Tarjeta de identificación del grupo sanguíneo caracterizada porque en el anverso figuran dos recuadros en uno de los cuales dividido en dos secciones se anota el nombre apellidos y fecha de nacimiento del titular y en la parte inferior su dirección; y en el otro re cuadro dividido en cuatro zonas se anotan el grupo sanguíneo del titular el Rh la fecha el nombre del facultativo que realice el análisis y la firma del mismo que garantice la autenticidad de los datos consignados. En el reverso de la tarjeta se hace figurar mediante estampación una somera explicación científica de la importancia de la determinación del grupo sanguíneo.
