Abstract
Τα στοιχεία (τμήματα) προθέσεων τεχνητών αρθρώσεων ισχίου και γόνατος αγκυρούνται εις το οστούν, δι'εφαρμογής προεντάσεως. Η παρεμβολή αρθρώσεως μεταξύ προθέσεως και φορέως εφελκυσμού συμβάλλει εις την ελαστικότητα και διάρκειαν του συστήματος. Η αγκύρωσις των προθέσεων εις μεγάλην απόστασιν εξ αυτών δημιουργεί τάσεις θλίψεως εις το οστούν οι οποίες είναι οστεογεννετικές, και μειώνει την διαφοράν μέτρου ελαστικότητος προθέσεως και οστού. Με την εφαρμογήν προεντάσεως ελαχιστοποιούνται αι μεταξύ προθέσεως και οστού μικροκινήσεις και αποφεύγεται η εκ της αποξορτίσεως των οστικών δοκίδων οστεοπόρωση, που είναι αιτία χαλαρώσεως των αρθρώσεων. Η τεχνική τοποθετήσεως των προθέσεων αυτών, σύμφωνα με τις αξιώσεις 1-10, είναι απλή και δεν δημιουργεί τεχνικά χειρουργικά προβλήματα. Δια της εφαρμογής της προεντάσεως μειούται ο χρόνος βιολογικής προσαρμογής της προθέσεως με το οστούν και επιτρέπεται η πρώιμος φόρτισις των τεχνητών αρθρώσεων. Εξ άλλου, το οστούν προσαρμοζόμενο εις τας τάσεις θλίψεως της προθέσεως εκ της προεντάσεως, αντιδρά δι' οστεογεννέσεως και έτσι λειτουργεί το σύστημα και όταν παύση να υπάρχη η αρχικώς εφρμοσθείσα προέντασις. Η ελαχιστοποίησις των μικροκινήσεων μεταξύ προθέσεως και οστού, συμβάλλει εις την επί μακρόν παραμονήν των προθέσεων. Η ονομασία των προθέσεων αυτών είναι προθέσεις προεντεταμένες. ωThe elements (parts) of artificial hip and knee joint prostheses are anchored to the bone, by applying pretension. The interposition of articulation between the prosthesis and the tension carrier contributes to the flexibility and durability of the system. The anchoring of the prostheses at a long distance from them creates compressive stresses in the bone which are osteogenic, and reduces the difference in elasticity measure of the prosthesis and bone. By applying pre-tensioning, the micro-movements between the prosthesis and the bone are minimized and osteoporosis, which is the cause of loosening of the joints, is avoided due to the detachment of the bone trabeculae. The technique of placing these prostheses, according to claims 1-10, is simple and does not create technical surgical problems. Through the application of pretensioning, the time for biological adaptation of the prosthesis to the bone is reduced and early loading of the artificial joints is allowed. On the other hand, the bone adapting to the compressive stresses of the prosthesis from the pretension, reacts by ossification and this is how the system works even when the initially applied pretension ceases to exist. The minimization of the micro-movements between the prosthesis and the bone contributes to the long-term stay of the prostheses. The name of these prepositions are pre-tense prepositions. oh
