Abstract
Description
The invention relates to an instrument for the dorsal bracing of the spine damage. This makes it possible to perform percutaneous surgery instead of surgery to set and thus perform an elastic bracing of the dome processes. The invention can be used in orthopedics, as well as in accident and neurosurgery.
Unstable spinal fractures are an absolute indication for surgery because there are always injuries to the tape. The operation takes this into account to wear. If there is an osteoligamentous injury, it should be ventral and dor be operated on. The intervention is initially carried out ventrally, as a second intervention stabilization is performed dorsally. So far, the latter ope has been performed ration so that the dome processes are exposed from a large skin incision and one does a loop under sight. This is for a seriously injured Second operation very stressful.
Thanks to the new, percutaneous method, this intervention is quick and effortless feasible. If there is only a rupture of a band, the intervention is considered the sole ope ration applied. It enables biological tension by replacing the Ribbon by means of a Dacron loop soaked with your own blood. A good attachment to the injured interspinosum ligament and adhesion are thus secured. The percutaneous method is also in the event of instability of a spinal column segment follow intervertebral disc damage or can be used after intervertebral disc surgery. they can be used in both the cervical and lumbar areas.
The instrument according to the invention consists of a guide tube, which has a cap rotates. This cap is with a Zen running axially in the guide tube tralstab connected. This extends to the lower end of the guide tube and has one Longitudinal slot on. Correspondingly, there is a side opening in the guide pipe. A flat spring steel is inserted upright through this opening and in Longitudinal slot of the central rod anchored. The flat spring steel is around the central rod wrapped and placed in the guide tube, whereby between flat spring steel and Füh a Teflon sliding ring. The lower part of the guide tube is through locked a conical closure that runs out lancet-shaped. The guide tube there is an adjusting sleeve which is rotatably supported by a guide ring. The Adjusting sleeve has a side slot at the same height as the guide tube. Immit Telbar above there is a firmly fixed thin in the longitudinal direction of the adjusting sleeve Pipe through which steel silk, hereinafter referred to as guide wire, is passed. The end of Guide wire is connected to a slider, which through the longitudinal slot of the Rohres is performed. The initial part of the rolled flat spring steel is outside of the guide tube. The tip of the flat spring steel is based on the principle of the eye of the needle designed. The guide wire lies on the inside of the flat spring steel and is replaced by a concealed gutter, the front end is fixed to the rear eye section. In the In the middle of the guide tube there is a tiltable metal clamp, in which a small metal tube with an integrated rod is inserted. This rod, below Called hook bar, is freely movable and carries a notch hook at the top. The height of the hook bar can be adjusted using a horizontally attached lever and be rotated.
The operation of the instrument will be described using the following embodiment will. The operation takes place under image converter control. About a little stitch the instrument is advanced to the vertebral arch. By rotating the The central spring releases the flat spring steel. This tries according to the given shape to bend. By turning the adjusting sleeve, the Flachfe derstahl be deformed so that it emerges in an elongated form. The tip of the Flachfe derstabs penetrates the ligamentum interspinosum and reaches the opposite side of the Spinous processes. The twist of the adjusting sleeve is gradually reduced and the Flat spring steel by rotating the central rod along the dome extensions led above. The process is controlled under image converter control. When reaching the adjusting sleeve becomes the upper edge of the spinous process above the injury brought into zero position and the curvature of the flat spring steel by tightening the Conducting wire. At the same time, the flat spring steel is further turned. The tip reaches the exit side through the interspinosum ligament the spinous processes. Through a precise coordination between pre-turning and Kor rectification of the flat spring steel, its tip is guided to the adjusting sleeve and is supported from there. The implant is inserted over the hook rod and into the eye of the Flachfe anchored. By turning back the guide tube, the flat spring steel becomes in the starting position and the implant in the desired position.
