Description
Die vorliegende Erfindung betrifft ein Mittel gegen obstruktive Blasenentlee
rungsstörung, das als Wirkstoff 4-{3-[3-(4-Fluorbenzoylpiperidino)-propo
xyl]-4-methoxy-phenyl}-2-pyrrolidon enthält.
In der deutschen Offenlegungsschrift DE-OS 28 34 114 wird die Verbindung 4-
{3-[-(4-Fluorbenzoylpiperidino)-propoxyl]-4-methoxyphenyl}-2-pyrrolidon
(I) mit gefäßerweiternder und blutdrucksenkender Wirkung beschrieben.
In vitro Untersuchungen an Prostatapräparationen zeigen, daß die Verbindung I
einen positiven Effekt auf pharmakologisch induzierte Prostatakontraktionen hat
und somit die Symptomatik der benignen Prostatahyperplasie (obstruktive Bla
senentleerungsstörungen) günstig beeinflußt. Die Verbindung I wirkt in einem
Dosisbereich, der im Vergleich zu bekannten Verbindungen auf die Gefäße noch
nicht so wirksam ist, so daß die gefäßbedingten Effekte wie orthostatische Dys
regulation und Blutdruckabfall als Nebenwirkungen noch nicht so stark in Er
scheinung treten.
Die Herstellung der Verbindung I wird ausführlich in der deutschen Offenle
gungsschrift DE-OS 34 27 797 beschrieben. Die neue Wirkung der Verbindung I
wurde an isolierten Streifenpräparaten der Kaninchenprostata, die sich bekann
termaßen sehr ähnlich wie die menschliche Prostata verhält, untersucht.
Für therapeutische Zwecke wird die Verbindung I oral in einer täglichen Dosis
von 0,1-500 mg, vorzugsweise von 0,25-50 mg, verabreicht.
Die oral zu verabreichenden Darreichungsformen enthalten den Wirkstoff in einer
Menge 0,05 bis etwa 20 mg pro Dosiseinheit.
Für die Verabreichung des Arzneimittels können die Wirkstoffe in Form von
Tabletten, eines Granulats, eines Pulvers, von Kapseln u. ä. vorliegen. Der
Wirkstoff wird in der Regel zusammen mit Hilfsstoffen oder Träger verabreicht,
wie beispielsweise Lactose, Magnesiumstearat, Kaolin, Saccharose, Maisstärke,
Talk, Stearinsäure, Gelatine, Agar-Agar, Pektin usw.
Es wurden isolierte Prostatastreifen der Kaninchenaorta mit Phenylephrin kon
trahiert und nach Zugabe unterschiedlicher Dosen der Verbindung I, von Prazosin
und Ketanserin eine Verschiebung der für Phenylephrin erhaltenen Konzentra
tionswirkungskurve ermittelt. Es wurde der pA2-Wert nach Arunlakshana und
Schild bestimmt.
Für die Verbindung I wurde an der Prostata ein pA2-Wert von 9,08 ermittelt. An
Gefäßen der Rattenaorta beträgt der pA2-Wert 9,16.
Für die Vergleichsverbindungen, Prazosin und Ketanserin, betragen die pA2-Werte
an der Prostata 8,84 und 8,04 und an den Gefäßen 9,84 und 8,10.
Diese Ergebnisse zeigen, daß Verbindungen mit α 1- und 5-HT2- antagonisitischer
Komponente an Gefäßen und an Prostatagewebe sehr ähnliche Potenz aufweisen,
während der α 1-Antagonist Prazosin eine um den Faktor 10 schwächere Potenz an
der Prostata im Vergleich zu den Gefäßen hat.
Claims (2)
1. Mittel zur Behandlung der obstruktiven Blasenentleerungsstörung, enthaltend
4-{3-[3-(4-Fluorbenzoylpiperidino)-propoxyl]-4-methoxyphenyl}2-pyrroli
don und übliche Hilfs- und Trägerstoffe.
2. Verwendung von 4-{3-[-(4-Fluorbenzoylpiperidino)-propoxyl]-4-methoxyphe
nyl}-2-pyrrolidon zur Behandlung von obstruktiver Blasenentleerungs
störung.
