The present invention provides a comprehensive computerized patient record (CPR) system including a graphical user interface (50) with a three-screen organization method. The method organizes the medical data entry to conform with the way that medical care providers are trained to gather and evaluate data. The present invention also includes the integration of the patient encounter and medical data entry (90) with user customizable Visit Outlines (84). Order Sets, carepath regimens, wellness guides (74), or the like.
