIONE dell’invenzione industriale avente per titolo : VALVE PER PINZA DA BIOPSIA IN ENDOSCOPIA DIGESTIVA.
RIASSUNTO
La presente invenzione riguarda le valve di pinze che vengono utilizziate per la biopsia endoscopica .
Il ritrovato riguarda un nuovo tipo di valva fornita di un grosso ostie che mette in comunicazione, a pinza chiusa , lo spazio fra le valve ed un sistema tubulare di varia lunghezza che funge da deposito, permettendo in tal modo un numero indefinito di prelievi senza estrarre la pinza dall· endoscopio.
DESCRIZIONE
In Endoscopia digestiva, nelle manovre diagnostiche, il prelievo di frammenti di mucosa degli organi che si esplorano ( Esofago, stomaco, duodeno, colon) si effettua mediante la biopsia endoscopica che viene eseguita con l’uso di particolari e specifiche pinze che vengono fatte passare attraverso il canale operatore dell’endoscopio e sono munite di due valve che vengono appoggiate sulle mucose da bioptizzare quando queste valve vengono chiuse il pezzettino di mucosa viene tagliato e compresso fra le due valve.
Quando le valve della pinza vengono chiuse il numero dei pezzi che esse possono contenere e’ molto limitato ; due, massimo tre.
Esistono delle non infrequenti patologie in cui il numero dei prelievi deve essere più consistente come numero di pezzi prelevati, basti, per esempio, pensare ad una ulcera gastrica oppure al numero di prelievi necessari per evidenziare la possibilità’ di una displasia o neoplasia in una pancolite ulcerativa ( severa malattia cronica del Colon ).
Il prelievo endoscopico talvolta viene fatto in condizioni particolari e pertanto il tempo disponibile diventa allora molto limitato, come avviene in soggetti che debbono eseguire una gastroscopia ma sono marcatamente intolleranti e non possono fare dei sedativi.
Altro problema dell’attuale tecnica dei prelievi di tessuto multipli per via endoscopico e’ il fatto che la pinza viene passata numerose volte nel canale dell’ endoscopio con usura e deformazione dello stesso.
E’ possibile in queste condizioni la rottura del canale con gravissimi danni all’endoscopio per in filtrazione di liquido .
Un ulteriore problema dell’attuale tecnica e’ lo svuotamento delle valve della pinza dopo che e’ stato eseguito il prelievo , fatto che richiede, a causa della piccolezza dei frammenti, un tempo non irrilevante da parte dell’operatore.
La presente invenzione prevede nuove valve con una funzione diversa dalle valve attualmente in uso. la funzione delle valve di ogni pinza endoscopico e’ quella di tagliare il pezzo di mucosa senza una minima funzione di deposito. Quando si e’ voluto dare una minima funzione di deposito si e’ tentato di costruire delle valve più* lunghe essendo lo sviluppo in larghezza condizionato dalla capienza del canale operatore.
lì nostro ritrovato consiste nel posizionare attaccato ad una delle valve idoneamente traforate (Tav. 1 : fìg. 1: lett. b ) un serbatoio cilindrico in lattice sottile ( Tav. 1 : fig. 1, 2, 3 : lett. c ) con fori che funzionano da sfiatatoi ( Tav. 1 : fìg. 1,2,3 : lett d) o un serbatoio cilindrico composta da rete sottile ( Tav. 3: fig. 1,2,3: let. c).
Il serbatoio in lattice e ’ a fondo cieco, di lunghezza varia ed e’ dotato di fori nella parete che favoriscono il deposito del pezzo istologico verso il fondo del cilindreto in latice.
I fori servono ossia ad impedire il fenomeno dello “ air trapping” che rallenta la progressione del bolo istologico.
Quando le valve si stringono intorno al pezzo di mucosa questo viene tagliato e spinto nel serbatoio sia dalla spinta stessa della valva non forata sia dal meccanismo di mungitura che avviene quando con la manovra di strappo della mucosa compressa fra le valve la pinza rientra per qualche centimetro nel canale dell· endoscopio , in questo modo il serbatoio che e* solidale con la pinza entra anche lui nel canale ed il pezzo della biopsia viene spinto verso il fondo del serbatoio lasciando spazio ad altri pezzi
Quando la pinza viene estrata il serbatoio in cui sono custoditi i pezzi bioptici li mantiene isolati dal canale e questo ef un ulteriore vantaggio. In questo modo non vi e’ nessuna commistione con precedenti pezzi istologici presenti per errore nel canale operatore.
