Description
DOMAINE TECHNIQUE TECHNICAL AREA
Il s'agit d'un innovation médicale concernant les hémorragies survenant dans les suites des accouchements. Il s'agit d'une prothèse en tissu résorbable à la forme d'un sac qui permet d'assurer une compression mécanique de l'utérus. Les hémorragies sévères dans les suites immédiates de l'accouchement constituent la première cause de mortalité maternelle néo-natale. Elles sont liées dans quatre-vingt pour cent des cas à une rétraction insuffisante de l'utérus qui survient à la suite de l'accouchement et à la délivrance du placenta. De façon physiologique, l'utérus se contracte naturellement sous l'effet d'une hormone appelée ocytocine . Lorsque cette réaction physiologique ne survient pas, l'utérus ne se contracte pas et une hémorragie sévère survient. Le sac utérin hémostatique permet d'obtenir de façon mécanique cette contraction de l'utérus afin d'arrêter l'hémorragie. 1 ÉTAT DE LA TECHNIQUE ANTÉREURE This is a medical innovation concerning haemorrhages occurring as a result of childbirth. It is a resorbable fabric prosthesis in the shape of a bag that provides mechanical compression of the uterus. Severe haemorrhage in the immediate aftermath of childbirth is the leading cause of neonatal maternal mortality. In eighty percent of cases, they are associated with insufficient retraction of the uterus that occurs as a result of delivery and delivery of the placenta. Physiologically, the uterus naturally contracts under the effect of a hormone called oxytocin. When this physiological reaction does not occur, the uterus does not contract and severe bleeding occurs. The hemostatic uterine sac allows to mechanically obtain this contraction of the uterus in order to stop the hemorrhage. 1 STATE OF THE TECHNIQUE ANTÉREURE
Actuellement, la prise en charge chirurgical en urgence comporte quatre méthodes . Premièrement : l'embolisation sous contrôle radiologique( l'obturation des vaisseaux utérins par injection de particules dans les artères utérines . Cette technique , efficace, est difficile à mettre en place car nécessite un important plateau technique bien souvent à distance des maternités . Le délai de prise en charge trop long ne permet pas de proposer cette technique de façon courante . Deuxièmement : la ligature chirurgicale des artères nourricières de l'utérus ( artères hypogastriques et utérines) . Cette technique est rapide mais techniquement difficile. Troisièmement : les plicatures par fils résorbables trans-fixants l'utérus . Cette technique est difficile et assez longue à réaliser. Le passage des fils à travers le muscle est source de saignements au niveau des points de passage des aiguilles. Quatrièmement : l'hystérectomie d'hémostase consiste à réaliser l'ablation radicale de l'utérus. Cette technique est encore largement utilisée après échec des précédentes techniques ou urgence vitale non contrôlés. Cette technique chirurgicale doit être évitée en raison du caractère irréversible pour des patientes en âge de procréer . 2 EXPOSE DE L'INVENTION Currently, urgent surgical management has four methods. Le sac hémostatique est constitué d'une prothèse (1) en fils tressés résorbables constituant un treillis. Elle dispose de plusieurs fils de serrage verticaux (2) (figure 1. figure 3) , horizontaux (3) (figure 1. figure 2) et circulaires (4) ( figure 1. figure 3). Cette technique très simple vise à réaliser une compression externe sans trans-fixation grâce à des fils placés sur un sac résorbable. Dans un premier temps, l'utérus (7) est placé dans le sac hémostatique utérin.(1) ( figure 4). Le fil circulaire (4) à l'entrée du sac utérin hémostatique est serré en premier afin de permettre aux deux autres serrage d'exercer une compression. Les fils verticaux (2) sont fixés à l'arrière au niveau des ligaments utéro-sacrés (6) proches de leur insertion sur l'utérus puis serrés sur la face antérieur( figure 3 et figure 5). Les fils horizontaux (3)sont serrés et permettent une compression latérale du sac hémostatique utérin(1).( fig. 2 et figure 5 ).
