Description
COMPOSANT PROTHETIQUE PARTIELLEMENT EN POLYMERE, NOTAMMENT EN POLYETHYLENE PARTIALLY POLYMERIC PROTHETIC COMPONENT, IN PARTICULAR POLYETHYLENE
La présente invention concerne un composant prothétique, notamment un implant orthopédique, tel qu'un composant glénoïdien de prothèse d'épaule. Elle concerne également un ensemble d'implantation comprenant un tel composant. L'invention s'intéresse plus précisément aux composants incluant un corps en polymère, notamment en polyéthylène à fixer à un os. Dans la mesure où le polyéthylène n'établit que peu de liaisons physico-chimiques avec la matière osseuse constituant l'os sur lequel le corps du composant est à fixer, la transmission des contraintes au niveau de l'interface de fixation entre le corps du composant et l'os est en général basée principalement sur une complémentarité de formes et/ou sur des éléments mécaniques rapportés assurant une fixation primaire. Une autre solution, alternative ou complémentaire, consiste à interposer une couche de ciment entre l'os et la face de fixation du corps en polyéthylène. Dans tous les cas, la fixation sur le long terme, généralement qualifiée de fixation secondaire, est de mauvaise qualité, voire quasiment inexistante. Le but de la présente invention est de proposer un composant prothétique partiellement en polymère, notamment en polyéthylène, dont la face de fixation à un os permet une fixation secondaire fiable et importante. A cet effet, l'invention a pour objet un composant prothétique comportant un corps en polymère, notamment en polyéthylène, qui présente une face de fixation à un os, caractérisé en ce qu'un textile est fixé à la face de fixation. L'idée à la base de l'invention est d'utiliser la capacité de fixation du polymère pour rapporter fixement, notamment par soudage, sur la face de fixation du corps du composant un textile technique qui, avantageusement, peut être colonisé par de la matière osseuse provenant de l'os sur lequel est implanté le composant et/ou peut être utilisé avec une matière de scellement à interposer entre le corps du composant et l'os précité. Ce textile va, en quelque sorte, se comporter pour l'os comme un support de croissance jusqu'au corps et/ou comme support de la matière de scellement, ce qui fournit une fixation secondaire remarquable. The idea underlying the invention is to use the fixing capacity of the polymer to fix, particularly by welding, on the fastening face of the component body a technical textile which, advantageously, can be colonized by the bone material from the bone on which the component is implanted and / or may be used with a sealing material to be interposed between the body of the component and the aforementioned bone. This textile will, in a way, behave for the bone as a growth support to the body and / or as a support for the sealing material, which provides a remarkable secondary fixation. At the same time, the bond, particularly welded, between the polymer body and the textile is particularly strong and reliable.
L'invention s'applique à des composants prothétiques divers, en particulier à des composants de prothèses orthopédiques qui, en service, sont soumis à des contraintes à transmettre à des os sur lesquels sont fixés ces composants. C'est notamment le cas pour les composants glénoïdiens de prothèse d'épaule dont le corps en polymère, notamment en polyéthylène, est à fixer à la glène osseuse de l'omoplate d'un patient. On notera que la matière polymère évoquée ici n'est pas limitée au polyéthylène « pur », mais vise, de manière générale, les matières synthétiques à base de polyéthylène, telles que le PERD (polyéthylène haute densité), ainsi que d'autres polymères biocompatibles. En pratique, la présence du textile sur la face de fixation du corps du composant selon l'invention est avantageusement compatible avec la présence de moyens de fixation primaire dont est potentiellement pourvue cette face de fixation. Ainsi, pour reprendre l'exemple des composants glénoïdiens de prothèse d'épaule, la face de fixation sur laquelle est soudé le textile de colonisation osseuse peut être également pourvue d'une quille ou de pions d'ancrage dans la glène de l'omoplate. Suivant des caractéristiques avantageuses du composant prothétique conforme à l'invention, prises isolément ou selon toutes les combinaisons techniquement possibles : - le textile est fixé par soudage sur la face de fixation ; - le textile est un textile de colonisation osseuse ; - le textile de colonisation osseuse est imprégné d'une substance liquide ou visqueuse ; - la substance liquide ou visqueuse est choisie parmi des dérivés de collagène, une solution contenant des facteurs de croissance osseuse, une solution contenant des cellules souches et une solution contenant des protéines ; - le textile comprend un tissu, qui est notamment constitué d'un patch tissé ou tressé ; - le textile comprend un non tissé, qui est notamment constitué de fibres lâches pulvérisées ; - le textile inclut des fibres en partie libres, qui s'étendent de manière sensiblement perpendiculaire par rapport au reste du textile ; - le textile présente une structure multicouche. L'invention a également pour objet un ensemble d'implantation, comprenant un composant prothétique tel que défini ci-dessus et une matière de scellement adaptée pour être rapportée de manière interposée entre la face de fixation du composant et un os, en s'insinuant dans le textile. L'invention sera mieux comprise à la lecture de la description qui va suivre, donnée uniquement à titre d'exemple et faite en se référant aux dessins sur lesquels : - la figure 1 est une vue schématique d'un composant prothétique conforme à l'invention, associé à un os sur lequel ce composant est à fixer ; - la figure 2 est une section à plus grande échelle du détail cerclé Il sur la figure 1 ; - les figures 3 et 4 sont des vues analogues à la figure 2, illustrant respectivement deux variantes conformes à l'invention ; et - la figure 5 est une vue analogue à la figure 2, illustrant un ensemble d'implantation conforme à l'invention. Sur la figure 1 est représenté un composant glénoïdien 1 appartenant à une prothèse d'épaule. Ce composant 1 comporte un corps 2 qui, d'un côté, présente une face 2A destinée à être appuyée et fixée contre la glène G de l'omoplate d'un patient. Du côté opposé, le corps 2 présente une face 2B destinée à s'articuler contre une tête humérale soit naturelle, soit prothétique, non représentée. Dans l'exemple de réalisation considéré ici, le corps 2 est réalisé intégralement en polyéthylène. Toutefois, en variante non représentée, seule une partie de ce corps peut être prévue en polyéthylène, du moment que cette partie en polyéthylène présente la face de fixation 2A. A titre optionnel, la face 2A est pourvue d'une quille 4 d'ancrage osseux dans la glène G. A titre de variante non représentée, d'autres éléments de fixation primaire que cette quille 4 peuvent être envisagés, en vue de fixer le corps 2 à la glène G par coopération mécanique avec cette dernière, par exemple par coincement comme dans le cas de la quille 4. Selon l'invention, le composant glénoïdien 1 comporte un textile de colonisation osseuse 6 rapporté à demeure par soudage sur la face de fixation 2A. Comme représenté à plus grande échelle sur la figure 2, le textile 6 considéré dans ce mode de réalisation comprend, voire consiste en un tissu, c'est-à-dire en un entrelacement de fils 61, obtenu notamment par tissage et/ou tressage. En pratique, le tissu formant le textile 6 se présente sous la forme d'un patch tissé et/ou tressé, qui est rapporté et soudé sur la face de fixation 2A du corps 2. A cette fin, la couche superficielle du corps 2, au niveau de sa face 2A, est suffisamment échauffée pour fondre localement et ainsi réaliser des jonctions de soudure 21 avec au moins une partie des fils 61, comme montré de manière schématique sur la figure 2. La fusion superficielle du corps 2 est réalisée de diverses manières, notamment par chauffage superficiel, par applications d'ultrasons, à l'aide d'un laser, etc. Le matériau constituant les fils 61 du tissu du textile 6 est choisi pour, d'une part, supporter sans endommagement la soudure avec le polyéthylène du corps 2 et, d'autre part, permettre, voire favoriser la colonisation des espaces libres entre les fils 61 par de la matière osseuse provenant de la glène G après implantation du composant glénoïdien 1, c'est-à-dire après mise en place de la face de fixation 2A contre la glène. Par ailleurs, ce matériau peut aussi bien être prévu non résorbable, que bio-résorbable. En pratique, le choix du matériau précité est vaste, dans le sens où de nombreux matériaux présentent une bonne