Le due valve possono essere delle stesse dimensioni, nel qual caso un terzo della profondità ’ della valva viene riempito per evitare che possa permetere la permanenza di prelievi istologici durante Tato bioptico e non avviarli verso il deposito ( Tav. l,fìg. 2 : let. a, fìg. 3 :lett. a )
Le valve possono essere anche costruite di dimensioni diverse in cui la valva b prevale sulla valva a.
Non ci sarà ’ in quest’ ultima ipotesi di costruzione necessita ’ di obliterare un terzo della profondità ’ della concavità ’ della valva a ( Tav. 2: fìg. 5 : lett. a, fìg. 6 : lett a ).
La valva b e’ quella dotata di serbatoio, non ha più’ forma concava allungata tale da formare una meta’ di un ovoide ma ha le pareti verticali per favorire l’ingresso del pezzo istologico nel deposito.
Le pareti della valva dotata del dispositivo per il deposito nella parte anteriore ed inferiore sono leggermente smusse per evitare un trauma eccessivo sul canale operatore.
Consideriamo la valva nella prima ipotesi, ossia che essa venga strutturata per essere attaccata al serbatoio con mezzi meccanici
La valva b (Tav. 1 :fig. 2) e’ costruita per guadagnare spazio in altezza a svantaggio della valva a in modo tale da poter alloggiare il dispositivo di aggancio .
Una delle soluzioni potrebbe essere quella di far passare sul margine del serbatoio in lattice un filo sottile che viene poi riportato e fatto passare in fori idonei posti nella parete della valva b e disegnati per trattenere il filo e quindi il serbatoio durante la trazione.
Un’altra soluzione potrebbe essere quella di dotare la parte prossimale del serbatoio in lattice di uno ispessimento che viene spinto in idoneo alloggiamento delle pareti della valva b di modo che rimanga in sede durante le manovre.
Il serbatoio in lattice può’ essere sostituito da un serbatoio fatto in sottilissima rete a maglie molto strette che permette più ’ facilmente l’aggancio alla valva b.
Il bordo del cilindretto di rete a maglie sottili viene ribaltato, dopo che il cilindretto medesimo e’ stato infilato per quasi tutta la sua lunghezza neirapposito foro della valva b . Il ribaltamento sul bordo seghettato della valva b e’ un idoneo freno per mantenerlo in sede durante i movimenti costruendo il tagliente della lama della valva b seghettato , o dentato, o merlato. ( Tav. 3,fig. 8,9, lett. b ) le maglie della rete si impiglieranno fra dente e dente lasciando cosi ’ libera la funzione della valva rappresentata dalla punta dei denti .
In questa ipotesi e ’ vantaggioso realizzare le due valve con precise caratteristiche reciproche sia per quanto riguarda la forma del tagliente del tagliente che sarà' non dentato per la valva a e dentato per la valva b sia per quanto riguarda proprio le dimensioni che saranno più * grandi per la valva b dentata .le dimensioni diverse riguardano rattezza in quanto la valva b dentata sopravanzo la valva a non dentata , la larghezza in cui la valva b dentata e’ più' grande della valva a non dentata ( Tav. 3: fig.
7,8,9 : lett. a, b.)
Le due valve costruite di dimensioni diverse faranno in modo che quando la pinza viene chiusa la valva a con lama tagliente ma non dentata penetrerà’ nella valva b dentata per un tratto pari all’altezza della dentatura della valva b.
Il pezzo istologico quindi sarà’ spinto verso il serbatoio e non tenderà’ a rimanere incastrato in mezzo ai denti della valva b .
Le valve a , b possono essere entrambe dentate e delle stesse dimensioni con i denti che lavorano ad incastro ( Tav. 4 : fig. 11,12 : lett a ,b.).
Le dimensioni delle valve possono variare in relazione al fatto che possono essere utilizzate in endoscopi con canale normale o canale largo in colonscopi a canale normale o largo. Possono anche raggiungere dimensioni ragguardevoli se si tiene presente che attualmente esistono in commercio apparecchi con sei millimetri di diametro.
