Abstract

The device i.e. hemostatic uterine bag (1), has an absorbable enlaced prosthesis formed of a mesh, and provided with an orifice through which a uterus is entered. The prosthesis is equipped with vertical wires (2), horizontal wires (3) and circular wires (4), where the wires are passed through the mesh and exert compression on the uterus placed inside by clamping. The vertical wires are attached to a posterior surface of the prosthesis to allow fixation with uterine ligaments. The circular wires maintain the uterus in a combustion chamber.

Description

DOMAINE TECHNIQUE TECHNICAL AREA

1 ÉTAT DE LA TECHNIQUE ANTÉREURE This is a medical innovation concerning haemorrhages occurring as a result of childbirth. It is a resorbable fabric prosthesis in the shape of a bag that provides mechanical compression of the uterus. Severe haemorrhage in the immediate aftermath of childbirth is the leading cause of neonatal maternal mortality. In eighty percent of cases, they are associated with insufficient retraction of the uterus that occurs as a result of delivery and delivery of the placenta. Physiologically, the uterus naturally contracts under the effect of a hormone called oxytocin. When this physiological reaction does not occur, the uterus does not contract and severe bleeding occurs. The hemostatic uterine sac allows to mechanically obtain this contraction of the uterus in order to stop the hemorrhage. 1 STATE OF THE TECHNIQUE ANTÉREURE

First: embolization under radiological control (filling the uterine vessels by injecting particles into the uterine arteries) This technique, effective, is difficult to implement because requires a large technical platform often far from maternity. too long does not allow this technique to be used routinely Secondly: the surgical ligation of the uterine feeder arteries (hypogastric and uterine arteries) This technique is fast but technically difficult Third: the plication by wire Absorbable trans-fixing uterus. This technique is difficult and long enough to achieve. The passage of son through the muscle is a source of bleeding at the points of passage of needles.Fourth: Hysterectomy haemostasis is to achieve radical removal of the uterus. This technique is still widely used after with previous techniques or vital emergency not controlled. This surgical technique should be avoided because of the irreversible nature of patients of childbearing age. 2 SUMMARY OF THE INVENTION

Figure 5: Hemostatic uterine sac, compression of the uterus after complete tightening.

The hemostatic bag consists of a prosthesis (1) of resorbable braided son constituting a lattice. It has several vertical clamping wires (2) (Figure 1. Figure 3), horizontal (3) (Figure 1. Figure 2) and circular (4) (Figure 1. Figure 3).

The vertical threads (2) are attached to the posterior surface of the prosthesis allowing fixation to the uterine ligaments (6) (figure 3). Their tightening makes it possible to exert compression in the direction of the height of the bag (FIG. 3 and FIG. 5).

The horizontal son placed on the posterior face allow after tightening to exert a compression in the direction of the width (figure 1. figure 2. figure 5).

The circular wire (4) at the entrance to the bag keeps the uterus (7) in the compression chamber (Figure 1, Figure 3 and Figure 5).

This very simple technique aims to achieve external compression without trans-fixation through son placed on a resorbable bag. A series of son placed in horizontal and vertical and circular loops allow a mechanical compression of the uterus. This technique is simple, fast and reproducible.

The size of the wide bag with the following dimensions: 20 centimeters in height (h), 17 centimeters in width (1), rounded bottom. 3 The small bag size with the following dimensions: 18 centimeters in height (h), 15 centimeters in width (1), rounded bottom.

At first, the uterus (7) is placed in the uterine hemostatic sac (1) (Figure 4). The circular wire (4) at the entrance to the hemostatic uterine sac is tightened first to allow the other two clamps to exert compression. The vertical threads (2) are attached to the uterus at the level of the uterosacral ligaments (6) close to their insertion on the uterus and then tightened on the anterior face (Figure 3 and Figure 5). The horizontal threads (3) are tight and allow lateral compression of the uterine hemostatic sac (1) (Fig. 2 and Fig. 5).