affinité avec la matière osseuse, cette dernière ayant naturellement tendance à coloniser des structures alvéolaires placées au contact d'un os sain. Ainsi, à titre d'exemple, le matériau constituant les fils 61 est du polyéthylène, du polyuréthane, du polypropylène, etc. Sur la figure 3 est représenté un autre mode de réalisation qui ne se distingue du mode de réalisation de la figure 2 que par la nature du textile de colonisation osseuse soudé sur la face de fixation 2A du corps 2 : en effet, ce textile, référencé 8 sur la figure 3, comprend, voire consiste en un non tissé. En particulier, le non tissé du textile 8 est obtenu par pulvérisation de fibres lâches 81, directement déposées sur la face de fixation 2A, pour établir avec cette dernière les jonctions de soudure 21. Bien entendu, le non tissé du textile 8 peut, en variante, être d'abord fabriqué de manière ad hoc, puis être rapporté et soudé à la face de fixation 2A, de façon similaire à ce qui a été décrit plus haut pour le textile tissé 6. The invention also relates to an implant assembly, comprising a prosthetic component as defined above and a sealing material adapted to be inserted interposed between the attachment face of the component and a bone, by insinuating in the textile. The invention will be better understood on reading the description which follows, given solely by way of example and with reference to the drawings, in which: FIG. 1 is a diagrammatic view of a prosthetic component conforming to invention, associated with a bone on which this component is to fix; FIG. 2 is a section on a larger scale of the circled detail II in FIG. 1; FIGS. 3 and 4 are views similar to FIG. 2, respectively illustrating two variants in accordance with the invention; and FIG. 5 is a view similar to FIG. 2, illustrating an implantation assembly according to the invention. According to the invention, the glenoid component 1 comprises a bone colonization textile 6 permanently attached by welding on the face fixing 2A. As shown on a larger scale in FIG. 2, the textile 6 considered in this embodiment comprises, or even consists of, a fabric, that is to say an intertwining of threads 61, obtained in particular by weaving and / or braiding . In practice, the fabric forming the textile 6 is in the form of a woven and / or braided patch, which is attached and welded to the fixing face 2A of the body 2. For this purpose, the surface layer of the body 2, at its face 2A, is sufficiently heated to locally melt and thus achieve solder joints 21 with at least a portion of the son 61, as shown schematically in Figure 2. In particular, the non-woven textile 8 is obtained by spraying loose fibers 81, directly deposited on the fixing face 2A, to establish with it the solder joints 21. De même, à titre d'option avantageuse, le textile 6 ou 8 inclut des fibres en partie libres, qui s'étendent de manière sensiblement perpendiculaire par rapport au reste du textile, en direction opposée à la face de fixation 2A. Un exemple de mise en oeuvre de cette option est montré sur la figure 4, avec le textile tissé 6 présentant de telles fibres référencées 9. En pratique, ces fibres en partie libres peuvent être aussi bien crochues à leur extrémité libre, comme montré sur la figure 4 pour les fibres 9, que lisses sur toute leur étendue en saillie du reste du textile 6 ou 8. Dans tous les cas, ces fibres en partie libres forment entre elles des espaces libres propices à la croissance osseuse. Divers aménagements et variantes au composant glénoïdien 1 décrit jusqu'ici sont par ailleurs envisageables. A titre d'exemples : - plutôt que le textile 6 ou 8 soit soudé sur la face de fixation 2A du corps 2, ce textile peut être fixé à demeure à cette face 2A par d'autres moyens rapportés ad hoc, tels que de la colle par exemple ; - plutôt que le textile fixé à la face de fixation 2A du corps 2 soit monocouche, comme les textiles 6 et 8 montrés sur les figures 2 à 3, ce textile peut être constitué de plusieurs couches superposées, autrement dit présenter une structure multicouche, en combinant d'ailleurs éventuellement des couches de tissu et de non-tissé ; et/ou - les aménagements décrits pour le composant glénoïdien 1 sont applicables à divers composants prothétiques dont les corps sont en polymère biocompatible, au moins pour ce qui concerne leur face de fixation à un os.