Claims (14)
- RIVENDICAZIONI 1) Valve per pinze da biopsia endoscopica caratterizzate dal fatto che una delle valve e ’ completamente ed ampiamente traforata ed e’ dotata di un cilindretto in lattice chiuso dal lato non connesso alla valva o da un sacchetto in rete ugualmente chiuso dal lato non connesso alla valva.
- 2) Valve per pinze da biopsia endoscopica secondo la rivendicazione 1 caratterizzate, inoltre, dal fatto che l’altra valva non portatrice del serbatoio per i pezzi endoscopici e’ costruita con la concavità’ obliterata per un terzo della sua altezza.
- 3) Valve per pinze da biopsia endoscopica secondo la rivendicazione 1, caratterizzate dal fatto che le due valve possono essere costruite di dimensioni diverse : la valva portatrice del serbatoio per pezzi istologici può’ essere ossia più’ alta rispetto a quella che ne e’ sprovvista.
- 4 ) Valve per pinze da biopsia endoscopica secondo lo rivendicazioni 1 e 3 caratterizzate dal fatto che la valva non dotata di dispositivo per il recupero dei pezzi istologici può ’ essere costruita più’ piccola, con una circonferenza inferiore all’altra di modo che durante la chiusura della pinza per effettuare il prelievo penetri per un certo tratto nelValtra al fine di spingere il prelievo verso il serbatoio.
- 5) Valve per pinze da biopsia endoscopia secondo le rivendicazioni 1,2, 3, 4, caratterizzate dal fatto che il tagliente o di una valva o di entrambe può’ essere seghettato o dentato o merlato o dì qualsiasi altra forma desiderata.
- 6) Valve per pinze da biopsia endoscopia secondo le rivendicazioni 1,2, 3, 4, 5, caratterizzate dal fatto che il serbatoio può’ essere attaccato alla valva in qualsiasi modo e che la valva stessa può’ essere modificata in qualsiasi modo per permettere l’aggancio del serbatoio sia in modo fisso sia non fìsso.
- 7) Valve per pinze da biopsia endoscopia secondo le rivendiazioni 1,2, 3, 4, 5, 6 caratterizzate dal fatto che tutte e due le valve possono essere traforate e dotate ciascuna di un serbatoio.
- 8) Valve per pinze da biopsia endoscopia secondo le rivendicazioni 1,2, 3, 4, 5, 6 caratterizzate dal fatto che il serbatoio può ’ essere costruito in lattice o in rete di poliuretano o di qualsiasi altra sostanza.
- 9) Valve per pinze da biopsia endoscopia secondo le rivendicazioni 1,2, 3, 4, 5,6, 7,8, caratterizzate dal fatto che il serbatoio può ’ essere costruito in qualsiasi forma e lunghezza.
- 10) Valve per pinze da biopsia endoscopia secondo le rivendicazioni 1,2, 3, 4, 5, 6, 7, 8, 9 caratterizzate dal fatto che il serbatoio, se costruito in lattice o in qualsiasi altra sostanza ad assetto non traforato o reticolare tale da permettere il libero passaggio di aria e’ dotato di un numero variabile dì fori nella parete del cilindretto stesso che funge da deposito .
- 11) Valve per pinza da biopsia endoscopica secondo le rivendicazioni 1,2,3,4,5,6,7,8,9,10 caraterizzate dal fato che la forma dei fori e la posizione sulle parete dei cilindreti in latice per il deposito dei pezzi istologici può ’ essere quella migliore desiderata..
- 12) Valve per pinza da biopsia endoscopica secondo le precedenti rivendicazioni 1,2,3,4,5,6,7,8,9,10,11, e che sono caraterizzate dal fatto che possono avere qualsiasi dimensioni in relazione al diametro del canale dell’endoscopio per il quale vengono costruito.
- 13) Valve per pinze da biopsia endoscopica secondo le precedenti rivendicazioni 1,2,3,4,5,6,7,8,9,10,11,12. e caratterizzate dal fatto che può’ esistere qualsiasi rapporto di forma e di dimensione fra le due valve.
- 14) Valva per pinza da biopsia endoscopica secondo le precedenti rivendicazioni 1,2,3,4,5,6,7,8,9,10,11,12,13, e caraterizzate dal fato che l’angolo di apertura delle due valve può essere di qualsiasi ampiezza come di qualsiasi tipo può essere il sistema di aggancio delle due valve alla strutura principale della pinza .
